  • ECCO Topical Review Optimising Reporting in Surgery, Endoscopy, and Histopathology Collaboration Between S-ECCO, EduCom, H-ECCO

    • Background and Aims: Diagnosis and management of inflammatory bowel diseases [IBD] requires a lifelong multidisciplinary approach.The quality of medical reporting is crucial in this context.The present topical review addresses the need for optimised reporting in endoscopy, surgery, and histopathology. Methods: A consensus expert panel consisting of gastroenterologists, surgeons, and pathologists, convened by the European Crohns and Colitis Organisation, performed a systematic literature review. The following topics were covered: in endoscopy: [i] general IBD endoscopy; [ii] disease activity and surveillance; [iii] endoscopy treatment in IBD; in surgery: [iv] medical history with surgical relevance, surgical indication, and strategy; [v] operative approach; [vi] intraoperative disease description; [vii] operative steps; in pathology: [viii] macroscopic assessment and interpretation of resection specimens; [ix] IBD histology, including biopsies, surgical resections, and neoplasia; [x] IBD histology conclusion and report. Statements were developed using a Delphi methodology incorporating two consecutive rounds. Current practice positions were set when >= 80% of participants agreed on a recommendation. Results: Thirty practice positions established a standard terminology for optimal reporting in endoscopy, surgery, and histopathology. Assessment of disease activity, surveillance recommendations, advice to surgeons for operative indication and strategies, including margins and extent of resection, and diagnostic criteria of IBD, as well as guidance for the interpretation of dysplasia and cancer, were handled. A standardised report including a core set of items to include in each specialty report, was defined. Conclusions: Interdisciplinary high-quality care requires thorough and standardised reporting across specialties.This topical review offers an actionable framework and practice recommendations to optimise reporting in endoscopy, surgery, and histopathology.

  • Face-Centered Cubic (Al1-xCrx)2O3 Thin Films : Deposition, Characterization, and Heat Treatment Studies

    • Face centered cubic (Al1-xCrx)2O3 solid solution films, with x in the range 0.602 discharge at a substrate temperature of 500 °C. The films have a strong <100> preferred orientation. The unit cell parameter is 4.04 Å determined by x-ray diffraction and high resolution transmission electron microscopy techniques. The (Al1-xCrx)2O3 films are suggested to have a non-stoichiometric NaCl structure with 33% vacancy occupancy on Al/Cr sites. Nanoindentation shows that the films exhibit hardness values up to 26 GPa and reduced modulus of 220-235 GPa. In the present work, ex-situ annealing studies were performed on as-deposited samples for a series of temperatures up to 1100 °C and annealing time of 2,4,6,8, and 10 hours. The fcc structure remains intact up to 950 °C. A gradual phase transformation from fcc to corundum at 925 °C is observed, where annealing for 2 h yields a partial transformation and annealing for >6 h results in complete transformation to a-(Al1-xCrx)2O3. There is no indication of any phase separation into a-Cr2O3 and Al2O3.

    • In this work, we report a detailed study of the electronic structure and transport properties of mono- and difluorinated edges of zigzag graphene nanoribbons (ZGNR) using density functional theory (DFT). The calculated formation energies at 0 K indicate that the stability of the nanoribbons increases with the increase in the concentration of difluorinated edge C atoms along with an interesting variation of the energy gaps between 0.0 to 0.66 eV depending on the concentration. This gives a possibility of tuning the band gaps by controlling the concentration of F for terminating the edges of the nanoribbons. The DFT results have been reproduced by density functional tight binding method. Using the nonequilibrium Green functional method, we have calculated the transmission coefficients of several mono- and difluorinated ZGNR as a function of unit cell size and degree of homogeneous disorder caused by the random placement of mono and difluorinated C atoms at the edges.

  • Neutron position sensitive detectors for the ESS

    • The European Spallation Source (ESS) in Lund, Sweden will become the world's leading neutron source for the study of materials. It will be a long pulse source, with an average beam power of 5 MW delivered to the target station. The ESS is in the construction phase, which started in 2013 with the completion of the Technical Design Report (TDR). The instruments are being selected from conceptual proposals submitted by groups from around Europe. These instruments present numerous challenges for detector technology in the absence of the availability of Helium-3, which is the default choice for detectors for instruments built until today and due to the extreme rates expected across the ESS instrument suite. Additionally a new generation of source requires a new generation of detector technologies to fully exploit the opportunities that this source provides. To meet this challenge at a green-field site, the detectors will be sourced from partners across Europe through numerous in-kind arrangements; a process that is somewhat novel for the neutron scattering community. This contribution presents briefly the current status of detectors for the ESS, and outlines the timeline to completion. For a conjectured instrument suite based upon instruments recommended for construction, a recently updated snapshot of the current expected detector requirements is presented. A strategy outline as to how these requirements might be tackled by novel detector developments is shown. In terms of future developments for the neutron community, synergies should be sought with other disciples, as recognized by various recent initiatives in Europe, in the context of the fundamentally multi-disciplinary nature of detectors. This strategy has at its basis the in-kind and collaborative partnerships necessary to be able to produce optimally performant detectors that allow the ESS instruments to be world-leading. This foresees and encourages a high level of collaboration and interdependence at its core, and rather than each group being all-rounders in every technology, the further development of centres of excellence across Europe for particular technologies and niches.

  • Combined quantification of anisotropy and inhomogeneity of magmatic rock fabrics - An outcrop scale analysis recorded in high resolution

    • Magmatic mineral distribution patterns in a syntectonic syenite pluton have been recorded at high resolution over several square metres on quarried faces. The anisotropy and inhomogeneity of K-feldspar and mafic mineral distribution patterns have been quantified using two methods originally based on fractal geometry. (1) Map-counting, based on box-counting, illustrates an inhomogeneity on the decimetre to metre scale and highlights diffuse structures that can be related to mafic schlieren or felsic dykes that are not visible on the rock surface. (2) We have developed a mapping of rock fabric anisotropy (MORFA) method that leads to the detection of further magmatic structures that are not visible in the field. With MORFA a magmatic lineation and its variation over large areas is determined, as well as fabrics on the decimetre to 1 m scales, which possibly represent flow or fracture structures in the crystallising magma. © 2011 Elsevier Ltd.

    • The genesis of cardiogenic oscillations, i.e. The small waves in airway pressure (COSpaw) and flow (COSflow) signals recorded at the airway opening is under debate. We hypothesized that these waves are originated from cyclic changes in pulmonary artery (PA) pressure and flow but not from the physical transmission of heartbeats onto the lungs. The aim of this study was to test this hypothesis. In 10 anesthetized pigs, COS were evaluated during expiratory breath-holds at baseline with intact chest and during open chest conditions at: (1) close contact between heart and lungs; (2) no heart-lungs contact by lifting the heart apex outside the thoracic cavity; (3) PA clamping at the main trunk during 10 s; and (4) during manual massage after cardiac arrest maintaining the heart apex outside the thorax, with and without PA clamping. Baseline COSpaw and COSflow amplitude were 0.70 ± 0.08 cmH2O and 0.51 ± 0.06 L/min, respectively. Both COS amplitude decreased during open chest conditions in step 1 and 2 (p < 0.05). However, COSpaw and COSflow amplitude did not depend on whether the heart was in contact or isolated from the surrounding lung parenchyma. COSpaw and COSflow disappeared when pulmonary blood flow was stopped after clamping PA in all animals. Manual heart massages reproduced COS but they disappeared when PA was clamped during this maneuver. The transmission of PA pulsatilty across the lungs generates COSpaw and COSflow measured at the airway opening. This information has potential applications for respiratory monitoring.

  • The effect of periodontal therapy on intra-oral halitosis : a case series

    • AIM: The aim of this study was to evaluate the effects of non-surgical periodontal therapy on intra-oral halitosis 3 months after therapy.MATERIAL AND METHODS: Sixty-eight adults with intra-oral halitosis were included in a case series. Intra-oral halitosis was evaluated at baseline, and at 3 months after treatment using the organoleptic scores (OLS), Halimeter® , and a gas chromatograph.RESULTS: Significant reductions for OLS (p < 0.01), total sum of volatile sulphur compounds (T-VSC) (p < 0.01) and methyl mercaptan (MM) (p < 0.05) values were found after treatment. Hydrogen sulphide (H2 S) levels were not significantly reduced. The numbers of probing pockets 4 mm, 5 mm and 6 mm were significantly reduced as a result of therapy (p < 0.001). Bleeding on probing (BOP) and plaque indices were also significantly reduced (p < 0.001). For the 34 individuals with successful periodontal treatment (BOP<20% and a ≥50% reduction of total pocket depth) reductions in OLS (p < 0.01) and T-VSC scores (p < 0.01) were found. Eleven individuals were considered effectively treated for intra-oral halitosis presenting with a T-VSC value <160 ppb, a H2 S value <112 ppb and a MM value <26 ppb.CONCLUSION: Non-surgical periodontal therapy resulted in reduction of OLS, MM and T-VSC values 3 months after therapy. Few individuals were considered as effectively treated for intra-oral halitosis.
