  • Degenerative inter-vertebral disc disease osteochondrosis intervertebralis in Europe : Prevalence, geographic variation and radiological correlates in men and women aged 50 and over

    • Objectives. To assess the prevalences across Europe of radiological indices of degenerative inter-vertebral disc disease (DDD); and to quantify their associations with, age, sex, physical anthropometry, areal BMD (aBMD) and change in aBMD with time. Methods. In the population-based European Prospective Osteoporosis Study, 27 age-stratified samples of men and women from across the continent aged 50+ years had standardized lateral radiographs of the lumbar and thoracic spine to evaluate the severity of DDD, using the Kellgren-Lawrence (KL) scale. Measurements of anterior, mid-body and posterior vertebral heights on all assessed vertebrae from T4 to L4 were used to generate indices of end-plate curvature. Results. Images from 10 132 participants (56% female, mean age 63.9 years) passed quality checks. Overall, 47% of men and women had DDD grade 3 or more in the lumbar spine and 36% in both thoracic and lumbar spine. Risk ratios for DDD grades 3 and 4, adjusted for age and anthropometric determinants, varied across a three-fold range between centres, yet prevalences were highly correlated in men and women. DDD was associated with flattened, non-ovoid inter-vertebral disc spaces. KL grade 4 and loss of inter-vertebral disc space were associated with higher spine aBMD. Conclusion. KL grades 3 and 4 are often used clinically to categorize radiological DDD. Highly variable European prevalences of radiologically defined DDD grades 3+ along with the large effects of age may have growing and geographically unequal health and economic impacts as the population ages. These data encourage further studies of potential genetic and environmental causes.

  • Pelvic trauma : WSES classification and guidelines

    • Complex pelvic injuries are among the most dangerous and deadly trauma related lesions. Different classification systems exist, some are based on the mechanism of injury, some on anatomic patterns and some are focusing on the resulting instability requiring operative fixation. The optimal treatment strategy, however, should keep into consideration the hemodynamic status, the anatomic impairment of pelvic ring function and the associated injuries. The management of pelvic trauma patients aims definitively to restore the homeostasis and the normal physiopathology associated to the mechanical stability of the pelvic ring. Thus the management of pelvic trauma must be multidisciplinary and should be ultimately based on the physiology of the patient and the anatomy of the injury. This paper presents the World Society of Emergency Surgery (WSES) classification of pelvic trauma and the management Guidelines.

    • Acute appendicitis (AA) is among the most common cause of acute abdominal pain. Diagnosis of AA is challenging; a variable combination of clinical signs and symptoms has been used together with laboratory findings in several scoring systems proposed for suggesting the probability of AA and the possible subsequent management pathway. The role of imaging in the diagnosis of AA is still debated, with variable use of US, CT and MRI in different settings worldwide. Up to date, comprehensive clinical guidelines for diagnosis and management of AA have never been issued. In July 2015, during the 3rd World Congress of the WSES, held in Jerusalem (Israel), a panel of experts including an Organizational Committee and Scientific Committee and Scientific Secretariat, participated to a Consensus Conference where eight panelists presented a number of statements developed for each of the eight main questions about diagnosis and management of AA. The statements were then voted, eventually modified and finally approved by the participants to The Consensus Conference and lately by the board of co-authors. The current paper is reporting the definitive Guidelines Statements on each of the following topics: 1) Diagnostic efficiency of clinical scoring systems, 2) Role of Imaging, 3) Non-operative treatment for uncomplicated appendicitis, 4) Timing of appendectomy and in-hospital delay, 5) Surgical treatment 6) Scoring systems for intra-operative grading of appendicitis and their clinical usefulness 7) Non-surgical treatment for complicated appendicitis: abscess or phlegmon 8) Pre-operative and post-operative antibiotics.

  • WSES consensus conference guidelines: monitoring and management of severe adult traumatic brain injury patients with polytrauma in the first 24 hours

    • The acute phase management of patients with severe traumatic brain injury (TBI) and polytrauma represents a major challenge. Guidelines for the care of these complex patients are lacking, and worldwide variability in clinical practice has been documented in recent studies. Consequently, the World Society of Emergency Surgery (WSES) decided to organize an international consensus conference regarding the monitoring and management of severe adult TBI polytrauma patients during the first 24 hours after injury. A modified Delphi approach was adopted, with an agreement cut-off of 70%. Forty experts in this field (emergency surgeons, neurosurgeons, and intensivists) participated in the online consensus process. Sixteen recommendations were generated, with the aim of promoting rational care in this difficult setting.

    • Fault Detection, Isolation and Recovery (FDIR) are becoming increasingly indispensable for real-time systems. Active diagnosis systems, therefore, are endeavoring to increase the reliability of real-time systems by computing at run-time the diagnostic information within FDIR systems, to actively decide on recovery actions. For embedded systems that contain various components, especially those including sensors and actuators, it is essential to provide a platform in which these components are able to communicate and also serve for an active diagnosis system. This paper proposes the ADISTES ontology (Active Diagnosis based on Semantic Web Technologies for Distributed Embedded Real-Time Systems) for an active diagnosis system of sensors and actuators. The proposed ADISTES ontology is a general ontology that can be used by different cases and systems to integrate their sensors and actuators; it can be instantiated from heterogeneous sources. As a characteristic of ontologies, they provide information in a machine interpretable format. Hence, the ADISTES ontology can also play a role for different sensors and actuators to share and communicate their status with one another.

    • Today, thanks to the advancement of robotics and intelligent new technologies, robots are entering more and more into different areas of human life. In doing so, the interaction between humans and robots, involving solving tasks both supervised and collaborative, becomes increasingly important as they both rely on smart approaches for communication. Throughout the human-robot and robot-robot communication, the utilized technology has to be able to describe information that is semantically rich, structurally efficient concerning description and time efficient regarding the retrieval of information. This work utilizes the resource description framework (RDF), as one of the core technologies of the semantic web to describe the navigation of a mobile robot semantically in an ontology which is populated in real-time. This research proposes a new and efficient way of expressing semantic relationships within the navigation ontology by using helper nodes. In contrast to existing approaches, it could be shown that the use of helper nodes can cut down the generation of nodes by a factor of two, improving the storage but also the utilization for a time efficient communication.

    • Today, thanks to the advancement of robotics and the achievements that are made in the light of the development of automated and intelligent technologies, we are increasingly witnessing the arrival of robotic devices and machines into different areas of human life. With all of the achievements and their intelligence, the degree of their proficiency relies in many cases on their ability to interact with human beings. To do so, for a mobile robot the ability to understand its surroundings and also to describe it in a way that human can easily understand is vitally important. “Above”, “below”, “in front of”, “behind of”, “left of” and “right to” are prepositions that human uses in daily life to describe not only the relationship between objects but also to provide one's location implicitly. The Resource Description Framework, RDF, as one of the semantic web core components, can be employed for forming those prepositions. The idea is to utilize a smart sensor of a simulated, mobile robot for creating semantic information in RDF format that represents inter-object relations by using the same spatial prepositions that human uses in his daily life to describe his environment.
