    • Many long-term prospective studies have reported on associations of cardiovascular diseases with circulating lipid markers and/or inflammatory markers. Studies have not, however, generally been designed to provide reliable estimates under different circumstances and to correct for within-person variability. The Emerging Risk Factors Collaboration has established a central database on over 1.1 million participants from 104 prospective population-based studies, in which subsets have information on lipid and inflammatory markers, other characteristics, as well as major cardiovascular morbidity and cause-specific mortality. Information on repeat measurements on relevant characteristics has been collected in approximately 340,000 participants to enable estimation of and correction for within-person variability. Re-analysis of individual data will yield up to approximately 69,000 incident fatal or nonfatal first ever major cardiovascular outcomes recorded during about 11.7 million person years at risk. The primary analyses will involve age-specific regression models in people without known baseline cardiovascular disease in relation to fatal or nonfatal first ever coronary heart disease outcomes. This initiative will characterize more precisely and in greater detail than has previously been possible the shape and strength of the age- and sex-specific associations of several lipid and inflammatory markers with incident coronary heart disease outcomes (and, secondarily, with other incident cardiovascular outcomes) under a wide range of circumstances. It will, therefore, help to determine to what extent such associations are independent from possible confounding factors and to what extent such markers (separately and in combination) provide incremental predictive value.

  • Plasma fibrinogen level and the risk of major cardiovascular diseases and nonvascular mortality: an individual participant meta-analysis

    • CONTEXT: Plasma fibrinogen levels may be associated with the risk of coronary heart disease (CHD) and stroke. OBJECTIVE: To assess the relationships of fibrinogen levels with risk of major vascular and with risk of nonvascular outcomes based on individual participant data. DATA SOURCES: Relevant studies were identified by computer-assisted searches, hand searches of reference lists, and personal communication with relevant investigators. STUDY SELECTION: All identified prospective studies were included with information available on baseline fibrinogen levels and details of subsequent major vascular morbidity and/or cause-specific mortality during at least 1 year of follow-up. Studies were excluded if they recruited participants on the basis of having had a previous history of cardiovascular disease; participants with known preexisting CHD or stroke were excluded. DATA EXTRACTION: Individual records were provided on each of 154,211 participants in 31 prospective studies. During 1.38 million person-years of follow-up, there were 6944 first nonfatal myocardial infarctions or stroke events and 13,210 deaths. Cause-specific mortality was generally available. Analyses involved proportional hazards modeling with adjustment for confounding by known cardiovascular risk factors and for regression dilution bias. DATA SYNTHESIS: Within each age group considered (40-59, 60-69, and > or =70 years), there was an approximately log-linear association with usual fibrinogen level for the risk of any CHD, any stroke, other vascular (eg, non-CHD, nonstroke) mortality, and nonvascular mortality. There was no evidence of a threshold within the range of usual fibrinogen level studied at any age. The age- and sex- adjusted hazard ratio per 1-g/L increase in usual fibrinogen level for CHD was 2.42 (95% confidence interval [CI], 2.24-2.60); stroke, 2.06 (95% CI, 1.83-2.33); other vascular mortality, 2.76 (95% CI, 2.28-3.35); and nonvascular mortality, 2.03 (95% CI, 1.90-2.18). The hazard ratios for CHD and stroke were reduced to about 1.8 after further adjustment for measured values of several established vascular risk factors. In a subset of 7011 participants with available C-reactive protein values, the findings for CHD were essentially unchanged following additional adjustment for C-reactive protein. The associations of fibrinogen level with CHD or stroke did not differ substantially according to sex, smoking, blood pressure, blood lipid levels, or several features of study design. CONCLUSIONS: In this large individual participant meta-analysis, moderately strong associations were found between usual plasma fibrinogen level and the risks of CHD, stroke, other vascular mortality, and nonvascular mortality in a wide range of circumstances in healthy middle-aged adults. Assessment of any causal relevance of elevated fibrinogen levels to disease requires additional research.

    • A comprehensive phylogenetic classification of the kingdom Fungi is proposed, with reference to recent molecular phylogenetic analyses, and with input from diverse members of the fungal taxonomic community. The classification includes 195 taxa, down to the level of order, of which 16 are described or validated here: Dikarya subkingdom nov.; Chytridiomycota, Neocallimastigomycota phyla nov.; Monoblepharidomycetes, Neocallimastigomycetes class. nov.; Eurotiomycetidae, Lecarioromycetidae, Mycocaliciomycetidae subclass. nov.; Acarosporales, Corticiales, Baeomycetales, Candelariales, Gloeophyllales, Melanosporales, Trechisporales, Umbilicariales ords. nov. The clade containing Ascomycota and Basidiomycota is classified as subkingdom Dikarya, reflecting the putative synapomorphy of dikaryotic hyphae. The most dramatic shifts in the classification relative to previous works concern the groups that have traditionally been included in the Chytridiomycota and Zygomycota. The Chytridiomycota is retained in a restricted sense, with Blastocladiomycota and Neocallimastigomycota representing segregate phyla of flagellated Fungi. Taxa traditionally placed in Zygomycota are distributed among Glomeromycota and several subphyla incertae sedis, including Mucoromycotina, Entomophthoromycotina, Kickxellomycotina, and Zoopagomycotiria. Microsporidia are included in the Fungi, but no further subdivision of the group is proposed. Several genera of 'basal' Fungi of uncertain position are not placed in any higher taxa, including Basidiobolus, Caulochytrium, Olpidium, and Rozella. (c) 2007 The British Mycological Society.     • Environmental processes have been modelled for decades. However. the need for integrated assessment and modeling (IAM) has,town as the extent and severity of environmental problems in the 21st Century worsens. The scale of IAM is not restricted to the global level as in climate change models, but includes local and regional models of environmental problems. This paper discusses various definitions of IAM and identifies five different types of integration that Lire needed for the effective solution of environmental problems. The future is then depicted in the form of two brief scenarios: one optimistic and one pessimistic. The current state of IAM is then briefly reviewed. The issues of complexity and validation in IAM are recognised as more complex than in traditional disciplinary approaches. Communication is identified as a central issue both internally among team members and externally with decision-makers. stakeholders and other scientists. Finally it is concluded that the process of integrated assessment and modelling is considered as important as the product for any particular project. By learning to work together and recognise the contribution of all team members and participants, it is believed that we will have a strong scientific and social basis to address the environmental problems of the 21st Century.

  • Alginate impressions for fixed prosthodontics : A 20 year follow up study

    • The aim of this study was to estimate whether the survival ratios after 20 years of fixed prosthodontics made of alginate impressions was higher, equivalent or lower, compared to the survival ratios, shown in studies, where different impression materia Is were used. 151 females and 104 males were screened regarding the condition and age of the restorations at the an nu a I check-up in one of the authors surgeries. Average ages were 55 and 54 years respectively, when the fixed prosthodontics were seated. A total of 1.271 units were produced during the twenty years, 911 abutment teeth and 360 pontics. The type of prosthetic work was divided into three groups: 1) larger fixed prosthodontics 6-14 units (469), 2) smaller fixed prosthodontics 2-5 units (541) and 3) single crowns (261). The results show that alginate impressions can produce fixed prosthodontics with survival ratios similar to those presented in other studies, after 5 years (99%) 10 years (93-96%) and 15 years (74-96%). After 20 years the survival ratio was 61-63%. In conclusion, fixed prosthodontics made according to the syringe-tray alginate impression method may have the same success rates after 20 years compared to that of fixed prosthodontics presented in previous longitudinal clinical studies where other impression materials had been used. In this study, caries and root fractures were the main reasons for removing abutment teeth and pontics.

  • Estimation of the warfarin dose with clinical and pharmacogenetic data

    • BACKGROUND: Genetic variability among patients plays an important role in determining the dose of warfarin that should be used when oral anticoagulation is initiated, but practical methods of using genetic information have not been evaluated in a diverse and large population. We developed and used an algorithm for estimating the appropriate warfarin dose that is based on both clinical and genetic data from a broad population base.METHODS: Clinical and genetic data from 4043 patients were used to create a dose algorithm that was based on clinical variables only and an algorithm in which genetic information was added to the clinical variables. In a validation cohort of 1009 subjects, we evaluated the potential clinical value of each algorithm by calculating the percentage of patients whose predicted dose of warfarin was within 20% of the actual stable therapeutic dose; we also evaluated other clinically relevant indicators.RESULTS: In the validation cohort, the pharmacogenetic algorithm accurately identified larger proportions of patients who required 21 mg of warfarin or less per week and of those who required 49 mg or more per week to achieve the target international normalized ratio than did the clinical algorithm (49.4% vs. 33.3%, P<0.001, among patients requiring < or = 21 mg per week; and 24.8% vs. 7.2%, P<0.001, among those requiring > or = 49 mg per week).CONCLUSIONS: The use of a pharmacogenetic algorithm for estimating the appropriate initial dose of warfarin produces recommendations that are significantly closer to the required stable therapeutic dose than those derived from a clinical algorithm or a fixed-dose approach. The greatest benefits were observed in the 46.2% of the population that required 21 mg or less of warfarin per week or 49 mg or more per week for therapeutic anticoagulation.

  • Toroidal and poloidal momentum transport studies in tokamaks

    • The present status of understanding of toroidal and poloidal momentum transport in tokamaks is presented in this paper. Similar energy confinement and momentum confinement times, i.e. tau(E)/tau(phi)approximate to 1 have been reported on several tokamaks. It is more important though, to study the local transport both in the core and edge plasma separately as, for example, in the core plasma, a large scatter in the ratio of the local effective momentum diffusivity to the ion heat diffusivity chi(phi eff)/chi(i.eff) among different tokamaks can be found. For example, the value of effective Prandtl number is typically around chi(phi eff)/chi(i.eff)approximate to 0.2 on JET while still tau(E)/tau(phi)approximate to 1 holds. Perturbative NBI modulation experiments on JET have shown, however, that a Prandtl number chi(phi)/chi(i) of around 1 is valid if there is an additional, significant inward momentum pinch which is required to explain the amplitude and phase behaviour of the momentum perturbation. The experimental results, i.e. the high Prandtl number and pinch, are in good qualitative and to some extent also in quantitative agreement with linear gyro-kinetic simulations. In contrast to the toroidal momentum transport which is clearly anomalous, the poloidal velocity is usually believed to be neo-classical. However, experimental measurements on JET show that the carbon poloidal velocity can be an order of magnitude above the predicted value by the neo-classical theory within the ITB. These large measured poloidal velocities, employed for example in transport simulations, significantly affect the calculated radial electric field and therefore the E x B flow shear and hence modify and can significantly improve the simulation predictions. Several fluid turbulence codes have been used to identify the mechanism driving the poloidal velocity to such high values. CUTIE and TRB turbulence codes and also the Weiland model predict the existence of an anomalous poloidal velocity, peaking in the vicinity of the ITB and driven dominantly by the flow due to the Reynold's stress. It is worth noting that these codes and models treat the equilibrium in a simplified way and this affects the geodesic curvature effects and geodesic acoustic modes. The neo-classical equilibrium is calculated more accurately in the GEM code and the simulations suggest that the spin-up of poloidal velocity is a consequence of the plasma profiles steepening when the ITB grows, following in particular the growth of the toroidal velocity within the ITB.

    • Observations of electron phase-space holes (EHs) in Earth's plasma sheet by the THEMIS satellites include the first detection of a magnetic perturbation (delta B-parallel to) parallel to the ambient magnetic field (B-0). EHs with a detectable delta B-parallel to have several distinguishing features including large electric field amplitudes, a magnetic perturbation perpendicular to B-0, high speeds (similar to 0.3c) along B-0, and sizes along B-0 of tens of Debye lengths. These EHs have a significant center potential (Phi similar to k(B)T(e)/e), suggesting strongly nonlinear behavior nearby such as double layers or magnetic reconnection.

    • Broadband waves around the lower hybrid frequency (around 10 Hz) near the magnetopause are studied, using the four Cluster satellites. These waves are common at the Earthward edge of the boundary layer, consistent with earlier observations, and can have amplitudes at least up to 5 mV/m. These waves are similar on all four Cluster satellites, i.e. they are likely to be distributed over large areas of the boundary. The strongest electric fields occur during a few seconds, i.e. over distances of a few hundred km in the frame of the moving magnetopause, a scale length comparable to the ion gyroradius. The strongest magnetic oscillations in the same frequency range are typically found in the boundary layer, and across the magnetopause. During an event studied in detail, the magnetopause velocity is consistent with a large-scale depression wave, i.e. an inward bulge of magnetosheath plasma, moving tailward along the nominal magnetopause boundary. Preliminary investigations indicate that a rather flat front side of the large-scale wave is associated with a rather static small-scale electric field, while a more turbulent backside of the large-scale wave is associated with small-scale time varying electric field wave packets.

    • Stem cells generate neurons in discrete regions in the postnatal mammalian brain. However, the extent of neurogenesis in the adult human brain has been difficult to establish. We have taken advantage of the integration of (14)C, generated by nuclear bomb tests during the Cold War, in DNA to establish the age of neurons in the major areas of the human cerebral neocortex. Together with the analysis of the neocortex from patients who received BrdU, which integrates in the DNA of dividing cells, our results demonstrate that, whereas nonneuronal cells turn over, neurons in the human cerebral neocortex are not generated in adulthood at detectable levels but are generated perinatally.

    • Atomic, molecular and surface physics plays an important role in simulations of the edge plasma in present day tokamaks, and in the predictive simulations of new devices. The edge plasma-in this context, the Scrape-Off Layer (SOL), the Private Flux Region (PFR) and core region close to the separatrix (or Last Closed Flux Surface, LCFS)-provides the boundary conditions for the main plasma, and is the region where much of the power and all of the particle exhaust occurs. It is also the region where the plasma interacts with solid surfaces, puffed gases and gas arising from recycling. The results of plasma edge simulations can depend strongly on the availability and quality of the atomic, molecular and surface data (the peak plasma temperature at the divertor was found to vary by a factor of five dependent on the choice of atomic physics data in a recent sensitivity analysis). The current material choice for ITER with Plasma Facing Components (PFCs) consisting of C, Be and W also presents challenges, both in the availability of the necessary data for W, and in the plethora of charge states for W. Another challenge presented by the material choice is the likely presence of mixed materials formed by the migration of material from one surface to another. These introduce effects like alloying and preferential sputtering as well as new (much longer) time-scales in the problem. Efforts to incorporate a bundled charge state model within one of the present edge simulation codes, SOLPS, will he described, as well as efforts to address some of the questions raised by mixed materials. Some issues related to data consistency and traceability within the context of the European effort on Integrated Tokamak Modelling will also be addressed.

    • We report the first direct observations of parallel electric fields (E-parallel to) carried by double layers (DLs) in the plasma sheet of Earth's magnetosphere. The DL observations, made by the THEMIS spacecraft, have E-parallel to signals that are analogous to those reported in the auroral region. DLs are observed during bursty bulk flow events, in the current sheet, and in plasma sheet boundary layer, all during periods of strong magnetic fluctuations. These observations imply that DLs are a universal process and that strongly nonlinear and kinetic behavior is intrinsic to Earth's plasma sheet.

    • Changes of the toroidal plasma rotation induced by directed waves in the ion-cyclotron range of frequencies (ICRF) have been identified experimentally for the first time on the JET tokamak. The momentum carried by the waves is initially absorbed by fast resonating ions, which subsequently transfer it to the bulk plasma. Thus, the results provide evidence for the influence of ICRF heated fast ions on plasma rotation.

    • Fast wave current drive has been performed in JET plasmas with internal transport barriers, ITBs, and strongly reversed magnetic shear. Although the current drive efficiency of the power absorbed on the electrons is fairly high, only small effects are seen in the central current density. The main reasons are the parasitic absorption of RF power, the strongly inductive nature of the plasma and the interplay between the fast wave driven current and bootstrap current. The direct electron heating in the FWCD experiments is found to be strongly degraded compared to that with the dipole phasing.
