    • This study tests the reliability and validity of the European Organization for Research and Treatment of Cancer (EORTC) head and neck cancer module (QLQ-H&N35) and version 3.0 of the EORTC Core Questionnaire (QLQ-C30) in 622 head and neck cancer patients from 12 countries. The patients completed the QLQ-C30, the QLQ-H&N35 and a debriefing questionnaire before antineoplastic treatment or at a follow-up. 232 patients receiving treatment completed a second questionnaire after treatment. Compliance was high and the questionnaire was well accepted by the patients. Multitrait scaling analysis confirmed the proposed scale structure of the QLQ-H&N35. The QLQ-H&N35 was responsive to differences between disease status, site and patients with different Karnofsky performance status, and to changes over time. The new physical functioning scale (with a four-point response format) of version 3.0 of the QLQ-C30 was shown to be more reliable than previous versions. Thus, the QLQ-H&N35, in conjunction with the QLQ-C30, appears to be reliable, valid and applicable to broad multicultural samples of head and neck cancer patients.

  • Global and local detection of blood vessel elasticity

    • The present invention is a non-invasive analysis system for detection of global and local blood vessel elasticity. The analysis system comprises two subsystems where subsystem 1 is data collecting unit and subsystem 2 is an analysis unit. The data collecting unit comprises one or many non-invasive image generating systems, or the data collecting system makes the registration possible of movement parameters that quantifies the dynamics of the blood vessel wall in longitudinal and radial directions. Subsystem 2 performs Wave Intensity Analysis which is an analysis method using co-related parts of the circulation system by measuring the intensity change (dl) of the blood vessel during a heart cycle. The intensity change is calculated as the product of the pressure derivate and the flow velocity derivate. In subsystem 2 the changes of pressure and flow are approximated by the deformation velocity or velocity of the radial and longitudinal direction, respectively. By calculating time constants and amplitudes of the intensity change graph a measure is obtained of the local and global elasticity of the blood vessel and atherosclerotic constrictions of arterial vessels may then be identified at an early stage.

    • Cultural ecosystem services (ES) are consistently recognized but not yet adequately defined or integrated within the ES framework. A substantial body of models, methods, and data relevant to cultural services has been developed within the social and behavioral sciences before and outside of the ES approach. A selective review of work in landscape aesthetics, cultural heritage, outdoor recreation, and spiritual significance demonstrates opportunities for operationally defining cultural services in terms of socioecological models, consistent with the larger set of ES. Such models explicitly link ecological structures and functions with cultural values and benefits, facilitating communication between scientists and stakeholders and enabling economic, multicriterion, deliberative evaluation and other methods that can clarify tradeoffs and synergies involving cultural ES. Based on this approach, a common representation is offered that frames cultural services, along with all ES, by the relative contribution of relevant ecological structures and functions and by applicable social evaluation approaches. This perspective provides a foundation for merging ecological and social science epistemologies to define and integrate cultural services better within the broader ES framework.

    • Cities are a key nexus of the relationship between people and nature and are huge centers of demand for ecosystem services and also generate extremely large environmental impacts. Current projections of rapid expansion of urban areas present fundamental challenges and also opportunities to design more livable, healthy and resilient cities (e.g. adaptation to climate change effects). We present the results of an analysis of benefits of ecosystem services in urban areas. Empirical analyses included estimates of monetary benefits from urban ecosystem services based on data from 25 urban areas in the USA, Canada, and China. Our results show that investing in ecological infrastructure in cities, and the ecological restoration and rehabilitation of ecosystems such as rivers, lakes, and woodlands occurring in urban areas, may not only be ecologically and socially desirable, but also quite often, economically advantageous, even based on the most traditional economic approaches.

    • The central challenge of the 21st century is to develop economic, social, and governance systems capable of ending poverty and achieving sustainable levels of population and consumption while securing the life-support systems underpinning current and future human well-being. Essential to meeting this challenge is the incorporation of natural capital and the ecosystem services it provides into decision-making. We explore progress and crucial gaps at this frontier, reflecting upon the 10 y since the Millennium Ecosystem Assessment. We focus on three key dimensions of progress and ongoing challenges: raising awareness of the interdependence of ecosystems and human well-being, advancing the fundamental interdisciplinary science of ecosystem services, and implementing this science in decisions to restore natural capital and use it sustainably. Awareness of human dependence on nature is at an all-time high, the science of ecosystem services is rapidly advancing, and talk of natural capital is now common from governments to corporate boardrooms. However, successful implementation is still in early stages. We explore why ecosystem service information has yet to fundamentally change decision-making and suggest a path forward that emphasizes: (i) developing solid evidence linking decisions to impacts on natural capital and ecosystem services, and then to human well-being; (ii) working closely with leaders in government, business, and civil society to develop the knowledge, tools, and practices necessary to integrate natural capital and ecosystem services into everyday decision-making; and (iii) reforming institutions to change policy and practices to better align private short-term goals with societal long-term goals.

  • Towards an integrated understanding of green space in the European built environment

    • In recent years social, economic and environmental considerations have led to a reevaluation of the factors that contribute to sustainable urban environments. Increasingly, urban green space is seen as an integral part of cities providing a range of services to both the people and the wildlife living in urban areas. With this recognition and resulting from the simultaneous provision of different services, there is a real need to identify a research framework in which to develop multidisciplinary and interdisciplinary research on urban green space. In order to address these needs, an iterative process based on the delphi technique was developed, which comprised email-mediated discussions and a two-day symposium involving experts from various disciplines. The two outputs of this iterative process were (i) an integrated framework for multidisciplinary and interdisciplinary research and (ii) a catalogue of key research questions in urban green space research. The integrated framework presented here includes relevant research areas (i.e. ecosystem services, drivers of change, pressures on urban green space, human processes and goals of provision of urban green space) and emergent research themes in urban green space studies (i.e. physicality, experience, valuation, management and governance). Collectively these two outputs have the potential to establish an international research agenda for urban green space, which can contribute to the better understanding of people's relationship with cities.

  • Quality of life in advanced cancer patients: the impact of sociodemographic and medical characteristics

    • Population-based surveys have shown that health-related quality of life (HRQL) is influenced by patients' characteristics such as age, gender, living situation and diagnoses. The present study explores the impact of such factors on the HRQL of severely ill cancer patients. The study sample included 395 cancer patients who participated in a cluster randomised trial of palliative care. Median survival was 13 weeks. HRQL assessments (using the EORTC QLQ-C30 questionnaire) were compared among subgroups of relevant patients' characteristics (ANOVA), and the significance of individual covariates was explored by multivariate linear regression. Most EORTC QLQ-C30 scores showed minor differences between genders. Higher age was associated with less sleeping disturbance, less pain and better emotional functioning. No positive impact of living with a partner was found. Performance status and/or time from assessment to death were significantly associated with most functioning and symptom scores. We concluded that although the overall impact of sociodemographic characteristics may seem less important to HRQL scores among advanced cancer patients than in general populations, age and gender should be allowed for. Performance status and closeness to death also need to be reported.

    • The Cardiotom is a mobile gamma camera that uses ectomography, an alternative method of acquisition to SPECT. It is designed for early diagnosis of myocardial and cerebral infarctions in the emergency room. Ectomography is a limited view angle method, using a rotating slant hole collimator and a stationary camera head, to acquire projection images. The aim of this work was to validate a fully 3D ML-EM iterative reconstruction algorithm for the Cardiotom. Validation measurements were performed with Tc-99m point sources. Resolution in the reconstructed volume was determined in X, Y. and Z directions from the point spread functions. The results were compared with the values for the formerly used filtered back projection (FBP). The new reconstruction algorithm provides greatly improved depth resolution with respect to the FBP method previously implemented on the Cardiotom. Furthermore, for clinical examinations, images can be available for interpretation within 15 min of the injection, therefore, valuable information can be obtained without delaying treatment of the patient.
