  • A multilayer biomaterial for osteochondral regeneration shows superiority vs microfractures for the treatment of osteochondral lesions in a multicentre randomized trial at 2 years

    • Purpose The increasing awareness on the role of subchondral bone in the etiopathology of articular surface lesions led to the development of osteochondral scaffolds. While safety and promising results have been suggested, there are no trials proving the real potential of the osteochondral regenerative approach. Aim was to assess the benefit provided by a nanostructured collagen-hydroxyapatite (coll-HA) multilayer scaffold for the treatment of chondral and osteochondral knee lesions. Methods In this multicentre randomized controlled clinical trial, 100 patients affected by symptomatic chondral and osteochondral lesions were treated and evaluated for up to 2 years (51 study group and 49 control group). A biomimetic coll-HA scaffold was studied, and bone marrow stimulation (BMS) was used as reference intervention. Primary efficacy measurement was IKDC subjective score at 2 years. Secondary efficacy measurements were: KOOS, IKDC Knee Examination Form, Tegner and VAS Pain scores evaluated at 6, 12 and 24 months. Tissue regeneration was evaluated with MRI MOCART scoring system at 6, 12 and 24 months. An external independent agency was involved to ensure data correctness and objectiveness. Results A statistically significant improvement of all clinical scores was obtained from basal evaluation to 2-year follow-up in both groups, although no overall statistically significant differences were detected between the two treatments. Conversely, the subgroup of patients affected by deep osteochondral lesions (i.e. Outerbridge grade IV and OCD) showed a statistically significant better IKDC subjective outcome (+12.4 points, p = 0.036) in the coll-HA group. Statistically significant better results were also found for another challenging group: sport active patients (+16.0, p = 0.027). Severe adverse events related to treatment were documented only in three patients in the coll-HA group and in one in the BMS group. The MOCART score showed no statistical difference between the two groups. Conclusions This study highlighted the safety and potential of a biomimetic implant. While no statistically significant differences were found compared to BMS for chondral lesions, this procedure can be considered a suitable option for the treatment of osteochondral lesions.

  • A decentralized optimization framework for energy harvesting devices

    • Designing decentralized policies for wireless communication networks is a crucial problem, which has only been partially solved in the literature so far. In this paper, we propose a Decentralized Markov Decision Process (Dec-MDP) framework to analyze a wireless sensor network with multiple users which access a common wireless channel. We consider devices with energy harvesting capabilities, that aim at balancing the energy arrivals with the data departures and with the probability of colliding with other nodes. Over time, an access point triggers a SYNC slot, wherein it recomputes the optimal transmission parameters of the whole network, and distributes this information. Every node receives its own policy, which specifies how it should access the channel in the future, and, thereafter, proceeds in a fully decentralized fashion, with no interactions with other entities in the network. We propose a multi-layer Markov model, where an external MDP manages the jumps between SYNC slots, and an internal Dec-MDP computes the optimal policy in the short term. We numerically show that, because of the harvesting, stationary policies are suboptimal in energy harvesting scenarios, and the optimal trade-off lies between an orthogonal and a random access system.

  • Pallidal Deep Brain Stimulation in DYT6 Dystonia: Clinical Outcome and Predictive Factors for Motor Improvement

    • Pallidal deep brain stimulation is an established treatment in dystonia. Available data on the effect in DYT-THAP1 dystonia (also known as DYT6 dystonia) are scarce and long-term follow-up studies are lacking. In this retrospective, multicenter follow-up case series of medical records of such patients, the clinical outcome of pallidal deep brain stimulation in DYT-THAP1 dystonia, was evaluated. The Burke Fahn Marsden Dystonia Rating Scale served as an outcome measure. Nine females and 5 males were enrolled, with a median follow-up of 4 years and 10 months after implant. All benefited from surgery: dystonia severity was reduced by a median of 58% (IQR 31-62, p = 0.001) at last follow-up, as assessed by the Burke Fahn Marsden movement subscale. In the majority of individuals, there was no improvement of speech or swallowing, and overall, the effect was greater in the trunk and limbs as compared to the cranio-cervical and orolaryngeal regions. No correlation was found between disease duration before surgery, age at surgery, or preoperative disease burden and the outcome of deep brain stimulation. Device- and therapy-related side-effects were few. Accordingly, pallidal deep brain stimulation should be considered in clinically impairing and pharmaco-resistant DYT-THAP1 dystonia. The method is safe and effective, both short- and long-term.

  • A joint power and information transfer system using retransmissions

    • In this paper, we develop a green joint power and information transfer system. To guarantee reliable communication, we implement hybrid automatic repeat request (HARQ) feedback both in the power and information transmission phases. Moreover, with a limit on the packet transmission delay, we derive the optimal power allocation minimizing the energy-constrained outage probability. The simulation and analytical results indicate that the diversity gain of the joint power and information transfer system is the same as the diversity gain of the conventional non-green information transfer systems. Also, the implementation of HARQ improves the power efficiency significantly. For instance, consider a Rayleigh fading channel, outage probability 10-3, code rate 1 nats-per-channel-use and the case where the receiver requires to receive 0.1 Joule-per-channel-use energy per codeword length from the transmitter to process the information. Then, with a maximum of two retransmissions in the power and information transfer phases, the HARQ reduces the average required energy by 4 dB, compared to the open-loop communication setup.

  • Finite Block-Length Analysis of Spectrum Sharing Networks: Interference-Constrained Scenario

    • This letter studies the interference-constrained performance of spectrum sharing networks using adaptive power allocation. The throughput of the secondary user is derived in different cases with constraints on the primary user average or instantaneous received interference power. The analysis is based on some recent results on the achievable rates of finite-length codes and shows the effect of the codeword length on the performance of spectrum sharing networks. The results indicate that, with codewords of finite length and different primary user's interference constraints, there is potential for spectrum sharing, if the transmission power is properly adapted.

    • To accurately estimate locations and velocities of surrounding targets (cars) is crucial for advanced driver assistance systems based on radar sensors. In this paper we derive methods for fusing data from multiple radar sensors in order to improve the accuracy and robustness of such estimates. First we pose the target estimation problem as a multivariate multidimensional spectral estimation problem. The problem is multivariate since each radar sensor gives rise to a measurement channel. Then we investigate how the use of the cross-spectra affects target estimates. We see that the use of the magnitude of the cross-spectrum significantly improves the accuracy of the target estimates, whereas an attempt to compensate the phase lag of the cross-spectrum only gives marginal improvement. This paper may be viewed as a first step towards applying high-resolution methods that builds on multidimensional multivariate spectral estimation for sensor fusion.
