  • U-CAN : a prospective longitudinal collection of biomaterials and clinical information from adult cancer patients in Sweden.

    • Background: Progress in cancer biomarker discovery is dependent on access to high-quality biological materials and high-resolution clinical data from the same cases. To overcome current limitations, a systematic prospective longitudinal sampling of multidisciplinary clinical data, blood and tissue from cancer patients was therefore initiated in 2010 by Uppsala and Umeå Universities and involving their corresponding University Hospitals, which are referral centers for one third of the Swedish population.Material and Methods: Patients with cancer of selected types who are treated at one of the participating hospitals are eligible for inclusion. The healthcare-integrated sampling scheme encompasses clinical data, questionnaires, blood, fresh frozen and formalin-fixed paraffin-embedded tissue specimens, diagnostic slides and radiology bioimaging data.Results: In this ongoing effort, 12,265 patients with brain tumors, breast cancers, colorectal cancers, gynecological cancers, hematological malignancies, lung cancers, neuroendocrine tumors or prostate cancers have been included until the end of 2016. From the 6914 patients included during the first five years, 98% were sampled for blood at diagnosis, 83% had paraffin-embedded and 58% had fresh frozen tissues collected. For Uppsala County, 55% of all cancer patients were included in the cohort.Conclusions: Close collaboration between participating hospitals and universities enabled prospective, longitudinal biobanking of blood and tissues and collection of multidisciplinary clinical data from cancer patients in the U-CAN cohort. Here, we summarize the first five years of operations, present U-CAN as a highly valuable cohort that will contribute to enhanced cancer research and describe the procedures to access samples and data.

  • Operationalising and incorporating ethical considerations into a tool for multi-criteria decision making

    • In this study, an existing method for ethical analysis was extended by adding aspects of human rights and global distributional issues as framed by the United Nations (UN) Universal Declaration of Human Rights (UDHR) and the UN Millennium Development Goals (MDGs). A procedure for incorporating these issues together with environmental, safety and cost considerations into a decision making framework using multi-criteria decision analysis (MCDA) was devised. This includes a scoring system to convert qualitative ethical issues into semi-quantitative issues. The refined framework tool was tested in an illustrative case on the destruction of ammunition. The results showed that the extended framework for ethical analysis added value, for example as a way of highlighting ethical considerations in decision making. Use of the DecideIT software with its integrated MCDA improved the analysis by allowing for uncertain and imprecise values and estimations. The illustrative case results indicated that a combined alternative including recycling of metals and energetic materials had clear advantages over open detonation.

  • Allogeneic stem cell transplantation for chronic myeloid leukemia in the TKI era : population-based data from the Swedish CML registry

    • Two decades after the introduction of tyrosine kinase inhibitors (TKI), a sizeable portion of patients with chronic myeloid leukemia (CML) in chronic phase (CP) still undergo allogeneic stem cell transplantation (allo-HSCT). We investigated the indications for allo-HSCT, clinical outcome, management of relapse, and post-transplant TKI treatment in a population-based setting using the Swedish CML registry. Of 118 CML patients transplanted between 2002 and 2017, 56 (47.4%) received allo-HSCT in first CP, among whom TM resistance was the most common transplant indication (62.5%). For patients diagnosed with CML in CP at <65 years of age, the cumulative probability of undergoing allo-HSCT within 5 years was 9.7%. Overall 5-year survival was 96.2%, 70.1% and 36.9% when transplanted in first CP, second or later CP, and in accelerated phase or blast crisis, respectively. Risk factors for relapse were EBMT score >2 and reduced intensity conditioning, and for death, CP > 2 at time point of allo-HSCT only. Non-relapse mortality for patients transplanted in CP was 11.6%. Our data indicate that allo-HSCT still constitutes a reasonable therapeutic option for patients with CML in first CP, especially those resistant to TKI treatment, providing high long-term survival and low non-relapse mortality.

    • This article reviews suggestions for how ethical tools are to be evaluated and argues that the concept of ethical soundness as presented by Kaiser etal. (2007) is unhelpful. Instead, it suggests that the quality of an ethical tool is determined by how well it achieves its assigned purpose(s). Those are different for different tools, and the article suggests a categorization of such tools into three groups. For all ethical tools, it identifies comprehensiveness and user-friendliness as crucial. For tools that have reaching a decision in a democratic context as a main purpose, it identifies transparency, guiding users toward a decision and justification of the decision-supporting mechanism. For tools that aim to engage the public, it identifies procedural fairness as essential. It also notes that the scope of use for ethical tools is limited to the same moral community, and that this feature is frequently overlooked.

    • Sverige och världen står inför stora samhällsutmaningar, inte minst inom energi- och klimatområdet. För att möta dessa utmaningar och nå målsättningarna i Parisavtalet och Agenda 2030 krävs genomgripande förändringar av dagens samhälle – en utveckling som fordrar transformativa, kunskapsbaserade och samordnade processer för omställning. Rätt utformade och i effektiva kombinationer förväntas forsknings- och styrmedelsinsatser kunna åstadkomma en transformativ omställning av energisystemet. Omställningen förväntas bygga på forskningsinsatsersom leder till ny teknik och ny kunskap, samt olika kombinationer av styrmedel som riktas mot existerande marknadsmisslyckanden. För att försäkraoss om att kombinationerna av dessa insatser är effektiva och bidrar till en önskad omställning behöver vi även utvärderingsstrategier som genererar kunskap om hur olika forsknings- och styrmedelsinsatser påverkar och bidrar till omställningsarbetet.

    • Circulating IgA class anti-neutrophil cytoplasm antibodies (ANCA) directed against proteinase 3 (PR3) have been reported in ANCA-associated vasculitis (AAV) with mucosal involvement. However, secretory IgA (SIgA) PR3-ANCA has not been reported previously. In this study we compared serum levels of SIgA PR3-ANCA and IgA PR3-ANCA with IgG PR3-ANCA in relation to disease characteristics. Among 73 patients with AAV and PR3-ANCA at diagnosis, 84% tested positive for IgG PR3-ANCA, 47% for IgA-ANCA and 36% for SIgA PR3-ANCA at the time of sampling for the present study. IgA and IgG PR3-ANCA were similarly represented among patients with different organ manifestations, i.e. upper airway, lung or kidney at time of sampling. However, SIgA PR3-ANCA was significantly less represented among patients with upper airway involvement. During active disease, the proportions of IgA PR3-ANCA and SIgA PR3-ANCA positive patients were significantly higher as compared to inactive disease. Eight patients were prospectively sampled during 24 months from onset of active disease. In these patients, IgA PR3-ANCA and SIgA PR3-ANCA more often turned negative after remission induction as compared to IgG PR3-ANCA. Our findings suggest that serum IgA PR3-ANCA and SIgA PR3-ANCA are more closely related to disease activity in AAV as compared to IgG PR3-ANCA. Further studies are required to reveal if this has implications for disease activity monitoring. The mean number of PR3-ANCA isotypes increased along with disease activity, suggesting a global B-cell activation during active disease. This article is protected by copyright. All rights reserved.
