  • Immunoglobulin-A producing probably primary lymphoma of the breast

    • A breast tumour from a 65-year old woman was found to be a primary non-Hodgkin lymphoma, a very rare primary malignancy in this location. The lymphoma was of a diffuse histiocytic type according to the classification of Rappaport, or polymorphic immunocytoma according to the Kiel classification. Immunohistochemistry, not previously reported for breast lymphomas, revealed the production of IgA. In the serum this appeared as an IgA M-component which was greatly reduced after tumour removal. Immunological properties of primary breast lymphomas are reviewed, we suggest further extended studies with the immunohistochemical use of marker substances for the evaluation of prognosis. © 1983 Springer-Verlag.

    • In cancer tumor detection, tissue autofluorescence and characteristic features of injected hematoporphyrin derivative (HPD) can be utilized. The authors have studied the importance of the excitation wavelength for the achievable contrast between tumor and surrounding tissue using a rat tumor model. They have also compared the relative merits of two HPD preparations for tumor fluorescence detection. The real-time capability of the fluorescence technique is discussed.

    • Following systemic injection, hematoporphyrin derivative (HPD) is known to be selectively retained in malignant tissue. This property can be used in a two-fold way: a) for localizing tumours by observing the characteristic dual-peaked laser-induced fluorescence from HPD, and b) for photodynamic therapy (HPD-PDT) using a localized HPD-assisted singlet oxygen release induced by irradiation of 630 nm laser light. This rapidly developing field has recently been reviewed by DOUGHERTY.

  • Adenovirous type 9-induced tumorigenesis in the rat mammary gland related to sex hormonal state

    • Adenovirus type 9 was inoculated sc into newborn Wistar/Furth rats, divided into four groups: (1) six male rats, not treated further; (2) 11 male rats, castrated at 4 weeks of age; (3) 12 male rats, castrated at 4 weeks of age and subsequently treated repeatedly with estrogen; and (4) 12 female rats, not treated further. All of the rats in group 3 developed mammary hyperplasia and tumors (fibroadenomas and lipomas), in some cases with malignant histologic structure. Rats in group 4 developed similar mammary tumors, but with later appearance and significantly slower growth. A fifth group of rats, not virus inoculated but castrated and estrogen treated as in group 3, did not develop any demonstrable mammary lesions. The results show that the effects of the virus on the mammary gland are dependent upon an estrogenic background, which by itself cannot cause tumor development in males. It is suggested that viral DNA is incorporated into the cellular DNA in such a way that it influences the synthesis and/or activity of steroid receptors, triggering tumor development.

  • Complete remission in a patient with acute myelogenous leukemia treated with leukocyte α-interferon and cimetidine

    • A 76-year-old woman with acute myelogenous leukemia with approximately 65% myeloblasts on bone marrow examination was treated daily with a combination of 4 megaU of leukocyte interferon IM and 1,000 mg cimetidine PO. During therapy there was a gradual decrease of bone marrow myeloblasts down to 9% and a normalization of peripheral white blood cells. The treatment was discontinued after 6 weeks because of increasing fatigue and anorexia. The general condition improved greatly during the following weeks and the patient entered complete remission, which has continued for 6 months so far. In the course of therapy there was a gradual appearance of antibodies showing a selective binding capacity to autochthonous leukemic cells with no tendency to increased binding to remission cells. The aim of this report is to stimulate a further evaluation of this form of therapy in additional AML patients whenever this might be justified as an alternative to conventional chemotherapy.

    • Functional activity of granulocytes from healthy individuals and from patients with acute leukemia in remission was studied. The increase of heat production rate (metabolic activity) after stimulation of the blood cells with in vitro formed immune complexes was measured by microcalorimeters of heat conduction type. It was demonstrated that increased heat production rate after exposure to immune complexes was significantly lower (p<0.0005) in 9 patients with acute leukemia with a remission duration of less than 6 months than in 25 healthy volunteers.

    • Selenium in its organic and inorganic forms has been shown to inhibit the development of chemically induced, spontaneous and transplanted tumors. The present investigation was performed to study the effect of selenium (4 μg per ml of drinking water) on tumorigenesis of adenovirus‐type‐9‐induced breast fibroadenomas and on 1,2‐dimethylhydrazine‐induced bowel carcinogenesis in WF rats. It was found that identical treatment with Se under identical conditions and with no obvious toxic effects on the rats (I) resulted in inhibition of DMH‐induced large‐bowel carcinogenesis; (2) facilitated induction of small‐bowel cancer by the same carcinogen in the same animals, and (3) greatly facilitated induction of breast fibroadenoma by adenovirus type 9 in the same strain of rats. The effect of Se treatment on DMH‐induced largebowel carcinogenesis confirms previous findings and proves that the opposite effect on fibroadenoma development is not due to differences in e.g. effective dose, animal strains or condition of the animals. It is not yet clear through which mechanisms Se exerts these effects.

  • Microcalorimetry as a tool for the detection of complement-dependent cytotoxicity

    • Microcalorimetry using a 4-channel static ampoule microcalorimeter of thermopile type has been evaluated as a tool for the detection of complement-dependent cytotoxicity against the surface antigens of living cells. Cytotoxic reactions mediated by a rabbit antiserum against human white blood cells and by 2 different monoclonal antibodies recognizing a melanoma-associated antigen on a human melanoma cell line were studied. The cytotoxic reactions were registered as a decrease of the heat production rate when the cells were exposed to antibodies in the presence of active complement as compared to the heat production rate of the cells exposed to the same antibodies in the presence of inactive complement. This investigation shows that microcalorimetry can be used as a highly sensitive method for the detection of complement-dependent immune reactions, detecting antibody dilutions higher than 10-5. It also indicates that microcalorimetry may become a particularly important technique in the analysis of the kinetics of cytotoxic immune reactions in vitro.

Contrast enhancement techniques have been developed which improve the ability to localize a tumor when using fluorescence from the tumor-seeking substance hematoporphyrin derivative (HPD). A multi-color fluorescence imaging technique has been tested. A discussion of clinical applications is included.

  • Use of microcalorimetry in analysing the kinetics of ADCC

    • Microcalorimetry was found to be a useful technique for the demonstration of antibody-dependent cellular cytotoxicity (ADCC) against human melanoma cells mediated by a heterologous rabbit antiserum and two monoclonal antibodies in combination with human peripheral blood lymphocytes as effector cells. The rabbit antiserum and the monoclonal IgG3 antibody 2B2 directed against the GD3 ganglioside expressed cell-inhibitory effects resulting in a decreased heat production rate over 2-18 h of incubation. The 4.2 monoclonal IgM antibody to GD3 had no similar cell-inhibitory effect. In contrast, the 4.2 antibody expressed a much stronger effect than 2B2 in tests for complement-dependent cytotoxicity. The kinetics of these effects were quite reproducible. It is concluded that microcalorimetry is a sensitive and particularly suitable method for the analysis of cytotoxicity kinetics.
