  • Electrostatic interactions of peptides flanking the tyrosine kinase domain in the epidermal growth factor receptor provides a model for intracellular dimerization and autophosphorylation
  • 2006
  • Ingår i: Proteins. - : Wiley. - 0887-3585 .- 1097-0134. ; 62:4, s. 1036-1043
  • Tidskriftsartikel (refereegranskat)abstract
    • The mechanism by which ligand-activated EGFR induces autophosphorylation via dimerization is not fully understood. Structural studies have revealed an extracellular loop mediated receptor dimerization. We have previously presented experimental data showing the involvement of a positive 13 amino acid peptide (R645-R657, P13+) from the intracellular juxtamembrane domain (JM) of EGFR important for intracellular dimerization and autophosphorylation. A model was presented that suggest that P13+ interacts with a negative peptide (D979-E991, P13-) positioned distal to the tyrosine kinase domain in the opposite EGFR monomer. The present work shows additional data strengthening this model. In fact, by analyzing protein sequences of 21 annotated ErbB proteins from 9 vertebrate genomes, we reveal the high conservation of peptides P13+ and P13- with regard to their sequence as well as their position relative to the tyrosine kinase (TK) domain. Moreover in silico structure modeling of these ErbB intracellular domains supports a general electrostatic P13+/P13- interaction, implying that the C-terminal of one receptor monomer is facing the TK domain of the other monomer in the receptor dimer and vice versa. This model provides new insights into the molecular mechanism of ErbB receptor activation and suggests a new strategy to pharmacologically interfering with ErbB receptor activity. © 2005 Wiley-Liss, Inc.

  • Aggregation-Induced Folding of a de novo Designed Polypeptide Immobilized on Gold Nanoparticles
  • 2006
  • Ingår i: Journal of the American Chemical Society. - : ACS Publications. - 0002-7863 .- 1520-5126. ; 128:7, s. 2194 -2195
  • Tidskriftsartikel (refereegranskat)abstract
    • This communication reports the first steps in the construction of a novel, nanoparticle-based hybrid material for biomimetic and biosensor applications. Gold nanoparticles were modified with synthetic polypeptides to enable control of the particle aggregation state in a switchable manner, and particle aggregation was, in turn, found to induce folding of the immobilized peptides.

  • 2008
  • Ingår i: Proceedings SPIE, Vol. 6885, Photonic Biosensing and Microoptics. - : SPIE. ; , s. 688506-1-688506-8
  • Konferensbidrag (refereegranskat)abstract
    • Heterodimerization between designed helix-loop-helix polypeptides was utilized in order to assemble gold nanoparticles on planar substrates. The peptides were designed to fold into four-helix bundles upon dimerization. A Cys-residue in the loop region was used to immobilize one of the complementary peptides on a maleimide containing SAM on planar gold substrates whereas the second peptide was immobilized directly on gold nanoparticles. Introducing the peptide decorated particles over a peptide functionalized surface resulted in particle assembly. Further, citrate stabilized particles were assembled on amino-silane modified glass and silicon substrates. By subsequently introducing peptides and gold nanoparticles, particle-peptide hybrid multi layers could be formed.

  • Folding Induced Assembly of Polypeptide Decorated Gold Nanoparticles
  • 2008
  • Ingår i: Journal of the American Chemical Society. - : American Chemical Society (ACS). - 0002-7863 .- 1520-5126. ; 130:17, s. 5780-5788
  • Tidskriftsartikel (refereegranskat)abstract
    • Reversible assembly of gold nanoparticles controlled by the homodimerization and folding of an immobilized de novo designed synthetic polypeptide is described. In solution at neutral pH, the polypeptide folds into a helix-loop-helix four-helix bundle in the presence of zinc ions. When immobilized on gold nanoparticles, the addition of zinc ions induces dimerization and folding between peptide monomers located on separate particles, resulting in rapid particle aggregation. The particles can be completely redispersed by removal of the zinc ions from the peptide upon addition of EDTA. Calcium ions, which do not induce folding in solution, have no effect on the stability of the peptide decorated particles. The contribution from folding on particle assembly was further determined utilizing a reference peptide with the same primary sequence but containing both D and L amino acids. Particles functionalized with the reference peptide do not aggregate, as the peptides are unable to fold. The two peptides, linked to the nanoparticle surface via a cysteine residue located in the loop region, form submonolayers on planar gold with comparable properties regarding surface density, orientation, and ability to interact with zinc ions. These results demonstrate that nanoparticle assembly can be induced, controlled, and to some extent tuned, by exploiting specific molecular interactions involved in polypeptide folding.

  • Assise: Performance and Availability via Client-local NVM in a Distributed File System
  • 2020
  • Konferensbidrag (refereegranskat)abstract
    • The adoption of low latency persistent memory modules (PMMs) upends the long-established model of remote storage for distributed file systems. Instead, by colocating computation with PMM storage, we can provide applications with much higher IO performance, sub-second application failover, and strong consistency. To demonstrate this, we built the Assise distributed file system, based on a persistent, replicated coherence protocol that manages client-local PMM as a linearizable and crash-recoverable cache between applications and slower (and possibly remote) storage. Assise maximizes locality for all file IO by carrying out IO on process-local, socket-local, and client-local PMM whenever possible. Assise minimizes coherence overhead by maintaining consistency at IO operation granularity, rather than at fixed block sizes. We compare Assise to Ceph/BlueStore, NFS, and Octopus on a cluster with Intel Optane DC PMMs and SSDs for common cloud applications and benchmarks, such as LevelDB, Postfix, and FileBench. We find that Assise improves write latency up to 22x, throughput up to 56x, fail-over time up to 103x, and scales up to 6x better than its counterparts, while providing stronger consistency semantics.

  • 1991
  • Ingår i: Social Approaches to Viking Studies. - 1873448007 ; , s. 53-64

    • The article demonstrates how the Swedish view of the Nordic Viking Age woman developed within a specific social situation at the end of the nineteenth century. The dominant gender ideology affected the formation of this image, which became a long-lived stereotype. Recently, its content partly merged with another image derived from completely different social and gender ideological contexts. A lack of conciousness of the different historical situations, of which the ideas are a part, has resulted in a contamination of gender ideologies.

  • Oral vancomycin treatment does not alter markers of postprandial inflammation in lean and obese subjects
  • 2019
  • Ingår i: Physiological Reports. - : Wiley. - 2051-817X. ; 7:16
  • Tidskriftsartikel (refereegranskat)abstract
    • Intake of a high-fat meal induces a systemic inflammatory response in the postprandial which is augmented in obese subjects. However, the underlying mechanisms of this response have not been fully elucidated. We aimed to assess the effect of gut microbiota modulation on postprandial inflammatory response in lean and obese subjects. Ten lean and ten obese subjects with metabolic syndrome received oral vancomycin 500 mg four times per day for 7 days. Oral high-fat meal tests (50 g fat/m(2) body surface area) were performed before and after vancomycin intervention. Gut microbiota composition, leukocyte counts, plasma lipopolysaccharides (LPS), LPS-binding protein (LBP), IL-6 and MCP-1 concentrations and monocyte CCR2 and cytokine expression were determined before and after the high-fat meal. Oral vancomycin treatment resulted in profound changes in gut microbiota composition and significantly decreased bacterial diversity in both groups (phylogenetic diversity pre- versus post-intervention: lean, 56.9 +/- 7.8 vs. 21.4 +/- 6.6, P < 0.001; obese, 53.9 +/- 7.8 vs. 21.0 +/- 5.9, P < 0.001). After intervention, fasting plasma LPS significantly increased (lean, median [IQR] 0.81 [0.63-1.45] EU/mL vs. 2.23 [1.33-3.83] EU/mL, P = 0.017; obese, median [IQR] 0.76 [0.45-1.03] EU/mL vs. 1.44 [1.11-4.24], P = 0.014). However, postprandial increases in leukocytes and plasma LPS were unaffected by vancomycin in both groups. Moreover, we found no changes in plasma LBP, IL-6 and MCP-1 or in monocyte CCR2 expression. Despite major vancomycin-induced disruption of the gut microbiota and increased fasting plasma LPS, the postprandial inflammatory phenotype in lean and obese subjects was unaffected in this study.

  • Pancreatic 18 F-FDG uptake is increased in type 2 diabetes patients compared to non-diabetic controls
  • 2019
  • Ingår i: Plos One. - : Public Library of Science (PLoS). - 1932-6203. ; 14:3
  • Tidskriftsartikel (refereegranskat)abstract
    • Introduction Increasing evidence indicates that the development of type 2 diabetes is driven by chronic low grade beta-cell inflammation. However, it is unclear whether pancreatic inflammation can be noninvasively visualized in type 2 diabetes patients. We aimed to assess pancreatic 18 F-FDG uptake in type 2 diabetes patients and controls using 18 F-fluorodeoxylglucose positron emission tomography/computed tomography ( 18 F-FDG PET/CT). Material and methods In this retrospective cross-sectional study, we enrolled 20 type 2 diabetes patients and 65 controls who had undergone a diagnostic 18 F-FDG PET/CT scan and obtained standardized uptake values (SUVs) of pancreas and muscle. Pancreatic SUV was adjusted for background uptake in muscle and for fasting blood glucose concentrations. Results The maximum pancreatic SUVs adjusted for background muscle uptake (SUV max.m ) and fasting blood glucose concentration (SUV glucose ) were significantly higher in diabetes patients compared to controls (median 2.86 [IQR 2.24–4.36] compared to 2.15 [IQR 1.51–2.83], p = 0.006 and median 2.76 [IQR 1.18–4.34] compared to 1.91 [IQR 1.27–2.55], p<0.001, respectively). In linear regression adjusting for age and body mass index, diabetes remained the main predictor of SUV max.m and SUV glucose . Conclusion Pancreatic 18 F-FDG uptake adjusted for background muscle uptake and fasting blood glucose concentration was significantly increased in type 2 diabetes patients. © 2019 Bakker et al. • Hybrid video quality prediction : Re-viewing video quality measurement for widening application scope
  • 2015
  • Ingår i: Multimedia tools and applications. - : Springer Science and Business Media LLC. - 1380-7501 .- 1573-7721. ; 74:2, s. 323-343
  • Tidskriftsartikel (refereegranskat)abstract
    • A tremendous number of objective video quality measurement algorithms have been developed during the last two decades. Most of them either measure a very limited aspect of the perceived video quality or they measure broad ranges of quality with limited prediction accuracy. This paper lists several perceptual artifacts that may be computationally measured in an isolated algorithm and some of the modeling approaches that have been proposed to predict the resulting quality from those algorithms. These algorithms usually have a very limited application scope but have been verified carefully. The paper continues with a review of some standardized and well-known video quality measurement algorithms that are meant for a wide range of applications, thus have a larger scope. Their individual artifacts prediction accuracy is usually lower but some of them were validated to perform sufficiently well for standardization. Several difficulties and shortcomings in developing a general purpose model with high prediction performance are identified such as a common objective quality scale or the behavior of individual indicators when confronted with stimuli that are out of their prediction scope. The paper concludes with a systematic framework approach to tackle the development of a hybrid video quality measurement in a joint research collaboration.

  • Solving Satisfaction Problems using Large-Neighbourhood Search
  • 2020
  • Ingår i: Principles and Practice of Constraint Programming. - Cham : Springer International Publishing. - 9783030584740 - 9783030584757 ; , s. 55-71
  • Konferensbidrag (refereegranskat)abstract
    • Large-neighbourhood search (LNS) improves an initial solution, hence it is not directly applicable to satisfaction problems. In order to use LNS in a constraint programming (CP) framework to solve satisfaction problems, we usually soften some hard-to-satisfy constraints by replacing them with penalty-function constraints. LNS is then used to reduce their penalty to zero, thus satisfying the original problem. However, this can give poor performance as the penalties rarely cause propagation and therefore do not drive each CP search, and by extension the LNS search, towards satisfying the replaced constraints until very late. Our key observation is that entirely replacing a constraint is often overkill, as the propagator for the replaced constraint could have performed some propagation without causing backtracking. We propose the notion of a non-failing propagator, which is subsumed just before causing a backtrack. We show that, by only making a few changes to an existing CP solver, any propagator can be made non-failing without modifying its code. Experimental evaluation shows that non-failing propagators, when used in conjunction with penalties, can greatly improve LNS performance compared to just having penalties. This allows us to leverage the power of the many sophisticated propagators that already exist in CP solvers, in order to use LNS for solving hard satisfaction problems and for finding initial solutions to hard-to-satisfy optimisation problems.

  • Udinese Calcio soccer club as a talents factory : Strategic agility, diverging objectives, and resource constraints
  • 2014
  • Ingår i: European Management Journal. - : Elsevier. - 0263-2373. ; 32:2, s. 319-336
  • Tidskriftsartikel (refereegranskat)abstract
    • Strategic agility can be defined as a firm-level ability to continuously adjust and adapt decisions to the changing circumstances of the external environment and thus nurture value creation (DozXX1Kosonen, 2008a). At its very heart, agility allows integrating diverging strategic objectives, i.e. focus and adaptability, commitment and flexibility. This paper aims at providing new empirical evidence that supports the strategic agility model and focuses on a case which fits the various dimensions described by Doz and Kosonen. Specifically, our study refers to the context of soccer, where professional teams struggle to balance divergent strategic objectives, i.e. to achieve both good sports and financial performances. We argue indeed that these objectives can be successfully balanced over time through strategic agility. The case we use to support this hypothesis is that of Udinese Calcio S.p.A., an Italian soccer team which in the last twenty years has adopted a business model based on strong investments and quick turnaround of young and promising players, which has turned it into a factory of talents. Udinese Calcio has been able to do so keeping its books in order and at the same time managing to obtain excellent results in a very competitive national soccer tournament such as the Italian Serie A. Udinese is a small organization based in a medium-sized city in the North-East of Italy, an area which is indeed peripheral from a geopolitical and business perspective. The team can count on limited resources, it is not owned by wealthy individuals with low spending constraints and an emotional attitude, and it is a quite unique case for both its sports and financial performances. These results have been achieved, despite the unique business model which relies on young talents more than on well-known, experienced players, through excellent management practices that have made Udinese Calcio a strategically agile organization. We argue that the case of Udinese Calcio strengthens the validity of the strategic agility concept and can be extended to other situations where the need to strike a balance between divergent objectives with limited resources is quintessential. Examples are to be found outside soccer or sport clubs, in contexts such as health management, education, political lobbying. © 2013 Elsevier Ltd.

  • Model-Checking-based vs. SMT-based Consistency Analysis of Industrial Embedded Systems Requirements : Application and Experience
  • 2018
  • Ingår i: Electronic Communications of the EASST. - Germany. - 1863-2122. ; 75, s. 1-20
  • Tidskriftsartikel (refereegranskat)abstract
    • Industry relies predominantly on manual peer-review techniques for assessing the correctness of system specifications. However, with the ever-increasing size, complexity and intricacy of specifications, it becomes difficult to assure their correctness with respect to certain criteria such as consistency. To address this challenge, a technique called sanity checking has been proposed. The goal of the technique is to assess the quality of the system specification in a systematic and rigorous manner with respect to a formally-defined criterion. Predominantly, the sanity checking criteria, such as for instance consistency, are encoded as reachability or liveness properties which can then be verified via model checking. Recently, a complementary approach for checking the consistency of a system's specification by reducing it to a satisfiability problem that can be analyzed using Satisfiability Modulo Theories has been proposed. In this paper, we compare the two approaches for consistency analysis, by applying them on a relevant industrial use case, using the same definition for consistency and the same set of requirements. Since the bottlenecks of analyzing large systems formally are most often the construction of the model and the time needed to return a verdict, we carry out the comparison with respect to the: i) required effort for generating the analysis model and the latter's complexity, and ii) consistency analysis time. Assuming checking only invariance properties, our results show no significant difference in analysis time between the two approaches when applied on the same system specification under the same definition of consistency. As expected, the main difference between the two comes from the required time and effort of creating the analysis models.

  • Pattern-based Specification and Formal Analysis of Embedded Systems Requirements and Behavioral Models
  • 2017
  • Licentiatavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • Since the first lines of code were introduced in the automotive domain, vehicles have transitioned from being predominantly mechanical systems to software intensive systems. With the ever-increasing computational power and memory of vehicular embedded systems, a set of new, more powerful and more complex software functions are installed into vehicles to realize core functionalities. This trend impacts all phases of the system development including requirements specification, design and architecture of the system, as well as the integration and testing phases. In such settings, creating and managing different artifacts during the system development process by using traditional, human-intensive techniques becomes increasingly difficult. One problem stems from the high number and intricacy of system requirements that combine functional and possibly timing or other types of constraints. Another problem is related to the fact that industrial development relies on models, e.g. developed in Simulink, from which code may be generated, so the correctness of such models needs to be ensured. A potential way to address of the mentioned problems is by applying computer-aided specification, analysis and verification techniques already at the requirements stage, but also further at later development stages. Despite the high degree of automation, exhaustiveness and rigor of formal specification and analysis techniques, their integration with industrial practice remains a challenge. To address this challenge, in this thesis, we develop the foundation of a framework, tailored for industrial adoption, for formal specification and analysis of system requirements specifications and behavioral system models. First, we study the expressiveness of existing pattern-based techniques for creating formal requirements specifications, on a relevant industrial case study. Next, in order to enable practitioners to create formal system specification by using pattern-based techniques, we propose a tool called SeSAMM Specifier. Further, we provide an automated Satisfiability Modulo Theories (SMT)-based consistency analysis approach for the formally encoded system requirements specifications. The proposed SMT-based approach is suitable for early phases of the development for debugging the specifications. For the formal analysis of behavioral models, we provide an approach for statistical model checking of Simulink models by using the UPPAAL SMC tool. To facilitate the adoption of the approach, we provide the SIMPPAAL tool that automates procedure of generating network of stochastic timed automata for a given Simulink model. For validation, we apply our approach on a complex industrial model, namely the Brake-by-Wire function from Volvo GTT.

    • The advanced technology used for developing modern automotive systems increases their complexity, making their correctness assurance very tedious. To enable analysis, but also enhance understanding and communication, by simulation, engineers use MATLAB/Simulink modeling during system development. In this paper, we provide further analysis means to industrial Simulink models by proposing a pattern-based, execution-order preserving transformation of Simulink blocks into the input language of UPPAAL Statistical Model checker, that is, timed (hybrid) automata with stochastic semantics. The approach leads to being able to analyze complex Simulink models of automotive systems, and we report our experience with two vehicular systems, the Brake-by-Wire and the Adjustable Speed Limiter.

    • As the complexity of industrial systems increases, it becomes dificult to ensure the correctness of system requirements specifications with respect to certain criteria such as consistency. Automated techniques for consistency checking of requirements, mostly by means of model checking, have been proposed in academia. However, such approaches can some-times be costly in terms of modeling and analysis time or not applicable for certain types of properties. In this paper, we present a complementary method that relies on pattern-based formalization of requirements and automated consistency checking using the state-of-the-art SMT tool Z3. For validation, we apply our method on a set of timed computation tree logic requirements of an industrial automotive subsystem called the Fuel Level Display.

  • Konferensbidrag (refereegranskat)abstract
    • With the ever-increasing size and complexity of the industrial software systems there is an imperative need for an automated, systematic and exhaustive verification of various software artifacts, such as system specifications, models, code, etc. A potential remedy for this need might lie in a pool of techniques for computer-aided verification of software related artifacts, including system specifications. The Achilles' heel of these techniques, and the main hinder for their wider adoption in industrial development process are the complexity and the specialized skill-set required for the formal encoding of specifications. To alleviate this problem, Specification Patterns that are based on the observation that the system specifications are framed within reoccurring solutions have been proposed. The approach has been shown to be expressive enough for capturing requirements in the automotive domain, however, there is a lack of empirical data that can be used to judge its practical usefulness. In this paper, we involve an existing specification-patterns-based tool, and propose a small-size evaluation of the approach with practitioners, on a case study conducted in cooperation with Scania, one of the world's leading manufacturers of heavy-load vehicles. Our results show that the specification patterns that are supported by an adequate tooling have the potential for adoption in industrial practice.

    • The progress in translational cancer research relies on access to well-characterized samples from a representative number of patients and controls. The rationale behind our biobanking are explorations of post-zygotic pathogenic gene variants, especially in non-tumoral tissue, which might predispose to cancers. The targeted diagnoses are carcinomas of the breast (via mastectomy or breast conserving surgery), colon and rectum, prostate, and urinary bladder (via cystectomy or transurethral resection), exocrine pancreatic carcinoma as well as metastases of colorectal cancer to the liver. The choice was based on the high incidence of these cancers and/or frequent fatal outcome. We also collect age-matched normal controls. Our still ongoing collection originates from five clinical centers and after nearly 2-year cooperation reached 1711 patients and controls, yielding a total of 23226 independent samples, with an average of 74 donors and 1010 samples collected per month. The predominant diagnosis is breast carcinoma, with 933 donors, followed by colorectal carcinoma (383 donors), prostate carcinoma (221 donors), bladder carcinoma (81 donors), exocrine pancreatic carcinoma (15 donors) and metachronous colorectal cancer metastases to liver (14 donors). Forty percent of the total sample count originates from macroscopically healthy cancer-neighboring tissue, while contribution from tumors is 12%, which adds to the uniqueness of our collection for cancer predisposition studies. Moreover, we developed two program packages, enabling registration of patients, clinical data and samples at the participating hospitals as well as the central system of sample/data management at coordinating center. The approach used by us may serve as a model for dispersed biobanking from multiple satellite hospitals. Our biobanking resource ought to stimulate research into genetic mechanisms underlying the development of common cancers. It will allow all available "-omics" approaches on DNA-, RNA-, protein- and tissue levels to be applied. The collected samples can be made available to other research groups.

  • Off-diagonal generalization of the mixed-state geometric phase
  • 2003
  • Ingår i: Physical Review A. Atomic, Molecular, and Optical Physics. - 1050-2947 .- 1094-1622. ; 68:4, s. 042112-
  • Tidskriftsartikel (refereegranskat)abstract
    • The concept of off-diagonal geometric phases for mixed quantal states in unitary evolution is developed. We show that these phases arise from three basic ideas: (1) fulfillment of quantum parallel transport of a complete basis, (2) a concept of mixed state orthogonality adapted to unitary evolution, and (3) a normalization condition. We provide a method for computing the off-diagonal mixed state phases to any order for unitarities that divide the parallel transported basis of Hilbert space into two parts: one part where each basis vector undergoes cyclic evolution and one part where all basis vectors are permuted among each other. We also demonstrate a purification based experimental procedure for the two lowest order mixed state phases and consider a physical scenario for a full characterization of the qubit mixed state geometric phases in terms of polarization-entangled photon pairs. An alternative second order off-diagonal mixed state geometric phase, which can be tested in single-particle experiments, is proposed.

    • We extend the off-diagonal geometric phase [Phys. Rev. Lett. 85, 3067 (2000)] to mixed quantal states. The nodal structure of this phase in the qubit (two-level) case is compared with that of the diagonal mixed state geometric phase [Phys. Rev. Lett. 85, 2845 (2000)]. Extension to higher dimensional Hilbert spaces is delineated. A physical scenario for the off-diagonal mixed state geometric phase in polarization-entangled two-photon interferometry is proposed.

    • Uhlmann's concept of quantum holonomy for paths of density operators is generalised to the off-diagonal case providing insight into the geometry of state space when the Uhlmann holonomy is undefined. Comparison with previous off-diagonal geometric phase definitions is carried out and an example comprising the transport of a Bell-state mixture is given.

  • Global Differences in Characteristics, Precipitants, and Initial Management of Patients Presenting With Acute Heart Failure
  • 2020
  • Ingår i: JAMA cardiology. - : AMER MEDICAL ASSOC. - 2380-6583 .- 2380-6591. ; 5:4, s. 401-410
  • Tidskriftsartikel (refereegranskat)abstract
    • Question What are the key similarities and differences in characteristics, comorbidities, therapies, and in-hospital outcomes in patients with chronic and acute heart failure? Findings In this cohort study including 18& x202f;553 patients from 7 global regions, there were similarities in many regions regarding prevalence of prior heart failure, ejection fraction, and comorbidities. However, there were key differences in outpatient treatment, hospital point of entry, acute heart failure precipitants, and timing and type of inpatient intravenous therapies. Meaning These data provide information on the current global burden of acute heart failure, identify region-specific gaps in management, and note differences in practice around the world associated with patient outcomes. This cohort study compares the characteristics and management of acute heart failure in global regions comprising 44 countries. Importance Acute heart failure (AHF) precipitates millions of hospital admissions worldwide, but previous registries have been country or region specific. Objective To conduct a prospective contemporaneous comparison of AHF presentations, etiologic factors and precipitants, treatments, and in-hospital outcomes among global regions through the International Registry to Assess Medical Practice with Longitudinal Observation for Treatment of Heart Failure (REPORT-HF). Design, Setting, and Participants A total of 18& x202f;553 adults were enrolled during a hospitalization for AHF. Patients were recruited from the acute setting in Western Europe (WE), Eastern Europe (EE), Eastern Mediterranean and Africa (EMA), Southeast Asia (SEA), Western Pacific (WP), North America (NA), and Central and South America (CSA). Patients with AHF were approached for consent and excluded only if there was recent participation in a clinical trial. Patients were enrolled from July 23, 2014, to March 24, 2017. Statistical analysis was conducted from April 18 to June 29, 2018; revised analyses occurred between August 6 and 29, 2019. Main Outcomes and Measures Heart failure etiologic factors and precipitants, treatments, and in-hospital outcomes among global regions. Results A total of 18& x202f;553 patients were enrolled at 358 sites in 44 countries. The median age was 67.0 years (interquartile range [IQR], 57-77), 11& x202f;372 were men (61.3%), 9656 were white (52.0%), 5738 were Asian (30.9%), and 867 were black (4.7%). A history of HF was present in more than 50% of the patients and 40% were known to have a prior left-ventricular ejection fraction lower than 40%. Ischemia was a common AHF precipitant in SEA (596 of 2329 [25.6%]), WP (572 of 3354 [17.1%]), and EMA (364 of 2241 [16.2%]), whereas nonadherence to diet and medications was most common in NA (306 of 1592 [19.2%]). Median time to the first intravenous therapy was 3.0 (IQR, 1.4-5.6) hours in NA; no other region had a median time above 1.2 hours (P < .001). This treatment delay remained after adjusting for severity of illness (P < .001). Intravenous loop diuretics were the most common medication administered in the first 6 hours of AHF management across all regions (65.4%-89.9%). Despite similar initial blood pressure across all regions, inotropic agents were used approximately 3 times more often in SEA, WP, and EE (11.3%-13.5%) compared with NA and WE (3.1%-4.3%) (P < .001). Older age (odds ratio [OR], 1.0; 95% CI, 1.00-1.02), HF etiology (ischemia: OR, 1.65; 95% CI, 1.11-2.44; valvular: OR, 2.10; 95% CI, 1.36-3.25), creatinine level greater than 2.75 mg/dL (OR, 1.85; 95% CI, 0.71-2.40), and chest radiograph signs of congestion (OR, 2.03; 95% CI, 1.39-2.97) were all associated with increased in-hospital mortality. Similarly, younger age (OR, -0.04; 95% CI, -0.05 to -0.02), HF etiology (ischemia: OR, 0.77; 95% CI, 0.26-1.29; valvular: OR, 2.01; 95% CI, 1.38-2.65), creatinine level greater than 2.75 mg/dL (OR, 1.16; 95% CI, 0.31-2.00), and chest radiograph signs of congestion (OR, 1.02; 95% CI, 0.57-1.47) were all associated with increased in-hospital LOS. Conclusions and Relevance Data from REPORT-HF suggest that patients are similar across regions in many respects, but important differences in timing and type of treatment exist, identifying region-specific gaps in medical management that may be associated with patient outcomes.

  • Intravenous ferric carboxymaltose in iron-deficient chronic heart failure patients with and without anaemia: a subanalysis of the FAIR-HF trial
  • 2013
  • Ingår i: European Journal of Heart Failure. - : Wiley. - 1879-0844 .- 1388-9842. ; 15:11, s. 1267-1276
  • Tidskriftsartikel (refereegranskat)abstract
    • Therapy with i.v. iron in patients with chronic heart failure (CHF) and iron deficiency (ID) improves symptoms, functional capacity, and quality of life. We sought to investigate whether these beneficial outcomes are independent of anaemia. FAIR-HF randomized 459 patients with CHF [NYHA class II or III, LVEF 40 (NYHA II) or 45 (NYHA III)] and ID to i.v. iron as ferric carboxymaltose (FCM) or placebo in a 2:1 ratio. We analysed the efficacy and safety according to the presence or absence of anaemia (haemoglobin 120 g/L) at baseline. Of 459 patients, 232 had anaemia at baseline (51). The effect of FCM on the primary endpoints of self-reported Patient Global Assessment (PGA) and NYHA class at week 24 was similar in patients with and without anaemia [odds ratio (OR) for improvement, 2.48 vs. 2.60, P 0.97 for PGA and 1.90 vs. 3.39, P 0.51 for NYHA). Results were also similar for the secondary endpoints, including PGA and NYHA at weeks 4 and 12, 6 min walk test distance, Kansas City Cardiomyopathy Questionnaire overall score, and European Quality of Life-5 Dimensions Visual Analogue Scale at most time points. Regarding safety, no differences were noticed in the rates of death or first hospitalization between FCM and placebo both in anaemic and in non-anaemic patients. Treatment of ID with FCM in patients with CHF is equally efficacious and shows a similar favourable safety profile irrespective of anaemia. Iron status should be assessed in symptomatic CHF patients both with and without anaemia and treatment of ID should be considered.

  • Konferensbidrag (refereegranskat)abstract
    • Nuclear magnetic resonance spectroscopy and rheological analysis have been used to investigate the stability of mechanically induced tri-ethylene-glycol-mono-n-decyl-ether (C10E3)/deuterium oxide (D2O) multi-lamellar vesicles (MLVs) and the transition from MLVs to planar lamellae. It was found that MLVs prepared by vortex stirring, relax back to the lamellar phase in a few hours while the relaxation of the shear induced MLVs takes days. Pulsed gradient spin echo and water self-diffusion coefficient experiments, revealed that the MLVs texture, obtained by vortex stirring, is composed of large size structures. These data indicate that the kinetics of lamellar re-formation depend on the MLVs number density.

  • Developmental Changes in the Association Between Cognitive Control and Anxiety
  • 2022
  • Ingår i: Child Psychiatry and Human Development. - : Springer Nature. - 0009-398X .- 1573-3327. ; 53:3, s. 599-609
  • Tidskriftsartikel (refereegranskat)abstract
    • Anxiety has been associated with reliance on reactive (stimulus-driven/reflexive) control strategies in response to conflict. However, this conclusion rests primarily on indirect evidence. Few studies utilize tasks that dissociate the use of reactive ('just in time') vs. proactive (anticipatory/preparatory) cognitive control strategies in response to conflict, and none examine children diagnosed with anxiety. The current study utilizes the AX-CPT, which dissociates these two types of cognitive control, to examine cognitive control in youth (ages 8-18) with and without an anxiety diagnosis (n = 56). Results illustrate that planful behavior, consistent with using a proactive strategy, varies by both age and anxiety symptoms. Young children (ages 8-12 years) with high anxiety exhibit significantly less planful behavior than similarly-aged children with low anxiety. These findings highlight the importance of considering how maturation influences relations between anxiety and performance on cognitive-control tasks and have implications for understanding the pathophysiology of anxiety in children.
