  • Intravenous Thrombolysis in Patients With Ischemic Stroke Aged ≥90 Years: A Cohort Study From the TRISP Collaboration
  • 2022
  • Ingår i: Stroke. - 1524-4628. ; 53:12, s. 3557-3563
  • Tidskriftsartikel (refereegranskat)abstract
    • BACKGROUND: The probability to receive intravenous thrombolysis (IVT) for treatment of acute ischemic stroke declines with increasing age and is consequently the lowest in very elderly patients. Safety concerns likely influence individual IVT treatment decisions. Using data from a large IVT registry, we aimed to provide more evidence on safety of IVT in the very elderly. METHODS: In this prospective multicenter study from the TRISP (Thrombolysis in Ischemic Stroke Patients) registry, we compared patients ≥90 years with those <90 years using symptomatic intracranial hemorrhage (ECASS [European Cooperative Acute Stroke Study]-II criteria), death, and poor functional outcome in survivors (modified Rankin Scale score 3-5 for patients with prestroke modified Rankin Scale score ≤2 and modified Rankin Scale score 4-5 for patients prestroke modified Rankin Scale ≥3) at 3 months as outcomes. We calculated adjusted odds ratio with 95% CI using logistic regression models. RESULTS: Of 16974 eligible patients, 976 (5.7%) were ≥90 years. Patients ≥90 years had higher median National Institutes of Health Stroke Scale on admission (12 versus 8) and were more often dependent prior to the index stroke (prestroke modified Rankin Scale score of ≥3; 45.2% versus 7.4%). Occurrence of symptomatic intracranial hemorrhage (5.7% versus 4.4%, odds ratioadjusted 1.14 [0.83-1.57]) did not differ significantly between both groups. However, the probability of death (odds ratioadjusted 3.77 [3.14-4.53]) and poor functional outcome (odds ratioadjusted 2.63 [2.13-3.25]) was higher in patients aged ≥90 years. Results for the sample of centenarians (n=21) were similar. CONCLUSIONS: The probability of symptomatic intracranial hemorrhage after IVT in very elderly patients with stroke did not exceed that of their younger counterparts. The higher probability of death and poor functional outcome during follow-up in the very elderly seems not to be related to IVT treatment. Very high age itself should not be a reason to withhold IVT.

  • Report on challenges for SCIs
  • 2021
  • Rapport (övrigt vetenskapligt/konstnärligt)abstract
    • This report discusses the challenges posed by four types of threats – terrorist attacks, cyber-attacks, extreme weather and social unrest – on the following eight smart critical infrastructure systems:a.ALPHA - Finance (financial system): The analysis focuses on disturbed information flow and disabling/manipulating IT and communication systems, including attacks on the “physical layer” using the example of IEMI/HPEM threats, as well as the software layer.b. BRAVO - Energy supply (system): The analysis focuses on disruption of “smart” energy supply in a “smart city”, caused by natural hazards, in this case flooding, leading to cascading effects and severe consequences for other energy-depending SCIs.c. CHARLIE - Health care (system):Focus of the analysis is on all threats that might cause large increases in the numbers of injuries or sick patients within a densely populated area. This will include indirect impacts, e.g. large numbers of injuries caused by a disaster or terrorist attacks or disease epidemics, but also direct impacts, e.g. service disruptions in critical health infrastructures, such as hospitals, due to attacks or disasters hitting the infrastructure itself.d.DELTA - Transportation (system) – airports: According to the framework situation, threats on Smart Airports will be assessed under circumstances of (i) blocked traffic, (ii) passenger and airplane traffic exceeding capacity (iii) flood.e. ECHO - Industry (in zones in cities) "Industrial Production Plants":The analysis focuses mainly on technological accidents within the refinery complex, but also accidents caused by natural hazards affecting refinery property outside the main refinery complex, e.g. accident on jetty belonging to refinery on the river Danube during unloading/loading oil products from barge to a tank, damages by a gale or storm on process installations (pipes, hoses) resulting in river pollution. Both scenarios could lead to cascading effects for other SCIs in close vicinity.f. FOXTROT - Water supply (systems): The analysis focuses on three cases of local and regional drinking water supply chains, with different kinds of vulnerabilities in terms of climate threats, ICT challenges, security issues and human error.g.GOLF - Urban flood protection (systems): The analysis focuses in the disruption of water and transport caused through tidal and fluvial flooding events.h. HOTEL: City of Helsinki - Flooding underground coal storage. Resilience of the energy infrastructure (city environment). The way this analysis was conducted was by assessing these threats using a 5x5 framework matrix. The two axes of the matrix were phases (understand risks, anticipate/prepare, absorb/withstand, respond/recover, adapt/learn) and dimensions (system/physical, information/data, organizational/business, societal/political, cognitive/decision-making).Each individual matrix block was discussed by subject experts who identified specific challenges and implications for each matrix element and rated its relevance (high, medium, low).In terms of the results, the system/physical dimension received the highest number of important challenges.Overall, the most important singular element was to understand risks in the organizational/business dimension. The least importance was attributed to the adapt/learn phase.

  • Association of prestroke metformin use, stroke severity, and thrombolysis outcome
  • 2020
  • Ingår i: Neurology. - : Ovid Technologies (Wolters Kluwer Health). - 0028-3878 .- 1526-632X. ; 95:4
  • Tidskriftsartikel (refereegranskat)abstract
    • Objective To evaluate whether pretreatment with metformin (MET) is associated with less stroke severity and better outcome after IV thrombolysis (IVT), we analyzed a cohort of 1,919 patients with stroke with type 2 diabetes mellitus in a multicenter exploratory analysis. Methods Data from patients with diabetes and ischemic stroke treated with IVT were collected within the European Thrombolysis in Ischemic Stroke Patients (TRISP) collaboration. We applied propensity score matching (PSM) to obtain balanced baseline characteristics of patients treated with and without MET. Results Of 1,919 patients with stroke with type 2 diabetes who underwent IVT, 757 (39%) had received MET before stroke (MET+), whereas 1,162 (61%) had not (MET-). MET+ patients were younger with a male preponderance. Hypercholesterolemia and pretreatment with statins, antiplatelets, or antihypertensives were more common in the MET+ group. After PSM, the 2 groups were well balanced with respect to demographic and clinical aspects. Stroke severity on admission (NIH Stroke Scale 10.0 +/- 6.7 vs 11.3 +/- 6.5), 3-month degree of independence on modified Rankin Scale (2 [interquartile range (IQR) 1.0-4.0] vs 3 [IQR 1.0-4.0]), as well as mortality (12.5% vs 18%) were significantly lower in the MET+ group. The frequency of symptomatic intracerebral hemorrhages did not differ between groups. HbA1c levels were well-balanced between the groups. Conclusions Patients with stroke and diabetes on treatment with MET receiving IVT had less severe strokes on admission and a better functional outcome at 3 months. This suggests a protective effect of MET resulting in less severe strokes as well as beneficial thrombolysis outcome.

  • Multiband optical variability of the TeV blazar PG 1553+113 in 2019
  • 2022
  • Ingår i: Monthly notices of the Royal Astronomical Society. - : Oxford University Press (OUP). - 0035-8711 .- 1365-2966. ; 519:2, s. 2796-2811
  • Tidskriftsartikel (refereegranskat)abstract
    • We report the flux and spectral variability of PG 1553 + 113 on intra-night (IDV) to short-term time-scales using BVRI data collected over 91 nights from 28 February to 8 November 2019 employing 10 optical telescopes: three in Bulgaria, two each in India and Serbia, and one each in Greece, Georgia, and Latvia. We monitored the blazar quasi-simultaneously for 16 nights in the V and R bands and 8 nights in the V, R, I bands and examined the light curves (LCs) for intra-day flux and colour variations using two powerful tests: the power-enhanced F-test and the nested ANOVA test. The source was found to be significantly (>99 per cent) variable in 4 nights out of 27 in R-band, 1 out of 16 in V-band, and 1 out of 6 nights in I-band. No temporal variations in the colours were observed on IDV time-scale. During the course of these observations the total variation in R-band was 0.89 mag observed. We also investigated the spectral energy distribution (SED) using B-, V-, R-, and I-band data. We found optical spectral indices in the range of 0.878 +/- 0.029 to 1.106 +/- 0.065 by fitting a power law (F-nu proportional to nu(-alpha)) to these SEDs of PG 1553 + 113. We found that the source follows a bluer-when-brighter trend on IDV time-scales. We discuss possible physical causes of the observed spectral variability.

  • Direct-imaging Discovery and Dynamical Mass of a Substellar Companion Orbiting an Accelerating Hyades Sun-like Star with SCExAO/CHARIS
  • 2022
  • Ingår i: Astrophysical Journal Letters. - : American Astronomical Society. - 2041-8205 .- 2041-8213. ; 934:2
  • Tidskriftsartikel (refereegranskat)abstract
    • We present the direct-imaging discovery of a substellar companion in orbit around a Sun-like star member of the Hyades open cluster. So far, no other substellar companions have been unambiguously confirmed via direct imaging around main-sequence stars in Hyades. The star HIP 21152 is an accelerating star as identified by the astrometry from the Gaia and Hipparcos satellites. We detected the companion, HIP 21152 B, in multiple epochs using the high-contrast imaging from SCExAO/CHARIS and Keck/NIRC2. We also obtained the stellar radialvelocity data from the Okayama 188 cm telescope. The CHARIS spectroscopy reveals that HIP 21152 B’s spectrum is consistent with the L/T transition, best fit by an early T dwarf. Our orbit modeling determines the semimajor axis and the dynamical mass of HIP 21152 B to be 17.5-+3.87.2 au and 27.8-+5.48.4 MJup, respectively. The mass ratio of HIP 21152 B relative to its host is ≈2%, near the planet/brown dwarf boundary suggested by recent surveys. Mass estimates inferred from luminosity-evolution models are slightly higher (33–42 MJup). With a dynamical mass and a well-constrained age due to the system’s Hyades membership, HIP 21152 B will become a critical benchmark in understanding the formation, evolution, and atmosphere of a substellar object as a function of mass and age. Our discovery is yet another key proof of concept for using precision astrometry to select directimaging targets.

  • SCExAO/CHARIS Direct Imaging Discovery of a 20 au Separation, Low-mass Ratio Brown Dwarf Companion to an Accelerating Sun-like Star
  • 2020
  • Ingår i: Astrophysical Journal Letters. - : American Astronomical Society. - 2041-8205 .- 2041-8213. ; 904:2
  • Tidskriftsartikel (refereegranskat)abstract
    • We present the direct imaging discovery of a substellar companion to the nearby Sun-like star, HD 33632 Aa, at a projected separation of similar to 20 au, obtained with SCExAO/CHARIS integral field spectroscopy complemented by Keck/NIRC2 thermal infrared imaging. The companion, HD 33632 Ab, induces a 10.5 sigma astrometric acceleration on the star as detected with the Gaia and Hipparcos satellites. SCExAO/CHARIS JHK (1.1-2.4 mu m) spectra and Keck/NIRC2 L-p (3.78 mu m) photometry are best matched by a field L/T transition object: an older, higher-gravity, and less dusty counterpart to HR 8799 cde. Combining our astrometry with Gaia/Hipparcos data and archival Lick Observatory radial velocities, we measure a dynamical mass of 46.4 8 M-J and an eccentricity of e < 0.46 at 95% confidence. HD 33632 Ab's mass and mass ratio (4.0% 0.7%) are comparable to the low-mass brown dwarf GJ 758 B and intermediate between the more massive brown dwarf HD 19467 B and the (near-)planet-mass companions to HR 2562 and GJ 504. Using Gaia to select for direct imaging observations with the newest extreme adaptive optics systems can reveal substellar or even planet-mass companions on solar system-like scales at an increased frequency compared to blind surveys.

  • SCExAO/CHARIS Near-infrared Integral Field Spectroscopy of the HD 15115 Debris Disk
  • 2020
  • Ingår i: Astronomical Journal. - : American Astronomical Society. - 0004-6256 .- 1538-3881. ; 160:4
  • Tidskriftsartikel (refereegranskat)abstract
    • We present new, near-infrared (1.1-2.4 mu m) high-contrast imaging of the debris disk around HD 15115 with the Subaru Coronagraphic Extreme Adaptive Optics (SCExAO) system coupled with the Coronagraphic High Angular Resolution Imaging Spectrograph (CHARIS). The SCExAO/CHARIS resolves the disk down to rho similar to 02 (r(proj) similar to 10 au), a factor of similar to 3-5 smaller than previous recent studies. We derive a disk position angle of PA similar to 2794-2805 and an inclination ofi similar to 853-86.2. While recent SPHERE/IRDIS imagery of the system could suggest a significantly misaligned two-ring disk geometry, CHARIS imagery does not reveal conclusive evidence for this hypothesis. Moreover, optimizing models of both one- and two-ring geometries using differential evolution, we find that a single ring having a Hong-like scattering phase function matches the data equally well within the CHARIS field of view (rho less than or similar to 1 ''). The disk's asymmetry, well evidenced at larger separations, is also recovered; the west side of the disk appears, on average, around 0.4 mag brighter across the CHARIS bandpass between 025 and 1 ''. Comparing Space Telescope Imaging Spectrograph (STIS) 50CCD optical photometry (2000-10500 A) with CHARIS near-infrared photometry, we find a red (STIS/50CCD-CHARIS broadband) color for both sides of the disk throughout the 04-1 '' region of overlap, in contrast to the blue color reported at similar wavelengths for regions exterior to similar to 2 ''. Further, this color may suggest a smaller minimum grain size than previously estimated at larger separations. Finally, we provide constraints on planetary companions and discuss possible mechanisms for the observed inner disk flux asymmetry and color.

  • High-resolution Near-infrared Polarimetry and Submillimeter Imaging of FS Tau A : Possible Streamers in Misaligned Circumbinary Disk System
  • 2020
  • Ingår i: Astrophysical Journal. - : American Astronomical Society. - 0004-637X .- 1538-4357. ; 889:2
  • Tidskriftsartikel (refereegranskat)abstract
    • We analyzed the young (2.8 Myr-old) binary system FS Tau A using near-infrared (H-band) high -contrast polarimetry data from Subaru/HiCIAO and submillimeter CO (J = 2-1) line emission data from Atacama Large Millimeter/submillimeter Array (ALMA). Both the near-infrared and submillimeter observations reveal several clear structures extending to similar to 240 au from the stars. Based on these observations at different wavelengths, we report the following discoveries. One arm-like structure detected in the near-infrared band initially extends from the south of the binary with a subsequent turn to the northeast, corresponding to two bar-like structures detected in ALMA observations with an local standard of rest kinematic (LSRK) velocity of 1.19-5.64 km s(-1). Another feature detected in the near-infrared band extends initially from the north of the binary, relating to an arm-like structure detected in ALMA observations with an LSRK velocity of 8.17-16.43 km s(-1). From their shapes and velocities, we suggest that these structures can mostly be explained by two streamers that connect the outer circumbinary disk and the central binary components. These discoveries will be helpful for understanding the evolution of streamers and circumstellar disks in young binary systems.

  • SiPP (Stroke in Pregnancy and Postpartum) : A prospective, observational, international, multicentre study on pathophysiological mechanisms, clinical profile, management and outcome of cerebrovascular diseases in pregnant and postpartum women
  • 2020
  • Ingår i: European Stroke Journal. - : SAGE Publications. - 2396-9873 .- 2396-9881. ; 5:2, s. 193-203
  • Tidskriftsartikel (refereegranskat)abstract
    • Rationale: Cerebrovascular diseases associated with pregnancy and postpartum period are uncommon; however, they can have an important impact on health of both women and foetus or newborn. Aims: To evaluate the frequency, characteristics and management of cerebrovascular events in pregnant/postpartum women, to clarify pathophysiological mechanisms underlying the occurrence of these events including biomolecular aspects, and to assess the short- and long-term cerebrovascular and global cardiovascular outcome of these patients, their predictors and infant outcome. Methods and design: This is an observational, prospective, multicentre, international case–control study. The study will include patients with cerebrovascular events during pregnancy and/or within six months after delivery. For each included case, two controls will be prospectively recruited: one pregnant or puerperal subject without any history of cerebrovascular event and one non-pregnant or non-puerperal subject with a recent cerebrovascular event. All controls will be matched by age, ethnicity and type of cerebrovascular event with their assigned cases. The pregnant controls will be matched also by pregnancy weeks/trimester. Follow-up will last 24 months for the mother and 12 months for the infant. Summary: To better understand causes and outcomes of uncommon conditions like pregnancy/postpartum-related cerebrovascular events, the development of multisite, multidisciplinary registry-based studies, such as the Stroke in Pregnancy and Postpartum study, is needed in order to collect an adequate number of patients, draw reliable conclusions and give definite recommendations on their management.

  • Subaru Near-infrared Imaging Polarimetry of Misaligned Disks around the SR 24 Hierarchical Triple System
  • 2020
  • Ingår i: Astronomical Journal. - : American Astronomical Society. - 0004-6256 .- 1538-3881. ; 159:1
  • Tidskriftsartikel (refereegranskat)abstract
    • The SR 24 multistar system hosts both circumprimary and circumsecondary disks, which are strongly misaligned with each other. The circumsecondary disk is circumbinary in nature. Interestingly, both disks are interacting, and they possibly rotate in opposite directions. To investigate the nature of this unique twin disk system, we present 01 resolution near-infrared polarized intensity images of the circumstellar structures around SR 24, obtained with HiCIAO mounted on the Subaru 8.2 m telescope. Both the circumprimary disk and the circumsecondary disk are resolved and have elongated features. While the position angle of the major axis and radius of the near-IR (NIR) polarization disk around SR 24S are 55° and 137 au, respectively, those around SR 24N are 110° and 34 au, respectively. With regard to overall morphology, the circumprimary disk around SR 24S shows strong asymmetry, whereas the circumsecondary disk around SR 24N shows relatively strong symmetry. Our NIR observations confirm the previous claim that the circumprimary and circumsecondary disks are misaligned from each other. Both the circumprimary and circumsecondary disks show similar structures in 12CO observations in terms of its size and elongation direction. This consistency is because both NIR and 12CO are tracing surface layers of the flared disks. As the radius of the polarization disk around SR 24N is roughly consistent with the size of the outer Roche lobe, it is natural to interpret the polarization disk around SR 24N as a circumbinary disk surrounding the SR 24Nb–Nc system.

  • Atmospheric Characterization and Further Orbital Modeling of kappa Andromeda b
  • 2020
  • Ingår i: Astronomical Journal. - : American Astronomical Society. - 0004-6256 .- 1538-3881. ; 159:2
  • Tidskriftsartikel (refereegranskat)abstract
    • We present kappa Andromeda b's photometry and astrometry taken with Subaru/SCExAO+HiCIAO and Keck/NIRC2, combined with recently published SCExAO/CHARIS low-resolution spectroscopy and published thermal infrared photometry to further constrain the companion's atmospheric properties and orbit. The Y/Y-K colors of kappa And b are redder than field dwarfs, consistent with its youth and lower gravity. Empirical comparisons of its Y-band photometry and CHARIS spectrum to a large spectral library of isolated field dwarfs reaffirm the conclusion from Currie et al. that it likely has a low gravity but admit a wider range of most plausible spectral types (L0-L2). Our gravitational classification also suggests that the best-fit objects for kappa And b may have lower gravity than those previously reported. Atmospheric models lacking dust/clouds fail to reproduce its entire 1-4.7 mu m spectral energy distribution (SED), and cloudy atmosphere models with temperatures of similar to 1700-2000 K better match kappa And b data. Most well-fitting model comparisons favor 1700-1900 K, a surface gravity of log(g) similar to 4-4.5, and a radius of 1.3-1.6 R-Jup; the best-fit model (Drift-Phoenix) yields the coolest and lowest-gravity values: T-eff = 1700 K and log g = 4.0. An update to kappa And b's orbit with ExoSOFT using new astrometry spanning 7 yr reaffirms its high eccentricity (0.77 0.08). We consider a scenario where unseen companions are responsible for scattering kappa And b to a wide separation and high eccentricity. If three planets, including kappa And b, were born with coplanar orbits, and one of them was ejected by gravitational scattering, a potential inner companion with mass greater than or similar to 10 M-Jup could be located at less than or similar to 25 au.
