  • Validation of a laboratory method of measuring postpartum blood loss

    • Laboratory methods give more accurate measurement of blood loss in the postpartum period than visual estimation. In order to evaluate a laboratory method used to quantify blood loss postpartum, blood lost at gynecological operations was collected in a measuring bottle. The measured amount of blood (50–1,000 ml) was then poured onto absorbent paper towels and sanitary pads, in order to mimic conditions when measuring blood loss in clinical trials in the postpartum period. The amount of blood absorbed onto the absorbent paper and sanitary pads was measured by a rapid method of automatic extraction and photometric measurement of alkaline hematin. The study shows that the method provides a reliable and accurate means of measuring blood loss. The error in each case was less than 10% with an intraclass correlation coefficient of almost 1.

  • Microstructural degradation of plain and platinum aluminide coatings on superalloy CM247 during isothermal oxidation

    • Isothermal oxidation at 1100°C of a high activity plain aluminide coating and a platinum aluminide coating, developed by the pack cementation technique, on cast nickel base superalloy CM247 has been carried out with the primary objective of systematically understanding the coating degradation process during oxidation. While the weight gains during oxidation for both plain aluminide and platinum aluminide coatings follow parabolic kinetics from the very beginning of oxidation exposure, the bare alloy was seen to exhibit a considerably long initial transient oxidation period (∼20 h), beyond which the parabolic law was followed. The parabolic rate constant for the platinum aluminide coating was found to be nearly two orders of magnitude lower than that for the plain aluminide coating. Alumina was identified as the only oxide phase that formed on both plain aluminide and platinum aluminide coatings during most of the oxidation exposure, although NiAl2O4 was also found in the case of the plain aluminide coating beyond ∼200 h. The oxide layer on the bare alloy, however, was found to consist of Al2O3, Cr2O3, and NiAl2O4. The microstructural degradation of both the plain aluminide and platinum aluminide coatings during oxidation was seen to occur in three distinct stages which, however, differed for each coating. This stagewise degradation, which involves final obliteration of the interdiffusion layer in each case, is discussed in detail. © 1999 IoM Communications Ltd.

  • A Practical Example Using VR in the Assessment of Brain Injury

    • Virtual Reality (VR) as a complementary tool for medical practitioners in the assessment and rehabilitation of people who have suffered a traumatic brain injury (TBI) is discussed. A pilot-study has been undertaken on a prototype VR assessment tool. The design involved nine occupational therapists with expertise in the care of traumatic brain injured patients and one (computer experienced) patient. The aim was to begin a dialogue and to ascertain the potential of a VR system. A common method for occupational therapists to assess function and ability is to ask a patient to brew coffee. From the performance of such a task, an individual’s “functional signature” can be determined. The prototype was built using Superscape, a personal computer based VR system, to be close to the real coffee making task, including effects of making mistakes, realistic graphics and sound effects. The world was designed to be as easy to use and intuitive as possible, though problems of mental abstraction level, transfer of training and realistic interaction have yet to be resolved. The comments from the test participants have highlighted problem areas, given positive insight and pointed out other scenarios where VR may be of use in the rehabilitation of people with a traumatic brain injury.

    • This paper deals with a new framework in analyzing the formal mathematical correspondence between quantum mechanics and time-frequency representations of a signal. It is also shown that joint time-frequency distributions have a close link with Heisenberg uncertainty relations if the observables are taken as fuzzy entities. This result contradicts the arguments of Cohen [IEEE Proc. 77(7):941 (1989)] regarding the time-frequency distributions and the uncertainty relation. It is postulated that these mechanisms will be of crucial importance in highly fragmented computation structures, such as neural networks, as they may exhibit a strong mutual interaction between data and operator.

    • Earlier in vitro studies suggest opposing roles of laminin and fibronectin in regulation of differentiated properties of vascular smooth muscle cells. To find out if this may also be the case in vivo, we used immunoelectron microscopy to study the distribution of these proteins during formation of intimal thickening after arterial injury. In parallel, cell structure and content of smooth muscle α-actin was analyzed. The results indicate that the cells in the normal media are in a contractile phenotype with abundant α-actin filaments and an incomplete basement membrane. Within 1 week after endothelial denudation, most cells in the innermost layer of the media convert into a synthetic phenotype, as judged by loss of actin filaments, construction of a large secretory apparatus, and destruction of the basement membrane. Some of these cells migrate through fenestrae in the internal elastic lamina and invade a fibronectin-rich network deposited on its luminal surface. Within another few weeks a thick neointima forms, newly produced matrix components replace the strands of fibronectin, and a basement membrane reappears. Simultaneously, the cells resume a contractile phenotype, recognized by disappearance of secretory organelles and restoration of α-actin filaments. These findings support the notion that laminin and other basement membrane components promote the expression of a differentiated smooth muscle phenotype, whereas fibronectin stimulates the cells to adopt a proliferative and secretory phenotype.
