  • Immunoreactive trypsin(Ogen) in the sera of children with recent-onset insulin-dependent diabetes and matched controls

    • To evaluate the exocrine pancreatic function at the time of diagnosis of insulin-dependent diabetes mellitus, we determined immunoreactive an-odal and cathodal trypsin(ogen) levels in sera from almost all children (n = 375) 0-14 years of age in Sweden in whom diabetes developed during 1 year, and in sex-, age-, and geographically matched control subjects (n = 312). The median level of anodal trypsin(ogen) was 5 (quartile range, 3-7) µg/L in children with newly diagnosed diabetes, compared with a median level of 7 (quartile range, 4-8) µg/L in control subjects (p < 0.0001). Similarly, the median level of cathodal trypsin(ogen) was 8 (quartile range, 4-10) µg/L in children with diabetes, compared with a median level of 11 (quartile range, 7-15) µg/L in control subjects (p < 0.0001). The median of the individual ratios between cathodal and anodal trypsin(ogen) was 1.4 in the diabetic patients and 1.7 in the control children (p < 0.001). In a multivariate test, however, only the decrease in cathodal trypsin(ogen) concentration was associated with diabetes. The levels of trypsin(ogen)s did not correlate with levels of islet cell antibodies, present in 81% of the diabetic children. Several mechanisms may explain our findings, for example, similar pathogenetic factors may affect both the endocrine and exocrine pancreas simultaneously, a failing local trophic stimulation by insulin on the exocrine cells may decrease the trypsinogen production, and there may be an increased elimination of trypsin(ogen) because of higher filtration through the kidneys in the hyperglycemic state.

  • Body composition in patients with Alzheimer's disease and healthy controls

    • In Alzheimer's disease (AD), decreased physical activity and nutritional problems are parts of the natural course with probable implications on body composition. Body fat and lean body mass were measured with a bioelectrical impedance method in 25 women who had AD according to the criteria of NINCDS- ADRDA and in 63 health age-matched controls. The patients with AD had 9.5 kg lower body weight (p<0.01), due to almost 4.0 kg lower body fat and 6.0 kg lower lean body mass (p<0.001). This might imply a higher risk for morbidity and mortality. Body weight and lean body mass decreased with age in AD patients but not in the healthy control women. In the control group, the unmarried women had more than 13 kg lower body weight and 9 kg lower body fat than the mean values of all other marital status groups (p<0.01). The maintenance of lean body mass in the very healthy old indicate the possibilities for physical activity.

  • Autoantibodies to leucocyte antigens in hydralazine-associated nephritis

    • Clinical and laboratory findings and drug history were studied in 17 patients with suspected hydralazine-associated nephritis, five of whom only had renal disease, while twelve also had extrarenal manifestations. Renal biopsies revealed extracapillary proliferative or focal segmental proliferative glomerulonephritis in 10 patients, and tubulo-interstitial nephritis in five patients. Antinuclear antibody (ANA) was found in 16 patients, but none of the 14 patients tested had antibodies to DNA. Tests for antibodies to myeloperoxidase (anti-MPO) and antibodies to neutrophil cytoplasm antigen (ANCA) were performed by ELISA. Twelve of the 14 patients tested had anti-MPO; five of these 14 patients had ANCA, while one had borderline levels. These findings suggest that hydralazine facilitates the induction of a systemic disease with multiple autoantibody production.

    • The effects of a 60 min exposure to photosynthetic photon flux densities ranging from 300 to 2200-mu-mol m-2 s-1 on the photosynthetic light response curve and on PS II heterogeneity as reflected in chlorophyll a fluorescence were investigated using the unicellular green alga Chlamydomonas reinhardtii. It was established that exposure to high light acts at three different regulatory or inhibitory levels; 1) regulation occurs from 300 to 780-mu-mol m-2 s-1 where total amount of PS II centers and the shape of the light response curve is not significantly changed, 2) a first photoinhibitory range above 780 up to 1600-mu-mol m-2 s-1 where a progressive inhibition of the quantum yield and the rate of bending (convexity) of the light response curve can be related to the loss of Q(B)-reducing centers and 3) a second photoinhibitory range above 1600-mu-mol m-2 s-1 where the rate of light saturated photosynthesis also decreases and convexity reaches zero. This was related to a particularly large decrease in PS II(alpha) centers and a large increase in spill-over in energy to PS I.

  • RECOVERY OF PHOTOSYNTHESIS AND PHOTOSYSTEM-II FLUORESCENCE IN CHLAMYDOMONAS-REINHARDTII AFTER EXPOSURE TO 3 LEVELS OF HIGH LIGHT

    • Recovery from 60 min of photoinhibitory treatment at photosynthetic photon flux densities of 500, 1400 and 2200-mu-mol m-2 s-1 was followed in cells of the green alga Chlamydomonas reinhardtii grown at 125-mu-mol m-2 s-1. These light treatments re resent photoregulation, moderate photoinhibition and strong photoinhibition, respectively. Treatment in photoregulatory light resulted in an increased maximal rate of oxygen evolution (P(max)) and an increased quantum yield (PHI), but a 15% decrease in F(V)/F(M). Treatment at moderately photoinhibitory light resulted in a 30% decrease in F(V)/F(M) and an approximately equal decrease in PHI. Recovery in dim light restored F(V)/F(M) within 15 and 45 min after high light treatment at 500 and 1 400-mu-mol m-2 s-1 respectively. Convexity (THETA), a measure of the extent of co-limitation between PS II turnover and whole-chain electron transport, and PHI approached, but did not reach the control level during recovery after exposure to 1 400-mu-mol m 2 s-1, whereas P(max) increased above the control. Treatment at 2200-mu-mol m-2 s-1 resulted in a strong reduction of the modeled parameters PHI, THETA and P(max). Subsequent recovery was initially rapid but the rate decreased, and a complete recovery was not reached within 120 min. Based on the results, it is hypothesized that exposure to high light results in two phenomena. The first, expressed at all three light intensities, involves redistribution within the different aspects of PS II heterogeneity rather than a photoinhibitory destruction of PS II reaction centers. The second, most strongly expressed at 2200-mu-mol m-2 s-1, is a physical damage to PS II shown as an almost total loss of PS II(alpha) and PS II Q(B)-reducing centers. Thus recovery displayed two phases, the first was rapid and the only visible phase in algae exposed to 500 and 1 400-mu-mol m-2 s-1. The second phase was slow and visible only in the later part of recovery in cells exposed to 2 200-mu-mol m-2 s-1.

    • Islet cell antibodies (ICA), fasting plasma C-peptide, and HbA(1c) were evaluated up to 10 years after diagnosis in 52 patients with diabetes diagnosed in adult age (mean age at diagnosis 53 ± 2 yr, range 21-76 yr) with a high (>20 IU/day) insulin requirement (group A) and in 50 matched control patients with diabetes diagnosed in adult age (mean age at diagnosis 54 ± 2 yr, range 24-73 yr) with low or no (<20 IU/day) insulin requirement (group B) during the first 3 yrs after diagnosis. Patients in group A showed a significantly higher frequency of ICA (37% [19/52] vs 10% [5/50]; p < 0.05), lower C-peptide (0.33 ± 0.06 nmol/l vs 0.56 ± 0.05 nmol/l; p < 0.001) and higher HbA(1c) (8.15 ± 0.35% vs 6.81 ± 0.25%; p < 0.01) values than group B patients. ICA positive group A patients (0.22 ± 0.06 nmol/l) showed significantly lower C-peptide values than ICA negative group A (0.40 ± 0.09 nmol/l; p < 0.05) patients. C-peptide values correlated with body mass index (BMI) in ICA negative patients both in group A (r = 0.66; p < 0.001) and in group B (r = 0.34; p < 0.05) but not in ICA positive group A patients in whom C-peptide values correlated with age (r = 0.48; p < 0.05). There was a correlation between HbA(1c) and C-peptide values in ICA negative (n = 78; r = -0.31; p < 0.01) but not in ICA positive patients (n = 24; r = -0.17; NS). ICA were most frequent in females (20/59 females vs 4/43 males; p < 0.01). ICA is associated with decreased pancreatic β-cell function in patients with diabetes diagnosed in adult age and may be detected up to 10 yr after diagnosis. Determination of ICA in patients with diabetes diagnosed in adult age improve the classification and etiological diagnosis of diabetes.
