  • Rationale and design of ACTIVE: the atrial fibrillation clopidogrel trial with irbesartan for prevention of vascular events

    • BACKGROUND: Atrial fibrillation (AF) is the most frequently occurring cardiac arrhythmia with often serious clinical consequences. Many patients have contraindications to anticoagulation, and it is often underused in clinical practice. The addition of clopidogrel to aspirin (ASA) has been shown to reduce vascular events in a number of high-risk populations. Irbesartan is an angiotensin receptor-blocking agent that reduces blood pressure and has other vascular protective effects. METHODS AND RESULTS: ACTIVE W is a noninferiority trial of clopidogrel plus ASA versus oral anticoagulation in patients with AF and at least 1 risk factor for stroke. ACTIVE A is a double-blind, placebo-controlled trial of clopidogrel in patients with AF and with at least 1 risk factor for stroke who receive ASA because they have a contraindication for oral anticoagulation or because they are unwilling to take an oral anticoagulant. ACTIVE I is a partial factorial, double-blind, placebo-controlled trial of irbesartan in patients participating in ACTIVE A or ACTIVE W. The primary outcomes of these studies are composites of vascular events. A total of 14000 patients will be enrolled in these trials. CONCLUSIONS: ACTIVE is the largest trial yet conducted in AF. Its results will lead to a new understanding of the role of combined antiplatelet therapy and the role of blood pressure lowering with an angiotensin II receptor blocker in patients with AF.

  • Mutations in a BTB-Kelch Protein, KLHL7, Cause Autosomal-Dominant Retinitis Pigmentosa

    • Retinitis pigmentosa (RP) refers to a genetically heterogeneous group of progressive neurodegenerative diseases that result in dysfunction and/or death of rod and cone photoreceptors in the retina. So far, 18 genes have been identified for autosomal-dominant (ad) RP. Here, we describe an adRP locus (RP42) at chromosome 7p15 through linkage analysis in a six-generation Scandinavian family and identify a disease-causing mutation, c.449G -> A (p.S150N), in exon 6 of the KLHL7 gene. Mutation screening of KLHL7 in 502 retinopathy probands has revealed three different missense mutations in six independent families. KLHL7 is widely expressed, including expression in rod photoreceptors, and encodes a 75 kDa protein of the BTB-Kelch Subfamily within the BTB superfamily. BTB-Kelch proteins have been implicated in ubiquitination through Cullin E3 ligases. Notably, all three putative disease-causing KLHL7 mutations are within a conserved BACK domain; homology modeling suggests that mutant amino acid side chains can potentially fill the cleft between two helices, thereby affecting the ubiquitination complexes. Mutations in an identical region of another BTB-Kelch protein, gigaxonin, have previously been associated with giant axonal neuropathy. Our studies suggest an additional role of the ubiquitin-proteasome protein-degradation pathway in maintaining neuronal health and in disease.

    • The authors address feature-size dependence in Ar/Cl-2, chemically assisted ion beam etching (CAIBE) in the context of the fabrication of photonic crystal (PhC) structures. They systematically investigate the influence of various parameters such as hole diameter (115-600 nm), etch duration (10-60 min), and ion beam energy (300-600 eV) on PhC etching in InP with Ar/Cl-2, CAIBE. For a 60 min etching at an Ar-ion energy of 400 eV, the authors report an etch depth of 5 mu m for hole diameters d larger than 300 nm; the etch depth is in excess of 3 mu m for d larger than 200 nm. The evolution of roughness at the bottom of the etched holes and its dependence on hole size and etching conditions,is discussed. The physical mechanism of the observed feature-size dependent etching (FSDE) is then discussed and the effect of the process parameters is qualitatively understood using a model combining the effect of ion sputtering and surface chemical reactions. Finally, the effect of FSDE on the PhC optical properties is assessed by measuring the quality factor of one-dimensional Fabry-Perot PhC cavities. The measured quality factors show a clear trend with the etch depth: the cavity Q increases as the etch depth increases.

    • This work addresses feature size effects (the lag-effect and roughness development) in chemically assisted ion beam etching (CAIBE) etching of InP-based photonic crystals. Photonic crystal fields with varying hole size and periods were etched with different etching times. The slope of the etch depth versus diameter curves (lag-curves) reveals a hole size dependence, with a critical aspect ratio higher than 25. A model for the etch rate specific to Ar/Cl-2 CAIBE is proposed. We calculate the etch rate using a physico-chemical model which takes in to account the effect of Ar-ion sputtering and surface chemical reactions. In addition, it combines the aspect ratio dependence of the gas conductance of the etched holes. The origin and evolution of the bottom roughness of the etched holes is examined. The impact of the feature size dependence of the etching on the photonic crystal optical properties is then assessed by measuring the quality-factor of one-dimensional Fabry-Perot cavities using the Internal Light Source method, and discussed in terms of hole shape and depth. A systematic trend between the determined quality factor (Q) and the lag-effect is evidenced: Q decreases from about 250 to 60 when the hole depth drops from 5 mu m to 2 mu m.

    • Feature size dependence in Ar/Cl-2 chemically assisted ion beam etching of InP-based photonic crystals (PhCs) and its influence on the optical properties of PhC devices operating in the band gap are investigated. The analysis of the measured quality factors, the determined mirror reflectivities, and losses of one-dimensional Fabry-Perot cavities clearly demonstrates the importance of feature-size dependent etching. The optical properties show a dramatic improvement up to a hole depth of about 3.5 mu m that is primarily due to a significant reduction in extrinsic losses. However, beyond this hole depth, the improvement is at a lower rate, which suggests that extrinsic losses, although present, are not dominant.

  • Premature truncation of a novel protein, RD3, exhibiting subnuclear localization is associated with retinal degeneration

    • The rd3 mouse is one of the oldest identified models of early-onset retinal degeneration. Using the positional candidate approach, we have identified a C -> T substitution in a novel gene, Rd3, that encodes an evolutionarily conserved protein of 195 amino acids. The rd3 mutation results in a predicted stop codon after residue 106. This change is observed in four rd3 lines derived from the original collected mice but not in the nine wild-type mouse strains that were examined. Rd3 is preferentially expressed in the retina and exhibits increasing expression through early postnatal development. In transiently transfected COS-1 cells, the RD3-fusion protein shows subnuclear localization adjacent to promyelocytic leukemia-gene-product bodies. The truncated mutant RD3 protein is detectable in COS-1 cells but appears to get degraded rapidly. To explore potential association of the human RD3 gene at chromosome 1q32 with retinopathies, we performed a mutation screen of 881 probands from North America, India, and Europe. In addition to several alterations of uncertain significance, we identified a homozygous alteration in the invariant G nucleotide of the RD3 exon 2 donor splice site in two siblings with Leber congenital amaurosis. This mutation is predicted to result in premature truncation of the RD3 protein, segregates with the disease, and is not detected in 121 ethnically matched control individuals. We suggest that the retinopathy-associated RD3 protein is part of subnuclear protein complexes involved in diverse processes, such as transcription and splicing.

    • A procedure for the infiltration of planar photonic crystals (PhCs) with liquid crystals (LCs) is presented. InP-based PhCs are infiltrated with the nematic LC-K15 in a specially designed high-vacuum chamber. The infiltration technique is validated and systematically characterized by measuring the transmission through the infiltrated PhCs at different temperatures and for different polarizations. The reproducibility and reliability of our procedure are demonstrated and a high filling efficiency is obtained.

    • Recently there has been a growing amount of attention devoted to tuneable photonic crystals (PhCs) where the optical response of PhC structures can be dynamically modified. We will show how infiltrating planar PhCs with a synthetic organic material allows the trimming and tuning of their optical properties. The potential of PhC infiltration will be demonstrated for InP-based planar PhCs consisting of a hexagonal array of air holes (hole diameter = 200 - 400 nm; air filling factor = 0.40-0.50) etched through a planar waveguide in which light emitters (i.e. quantum wells) were embedded to enable optical measurements. The PhC pores were infiltrated with LC-K15 (5CB) nematic liquid crystals (LCs) in a specifically designed vacuum chamber, thereby changing the refractive index contrast between the holes and the semiconductor (trimming). Moreover, the possibility of tuning the optical response of PhCs by an external perturbation (i.e. temperature) was demonstrated. The change of the PhC optical properties due to infiltration and temperature tuning was studied both experimentally and theoretically. Experimental measurements were compared to theoretical calculations in order to obtain information on the in-filling efficiency, the LC refractive index, and the molecule orientation inside the holes. In the first case, optical measurements were performed as a function of temperature, whilst the average LC director configuration was determined by comparing transmission spectra in the transverse electric and magnetic polarization directions.

    • BACKGROUND: Patients with heart failure (HF) and anaemia have greater functional impairment, worse symptoms, increased rates of hospital admission, and a higher risk of death, compared with non-anaemic HF patients. Whether correcting anaemia can improve outcomes is unknown. OBJECTIVE: The Reduction of Events with Darbepoetin alfa in Heart Failure trial (RED-HF; Clinical Trials.gov NCT 003 58215) was designed to evaluate the effect of the long-acting erythropoietin-stimulating agent darbepoetin alfa on mortality and morbidity (and quality of life) in patients with HF and anaemia. METHODS: Approximately 2600 patients with New York Heart Association class II-IV, an ejection fraction < or =40%, and a haemoglobin (Hb) consistently < or =12.0 g/dL but > or =9.0 g/dL will be enrolled. Patients are randomized 1:1 to double-blind subcutaneous administration of darbepoetin alfa or placebo. Investigators are also blinded to Hb measurements and darbepoetin alfa is dosed to achieve an Hb concentration of 13.0 g/dL (but not exceeding 14.5 g/dL) with sham adjustments of the dose of placebo. The primary endpoint is the time to death from any cause or first hospital admission for worsening HF, whichever occurs first. The study will complete when approximately 1150 subjects experience a primary endpoint.

  • Optical Bloch waves studied by near optical field microscopy

    • Photonic bandgap structures are very promising for the integration of optical function at the nanoscale level and particularly 2D structures perforated on a slab waveguide. In this context, photonic crystal waveguides obtained by removing rows of holes in the periodic structure are very interesting. We will present here, results of near field optical microscopy conducted on PCW. With a Fourier transform analysis of the image, we can image the Bloch waves propagating in the waveguide. The results are compared to FDTD simulations and we will show that the images obtained correspond to the electric field. Such result allows us to study the interaction of the optical tip with the electromagnetic field. 2D FDTD simulations of 1D photonic crystal interaction with a SNOM tip is presented. The results obtained confirm that the SNOM signal is mainly related to the electric field.

    • Coupling into the Bloch modes of a two-dimensional photonic crystal (PhC) field is investigated by Fourier optics. The PhC was designed to operate in the second band above the air-light line, close to the autocollimation regime for TE polarization. The sample was fabricated in an InP-based heterostructure and an access ridge waveguide provides in-plane excitation of the PhC. The spatial Fourier transform of the field maps obtained from finite-difference time-domain simulations and those calculated by plane-wave expansion are compared to the experimentally obtained equifrequency surfaces (EFS). The shape of the imaged EFS and its variation with the excitation wavelength is shown to be consistent with the theoretical simulations. Finally, the results indicate that if combined with different excitation geometries, Fourier optics can be a powerful technique to assess photonic crystal devices and to design efficient structures.

  • Circulating Tumor Cell Number and Prognosis in Progressive Castration-Resistant Prostate Cancer

    • PURPOSE: The development of tumor-specific markers to select targeted therapies and to assess clinical outcome remains a significant area of unmet need. We evaluated the association of baseline circulating tumor cell (CTC) number with clinical characteristics and survival in patients with castrate metastatic disease considered for different hormonal and cytotoxic therapies. EXPERIMENTAL DESIGN: CTC were isolated by immunomagnetic capture from 7.5-mL samples of blood from 120 patients with progressive clinical castrate metastatic disease. We estimated the probability of survival over time by the Kaplan-Meier method. The concordance probability estimate was used to gauge the discriminatory strength of the informative prognostic factors. RESULTS: Sixty-nine (57%) patients had five or more CTC whereas 30 (25%) had two cells or less. Higher CTC numbers were observed in patients with bone metastases relative to those with soft tissue disease and in patients who had received prior cytotoxic chemotherapy relative to those who had not. CTC counts were modestly correlated to measurements of tumor burden such as prostate-specific antigen and bone scan index, reflecting the percentage of boney skeleton involved with tumor. Baseline CTC number was strongly associated with survival, without a threshold effect, which increased further when baseline prostate-specific antigen and albumin were included. CONCLUSIONS: Baseline CTC was predictive of survival, with no threshold effect. The shedding of cells into the circulation represents an intrinsic property of the tumor, distinct from extent of disease, and provides unique information relative to prognosis.

  • Low loss InP based photonic crystal waveguides and resonators

    • We report on low-loss light propagation at the communication wavelength of 1.55 μm through straight two-dimensional photonic crystal waveguides patterned into InGaAsP/InP heterostructures. The linear defect waveguides along the ΓK direction of a triangular lattice of air holes were etched deeply into the semiconductor by Cl 2/Ar electron cyclotron resonance reactive ion etching. Efficient waveguiding was observed for both polarization directions, although a photonic band gap exists for only one of the polarization states. Propagation losses, determined by the Fabry-Pérot resonance method, are as low as 0.2 dB/mm and 1.5 dB/mm for waveguides based on seven and three missing rows of holes, respectively. Waveguide resonators with 100 GHz channel spacing and quality factors up to 15,000 have been realized by inserting photonic crystal mirrors into the waveguides. The dispersion of the resonators was measured using a phase shift technique. Values for the group velocity dispersion range from -250 ps/nm to +250 ps/nm at wavelengths around 1.55 μm, which is sufficient to compensate for the dispersion of 15 km standard single-mode fiber. Controlling the device temperature allows to tune the dispersive properties of the devices.

    • We have investigated the dispersion properties of photonic crystal waveguide resonators. A passive InGaAsP/InP slab waveguide structure was used for the fabrication of the samples. The PhC waveguide resonators were defined by the omission of several rows of holes along the ΓK or ΓM direction of a triangular photonic crystal lattice. In addition, mirrors with a thickness of 1 to 4 rows of holes were inserted into the waveguide. An optimized dry etch process was used to etch the patterns to a depth of 3.5 μm through the waveguide layer. The group delay of the PhC devices was measured using the phase shift technique. The signal of a tunable laser was modulated at 3 GHz using a LiNbO 3 Mach-Zehnder modulator and detected with a high-frequency lightwave receiver. A phase sensitive detection with a network analyer measured the phase shift of the transmitted signal, which is proportional to the group delay. Close to the center of the resonances, the chromatic dispersion reaches values of -250 ps/nm and 250 ps/nm. This corresponds to the chromatic dispersion of 15 km standard fiber.

    • We report in this paper results obtained from characterizations of photonic band gap waveguide, using near field optical microscopy. We will show evidence of a Bloch wave propagating within our W1 photonic crystal waveguides (PCWs) structure, using a 2D Fourier transform approach. The processed image will then be compared to simulations obtained from plane wave method. This comparison exhibits that near-field measurements, using tapered optical fibers as probes, are mainly sensitive to the electric field propagating within the structure.

    • AIMS/HYPOTHESIS: Oxidation of LDL in the arterial extracellular matrix is a key event in the development of atherosclerosis and autoantibodies against oxidised LDL antigens reflect disease severity and the risk of developing acute cardiovascular events. Since type 2 diabetes is associated with increased oxidative stress, we tested the hypothesis that autoantibodies against oxidised LDL antigens are biomarkers for vascular complications in diabetes. METHODS: We studied 497 patients with type 2 diabetes without clinical signs of coronary heart disease. Oxidised LDL autoantibodies were determined by ELISA detecting IgG and IgM specific for native and malondialdehyde (MDA)-modified apolipoprotein B-100 peptides p45 and p210. The severity of coronary disease was assessed as the coronary artery calcium score. RESULTS: Patients affected by retinopathy had significantly higher levels of IgG against MDA-p45 and MDA-p210. In contrast, high levels of autoantibodies against the corresponding native peptides were associated with less coronary calcification and a lower risk of progression of coronary disease. CONCLUSIONS/INTERPRETATION: Our observations suggest that LDL oxidation is involved in the pathogenesis of diabetic retinopathy and that autoantibodies against apolipoprotein B peptides may act as biomarkers for both micro- and macrovascular complications in diabetes.
