    • In managing hypertension, patient participation and understanding of the nature and significance of treatment are decisive. We analysed the communication between patient and physician with respect to antihypertensive medication at a follow-up appointment, and assessed patients' knowledge of their medication. The empirical data consist of audio-recordings from 51 hypertensive patients' follow-up appointment with their physicians. Thirty-three of these patients were interviewed in depth immediately after the appointment. The study was performed in primary health care centres and at a specialist clinic for hypertension. When discussing medications, patients mainly talked about experiences of being on medication, whereas physicians generally focused on the pharmacological effect and dosage of the drug. Physicians routinely asked about compliance with drug regimen, but seldom in any depth. Little effort was invested into discussing the effect and goal of therapy. The main finding was that patients had a very fragmentary understanding of the functional nature of their antihypertensive medication. This is unsatisfactory both from the point of view of treatment efficacy and also when considering the legal requirements of involving the patient in the decision making. The follow-up appointments studied gave few possibilities for the patient to learn about their antihypertensive medication.

    • OBJECTIVE: To investigate hypertensive patients' understanding of the circulatory system, in particular high blood pressure. DESIGN: Semi-structured audio-taped interviews of patients immediately after a regular follow-up appointment with their physician. SETTING: A primary health care centre and a specialist clinic (hypertension unit) in southern Sweden. PATIENTS: 33 hypertensive patients, consecutively selected. MAIN OUTCOME MEASURE: Focus was set on the exploration of patients' understanding/knowledge. RESULTS: In spite of a long history of hypertensive care, on average ten years, patients had a less than satisfactory understanding of their condition. Most patients knew their blood pressure values, but very few were able to give an account of what high blood pressure implies in functional terms. Knowledge of high blood pressure seems mainly to be derived from sources other than the health care system, in particular from the mass media. Knowledge of the risks associated with hypertension was quite good, as was the insight into how these risks could be managed. CONCLUSION: An assessment of patient knowledge of high blood pressure ought to be a starting point for educational strategies that aim to deepen patients' understanding of their state of health.

    • Two different cDNAs, homologous to genes for rotenone-insensitive NADH dehydrogenases of bacteria and yeast, were isolated from potato. The encoded proteins, called NDA and NDB, have calculated molecular masses of 55 and 65kDa, respectively. The N-terminal parts show similarity to mitochondrial targeting peptides and the polypeptides are in vitro imported into potato mitochondria. Import processing to a smaller polypeptide is seen for the NDA but not the NDB protein. After import, NDA is intramitochondrially sorted to the matrix side of the inner membrane, whereas NDB becomes exposed to the intermembrane space. Imported proteins are associated to membranes upon digitonin permeabilization. On expression in Escherichia coli, NDB is released from the bacterial membrane in the absence of divalent cations whereas detergents are necessary for solubilization of NDA. Both deduced amino-acid sequences contain the dual motifs for nucleotide binding with the characteristics of the core criteria, similar to the bacterial homologues. Unique among NADH dehydrogenases, the NDB amino-acid sequence contains a nonconserved insert, which is similar to EF-hand motifs for calcium binding. Phylogenetic analyses group the rotenone-insensitive NADH dehydrogenases largely by species, but suggest ancient gene duplications.

  • Internal Stresses in Wood Caused by Climate Variations

    • This thesis is the result of a series of investigations on the hygro-mechanical behaviour of clear wood in the radial and tangential directions. Approximately half of the effort put into the investigations was concentrated on laboratory tests while the remaining effort was concentrated on modelling the observations made in the tests. The results from the investigations are published or submitted for publication in eight papers. Paper I : In this paper methods for testing the hygro-mechanical behaviour of small clear wood specimen are evaluated and discussed. The equipment used for conducting the tests is thoroughly described. It was concluded that the equipment showed good potential to perform tests on the specimen in variable climate and that the methods for testing were reliable. Paper II : Results from performed tests on small clear wood specimens in climates representative for kiln drying of sawn timber are presented in this paper. Evaluations of the tests also showed that the hygro-mechanical behaviour at high temperatures and high moisture content consists of both mechano-sorption and time induced creep. Paper III : A material model for drying wood at constant temperatures on the material point level is presented in this paper. The model was calibrated against results. It was found that the material model predicted the response from climate and load action in small specimens. Paper IV : In this paper the one dimensional material model introduced in Paper III is verified against more test results on small clear wood specimen. Also, a two dimensional model was developed and is presented in this paper. Paper V : Results from tests on small clear wood specimens for indoor climates with single cycles of humidity are presented in this paper. The mechano-sorptive strain was found to be independent of moisture level and rate of moisture change, but strongly dependent on the magnitude of moisture change. Paper VI : Results from tests on small specimens subjected to indoor climates with multiple changes of humidity are presented in this paper. A material model describing the hygro-mechanical behaviour of clear wood in radial and tangential directions for indoor conditions was derived. Paper VII : Results from simulations with the one dimensional and two dimensional material models (developed in Papers III and IV) implemented into simulation programs are shown in this paper. Paper VIII : Application of the work on material behaviour in kiln drying conditions (see Papers I, II, III, IV and VII) for practical kiln drying is presented in this paper. It is shown how simulations can be used to alter drying schedules to reduce internal stresses in drying timber.

    • A number of rheological models of drying wood have been presented during the last ten years. At the material point level, most models show high accuracy. The models in themselves, however, have no value unless used for improving wood drying, technique, quality, etc. To achieve this, they have to be implemented in applicable programs/ methods. In this article, the implementation of a material model calibrated on small clear wood specimen is presented. The strain (cup-deformations) and the internal stresses (indicator of surface checks and internal collapse) of the boards cross-section are calculated for given moisture histories. Comparisons with test results concerning cupping show that the model can describe the deformation of a board cross-section. It is also shown that in many cases a simple analytical solution can be used to determine the cupping. Comparisons between two-dimensional and one-dimensional calculations show that the one-dimensional formulation in many cases is suf®cient to describe the maximum internal stresses during drying as function of time.
