Jones fracture. Surgical versus nonsurgical treatment

- Sixty-three patients with 66 transverse and short oblique fractures of the proximal shaft of the fifth metatarsal bone (Jones fracture) were evaluated an average of five years (range, one to ten years) after the injury. There were 27 acute fractures and 39 chronic, or stress, fractures. The primary treatment was surgical for one third of the injuries and nonsurgical for the others. Surgical treatment consisted of the insertion of medullary screws. Nonsurgical treatment consisted of the application of either a plaster cast or an elastic bandage. Almost one fourth of the fractures treated nonsurgically later had to be treated surgically because of delayed unions or refractures. Late surgery was required in 12% of acute fractures and in 50% of chronic fractures with sclerosis narrowing the medullary canal. Irrespective of the primary treatment, all the patients had full function at the time of the follow-up evaluation, and no nonunions were diagnosed.

- The mathematical analogy between the nonlinear Schrödinger equations describing propagation of temporal soliton pulses in optical fibres, and propagation of spatial soliton beams in planar optical wave guides is discussed. A straightforward variational method, based on trial functions and Rayleigh-Ritz optimization, is demonstrated and applied to incoherent interaction of solitons. We study incoherent soliton interaction in a wavelength-multiplexed optical-fibre communication system, as well as the analogous problem for crossed optical beams in a planar waveguide. The predictions of the variational approach are also shown to be in good agreement with numerical simulation results and with experiments.

- The dynamics that result from the combined effects of spatial diffraction, nonlinearity, and a parabolic graded index for wave propagation in optical fibers are presented. An approximate analytical solution of the nonlinear Schrödinger equation in a graded-index fiber is obtained by using a variational approach. Particular emphasis is put on the variation of both the pulse width and the longitudinal phase delay with the distance of propagation along the fiber.

- Twenty patients who had been treated with a silicone rubber trapezial implant for degenerative changes in the first carpometacarpal joint were evaluated radiologically and clinically. After a mean follow up of 4.5 years, 11 of the 20 required reoperation and removal of the prosthesis for persisting pain and weakness. In all hands in which the prosthesis remained radiographs showed bone resorption or carpal bone cysts typical of silicone particle synovitis. In the light of these results we now do not recommend this procedure for the treatment of arthritis of the first carpometacarpal joint.

- This retrospective case control study was done in order to investigate whether patients who sustain a "nonosteoporotic fracture" early in life also continue to sustain fragility fractures later in life. All patients who had been treated at the Department of Orthopedics in Malmo with a tibial shaft fracture from 1949 to 1963 (n = 767) or an ankle fracture from 1961 to 1965 (n = 786) were included in this study. At the time of follow-up in 1992, 231 of the patients who sustained a tibial shaft fracture and 260 of the patients who sustained an ankle fracture were still living in the city of Malmo. Objective registration was done of all subsequent fractures that these former patients had sustained. Comparison was done with corresponding data from double numbers of age- and sex-matched controls who at that time (1950s and 1960s) had no such fractures. At the time of the fracture as well as today, the controls were living in the area of Malmo. Individuals with earlier tibial or ankle fractures had an increased incidence of fractures generally classified as fragility fractures. There was no difference in this respect between men and women, nor whether the initial fracture had been diaphyseal or metaphyseal. We conclude that sustenance of fractures early in life may serve as a predictor for fragility fractures later in life.

- Because degenerative changes in the trapeziometacarpal joint are a common cause of pain and weakness of the thumb, particularly in postmenopausal women, we have evaluated the functional and radiological results of arthrodesis in 43 patients. Arthrodesis is a safe and effective way of treating isolated painful arthritis in the trapeziometacarpal joint; solid fusion occurred in 39 cases, and four patients had fibrous union. All but four patients were satisfied with the operation including three of the four patients with fibrous union. Thirty-one of the patients had no functional restrictions (decreased range of movement), 34 were free of pain at rest, and 25 were free of pain all the time.
