  • On the hypersensitivity syndrome induced by the selective serotonin reuptake inhibiting antidepressant Zimeldine : A clinical and experimental study

    • Zimeldine, the first selective neuronal serotonin (5-H1) reuptake inhibitor to be registered as an antidepressant was withdrawn from common use due to an acute flu-like adverse syndrome comprising fever, myalgia and/or arthralgia and signs of disturbed liver function. A few of these patients also developed a peripheral neuropathy, in the most serious form a Guillain-Barré syndrome (GBS). The reaction was tentatively termed a hypersensitivity syndrome (HSS).Patients who had developed a HSS during zimeldine therapy (HSS-patients) were compared to control patients who tolerated the drug.No predictor to the HSS emerged concerning demographic data, psychiatric illness, previous medical history, medications with zimeldine or other drugs, other diseases, professional or nutritional circumstances. Nor was there any sign that HSS-patients who also developed neurological symptoms had a different profile of the HSS than other HSS-patients. Earlier zin1eldine treatment per se was not seen to predispose for development of a HSS or any other kind of adverse experience during subsequent therapy. The spectrum of adverse reactions was in agreement with those reported in previous studies and no new case of the GBS was revealed. The estimated frequency of HSS ranged from 0.63 to 3.4 % in spontaneous reports to the regnlatory authority and 1.4- 13% in a written inquiry. Among closely monitored inpatients 31 %developed a HSS. The mechanism for the HSS does not seem to be related primarily to the 5-HT reuptake inhibition as such. No HLA-associated disposition for the HSS was found. Nor was there any support for a HLA-associated disposition for depressive disorders. The pathogenetic mechanism for the HSS seems to involve an inlmunological response to antigens related to zimeldine.Zimeldine and its main primary metabolite norzimeldine both suppressed clinical signs of experimental allergic neuritis (EAN) in Lewis rats. Imrnunomodulatory effects in vitro of zimeldine, its metabolites norzimeldine and CPP 200 as well as of other monoamine reuptake inhibiting antidepressants were identified in the same EAN model.These observations call for further research on immunological mechanisms in the pathogenesis of mental disorders as well as on the potential role of drugs acting on the monoamine systems in the treatment of autoimmune diseases. The findings also justify a discussion on the application of immunological methods in the testing of new psychopharmacological drugs.Based on the presented results the tentatively used term for the adverse reactions to zimeldine, the hypersensitivity syndrome, seems justified.

    • An interface in elastomeric poly(dimethylsiloxane) (PDMS) for on-line orthogonal coupling of packed capillary liquid chromatography (LC) (i.d. = 0.2 mm) with capillary electrophoresis (CE) in combination with sheathless electrospray ionization (ESI) time-of-flight mass spectrometric (TOFMS) detection is presented. The new interface has a two-level design, which in combination with a continuous CE electrolyte flow through the interface provides integrity of the LC effluent and the CE separation until an injection is desired. The transparent and flexible PDMS material was found to have a number of advantages when combined with fused silica column technology, including ease to follow the process and ease to exchange columns. By combining conventional microscale systems of LC, CE, and ESI−MS, respectively, the time scales of the individual dimensions were harmonized for optimal peak capacity per unit time. The performance of the LC−CE−TOFMS system was evaluated using peptides as model substances. A S/N of about 330 was achieved for leucine-enkephaline from a 0.5 μL LC injection of 25 μg/mL peptide standard.

    • Computational epidemiology is a form of spatiotemporal reasoning in which social link structures are employed, and spatially explicit models are specified and executed. We point to issues thus far addressed neither by engineers, nor scientists, in the light of a use case focusing on catastrophic scenarios that assume the emergence of a highly unlikely but lethal and contagious strain of influenza. Our conclusion is that important perspectives are missing when dealing with policy issues resulting from scenario execution and analyses in computational epidemiology.

    • Climate-induced mechanical damage to cultural heritage objects of hygroscopic materials is not yet fully understood. This is particularly true of objects in historic buildings with less climate-controlled indoor environments. Research aiming at clarifying the response of hygroscopic materials to changes of the ambient relative humidity and temperature is scarce. The objective of this study was to use a method to monitor relative humidity and temperature at three different depths inside samples of Scots pine (Pinus sylvestris L.), subjected to relative humidity and temperature fluctuations in a climate chamber. This approach is important because mechanical stress is related to the moisture content of the material. However, the knowledge on how moisture gradients in wood progress before reaching equilibrium has not been studied in depth in cultural heritage science. The monitored relative humidity and temperature data in the wooden samples were converted to moisture content using a method that took both temperature and the hysteresis effect into account. The samples were subjected to step changes and daily relative humidity fluctuations at different temperatures. Moreover, museum climate, a non-heated historic building climate, and intermittent heating of a typical church were simulated in the experiments. Low temperatures reduced the moisture diffusion rate, resulting in moisture content fluctuations of smaller amplitudes. A response delay was noted which gradually increased with depth in the wood and with low temperatures. A combination of a step-change and daily fluctuations increased the time to reach equilibrium due to the slower desorption process compared to adsorption process. Occasionally, the moisture content could also exceed equilibrium at some depths. The moisture content levels in wood during intermittent heating stayed stable overall. The conclusion is that moisture transport in wood is complex and is influenced by the moisture history of wood, as well as duration and amplitude of the combined fluctuations in relative humidity and temperature.
