  • Thrombin generation in vitro in the presence of by-passing agents in siblings with severe haemophilia A.

    • Summary. Previous data have shown an inter-individual difference in the thrombin generating capacity in vitro as well as phenotypic bleeding pattern among patients with severe haemophilia A (FVIII:C activity below 1%). The reason for this is not known. In addition, there are no reports on how thrombin generation may correlate between siblings. In this study, we evaluated and compared thrombin generation in vitro using plasma samples in the presence of by-passing agents (FEIBA((R)) and NovoSeven((R))) in 21 unrelated brother pairs with and without inhibitors enrolled in the Malmö International Brother Study (MIBS). Mean maximum thrombin formation in patients with a current inhibitor titer was 182.0 +/- 52.8 mmol mL(-1) (FEIBA((R))) and 130.7 +/- 54.9 mmol mL(-1) (rFVIIa), respectively, and somewhat higher in those without inhibitors, 222.7 +/-85.5 mmol mL(-1) (FEIBA((R))) and 142.8 +/-53.6mmol mL(-1) (rFVIIa) (P = 0.16 and 0.29). The variance regarding the maximum thrombin production within a family was significantly lower compared with the thrombin production between families (P < 0.001 for both FEIBA((R)) and NovoSeven((R))). Our data indicate that genetically determined factors, other than the FVIII:C activity seems to influence the phenotypic variation in thrombin formation in the presence of by-passing agents. The nature of these determinants remains to be identified.

  • On the identification of the unrestricted Thurstonian model for ranking data

    • The identification issues of the unrestricted Thurstonian model for ranking data is the focus of the current paper. Within the Thurstonian framework, each object among those to be ranked is associated with a latent continuous variable, often interpreted as the utility of the object. The unrestricted Thurstonian model, due to the discrete and comparative nature of ranking data, faces more serious identification problems than the indeterminacy of the latent scale origin and unit. Most researchers resort to the study of the unrestricted model referring to the differences of the object utilities but then the inference on object utilities becomes tricky. Maydeu-Olivares and Böckenholt (2005) suggest a strategy to overcome the identification problem of the unrestricted model referring to object utilities but this requires many extra identification constraints, additional to the ones needed for defining the scale origin and unit. In the current paper, we study the general applicability of the suggested identification approach. Our simulation study indicates that the estimates obtained can be seriously biased with relatively large mean squared errors (MSE) when the extra constraints deviate from the true values of the parameters. Besides, the bias and MSE do not seem to decrease with increase in the sample size, and the effect of the constraints is not uniform on all estimated parameters.

    • Needs assessment may be helpful in establishing health care priorities, especially when community financial resources are reduced and the need and demand for health services are increasing. As the starting point of empirical studies concerning needs assessment of rehabilitation and occupational therapy, this study aimed to establish the prevalence of self-perceived activity limitation and/or participation restrictions due to long-term or recurrent pain. The study sample (n = 10000) was taken at random from the Swedish population aged 18-58 years. After three reminders, the response rate to a postal questionnaire was 77.1%, by mail or to a telephone answering machine. The main results indicated prevalence frequencies of 26%, showing the actual state of self-perceived activity limitation and/or participation restrictions due to long-term and/or recurrent pain. Differences were found between genders, among almost all age-classes and between persons who had pain currently and those who had had pain previously. Suggested indicators from the study result were women born between 1940 and 1949 with long-term pain and previous pain.

    • Harman and Lewis credit Kripke with having formulated a puzzle that seems to show that knowledge entails dogmatism. The puzzle is widely regarded as having been solved. In this paper we argue that this standard solution, in its various versions, addresses only a limited aspect of the puzzle and holds no promise of fully resolving it. Analyzing this failure and the proper rendering of the puzzle, it is suggested that it poses a significant challenge for the defense of epistemic closure.
