  • The electric field and wave experiment for the Cluster mission

    • The electric-field and wave experiment (EFW) on Cluster is designed to measure the electric-field and density fluctuations with sampling rates up to 36 000 samples s(-1). Langmuir probe sweeps can also be made to determine the electron density and temperature. The instrument has several important capabilities. These include (1) measurements of quasi-static electric fields of amplitudes lip to 700 mV m(-1) with high amplitude and time resolution, (2) measurements over short periods of time of up to five simualtaneous waveforms (two electric signals and three magnetic signals from the seach coil magnetometer sensors) of a bandwidth of 4 kHz with high time resolution, (3) measurements of density fluctuations in four points with high time resolution. Among the more interesting scientific objectives of the experiment are studies of nonlinear wave phenomena that result in acceleration of plasma as well as large- and small-scale interferometric measurements. By using four spacecraft for large-scale differential measurements and several Langmuir probes on one spacecraft for small-scale interferometry, it will be possible to study motion and shape of plasma structures on a wide range of spatial and temporal scales. This paper describes the primary scientific objectives of the EFW experiment and the technical capabilities of the instrument.

  • Quality of life associated with varying degrees of chronic lower limb ischaemia: comparison with a healthy sample

    • OBJECTIVES: To assess quality of life in patients with varying degrees of ischaemia in comparison with controls, and to determine whether the degree of lower limb ischaemia and sense of coherence were associated with quality of life. MATERIALS AND METHODS: 168 patients, including 93 claudicants and 75 patients with critical ischaemia and 102 controls were studied. Quality of life was assessed using the Nottingham Health Profile in addition to the Sense of Coherence scale. MAIN RESULTS: Patients with lower limb ischaemia scored significantly reduced quality of life in all aspects compared to controls. Pain, physical mobility and emotional reactions were the significant independent factors when using logistic regression analysis. The grade of disease and low sense of coherence were significantly associated with low quality of life. Increasing lower limb ischaemia significantly conferred worse pain, sleeping disturbances and immobility. CONCLUSION: This study showed that the quality of life was impaired among patients with lower limb ischaemia, in all investigated respects. The degree to which quality of life was affected seems to represent an interplay between the grade of ischaemia and the patient's sense of coherence. This suggests the need for a multidimensional assessment prior to intervention.

  • Bone response to surface-modified titanium implants: studies on the early tissue response to machined and electropolished implants with different oxide thicknesses.

    • The bone formation around titanium implants with varied surface properties is investigated. Machined and electropolished samples with and without thick, anodically formed surface oxides were prepared, surface characterized and inserted in the cortical bone of rabbits (1, 3 and 6 weeks). Scanning electron microscopy, scanning Auger electron spectroscopy and atomic force microscopy revealed marked differences in oxide thickness, surface topography and roughness, but no significant differences in surface chemical composition, between the different groups of implants. Light microscopic morphology and morphometry showed that all implants were in contact with bone and had a large proportion of bone within the threads at 6 weeks. The smooth, electropolished implants, irrespective of anodic oxidation, were surrounded by less bone than the machined implants after 1 week. After 6 weeks the bone volume as well as the bone-implant contact were lower for the merely electropolished implants than for the other three groups. Our study shows that a high degree of bone contact and bone formation are achieved with titanium implants which are modified with respect to oxide thickness and surface topography. However, the result with the smooth (electropolished) implants indicates that a reduction of surface roughness, in the initial phase, decreases the rate of bone formation in rabbit cortical bone.

  • Secondary aortoenteric fistula : changes from 1973 to 1993

    • AIM: To investigate a series of patients with secondary aortoenteric fistulas and compare it with a previous series (1985-93 vs. 1973-84). DESIGN: Retrospective study of medical records. SETTING: Sixteen vascular surgical centers in Sweden. PATIENTS: Twenty-seven patients were identified making an overall incidence of 0.5% of all aortoiliac operations. Among aneurysm patients the incidence was significantly lower than in the previous series. One patient record could not be identified. Fourteen primary operations were for aortic aneurysm, 12 for occlusive disease and one was an aortorenal vein bypass. RESULTS: Symptoms of the fistula occurred after a median interval of 90 months which is significantly later than the previous series (32 months; p<0.05). The commonest presentation was bleeding followed by septis. The median diagnostic delay was 10.5 days, which was significantly shorter than in the previous series. Most fistulas involved the duodenum (88%). One patient died before surgery. The postoperative mortality was 28%, significantly lower than in the previous series (58%) (p<0.05). At the end of follow up (median 43 months) significantly more patients were alive than in the previous series (42% vs 18%) (p<0.05). CONCLUSION: Over a 21 year period there seems to have been a decrease in the frequency of secondary aortoenteric fistulas after aneurysm surgery, a longer interval before they occur, a shorter diagnostic delay, and a better survival.

  • Stimulation of nerve regeneration by macrophages in granulation tissue

    • The effects of granulation tissue exposure on regeneration of sensory axons after a test crush lesion in rat sciatic nerves were studied. Chromic catgut threads were applied subcutaneously in the back of rats. Three weeks later the surrounding granulation tissue was isolated and transposed to the sciatic or tibial nerve in the same rat. Immunocytochemical, light and electron microscopic evaluation of the isolated granulation tissue revealed numerous EDI and ED2 positive macrophages. After an additional three weeks the sciatic nerves were crushed and the axonal outgrowth lengths were measured by the pinch reflex test and by neurofilament staining 3 or 6 days later. The regeneration distances in the sciatic nerve exposed to granulation tissue were significantly longer compared with that of nerves to which only subcutaneous tissue was transposed. Thus, the nerve had become conditioned by the transposure of macrophage rich tissue. After the transposure macrophages were present close to the nerve at the time point of the test crush lesion, but no signs of degeneration or inflammation in the endoneurial space were observed. It was also found that the outgrowth lengths in the tibial nerve branch were consistently longer than in the peroneal branch of the sciatic nerve. It is concluded that granulation tissue, containing macrophages, can induce a conditioning effect in the sciatic nerve. We suggest that this effect is related to factors emanating from macrophages in the transposed tissue.

  • A comparative study of perceptions of British mental health nurses and psychiatrists of their work environment

    • This comparative study of the perceptions of mental health nurses and psychiatrists about aspects of their work environment was undertaken in the West Midlands in England. The aim of the study was to ascertain the extent to which the environment in which mental health professionals' work impacts on their own mental and physical well-being. Seventy-four psychiatrists and 301 mental health nurses responded to a postal questionnaire. Analysis of data indicated that significant differences exist between nurses and psychiatrists in their working conditions, their physical working environment, their sources of support with a work-related problem, and the effects of their work on their own mental and physical health. The main recommendation derived from this study was to improve communication between mental health professionals and their managers by giving more structured feedback and guidance about one's work performance. This may help to alleviate the mental strain many mental health professionals experience in their work.
