  • Age of onset of cerebral venous thrombosis: the BEAST study

    • Background: Cerebral venous thrombosis (CVT) is an uncommon cause of stroke in young adults. We aimed to determine the impact of age, gender and risk factors (including sex-specific) on CVT onset. Methods: We used data from the BEAST (Biorepository to Establish the Aetiology of Sinovenous Thrombosis), a multicentre multinational prospective observational study on CVT. Composite factors analysis (CFA) was performed to determine the impact on the age of CVT onset in males and females. Results: A total of 1309 CVT patients (75.3% females) aged > 18 years were recruited. The overall median (IQR-interquartile range) age for males and females was 46 (35-58) years and 37 (28-47) years (p < 0.001), respectively. However, the presence of antibiotic-requiring sepsis (p = 0.03, 95% CI 27-47 years) among males and gender-specific risk factors like pregnancy (p < 0.001, 95% CI 29-34 years), puerperium (p < 0.001, 95% CI 26-34 years) and oral contraceptive use (p < 0.001, 95% CI 33-36 years) were significantly associated with earlier onset of CVT among females. CFA demonstrated a significantly earlier onset of CVT in females, similar to 12 years younger, in those with multiple (> 1) compared to '0' risk factors (p < 0.001, 95% CI 32-35 years). Conclusions: Women suffer CVT 9 years earlier in comparison to men. Female patients with multiple (> 1) risk factors suffer CVT similar to 12 years earlier compared to those with no identifiable risk factors.

  • Vaccine hesitancy decreases in rheumatic diseases, long-term concerns remain in myositis: a comparative analysis of the COVAD surveys

    • ObjectiveCOVID-19 vaccines have a favorable safety profile in patients with autoimmune rheumatic diseases (AIRDs) such as idiopathic inflammatory myopathies (IIMs); however, hesitancy continues to persist among these patients. Therefore, we studied the prevalence, predictors and reasons for hesitancy in patients with IIMs, other AIRDs, non-rheumatic autoimmune diseases (nrAIDs) and healthy controls (HCs), using data from the two international COVID-19 Vaccination in Autoimmune Diseases (COVAD) e-surveys.MethodsThe first and second COVAD patient self-reported e-surveys were circulated from March to December 2021, and February to June 2022 (ongoing). We collected data on demographics, comorbidities, COVID-19 infection and vaccination history, reasons for hesitancy, and patient reported outcomes. Predictors of hesitancy were analysed using regression models in different groups.ResultsWe analysed data from 18 882 (COVAD-1) and 7666 (COVAD-2) respondents. Reassuringly, hesitancy decreased from 2021 (16.5%) to 2022 (5.1%) (OR: 0.26; 95% CI: 0.24, 0.30, P &lt; 0.001). However, concerns/fear over long-term safety had increased (OR: 3.6; 95% CI: 2.9, 4.6, P &lt; 0.01). We noted with concern greater skepticism over vaccine science among patients with IIMs than AIRDs (OR: 1.8; 95% CI: 1.08, 3.2, P = 0.023) and HCs (OR: 4; 95% CI: 1.9, 8.1, P &lt; 0.001), as well as more long-term safety concerns/fear (IIMs vs AIRDs – OR: 1.9; 95% CI: 1.2, 2.9, P = 0.001; IIMs vs HCs – OR: 5.4 95% CI: 3, 9.6, P &lt; 0.001). Caucasians [OR 4.2 (1.7–10.3)] were likely to be more hesitant, while those with better PROMIS physical health score were less hesitant [OR 0.9 (0.8–0.97)].ConclusionVaccine hesitancy has decreased from 2021 to 2022, long-term safety concerns remain among patients with IIMs, particularly in Caucasians and those with poor physical function.

    • The present study investigates the role of micro- and nano-CeO2 reinforcements on microstructural, mechanical and tribological properties of Cr3C2-NiCr coatings deposited using high-velocity-oxy-fuel (HVOF) spray process. A novel blending method involving ultrasonication, magnetic stirring and 3D-tumbler mixing was used to prepare the micro- and nano-CeO2 reinforced Cr3C2-NiCr feedstock powders. Unreinforced Cr3C2-NiCr coatings were also prepared for comparison purposes. X-ray diffraction, FE-SEM, EDS mapping, optical microscopy and optical profilometry were used to characterize the coatings. Tribological studies were performed on the coatings using modular ball-on-disc tribometer. The results showed that nano-CeO2 reinforced coatings yielded considerable reduction in porosity, surface roughness and CoF values while significantly enhancing the hardness and fracture toughness in comparison to the micro-CeO2 reinforced and unreinforced Cr3C2-NiCr coatings. The addition of nanoCeO2 to the coatings contributed to the microstructure refinement and to the formation of stable tribo-oxide layer, resulting in a lower specific wear rate under identical test conditions. Performance of micro-CeO2 reinforced coatings was intermediate to that of nano-CeO2 reinforced and unreinforced Cr3C2-NiCr coatings.

  • A collaborative comparison of international pediatric diabetes registries

    • Background An estimated 1.1 million children and adolescents aged under 20 years have type 1 diabetes worldwide. Principal investigators from seven well-established longitudinal pediatric diabetes registries and the SWEET initiative have come together to provide an international collaborative perspective and comparison of the registries. Work Flow Information and data including registry characteristics, pediatric participant clinical characteristics, data availability and data completeness from the Australasian Diabetes Data Network (ADDN), Danish Registry of Childhood and Adolescent Diabetes (DanDiabKids), Diabetes prospective follow-up registry (DPV), Norwegian Childhood Diabetes Registry (NCDR), National Paediatric Diabetes Audit (NPDA), Swedish Childhood Diabetes Registry (Swediabkids), T1D Exchange Quality Improvement Collaborative (T1DX-QI), and the SWEET initiative was extracted up until 31 December 2020. Registry Objectives and Outcomes The seven diabetes registries and the SWEET initiative collectively show data of more than 900 centers and around 100,000 pediatric patients, the majority with type 1 diabetes. All share the common objectives of monitoring treatment and longitudinal outcomes, promoting quality improvement and equality in diabetes care and enabling clinical research. All generate regular benchmark reports. Main differences were observed in the definition of the pediatric population, the inclusion of adults, documentation of CGM metrics and collection of raw data files as well as linkage to other data sources. The open benchmarking and access to regularly updated data may prove to be the most important contribution from registries. This study describes aspects of the registries to enable future collaborations and to encourage the development of new registries where they do not exist.

  • Graphene Oxide Reinforced Magnetic FeCoNiCuZn High Entropy Alloy through Electrodeposition

    • The production of high entropy alloy-based nanocomposites is an exciting yet challenging area in terms of its scalability and industrial applications. Here we developed graphene oxide (GO) reinforced FeCoNiCuZn high entropy alloy (HEA) nanocomposites through an electrochemical approach using aqueous medium in a single step. Transmission electron microscopy observations confirmed uniformly distributed nanocrystalline dual FCC phase quinary alloy nanoparticles throughout the GO layers. On the other hand, the presence of GO affects the electrochemical reduction of multiple elements during alloy formation in the deposition process, which often leads to dual phases with slight deviations in alloy composition, unlike the pure metal-GO composites. Additionally, incorporation of GO has not shown any effect on the ferromagnetic nature of FeCoNiCuZn HEA with saturation magnetization (Ms) ∼ 43.5 emu g−1. The obtained saturation magnetization is relatively higher compared to the existing reported magnetic nanoparticles with GO. Hence, this technique shows its potential applicability and provides an old technique yet a new approach for synthesizing GO-HEA nanocomposites for various magnetic applications.
