    • To identify child mental health problems in a mid-sized to large city in Iraqi Kurdistan, the Reporting Questionnaire for Children (RQC), followed by the Child Behaviour Checklist (CBCL) and the Post-traumatic Stress Symptom Checklist for Children (PTSS-C), were administered in interview form to the caregivers of 806 school-aged children. To cover different categories of children, four samples were randomly selected from among the general population (n = 201), orphans (n = 241), primary medical care patients (n = 199), and hospital patients (n = 165). The RQC revealed satisfactory validity against a deviant CBCL cut-off. The screening capacity of the RQC was further supported by its similarity to the CBCL in distribution of problem scores among the four samples and its positive correlation with the CBCL, but not with the trauma-related PTSS-C. Although the general population showed lower problem scores than the orphans and the two clinical samples, problem scores in all instruments were considerably higher than those reported from other societies. The RQC seems to be useful as a first-stage screening instrument for child mental health problems in Kurdistan.

    • The prevalence and correlates of post-traumatic stress disorder (PTSD) were assessed in random samples of school-aged Kurdistanian children and their parents in homeland and exile. Of the 376 eligible children at the two sites, 312 children and their parents (293 mothers and 248 fathers) completed the Harvard-Uppsala Trauma Questionnaire and Posttraumatic Stress Symptom interviews for children, and Harvard Trauma Questionnaire for parents. Unlike their children, fathers showed significantly higher PTSD frequencies in exile than in the homeland. The fathers' PTSD negatively correlated with the living standard and fathers' education, while child PTSD mostly correlated with maternal education and living in exile. Living in exile seems to have a negative impact on fathers' post-traumatic reactions, despite its positive influence on children. High drop-outs in exile limit the conclusions.

    • Background: Traumatic experiences and post-traumatic stress symptoms were assessed in Kurdish children in their native country and in exile. Method: 312 randomly selected school-age children at two sites completed assessments of traumatic experiences and post-traumatic symptoms. Results: Although traumatic experiences showed more similarities than differences between the two samples, the PTSD frequencies and post-traumatic stress symptom scores were higher in Kurdistan than in exile. Living in exile showed negative correlation with PTSD frequencies and post-traumatic stress symptom scores. Conclusions: Living in exile seems to have trauma healing effect on children of Kurdistan.

  • Tetrahydrobiopterin in the treatment of children with autistic disorder. A double-blind placebo-controlled crossover study

    • Twelve children, all boys, aged 4 to 7 years, with a diagnosis of autistic disorder and low concentrations of spinal 6R-l-erythro-5,6,7,8-tetrahydrobiopterin (tetrahydrobiopterin) were selected to participate in a double-blind, randomized, placebo-controlled, crossover study. The children received a daily dose of 3 mg tetrahydrobiopterin per kilogram during 6 months alternating with placebo. Treatment-induced effects were assessed with the Childhood Autism Rating Scale every third month. The results showed small nonsignificant changes in the total scores of Childhood Autism Rating Scale after 3- and 6-month treatment. Post hoc analysis looking at the 3 core symptoms of autism, that is, social interaction, communication, and stereotyped behaviors, revealed a significant improvement of the social interaction score after 6 months of active treatment. In addition, a high positive correlation was found between response of the social interaction score and IQ. The results indicate a possible effect of tetrahydrobiopterin treatment.

  • Overrepresentation of Internationally Adopted Adolescents in Swedish §12-institutions

    • In order to study internationally adopted delinquents, internationally adopted controls, delinquent controls and an additional group of healthy non-adopted, non-delinquent controls, the following tests were used: WISC/WAIS, TOL, WCST, a questionnaire, I think I am, ISSI, an attachment test, KSP, and SCL-90. In the register study, data were obtained from the registers of The National Board of Health and Welfare and Statistics Sweden and multivariate analyses were performed using logistic regression models. Odds ratios (OR) for different forms of out-of-home care placements were calculated.It was found that the adopted delinquents had a significantly lower full scale IQ (WISC/WAIS) and significantly lower results on several measurements in the WISC /WAIS compared to the adopted controls. In addition, both groups of adoptees scored low in the WISC/WAIS subscale arithmetics when compared to the population mean. The adopted delinquents clearly had disruptive and infectious relations to their parents which was demonstrated in I think I am, ISSI, the attachment test and the questionnaire. The adopted controls demonstrated good relations to adoptive parents. When personality and self-perception were measured and analyzed in a two-way ANOVA, the results clearly pointed to ”delinquency” as the explaining factor to the variance of the results as opposed to ”adoption”. Finally, the regression analyses of the register data demonstrated an OR of 3.0 (after adjustments for age and sex) for placements of intercountry adoptees in residental care from age 10 and an OR of 5.1 in model 2 (after adjustments for socio-demographic background variables). More over, higher child age at adoption, origin from Latin America, single parent adoption and maternal age above 35 at birth of the child were identified as significant predictors of out-of-home care from age 10.

  • Relation problems in internationally adopted juvenile delinquents

    • Objective: Internationally adopted delinquents are overrepresented in juvenile Swedish institutions. With the purpose of investigating possible reasons for this overrepresentation, this study compared adopted delinquent adolescents and internationally adopted controls in the structure and functioning of their current relations, especially with their parents. Methods: Internationally adopted adolescents admitted to institutional care (N=20) and non-delinquent internationally adopted controls (N=21) were compared through: a questionnaire; "family relations", a subscale in I think I am; "Family climate" ( from Karolinska Scale of Personality); Individual Schedule of Social Interaction; and an Attachment Test. Results: Bad relations with adoptive parents were more prevalent in internationally adopted delinquents compared to internationally adopted controls. Furthermore, the adopted delinquents and their parents blamed each other for the problems and the adopted delinquents reported physical and emotional abuse. Conclusions: Internationally adopted delinquents reported more problems in their relationships to their parents than adopted controls did.

    • The first meeting in psychotherapeutic and psychosocial work, has a big impacton the continuation. It is a less explored research field. Children’s ‘‘voices’’ tend to come inthe back-ground in family therapy and other settings. In a project at CAP (Child andAdolescent Psychiatry) the children’s views were collected in interviews with parents andtherapists present. The grounded theory analysis process was used. The children addressedthe importance of the therapist’s actions and positions in helping them to be able tocommunicate and to be in a dialogue. To be accepted and allowed to express feelings wasimportant, and so was how the therapist managed to adjust to each person in the room andgive space for various perspectives.
