    • Plants mediated synthesis of gold nanoparticles (AuNPs) containing desired characteristics for their suitable potential applications has been a challenging task, which is causing a major hindrance towards its commercialization. Therefore, herein phytosynthesis of AuNPs with required size and morphology has been achieved through manipulating the reaction conditions including reaction temperature and volume of Elaeis guineensis leaves (EGL) extract. Furthermore, photocatalytic potential of EGL mediated AuNPs having different size and shape has also been explored for the removal of methylene blue (MB) under visible light irradiation. The reaction temperature and volume of EGL strongly influenced the size and morphology of AuNPs, which are directly associated with the photocatalytic activities. The experimental results revealed that predominantly spherical and ultra-smaller size AuNPs with particle size of 16.26 ± 5.84 nm, formed at 70 °C showed the highest removal efficiency up to 92.55 % in 60 min. This highest photocatalytic activity of AuNPs could be attributed to the availability of higher number of low-coordinated gold (Au) atoms in the MB aqueous solution, which might have boosted the adsorption of the MB on the surface of particles and accelerated the degradation phenomenon. The proposed photocatalytic degradation mechanism of AuNPs for MB was also explained. The highly photoactive EGL mediated AuNPs with controllable morphology and size could be an advance step in future in chemical and biomedical applications.

  • Prediction of exposure-driven myelotoxicity of continuous infusion 5-fluorouracil by a semi-physiological pharmacokinetic-pharmacodynamic model in gastrointestinal cancer patients

    • PurposeTo describe 5-fluorouracil (5FU) pharmacokinetics, myelotoxicity and respective covariates using a simultaneous nonlinear mixed effect modelling approach.MethodsThirty patients with gastrointestinal cancer received 5FU 650 or 1000 mg/m2/day as 5-day continuous venous infusion (14 of whom also received cisplatin 20 mg/m2/day). 5FU and 5-fluoro-5,6-dihydrouracil (5FUH2) plasma concentrations were described by a pharmacokinetic model using NONMEM. Absolute leukocyte counts were described by a semi-mechanistic myelosuppression model. Covariate relationships were evaluated to explain the possible sources of variability in 5FU pharmacokinetics and pharmacodynamics.ResultsTotal clearance of 5FU correlated with body surface area (BSA). Population estimate for total clearance was 249 L/h. Clearances of 5FU and 5FUH2 fractionally changed by 77%/m2 difference from the median BSA. 5FU central and peripheral volumes of distribution were 5.56 L and 28.5 L, respectively. Estimated 5FUH2 clearance and volume of distribution were 121 L/h and 96.7 L, respectively. Baseline leukocyte count of 6.86 × 109/L, as well as mean leukocyte transit time of 281 h accounting for time delay between proliferating and circulating cells, was estimated. The relationship between 5FU plasma concentrations and absolute leukocyte count was found to be linear. A higher degree of myelosuppression was attributed to combination therapy (slope = 2.82 L/mg) with cisplatin as compared to 5FU monotherapy (slope = 1.17 L/mg).ConclusionsBSA should be taken into account for predicting 5FU exposure. Myelosuppression was influenced by 5FU exposure and concomitant administration of cisplatin.

  • Space-Time Clustering Characteristics of Tuberculosis in Khyber Pakhtunkhwa Province, Pakistan, 2015–2019

    • The number of tuberculosis (TB) cases in Pakistan ranks fifth in the world. The National TB Control Program (NTP) has recently reported more than 462,920 TB patients in Khyber Pakhtunkhwa province, Pakistan from 2002 to 2017. This study aims to identify spatial and space-time clusters of TB cases in Khyber Pakhtunkhwa province Pakistan during 2015–2019 to design effective interventions. The spatial and space-time cluster analyses were conducted at the district-level based on the reported TB cases from January 2015 to April 2019 using space-time scan statistics (SaTScan). The most likely spatial and space-time clusters were detected in the northern rural part of the province. Additionally, two districts in the west were detected as the secondary space-time clusters. The most likely space-time cluster shows a tendency of spread toward the neighboring districts in the central part, and the most likely spatial cluster shows a tendency of spread toward the neighboring districts in the south. Most of the space-time clusters were detected at the start of the study period 2015–2016. The potential TB clusters in the remote rural part might be associated to the dry–cool climate and lack of access to the healthcare centers in the remote areas. © 2020 by the authors.
