    • A laser photoacoustic spectroscopy (LPAS) instrument was developed and used for aircraft measurements of ethene from industrial sources near Houston, Texas. The instrument provided 20 s measurements with a detection limit of less than 0.7 ppbv. Data from this instrument and from the GC-FID analysis of air samples collected in flight agreed within 15% on average. Ethene fluxes from the Mt. Belvieu chemical complex to the northeast of Houston were quantified during 10 different flights. The average flux was 520 +/- 140 kg h(-1) in agreement with independent results from solar occultation flux (SOF) measurements, and roughly an order of magnitude higher than regulatory emission inventories indicate. This study shows that ethene emissions are routinely at levels that qualify as emission upsets, which need to be reported to regional air quality managers.

  • A national stroke quality register : 12 years experience from a participating hospital

    • Registration of all hospitalized stroke patients is practiced in Sweden in order to assess care quality. Data in this register, Riks-Stroke (RS), may be biased due to incomplete registration. The purpose of this paper was to report changes in stroke outcome in relation to fluctuations in registration. Patients registered in RS at a hospital during the period 1994-2005 were analyzed. Case fatality at 28 days, living conditions, and activities of daily living (ADL) performance at 3 months were correlated to the number of patients registered and follow-up frequency. A total of 4994 stroke cases were registered during the period. A high annual registration rate was significantly correlated to a high case fatality ratio. A low annual follow-up rate was associated with a low proportion of patients living in their own home without any need of help. Quality parameters are sensible for selection bias, which make them difficult to compare over time and between hospitals. We suggest that by weighing outcome data against stroke severity, safer conclusions may be drawn. Additionally, hospitals considering setting up quality registers should make every effort to attain complete case ascertainment at all times, including patients managed outside the hospital, in order to avoid selection bias.

  • Quantum pump driven fermionic Mach-Zehnder interferometer

    • We have investigated the characteristics of the currents in a pump driven fermionic Mach-Zehnder interferometer. The system is implemented in a conductor in the quantum Hall regime, with the two interferometer arms enclosing an Aharonov-Bohm flux Phi. Two quantum point contacts with transparency modulated periodically in time drive the current and act as beam splitters. The current has a flux-dependent part I-(Phi) as well as a flux-independent part I-(0). Both current parts show oscillations as a function of frequency on the two scales determined by the lengths of the interferometer arms. In the nonadiabatic, high-frequency regime I-(Phi) oscillates with a constant amplitude while the amplitude of the oscillations of I-(0) increases linearly with frequency. The flux-independent part I-(0) is insensitive to temperature while the flux-dependent part I-(Phi) is exponentially suppressed with increasing temperature. We also find that for low amplitude, adiabatic pumping rectification effects are absent for semitransparent beam splitters. Inelastic dephasing is introduced by coupling one of the interferometer arms to a voltage probe. For a long charge relaxation time of the voltage probe, giving a constant probe potential, I-(Phi) and the part of I-(0) flowing in the arm connected to the probe are suppressed with increased coupling to the probe. For a short relaxation time, with the potential of the probe adjusting instantaneously to give zero time-dependent current at the probe, only I-(Phi) is suppressed by the coupling to the probe.

    • Moderation of the level of genetic influence on children’s high reading ability by environmental influences associated with parental education was explored in two independent samples of identical and fraternal twins from the United States and Great Britain. For both samples, the heritability of high reading performance increased significantly with lower levels of parental education. Thus, resilience (high reading ability despite lower environmental support) is more strongly influenced by genotype than is high reading ability with higher environmental support. This result provides a coherent account when considered alongside results of previous research showing that heritability for low reading ability decreased with lower levels of parental education.

    • A susceptibility locus for psoriasis, PSORS4, has been mapped to chromosome 1q21 in the region of the epidermal differentiation complex. The region has been refined to a 115 kb interval around the loricrin (LOR) gene. However, no evidence of association between polymorphisms in the LOR gene and psoriasis has been found. Therefore, we have analysed association to three candidate gene clusters of the region, the S100, small proline-rich protein (SPRR) and PGLYRP (peptidoglycan recognition protein) genes, which all contain functionally interesting psoriasis candidate genes. In previous studies, the SPRR and S100 genes have shown altered expression in psoriasis. Also polymorphisms in the PGLYRP genes have shown to be associated with psoriasis. We genotyped altogether 29 single nucleotide polymorphisms (SNPs) in 255 Finnish psoriasis families and analysed association with psoriasis using transmission disequilibrium test. A five-SNP haplotype of PGLYRP SNPs associated significantly with psoriasis. There was also suggestive evidence of association to SPRR gene locus in Finnish families. To confirm the putative associations, selected SNPs were genotyped also in a family collection of Swedish and Irish patients. The families supported association to the two gene regions, but there was also evidence of allelic heterogeneity.

    • Seven patients underwent treatment with mesenchymal stem cells (MSCs), together with allogeneic hematopoietic stem cell transplantation (HSCT). MSCs were given to three patients for graft failure and four patients were included in a pilot study. HSCT donors were three human leukocyte antigen (HLA)-identical siblings, three unrelated donors and one cord blood unit. The conditioning was myeloablative in four patients and reduced in three patients. MSC donors were HLA-identical siblings in three cases and haploidentical in four cases. Neutrophil counts >0.5 ×109/l was reached at a median of 12 (range 10-28) days. Platelet counts >30 x 109/l was achieved at a median of 12 (8-36) days. Acute graft-versus-host disease (GVHD) grade 0-l was seen in five patients. Two patients developed grade II, which in one patient evolved into chronic GVHD. One severe combined immunodeficiency (SCID) patient died of aspergillosis, the others are alive and well. One patient, diagnosed with aplastic anemia had graft failure after her first transplantation and severe Henoch-Schönlein Purpura (HSP). After retransplantation of MSCs and HSCs, she recovered from both the HSP and aplasia. Thus, co-transplantation of MSC resulted in fast engraftment of absolute neutrophil count (ANC) and platelets and 100% donor chimerism, even in three patients regrafted for graft failure/rejection.

    • Myelodysplastic syndromes (MDSs) are a group of hematopoietic stem cell disorders characterized by ineffective hernatopoiesis and peripheral blood cytopenias. Lenalidomide has dramatic therapeutic effects in patients with low-risk MDS and a chromosome 5q31 deletion, resulting in complete cytogenetic remission in >60% of patients. The molecular basis of this remarkable drug response is unknown. To gain insight into the molecular targets of lenaliclomide we investigated its in vitro effects on growth, maturation, and global gene expression in isolated erythroblast cultures from MDS patients with clel(5)(q3l). Lenalidomide inhibited growth of differentiating del(5q) erythroblasts but did not affect cytogenetically normal cells. Moreover, lenalidomide significantly influenced the pattern of gene expression in del(5q) intermediate erythroblasts, with the V51G4, PPIC, TPBG, activin A, and SPARC genes up-regulated by >2-fold in all samples and many genes involved in erythropoiesis, including HBA2, GYPA, and KLF1, down-regulated in most samples. Activin A, one of the most significant differentially expressed genes between lenalidomide-treated cells from MDS patients and healthy controls, has pleiotropic functions, including apoptosis of hematopoietic cells. Up-regulation and increased protein expression of the tumor suppressor gene SPARC is of particular interest because it is anti proliferative, antiadhesive, and antiangiogenic and is located at 5q3l-q32, within the commonly deleted region in MDS 5q- syndrome. We conclude that lenalidomide inhibits growth of del(5q) erythroid progenitors and that the up-regulation of SPARC and activin A may underlie the potent effects of lenalidomide in MDS with clel(5)(q3l). SPARCmay play a role in the pathogenesis of the 5q- syndrome.

    • The aim of the study was to investigate how starting to use dialectical behavioural therapy (DBT) in the work with young self-harming women showing symptoms of borderline personality disorder affected the psychiatric professionals (n = 22) experience of occupational stress and levels of professional burnout. The study was carried out in relation to an 18-month clinical psychiatric development project, and used a mix of quantitative and qualitative research methods [a burnout inventory, the Maslach burnout inventory-General Survey (MBI-GS), free format questionnaires and group interviews]. The result confirms previous reports that psychiatric health professionals experience treatment of self-harming patients as very stressful. DBT was seen as stressful in terms of learning demands, but decreased the experience of stress in the actual treatment of the patients. The teamwork and supervision were felt to be supportive, as was one particular facet of DBT, namely mindfulness training which some therapists felt also improved their handling of other work stressors not related to DBT. The inventory for professional burnout, the MBI-GS, showed no significant changes over the 18-month period, although there was a tendency for increased burnout levels at the 6-month assessment, which had returned to baseline levels at 18 months.

    • We present a stochastic path integral method to calculate the full-counting statistics of conductors with energy conserving dephasing probes and dissipative voltage probes. The approach is explained for the experimentally important case of a Mach-Zehnder interferometer, but is easily generalized to more complicated setups. For all geometries where dephasing may be modeled by a single one-channel dephasing probe we prove that our method yields the same full-counting statistics as phase averaging of the cumulant generating function.

    • The aim of ADINE project is to develop and to demonstrate new methods for the electric distribution network management including distributed generation (DG). The active network management (ANM) method needs enabling solutions such as protection, voltage and reactive power control and planning and information systems of networks. The actual devices and monitored data from the real-life demonstrations are taken into a combined real-time simulation environment in order to study the interaction of the demonstrations. The paper describes shortly the objectives and tasks of ADINE project. It includes a description of active distribution network and the ANM method to be developed. The combined real-time simulation environment and the way of utilising it at the development of the ANM method are also described.

  • A phase II trial of pegylated interferon a-2b therapy for polycythemia vera and essential thrombocythemia : Feasibility, clinical and biologic effects, and impact on quality of life

    • BACKGROUND. Conventional interferon-a (IFN) is an effective treatment for patients with myeloproliferative disorders. However, many patients discontinue therapy because of side effects. METHODS. In this 24-month, Phase II feasibility study of pegylated interferon a-2b (PEG-IFN) treatment, a starting dose of 0.5 µg/kg per week was received by 21 patients with polycythemia vera (PV) and 21 patients with essential thrombocythemia (ET). The treatment objective, a complete platelet response (CR), was a platelet count <400 × 109/L in symptomatic patients and <600 in asymptomatic patients. Neutrophil polycythemia rubra vera-1 (PRV-1) messenger RNA expression was analyzed prior to and during therapy. Quality of life (QoL) was investi-gated by using the European Organization for Research and Treatment of Cancer QLQ-C30 questionnaire. RESULTS. At 6 months, 29 of 42 patients (69%) had achieved a CR after a median of 83 days. The CR rate was not related to diagnosis, gender, or previous therapy. Nineteen patients completed the planned 2-year treatment in CR. No thromboembolic or bleeding complications were observed. Phlebotomy requirements were reduced in the majority of patients with PV. Five of 14 patients (36%) who initially were positive for PRV-1 achieved normalized PRV-1 expression under PEG-IFN treatment. Side effects were the cause of therapy failure in 16 of 23 patients. However, only 8 of 19 patients reported any side effects at 2 years. The QLQ-C30 revealed clinically significant impairments in several aspects of QoL at 6 months, however, at 2 years, QoL measurements were not different from baseline. CONCLUSIONS. PEG-IFN effectively reduced platelet counts in 29 of 42 patients, but only 19 patients maintained a CR at 2 years. The reversal of PRV-1 positivity noted in a subset of patients suggested that PEG-IFN may have an effect on the malignant clone. PEG-IFN is a valuable therapeutic alternative for patients who tolerate its initial side effects. © 2006 American Cancer Society.

  • Noise and full counting statistics of incoherent multiple Andreev reflection

    • We present a general theory for the full counting statistics of multiple Andreev reflections in incoherent superconducting-normal-superconducting contacts. The theory, based on a stochastic path integral approach, is applied to a superconductor-double-barrier system. It is found that all cumulants of the current show a pronounced subharmonic gap structure at voltages V=2Δ/en. For low voltages V≪Δ/e, the counting statistics results from diffusion of multiple charges in energy space, giving the pth cumulant ⟨Qp⟩∝V2-p, diverging for p≥3. We show that this low-voltage result holds for a large class of incoherent superconducting-normal-superconducting contacts.

    • Genetic and environmental influences on prereading skills in preschool and on early reading and spelling development at the end of kindergarten were compared among samples of identical and fraternal twins from the U.S. (Colorado), Australia, and Scandinavia. Mean comparisons revealed significantly lower preschool print knowledge in Scandinavia, consistent with the relatively lower amount of shared book reading and letter-based activities with parents, and lack of emphasis on print knowledge in Scandinavian preschools. The patterns of correlations between all preschool environment measures and prereading skills within the samples were remarkably similar, as were the patterns of genetic, shared environment, and non-shared environment estimates: in all samples, genetic influence was substantial and shared environment influence was relatively weak for phonological awareness, rapid naming, and verbal memory, genetic influence was weak, and shared environment influence was relatively strong for vocabulary and print knowledge. In contrast, for reading and spelling assessed at the end of kindergarten in the Australian and U.S. samples, there was some preliminary evidence for country differences in the magnitude of genetic and environmental influences. We argue that the apparently higher genetic and lower shared environment influence in the Australian sample was related to a greater emphasis on formal reading instruction, resulting in more advanced reading and spelling skills at the end of kindergarten, and thus there was greater opportunity to observe genetic influences on response to systematic reading instruction among the Australian twins. © Springer Science+Business Media B.V. 2007.

    • We comment on the recent suggestion to use a family of local uncertainty relations as a standard way of quantifying entanglement in two-qubit systems. Some statements made on the applicability of the proposed measures are overly optimistic. We exemplify how these specific measures fall short, and present a minor modification of the general theory which uses the same experimentally gathered information, but in a slightly different, better way.

    • Five ocean vessels were investigated for the characterization and quantification of gaseous compounds emitted during ocean transportation of wood pellets in closed cargo hatches from Canada to Sweden. The study was initiated after a fatal accident with several injured during discharge in Sweden. The objective with the investigation was to better understand the off-gassing and issues related to workers' exposure. Air sampling was done during transport and immediately before discharge in the undisturbed headspace air above the wood pellets and in the staircase adjacent to each hatch. The samples were analyzed with Fourier transform infrared spectroscopy and direct reading instruments. The following compounds and ranges were detected in samples from the five ships: carbon monoxide (CO) 1460-14650 ppm, carbon dioxide (CO2) 2960-21570 ppm, methane 79.9-956 ppm, butane equivalents 63-842 ppm, ethylene 2-21.2 ppm, propylene 5.3-36 ppm, ethane 0-25 ppm and aldehydes 2.3-35 ppm. The oxygen levels were between 0.8 and 16.9%. The concentrations in the staircases were almost as high as in the cargo hatches, indicating a fairly free passage of air between the two spaces. A potentially dangerous atmosphere was reached within a week from loading. The conclusions are that ocean transportation of wood pellets in confined spaces may produce an oxygen deficient atmosphere and lethal levels of CO which may leak into adjacent access spaces. The dangerous combination of extremely high levels of CO and reduced oxygen produces a fast-acting toxic combination. Measurement of CO in combination with oxygen is essential prior to entry in spaces having air communication with cargo hatches of wood pellets. Forced ventilation of staircases prior to entry is necessary. Redesign, locking and labeling of access doors and the establishment of rigorous entry procedures and training of onboard crew as well as personnel boarding ocean vessels are also important.

    • BACKGROUND: Myotonic dystrophy type 1 (DM1) is associated with brain morphology changes including neurofibrillary degeneration. METHODS: We have examined cerebrospinal fluid (CSF) markers indicative of neuronal degeneration and amyloidogenesis; total tau (T-tau), phosphorylated tau (P-tau) and beta amyloid 1-42 (Abeta42), in 32 patients with DM1. RESULTS AND CONCLUSIONS: Associations between CSF markers and CTG repeat expansion size, brain MRI findings, and neuropsychological test results were analysed. As compared with matched controls Abeta42 was significantly decreased (P = 0.001), whilst levels of T-tau were increased (P < 0.001). No difference was found between measures considering P-tau levels. At present the clinical implications of these findings is unclear, because of an overlap between CSF values of DM1 patients and healthy controls, but also regarding modest associations between CSF markers and other measures. However notably, the Tau pathology, as seen in DM1, differs from Alzheimers disease, considering the lack of increased levels of P-tau.
