    • The aim was to study the perceived quality of life/life satisfaction in a sample of women with breast cancer who were treated in a hospital with alternative/complementary care and the same variables in individually matched patients who received only conventional medical treatment. A non-randomized controlled trial design with repeated measurements was used. Sixty women with breast cancer treated with anthroposophic medicine (ABCW) and 60 with conventional medicine (CBCW) were included and 36 matched pairs took part on all occasions. The quality of life was measured by the EORTC QLQ-C30 and the Life Satisfaction Questionnaire (LSQ). The comparisons were calculated as effect sizes (ES). The women in the ABCW group reported small or moderate effects, expressed as ES, on their quality of life/life satisfaction compared to their matched "twins'' in the CBCW group at the 1-year follow-up in 15 out of 21 scales/factors. It was concluded that the women who had chosen anthroposophic care increased their perceived quality of life/life satisfaction according to the methodology of the study.

    • The aim of the present study, which is part of a major clinical controlled study of the life situation of women with breast cancer, was to compare two groups of women concerning perceived quality of life and coping. The women were treated in two different cancer treatment programs: complementary treatment, which included anthroposophic therapy, and conventional cancer treatment. A total of 120 women were included, 60 women treated with anthroposophic medicine, and 60 individually matched women treated with conventional medicine only. Quality of life was measured by the European Organization for Research and Treatment of Cancer, Quality of Life Questionnaire, Core 30, and the Life Satisfaction Questionnaire. Coping was measured by the Mental Adjustment to Cancer scale. The results showed that the women who chose anthroposophic therapy perceived their quality of life to be lower on admission to the hospital and showed more anxious preoccupation than the women in conventional medicine. It can be concluded that, due to the careful matching procedure, the women in the two groups are comparable in a medical sense but not from the perspective of quality of life and coping.

  • Ethical reflection on patient care situations : a study in a group of Swedish nurse students

    • Introduction: In a nurse's future practical work, which is rapidly developing and changing, ethics will play an important role. A nurse student needs her/his own experience from patient care as well as training in reflecting upon ethical matters to manage future work situations. The aim of the study behind this article was to find the ethical problems that engage students, how they can correlate theoretical knowledge with practical experience and what insights in ethics they may achieve by reflecting on the experiences.Method: The study entailed examining written assignments, submitted by the students during their clinical education in medicine and surgery. Data consisted of 46 students' reports describing practical patient care situations on which the students had made reflections from an ethical point of view.Results: The most common ethical problem described by the students was the violation of the integrity of either a patient or his relative as well as disrespect of their autonomy. The actions taken by the students in these situations most often followed two lines: either taking a clear active part for the patient, or playing a passive role, not taking the action considered to be appropriate. Their passivity was due to fear of not being accepted by the staff. The insight and understanding gained by the students from these ethical dilemmas were: to act differently in a similar situation in the future, to be confident, and to listen to the patient in order to understand his/her needs. The students clearly showed their ability to correlate theoretical knowledge with practical experience the dynamics of their reasoning.Conclusion: The study indicated that reflections made by students can lead to an increased awareness of the need for knowledge about nursing, about own actions and behaviour, and the feelings and values that influence actions. It seems possible that the ethical competence gained by the students reflecting on situations with moral dilemmas, could be improved, if they were helped to develop and absorb certain virtues or abilities. In the students' reflections some qualities emerged as important, e.g. courage, sensitivity, judgement, and ability to perform a dialogue.

    • This is the first Swedish study to evaluate the health-related quality of life and sense of coherence in adult survivors of allogeneic, haematopoietic stem cell transplantation (HSCT). Twenty-five recipients completed three questionnaires 2-4 years after the transplantation. The questionnaires used were the Sickness Impact Profile (SIP), the Symptom Frequency Intensity and Distress (SFID-BMT) scale and the Sense of Coherence (SOC) scale measuring subjective functional status, symptom distress and coping ability. Impairments in functional status were found, as compared with a population norm. The most common impairments were found in the areas of social interaction and sleep and rest. Eye problems, dry mouth, cough, sexual problems, tiredness, anxiety and changes of taste were symptoms reported by more than half of the patients. Despite impaired functioning and a high incidence of symptoms, the general health was described as quite good or excellent by 80% (n = 20) of the patients. The majority (20/22) had also been able to return to work or to attend school. No difference in the sense of coherence was seen, as compared with the population norm. Functional impairments were significantly correlated to a lower degree of sense of coherence.
