  • Initial Experience From Implementation of Hand-Assisted Retroperitoneoscopic Live Donor Nephrectomy in Saudi Arabia

    • Introduction: Donor risks and morbidity are consequences of the invasiveness of donor nephrectomy procedure. The flank incision is currently the default donor nephrectomy procedure at the King Faisal Hospital in Saudi Arabia. In order to minimize the surgical-related trauma, we are implementing the hand-assisted retroperitoneoscopic live donor nephrectomy (HARS), which previously has been shown to promote donor safety. Here, we present our initial experience with this procedure. Material and Methods: The HARS technique was implemented at our center in 2010. We present a survey of our data regarding operative characteristics as well as donor/recipient outcome. Given the small number of cases, data are presented as median with range. Results: Between 2010 and 2013, 18 left -sided HARS nephrectomy procedures were performed. The median donor age and BMI were 26.5 (18-43) and 24.1 (18.7-30.7), respectively. The median hospitalization was 4 days (3-5). One donor presented wound seroma in the pfannenstiell incision with no need for intervention. Another donor presented unspecific thoracoabdominal pain on postoperative day 2. No intra-and postoperative bleeding was observed. The median creatinine at the current follow-up was 90 mu mol/L with 100% graft survival. Conclusion: HARS is a feasible and safe technique. However, for implementation of HARS as the default donor nephrectomy procedure more practice is needed.

    • Group B Streptococcus (GBS) causes invasive infections in human newborns. We recently showed that the GBS beta-protein attenuates innate immune responses by binding to sialic acid-binding immunoglobulin-like lectin 5 (Siglec-5), an inhibitory receptor on phagocytes. Interestingly, neutrophils and monocytes also express Siglec-14, which has a ligand-binding domain almost identical to Siglec-5 but signals via an activating motif, raising the possibility that these are paired Siglec receptors that balance immune responses to pathogens. Here we show that beta-protein-expressing GBS binds to both Siglec-5 and Siglec-14 on neutrophils and that the latter engagement counteracts pathogen-induced host immune suppression by activating p38 mitogen-activated protein kinase (MAPK) and AKT signaling pathways. Siglec-14 is absent from some humans because of a SIGLEC14-null polymorphism, and homozygous SIGLEC14-null neutrophils are more susceptible to GBS immune subversion. Finally, we report an unexpected human-specific expression of Siglec-5 and Siglec-14 on amniotic epithelium, the site of initial contact of invading GBS with the fetus. GBS amnion immune activation was likewise influenced by the SIGLEC14-null polymorphism. We provide initial evidence that the polymorphism could influence the risk of prematurity among human fetuses of mothers colonized with GBS. This first functionally proven example of a paired receptor system in the Siglec family has multiple implications for regulation of host immunity.

    • In this paper, we present the Cooperative Adaptive Cruise Control (CACC) architecture, which was proposed and implemented by the team from Chalmers University of Technology, Göteborg, Sweden, that joined the Grand Cooperative Driving Challenge (GCDC) in 2011. The proposed CACC architecture consists of the following three main components, which are described in detail: 1) communication; 2) sensor fusion; and 3) control. Both simulation and experimental results are provided, demonstrating that the proposed CACC system can drive within a vehicle platoon while minimizing the inter-vehicle spacing within the allowed range of safety distances, tracking a desired speed profile, and attenuating acceleration shockwaves.

    • Telecom operators of the high growth market of Bangladesh are facing several challenges in addressing the problem of load fluctuation and load shedding in the city areas which are already integrated in the national electricity grids. It is also facing severe difficulties in the un-tapped market of rural places which are located in the off electrical grid area. The operators need to invest a lot to maintain the uninterrupted supply of electrical power to the sites and to ensure the availability of their services. Considering the current situation of the power sector of Bangladesh, it can easily be predicted that the situation may get even worse in the coming days as the crisis of fossil fuel is building very rapidly all over the world. Under such conditions alternate power source has become a burning issue in this country. Out of the possible alternative sources, solar energy is evidenced very promising. Different technology leaders like Ericsson, Nokia, Siemens and Huawei also adopted the required solution for this type of market. This Solar powered site concept allows an operator to place their base stations with complete off grid independent energy solution. This notion will give a low total cost of ownership case, higher reliability than fossil fuel operated sites, virtually maintenance free site, free energy, and environmental friendly choices. This paper attempts to assess the probability of solar energy usage in the telecom sector of Bangladesh, its present scenario, its technical feasibility analysis and environomical analysis.

    • Cognitive Radio (CR) is a wireless advanced technology which can utilize an unlicensed as well as a licensed spectrum without a harmful interference to the primary users. An unbiased consistent spectrum estimator is required for the primary user’s detection (sensing) for distinguishing the narrow band signals in a noisy environment. Cognitive radio’s hardware solutions available in a commercial market are frequency band constrained at RF Front-End. In multi-dimensional radio spectrum space any of the dimension: time, frequency, code or space can be used as a transmission opportunity. The spectrum hole time opportunistic prediction is a promising solution to determine the free time slots for transmission within a frequency band. This paper presents classical and parametric statistical spectrum estimators for primary user’s detection. It also presents statistical auto-regressive and moving average predictive modeling for grey-hole spectrum opportunities prediction in a time domain for cognitive radios where frequency, code and space (geographical location) are operational constraints. A prototype system for a cognitive radio is built on top of the software-defined radio in a MATLAB/-Simulink and interfaced with an USRP2 main-board and RFX2400 daughter-board from Ettus Research LLC. © 2011 IEEE.
