  • Biological optimization of angle of incidence and intensity modulation in breast and cervix cancer radiation therapy

    • Biological treatment optimization aim at improving radiation therapy by accounting for the radiobiological tumour and normal tissues response properties when optimizing the dose delivery. Generally traditional methods, using only dosimetrical measures, disregard the nonlinear radiation response of different tumours and normal tissues. The accumulated knowledge on tissue response to radiation, in the form of more accurate dose response relations, cell survival models and their associated biological parameters, alongside with the tools for biological treatment plan optimization, has allowed the present investigation on the potential merits of biologically based treatment optimization in radiation therapy.With a more widespread implementation of intensity modulated radiation therapy in the clinic, there is an increasing demand for faster and safer treatment delivery techniques. In this thesis biological treatment plan optimization, using the probability to achieve complication free tumour control as the quantifier for treatment outcome, was applied to radiation therapy of early breast cancer and advanced cervix cancer. It is shown that very conformal dose distributions can generally be produced with 3 or 4 optimally orientated coplanar intensity modulated beams, without having clinically significant losses in treatment outcome from the optimal dose distribution.By using exhaustive search methods, the optimal coplanar beam directions for intensity modulated photon beams for early breast cancer and the optimal non-coplanar directions for an advanced cervix cancer were investigated. Although time consuming, exhaustive search methods have the advantage of revealing most features involving interactions between a small number of beams and how this may influence the treatment outcome. Thus phase spaces may serve as a general database for selecting an almost optimal treatment configuration for similar patients. Previous knowledge acquired with physically optimized uniform beam radiation therapy may not apply when intensity modulated biological optimization is used. Thus unconventional treatment directions were sometimes found.

    • The concept of awareness has become of increasing importance to both social and technical research in CSCW. The concept remains however relatively unexplored, and we still have little understanding of the ways in which people produce and sustain ‘awareness’ in and through social interaction with others. In this paper, we focus on a particular aspect of awareness, the ways in which participants design activities to have others unobtrusively notice and discover, actions and events, which might otherwise pass unnoticed. We consider for example how participants render visible selective aspects of their activities, how they encourage others to notice features of the local milieu, and how they encourage others to become sensitive to particular events. We draw examples from different workplaces, primarily centres of coordination; organisational environments which rest upon the participants’ abilities to delicately interweave a complex array of highly contingent, yet interdependent activities.

    • One of the most significant developments in healthcare over the past 25 years has been the widespread deployment of information and communication technologies. These technologies have had a wide-ranging impact on the organisation of healthcare, on professional practice and on patients’ experience of illness and its management. In this paper we discuss the ways in which Sociology of Health and Illness has provided a forum for the analysis of these new technologies in healthcare. We review a range of relevant research published in the Journal; papers that address such issues as dehumanisation and emotional labour, professional practice and identity, and the social and institutional shaping of technology. Despite these important initiatives, we suggest that information and communication technologies in healthcare remain relatively under-explored within the Journal and, more generally, by the sociology of health and illness and point to developments in cognate areas which may have some bearing upon the analysis of technology in action.

  • Wound infiltration with ropivacaine and fentanyl: Effects on postoperative pain and PONV after breast surgery

    • Study Objective: To determine whether Postoperative wound infiltration with local anesthetics combined with fentanyl improves analgesia following breast surgery; and to investigate awakening and Postoperative nausea/vomiting. Design: Prospectively randomized clinical study. Setting: University hospital. Patients: 45 ASA physical status I and II patients scheduled for breast surgery. Interventions: Patients were prospectively randomized and assigned to one of three treatments during general anesthesia: postsurgical wound infiltration with ropivacaine 0.375%; wound infiltration with ropivacaine 0.375% combined with fentanyl 0.5 mug/kg; and intravenous (IV) fentanyl 0.5 mug/kg before skin incision and no wound infiltration. Time to first verbal response, Pain at rest, Postoperative nausea and vomiting, and ketobemidone and dixyrazine utilization were compared. Measurements and Main Results: Time to first verbal response was significantly shorter in the IV fentanyl group compared to both infiltration groups (8.1 +/- 4.5 min vs. 15.3 +/- 4.3, and 12.0 +/- 5.0 min; p < 0.05, respectively). Postoperative Pain at rest, and nausea and vomiting occurred with similar frequencies in the groups. Ketobemidone utilization in both infiltration groups, (2.4 +/- 1.8 mg and 3.1 +/- 1.8 mg, respectively) was not different compared to the IV fentanyl group (2.9 +/- 2.0 mg; NS). There were no differences in Postoperative antiemetic requirements during the first, second and third two-hour periods postoperatively. The dixyrazine consumption was similar in the three groups, (0.9 +/- 1.5 mg, 0.8 +/- 1.3 mg, and 1.4 +/- 1.8 mg, respectively; NS). Conclusion: Postsurgical ropivacaine wound infiltration, with or without adding fentanyl, demonstrates no differences in postoperative Pain relief and nausea/vomiting compared to a balanced general anesthetic including IV fentanyl.     • The depletion of stratospheric ozone due to the effects of ozone-depleting substances, such as volatile organohalogens, emitted into the atmosphere from industrial and natural sources has increased the amount of ultraviolet radiation reaching the earth's surface. Especially in the subpolar and polar regions, where stratospheric ozone destruction is the highest, individual organisms and whole ecosystems can be affected. In a laboratory study, several species of marine macroalgae occurring in the polar and northern temperate regions were exposed to elevated levels of ultraviolet radiation. Most of the macroalgae released significantly more chloroform, bromoform, dibromomethane, and methyl iodide-all volatile organohalogens. Calculating on the basis of the release of total chlorine, bromine, and iodine revealed that, except for two macroalgae emitting chlorine and one alga emitting iodine, exposure to ultraviolet radiation caused macroalgae to emit significantly more total chlorine, bromine, and iodine. Increasing levels of ultraviolet radiation due to possible further destruction of the stratospheric ozone layer as a result of ongoing global atmospheric warming may thus increase the future importance of marine macroalgae as a source for the global occurrence of reactive halogencontaining compounds.

  • Detection of noroviruses in raspberries associated with a gastroenteritis outbreak

    • Following an acute foodborne gastroenteritis outbreak in southern Sweden, stool specimens from five of nine ill patients were found positive for norovirus using reverse transcriptase polymerase chain reaction. Epidemiological data pointed to raspberry cakes as the source of the outbreak. Using a combination of generic and patient-specific primers and novel food analysis methodology (with extraction efficiency control and inhibitor removal), norovirus strains from two different genogroups were directly identified in the contaminated raspberries.
