  • A randomized prospective clinical evaluation of two desensitizing agents on cervical dentine sensitivity : A pilot study

    • Syftet med denna prospektiva kliniska pilot studie var att utvärdera den upplevda smärtlindrande effekten av behandling mot känsliga tandhalsar med två desensibiliseringsprodukter (VivaSens® och Seal&Protect®) samt att utreda eventuella skillnader i den smärtlindrande effekten mellan materialen. Dessutom undersöktes försökspersonernas smärtupplevelse med hjälp av en enkät för att få indikation på i hur stor grad behandlingen gav förbättrad oral hälsa/livskvalité för respektive person. Total 30 patienter (23 kvinnor, 7 män) delades slumpmässigt in i två grupper i denna studie med sex månaders uppföljning. Varje grupp behandlades med en av de två produkterna. Känslighetsmätning genomfördes innan behandling (baseline) och vid vardera 1 vecka och 6 månader efter behandling. Patienterna fick ange den upplevda känsligheten i området vid luftblästring genom att markera på en Visuell Analog Skala (VAS). Efter 6 månader hade 27 patienter (90 %) fullföljt studien. Resultaten visade en signifikant minskning av känsliga tandhalsar efter en vecka respektive sex månader efter behandling med VivaSens® eller Seal&Protect®. Det fanns inga skillnader på behandlings effekten mellan de två produkterna. Resultaten från enkäten visade att patienterna upplevde en förbättrad oral hälsa/livskvalité efter behandling (0,000  p  0,021) och det fanns inga statistiska signifikant skillnader mellan grupperna. Slutsatsen är att båda produkterna som användes i studien var effektiva i reducering av känsliga tandhalsar under en 6 månaders period både enligt resultat från luftblästring som utifrån frågeformulär om försökspersonernas upplevelse av sin orala hälsa/livskvalité.

  • Fluoride toothpaste and toothbrushing; knowledge, attitudes and behaviour among Swedish adolescents and adults

    • The most effective way to administer fluoride is through the regular use of fluoride toothpaste. Adolescents and adults seem to have low awareness of toothbrushing procedures and use of fluoride toothpaste despite frequent dental care. The aim of this study was to describe knowledge, attitudes and behaviour concerning toothbrushing and use of fluoride toothpaste in three age groups in a Swedish population. A qualitative study design was used with the purpose of achieving a deeper understanding of the issue. Data were collected through interviews. A manifest and latent analysis of the text was performed using qualitative content analysis (Grounded theory). The informants were selected strategically to obtain the greatest possible variation in the data. Three age strata representing different stages in life were chosen: 15-16, 30-35 and 60-65 years. Informants were interviewed with support from an interview guide. Open-ended questions were used to focus on the individual’s knowledge, attitudes and behaviour concerning toothbrushing and fluoride toothpaste. Five people from each age group were interviewed in the study. The content areas were knowledge, attitudes and behaviour and the latent analysis identified the areas of empowerment, driving force and guidance as categories. Although the informants showed little knowledge about the reasons for and techniques of using fluoride toothpaste effectively, they described toothbrushing as important and the habit as a priority, giving the theme of this study: toothbrushing with fluoride toothpaste was a priority, despite the lack of knowledge about how to use toothpaste effectively and its positive effects on oral health. In conclusion the state of knowledge concerning toothbrushing and fluoride toothpaste needs to be improved. In addition, people’s desire for a fresh-feeling mouth and to fit in socially must be affirmed and utilized by dental staff in health promotion.

  • Tobacco cessation interventions by Swedish dental hygienists : a questionnaire study

    • The aim of the study was to investigate tobacco cessation interventions by Swedish dental hygienists and their perception of the importance of tobacco cessation to oral health. A questionnaire was mailed to 400 randomly selected dental hygienists (DH) in Sweden. The questions covered such topics as tobacco cessation interventions, perceived barriers, and their perception of the importance of tobacco cessation in relation to caries, gingivitis, periodontitis and dental implants. The response rate was 57%. Tobacco habits were routinely recorded by 94% of the respondents. 52% of the dental hygienists reported time constraints, 50% reported insufficient competence and 43% answered that they had lack of experience to work with tobacco cessation. All respondents perceived tobacco cessation to be an important determinant of treatment outcomes in patients with dental implants and periodontitis. Bivariate analysis showed an association between training courses in tobacco cessation and tobacco cessation interventions (OR 3.25, CI 95% 1.80-5.85). A logistic multivariate regression model disclosed two other factors significantly correlated with tobacco cessation interventions: competence (OR 2.4, 95% CI 1.16-4.85), and experience (OR 2.1, 95% CI 1.06-4.28). The analyses were adjusted for age, length of undergraduate training course, and dental care organization. The dental hygienists considered tobacco cessation to be very important in patients with periodontitis and in those with dental implants. Most of the DH in this study undertook some tobacco cessation interventions, though not extensive; the main barriers reported were lack of time, competence and experience.

  • Eating disorders and signs and symptoms of temporomandibular disorders. A matched case-control study

    • The aim of the study was to examine signs and symptoms related to temporomandibular disorders (TMD) in patients with eating disorders (ED) and to compare the prevalence with that in sex- and age-matched controls. During a 12-month period, all patients (n=65) who accepted and initiated psychiatric/medical outpatient treatment in an Eating Disorder Clinic/Erikbergsgården, Örebro, Sweden were invited to participate in the study. Of the ED patients, 54 (83 %) accepted participation. ED patients and controls underwent a comprehensive TMD questionnaire and clinical examination. Reported symptoms such as headache, facial pain, jaw tiredness, tongue thrusting, and lump feeling in the throat as well as dizziness, concentration difficulties and sleep disturbances were all significantly more prevalent among ED patients compared to controls. There was also a significantly higher prevalence of clinical TMD signs in the ED patients. Analyses within the ED group showed that those who reported self-induced vomiting reported significantly more heavy feeling in the head, nausea and snoring. Those with binge eating reported significantly more heavy feeling in the head, facial pain, dizzy feeling and concentration difficulties. No significant differences regarding subjective symptoms and clinical signs of TMD were found within the ED group with respect to duration of ED. In conclusion, orofacial pain and TMD related signs and symptoms are significantly more common in ED patients than in matched control subjects. Special emphasis should be made to those who reports vomiting and/or binge eating behaviors.

  • The prevalence and alterations of furcation involvements 13 to 16 years after periodontal treatment

    • The aim of this study was to evaluate the long term outcome of furcation involved molars in a population treated for periodontal disease. Initially, the study sample was 147 referred periodontal patients. Periodontal treatment consisted of oral hygiene instructions, supra- and subgingival scaling and periodontal surgery. After treatment 99 patients participated in a two year study on root caries. The patients got maintenance treatment every third to fourth month during 2 years. At the end of that study the patients were periodontally healthy and were referred back for supportive treatment to the referring dentist. Thirteen to 16 years after periodontal treatment 81 patients were still alive and 64 accepted a re-examination. At the start of the observation period the remaining 64 patients had in total 1537 teeth. During the 13 to 16 year follow up 217 teeth were lost. The number of molars at baseline was 361. The number of furcation involvement with different degrees were; 267 (0), 67 (I), 25 (II) and 2 (III) respectively. Totally 69 molars were lost during follow up. The proportion of molar loss according to the degree of furcation involvements 0 to III at baseline were 15%, 29%, 40% and 100% respectively. It was a significant greater risk of loosing an initially furcation involved molar than a single rooted tooth (pandlt;0.0001). The risk of loosing an initially furcated molar increased with the degree of furcation involvement (degree I; pandlt;0.05, degree II; pandlt;0.01). I N CONCLUSION: During a long term observation period molars with furcation involvements are more frequently lost than not furcation involved molars. However, two thirds are still in function 13 to 16 years after treatment which indicate that molars with furcation involvements might survive long after periodontal treatment.

  • Adverse events in Public Dental Service in a Swedish county - A survey of reported cases over two years

    • Adverse events cause suffering and increased costs in health care.The main way of registering adverse event is through dental personnel's reports, but reports from patients can also contribute to the knowledge of such occurrences. This study aimed to analyse the adverse events reported by dental personnel and patients in public dental service (PDS) in a Swedish county. The PDS has an electronic system for reporting and processing adverse events and, in addition, patients can report shortcomings, as regards to reception and treatment, to a patient committee orto an insurance company. The study material consisted of all adverse events reported in 2010 and 2011, including 273 events reported by dental personnel, 53 events reported by patients to the insurance company and 53 events reported by patients to the patient committee. Data concerning patients' age and gender, the nature, severity and cause of the event and the dental personnel's age gender and profession were collected and analysed. Furthermore the records describing the dental personnel's reports from 2011 were studied to investigate if the event had been documented and the patient informed. Age groups o to 9 and 20 to 39 years were underrepresented while those between the ages 10 to 19 and 60 to 69 years were overrepresented in dental personnel's reports. Amongyoung patients delayed diagnosis and therapy dominated and among patients over 20 years the most frequent reports dealt with inadequate treatments, especially endodontic treatments. In 29% of the events there was no documentation of the adverse event in the records and 49% of cases had no report about patient information.The majority of the reports from dental personnel were made by dentists (69%). Reporting adverse events can be seen as a reactive way of working with patient safety, but knowledge about frequencies and causes of incidents is the basis of proactive patient safety work.
