  • Cognition Seen Through the Eyes of Hearing Aid Users : Working Memory Resource Allocation for Speech Perception and Recall

    • This thesis investigates how hearing aid users allocate working (WM) memory resources under various task demands when listening to and storing speech in memory for later recall. This was done by combining an auditory recall task, the Sentence-final Word Identification and Recall (SWIR) test, with pupillometry. Different pupillary responses were used to obtain insights into momentary WM resource allocation and overall WM resource allocation over time. The task demands were manipulated by varying hearing aid noise reduction settings, as well as by varying the task difficulty of the SWIR test and the task difficulty predictability.The findings from the first two studies showed that recall performance in competing speech was better, and baseline pupillary responses were higher when noise reduction was activated compared to when it was not. This indicates that attenuating background noise frees up WM resources to be used for storing speech in memory rather than speech processing. While unpredictable task difficulty elicited higher baseline pupillary responses than predictable task difficulty, it did not have any effect on recall performance. This finding suggests that task difficulty predictability does not affect WM resource allocation. Instead, unpredictable task difficulty may lead to increased alertness in anticipation of the end of the SWIR test list. The findings of the third study showed that increased transient task-evoked pupillary responses, which reflect the momentary intensity of attention during encoding, were associated with a higher likelihood of subsequent recall. Moreover, higher WM capacity was also linked to higher likelihood of subsequent recall, presumably due to the ability to allocate more attentional resources during encoding. Lastly, the findings from the fourth study indicated that the combination of the SWIR test and pupillometry is suitable for capturing WM resource allocation. Although arousal decreased over time, recall performance remained stable, suggesting that participants did not reach the point of disengagement.Overall, a novel learning from this thesis is that increased pupillary responses may be a marker of “successful effort” when additional WM resources are allocated to achieve a better recall performance in the SWIR test. Furthermore, this thesis gives insights into which factors affect WM resource allocation and how to reduce the amount of processing resources required to understand speech, which may contribute to optimizing auditory rehabilitation in the future.

    • In 1944 special compulsory schooling for feeble-minded children was introduced in Sweden. This meant that these children could no longer be taught in ordinary elementary school classes. Instead they now had to be transferred to schools for the feeble-minded.The main purpose of the study is to investigate the meaning assigned to feeble-mindedness, the arguments in favour of compulsory schooling for the feeble-minded, and the significance that it subsequently had for the citizenship of these individuals in society. For this purpose, articles published in 1925-54 in four professional journals, reports of inquiries, legislation, and material from two schools for the feeble-minded have been studied.In the study the complexity that characterized compulsory tuition for the feeble-minded is revealed and related to contemporary welfare policy as a whole. Because of the special compulsory schooling, its institutional form, and the link to legislation on sterilization, the special education for the feeble-minded was to be both a form of tuition adjusted to the special needs of the children and an efficient instrument of control, at once a benefit and an imposition. The compulsory schooling made it possible to enrol feeble-minded children and keep them in the institutions, if necessary against their will and that of the parents. The sterilization laws also meant that sterilization could be made a condition for discharge. At the same time, the feeble-minded children were said to be given not only tuition adjusted to their abilities but also a suitable and sheltered environment in which to grow up with classmates of equal status and in many cases a better home than their parents could offer.Arguments in favour of special tuition for the feeble-minded were that it prevented them from growing up into asocial adults, while furthering their development into socially well-adjusted citizens. Work and social adjustment were also the characteristics of the feeble-minded individuals- citizenship, as this was described both in the professional debate and in the parents- correspondence with the Storängen institution for the feeble-minded. However, this citizenship did not include the right to form a family and have children.

  • Improving documentation using a nursing model

    • Background. The present investigation is part of a study where the Registered Nurses on three hospital wards received a 2 year intervention programme on nursing documentation in accordance with a keyword structure based on the nursing process. Aim. To describe the Registered Nurses’ perceptions of and attitudes towards the effects of the intervention, and to generate hypotheses for further research. Method. Focus group discussions were used to collect data, with a qualitative content analysis method for the processing of the data. Findings. The most interesting finding in these group discussions was the statements made by participants that the structured way of documenting nursing care made them think more, and think in a different way about their work with their patients. Two types of role changing were reported; from a medical technical focus to a more nursing expertise orientation and from a ‘hands on clinician’ to more of an administrator and secretary. Conclusion. A number of issues debated among the participants in this study could be seen as organizational matters and lead to the important issue of multidisciplinary and organizational work when implementing innovations within nursing.

  • Effects of leisure time physical activity on well-being among women in a 32-year perspective.

    • AIMS: To explore potential effects of physical activity on well-being in a population study. Results are from baseline and 32-year follow-ups. METHODS: In a population study of 1462 women in five age strata cross-sectional and prospective analysis were carried out. Activity levels were divided into low, intermediate and high. Well-being was based on self-reported well-being using a Likert-type 7-point scale. RESULTS: Cross-sectional analysis showed strong associations between level of physical activity and well-being. The odds ratio (OR) for poor well-being in women with low physical activity compared with physically more active women was, when studied cross-sectionally, after 12 years in 1980-81 3.94, 95% confidence interval (CI) 2.70-5.74, after 24 years in 1992-93 4.01, CI 2.61-6.17, and after 32 years in 2000-01 7.17, CI 3.56-14.44. Similar associations were observed when relating physical activity at baseline to subsequent well-being: after 12 years: OR 2.09, 95% CI 1.31-3.34, after 24 years: OR 2.74; 95% CI 1.56-4.83, and after 32 years: OR 1.49, 95% CI 0.77-2.88. There was a linear correlation between changes in the individual's physical activity level and her simultaneous changes in experience of well-being between 1980-81 and 1992-93 and between 1992-93 and 2000-01 as well as between 1980-81 and 2000-01. CONCLUSIONS: Strong associations were observed between leisure time physical activity level and reported experience of well-being cross-sectionally and prospectively. Well-being increased with concurrent changes in physical activity. Increased physical activity in sedentary individuals appears to promote not only health but also well-being.

    • Purpose: The aim of this study was to test the null hypothesis that there is no difference in implant failure rates, marginal bone loss, and postoperative infection for implants inserted in bone with different qualities and quantities according to the classification of Lekholm and Zarb. Materials and Methods: An electronic search was undertaken in January 2015 for randomized and nonrandomized human clinical studies. Results: A total of 94 publications were included. When bone sites of different qualities were considered, the results suggested the following comparative implant failure rates: 1 > 2, 1 > 3, 3 > 2, 4 > 1, 4 > 2, and 4 > 3. Sensitivity analyses suggested that when implants inserted in bone qualities 1 and 2 and 1 and 3 were compared, oxidized and sandblasted/acid-etched surfaces showed a decrease in significant difference in failures compared with turned implants. The same is not true for failure of implants inserted in bone quality 4 compared to failure of implants in all other bone qualities. When bone sites of different quantities were considered, the following comparative implant failure rates were observed: A > B, A > C, A < D, B < C, B < D, C < D, E > A, E > B, E > C, E > D. Due to insufficient information, meta-analyses for the outcomes postoperative infection and marginal bone loss were not performed. Conclusion: Sites with poorer bone quality and lack of bone volume may statistically affect implant failure rates. Implant surfaces may play a role in failure of implants in different bone qualities.

  • A ground-borne noise prediction model for railway traffic in tunnels in bedrock

    • Human life has become more manageable by the expansion of railway lines. However, despite providing convenience, railways increase noise and vibration in residential areas. Vibrations and noise generated by railways may harm human health, cause cosmetic damage and have an adverse impact on the environment. In order to reduce the effects of train-induced noise and vibration, efficient and accurate models for the prediction of ground-borne noise and vibration are required. Various analytical, theoretical, and experimental models have been developed to predict ground-borne noise. There is generally a lack of published information about parameters for ground-borne noise prediction concerning Swedish conditions with bedrock of high quality. Some investigations are reported, and a few consultancy companies have their own developed models, which are generally not publicly available. In fact, overall, the input data used to form these models and the methods of validation are not publicly available. Moreover, the statistical nature of the source and transfer paths requires that uncertainties are accurately handled in the model. This work aims to develop a model for ground-borne noise prediction for underground tunnels, to be used in Swedish Transport Administration projects. The methodology is formulated for three different stages based on precision and available information: location stage, planning stage, and construction stage. The first two stages correspond to planning and designing a railway track. The third stage involves the construction stage where more detailed information may be acquired. The prediction model presented here is developed for Swedish bedrock up to 1 kHz and formulated as a source term and several correction terms. These terms take into account various aspects, including train speed, distance attenuation, ground-to-building coupling, vibration levels on different floors and walls, how the room properties affect sound pressure levels within rooms, and different track treatments. Moreover, uncertainties are estimated using the standard deviation of each term. The required data are gathered from measurements in the Gårda tunnel in Gothenburg combined with existing data from measurements in the Åsa tunnel close to Varberg. As a result, a comprehensive model is suggested for ground-borne noise prediction in Swedish Transport Administration projects. However, the model is still under development, will be sent upon referral, and may undergo improvements.

  • Infinite hidden Markov model for short-term solar irradiance forecasting

    • Hidden state models are among the most widely used and efficient schemes for solar irradiance modeling in general and forecasting in particular. However, the complexity of such models – in terms of the number of states – is usually needed to be specified a priori. For solar irradiance data this assumption is very difficult to justify.In this paper, an infinite hidden Markov model (InfHMM) is introduced for short-term probabilistic forecasting of solar irradiance, where the assumption of fixed number of states a priori is relaxed and model complexity is determined during the model training. InfHMM is a non-parametric Bayesian model (NPB) indexed with an infinite dimensional parameter space which allows the automatic adaptation of the model to the “correct” complexity. This facilitates the automatic adaptation of the model to all weather conditions and locations. Posterior inference for InfHMM is performed using the Markov chain Monte Carlo algorithm, namely the beam sampler.Data from 13 different sources are used to validate the proposed model and subsequently it is compared to two well-established models in the literature: Markov-chain mixture distribution (MCM) and complete-history persistence ensemble (CH-PeEn) models. Important results are found, that cannot be derived from the existing finite models, such as the variation of the number of states within and across sites. The comparison of the models shows that the InfHMM is more consistent in term of the forecasting horizon.For reproducibility of the methodology presented in this paper, we have provided an R script for the InfHMM as supplementary material.

    • Agreement on the definition of talent management is not yet achieved. Reasons for this are overlapping human resources fields and the lack of conceptual boundaries to determine what should be under the concept of talent management and what not. However, the concept is getting stronger among academics and practioners, and talent is identified as a critical source of sustainable competitive advantage. This paper examines the progress of the definition of talent management in the literature and investigates if both mechanistic and political frames are taken into consideration when implementing talent management systems. Results of this study showed that systems can offer a complete panorama of talent management activities. However, its effectiveness is reduced by power relationships and political behavior. Awareness of this behavior when designing and dealing with information system can help to overcome, to certain extent, these obstacles.

    • One emerging form of communication discussed in the present article is the use of visual  self-representation as a tool for symbolic interaction between young people on the In-  ternet. Using examples of difference and similarity in young women’s and men’s visual  self-representation, the article offers an interpretation of these practices, pointing towards  both new visual conventions and references to pre-existing media representations, thus  revealing a process of hypervisuality. In this process of transformation, the involvement  of new technologies, such as webcam aesthetics and its form of intimacy and authenticity,  produces specific visual conventions within the frame of pre-existing media imagery, when  the self is presented in online communication.

    • Enzymatic side reactions can give rise to the formation of wasteful and toxic products that are removed by metabolite repair pathways. In this work, we identify and characterize a mitochondrial metabolic repair mechanism in Arabidopsis thaliana involving malate dehydrogenase (mMDH) and l-2-hydroxyglutarate dehydrogenase (l-2HGDH). We analyze the kinetic properties of both A. thaliana mMDH isoforms, and show that they produce l-2-hydroxyglutarate (l-2HG) from 2-ketoglutarate (2-KG) at low rates in side reactions. We identify A. thaliana l-2HGDH as a mitochondrial FAD-containing oxidase that converts l-2HG back to 2-KG. Using loss-of-function mutants, we show that the electrons produced in the l-2HGDH reaction are transferred to the mitochondrial electron transport chain through the electron transfer protein (ETF). Thus, plants possess the biochemical components of an l-2HG metabolic repair system identical to that found in mammals. While deficiencies in the metabolism of l-2HG result in fatal disorders in mammals, accumulation of l-2HG in plants does not adversely affect their development under a range of tested conditions. However, orthologs of l-2HGDH are found in all examined genomes of viridiplantae, indicating that the repair reaction we identified makes an essential contribution to plant fitness in as yet unidentified conditions in the wild. © 2015 The Author 2015.

  • Exploring factors related to non-adherence to exergaming in patients with chronic heart failure

    • AimsThis study aimed to explore factors related to non-adherence to exergaming in patients with heart failure.Methods and resultsData from patients in the exergame group in the HF-Wii trial were used. Adherence to exergaming was defined as playing 80% or more of the recommended time. Data on adherence and reasons for not exergaming at all were collected during phone calls after 2, 4, 8, and 12 weeks. Logistic regression was performed between patients who were adherent and patients who were non-adherent. Secondly, a logistic regression was performed between patients who not exergamed at all and patients who were adherent to exergaming. Finally, we analysed the reasons for not exergaming at all with manifest content analysis. Almost half of the patients were adherent to exergaming. Patients who were adherent had lower social motivation [odds ratio (OR) 0.072; 95% confidence interval (CI) 0.054-0.095], fewer sleeping problems (OR 0.84; 95% CI 0.76-0.092), and higher exercise capacity (OR 1.003; 95% CI 1.001-1.005) compared with patients who were non-adherent. Patients who not exergamed at all had lower cognition (OR 1.18; 95% CI 1.06-1.31) and more often suffered from peripheral vascular disease (OR 3.74; 95% CI 1.01-13.83) compared with patients who were adherent to exergaming. Patients most often cited disease-specific barriers as a reason for not exergaming at all.ConclusionsA thorough baseline assessment of physical function and cognition is needed before beginning an exergame intervention. It is important to offer the possibility to exergame with others, to be able to adapt the intensity of physical activity.

    • A heart failure team that treats heart failure patients often faces the challenge of managing multiple conditions requiring multiple medications and life style changes in an older patient group. A multidisciplinary team approach can optimally diagnose, carefully review and prescribe treatment, and educate and counsel patients and their families about medication use and life style changes. In this paper the possible role of the pharmacist, dietician, physical therapist, psychologist, primary care provider and social worker in heart failure management is discussed.

    • Ultrastrong coupling (USC) is a distinct regime of light-matter interaction in which the coupling strength is comparable to the resonance energy of the cavity or emitter. In the USC regime, common approximations to quantum optical Hamiltonians, such as the rotating wave approximation, break down as the ground state of the coupled system gains photonic character due to admixing of vacuum states with higher excited states, leading to ground-state energy changes. USC is usually achieved by collective coherent coupling of many quantum emitters to a single mode cavity, whereas USC with a single molecule remains challenging. Here, we show by time-dependent density functional theory (TDDFT) calculations that a single organic molecule can reach USC with a plasmonic dimer, consisting of a few hundred atoms. In this context, we discuss the capacity of TDDFT to represent strong coupling and its connection to the quantum optical Hamiltonian. We find that USC leads to appreciable ground-state energy modifications accounting for a non-negligible part of the total interaction energy, comparable to kBT at room temperature.

    • In many system identification methods, process model parameters are considered stochastic variables. Several methods do not only yield expectations of these, but in addition their variance, and sometimes higher moments. This paper proposes a method for robust synthesis of the proportional-integral-derivative (PID) controller, taking parametric process model uncertainty explicitly into account. The proposed method constitutes a stochastic extension to the well-studied minimization of integrated absolute error (IAE) under H∞-constraints on relevant transfer functions. The conventional way to find an approximate solution to the extended problem is through Monte Carlo (MC) methods, resulting in high computational cost. In this work, the problem is instead approximated by a deterministic one, through the unscented transform (UT), and its conjugate extension (CUT). The deterministic approximations can be solved efficiently, as demonstrated through several realistic synthesis examples.

    • This paper presents an automatic loop-shaping method for designing proportional integral derivative controllers. Criteria for load disturbance attenuation, measurement noise injection, set-point response and robustness to plant uncertainty are given. One criterion is chosen to be optimized with the remaining ones as constraints. Two cases are considered: M-constrained integral gain optimization and minimization of the cost of feedback according to quantitative feedback theory. Optimization is performed using a convex-concave procedure (CCP). The method that relies on solving a sequence of convex optimization problems converges to a local minimum or a saddle point. The proposed method is illustrated by examples.

    • Propolis is a resin-like material produced by bees fromthe budsof poplar and cone-bearing trees and is used in beehive construction.Propolis is a common additive in various biocosmetics and health-relatedproducts, despite the fact that it is a well-known cause of contactallergy. Caffeic acid and its esters have been the primary suspectsbehind the sensitization potency of propolis-induced contact allergy.However, the chemical structures of the protein adducts formed betweenthese haptens and skin proteins during the process of skin sensitizationremain unknown. In this study, the reactivity of three main contactallergens found in propolis, namely, caffeic acid (CA), caffeic acid1,1-dimethylallyl ester (CAAE), and caffeic acid phenethyl ester (CAPE),was investigated. These compounds were initially subjected to thekinetic direct peptide reactivity assay to categorize the sensitizationpotency of CA, CAAE, and CAPE, but the data obtained was deemed toounreliable to confidently classify their skin sensitization potentialbased on this assay alone. To further investigate the chemistry involvedin generating possible skin allergy-inducing protein adducts, modelpeptide reactions with CA, CAAE, and CAPE were conducted and analyzedvia liquid chromatography-high-resolution mass spectrometry.Reactions between CA, CAAE, and CAPE and a cysteine-containing peptidein the presence of oxygen, both in closed and open systems, were monitoredat specific time points. These studies revealed the formation of twodifferent adducts, one corresponding to thiol addition to the & alpha;,& beta;-unsaturatedcarbonyl region of the caffeic structure and the second correspondingto thiol addition to the catechol, after air oxidation to o-quinone.Observation of these peptide adducts classifies these compounds asprehaptens. Interestingly, no adduct formation was observed when thesame reactions were performed under oxygen-free conditions, highlightingthe importance of air oxidation processes in CA, CAAE, and CAPE adductformation. Additionally, through NMR analysis, we found that thioladdition occurs at the C-2 position in the aromatic ring of the CAderivatives. Our results emphasize the importance of air oxidationin the sensitization potency of propolis and shed light on the chemicalstructures of the resultant haptens which could trigger allergic reactionsin vivo.

    • Nasopharyngeal cancer (NPC) is associated with the Epstein-Barr virus (EBV). The clinical presentation and prognosis of NPC is well described, but not in relation to intralesional EBV-DNA load. In a retrospective design, 48 patients with NPC were examined. Patient history was re-evaluated, and diagnostic biopsies were re-examined. Furthermore, intralesional EBV-DNA was quantitated and HPV status determined. Cancer stage, disease-free survival (DFS), and overall survival (OS) were assessed. Of the 48 patients, 36 (75%) patients featured lesions that were positive for EBER (Epstein–Barr virus-encoded small RNA) and 40 (83%) were positive for EBV-DNA. Seven patients (15%) were HPV positive. The levels of EBV-DNA ranged from 0.0005 to 94617 copies/cell. An EBV-DNA load of more than 70 copies/cell was associated with a prolonged DFS for EBV-DNA positive patients treated with curative intent (p = 0.046). In conclusion, the EBV-DNA load in NPC lesions appears to vary greatly. For patients with EBV-DNA positive NPC treated with curative intent, an EBV-DNA load of more than 70 copies/cell is associated with a better outcome in terms of 7-year DFS.

    • Collapse and inward slumping of unconsolidated sedimentary strata expanded the Chesapeake Bay impact structure far beyond its central basement crater. During crater collapse, sediment-loaded water surged back to fill the crater. Here, we analyze clast frequency and granulometry of these resurge deposits in one core hole from the outermost part of the collapsed zone (i.e., Langley) as well as a core hole from the moat of the basement crater (i.e., Eyreville A). Comparisons of clast provenance and flow dynamics show that at both locations, there is a clear change in clast frequency and size between a lower unit, which we interpret to be dominated by slumped material, and an upper, water-transported unit, i.e., resurge deposit. The contribution of material to the resurge deposit was primarily controlled by stripping and erosion. This includes entrainment of fallback ejecta and sediments eroded from the surrounding seafloor, found to be dominant at Langley, and slumped material that covered the annular trough and basement crater, found to be dominant at Eyreville. Eyreville shows a higher content of crystalline clasts than Langley. There is equivocal evidence for an anti-resurge from a collapsing central water plume or, alternatively, a second resurge pulse, as well as a transition into oscillating resurge. The resurge material shows more of a debris-flow–like transport compared to resurge deposits at some other marine target craters, where the ratio of sediment to water has been relatively low. This result is likely a consequence of the combination of easily disaggregated host sediments and a relatively shallow target water depth.

    • The presence and vasoactive effects of native calcitonin gene-related peptide (CGRP), substance P (SP), and neurokinin A (NKA) were studied on isolated small branches of the coeliac artery from Atlantic cod, Gadus morhua, using immunohistochemistry and myograph recordings, respectively. Immunohistochemistry revealed nerve fibers containing CGRP- and SP/NKA-like material running along the wall of the arteries. CGRP induced vasorelaxation of precontracted arteries with a pD(2) value of 8.54 +/- 0.17. Relaxation to CGRP (10(-8) M) was unaffected by L-NAME (3 x 10(-4) M) and indomethacin (10(-6) M) suggesting no involvement of nitric oxide or prostaglandins in the CGRP-induced relaxation. SP and NKA (from 10(-10) to 3 x 10(-7) M) contracted the unstimulated arteries at concentrations from 10(-8) M and above in 42% and 33%, respectively, of the vessels. It is concluded that the innervation of the cod celiac artery includes nerves expressing CGRP-like and tachykinin-like material, and that a vasodilatory response to CGRP is highly conserved amongst vertebrates while the response to tachykinins is more variable.

    • Abstract: In this paper the occurrence of skeletal changes in joints has been investigated in cattleextremities. The bone elements derive from the archaeological site Eketorp ringfort on the Ölandisland in Sweden dated between Iron Age - Middle Age (ca 300 - 1200/50 A.D.). The analysis wasconducted in order to examine skeletal lesions and their connection to traction work. Different skeletallesions were recorded and the joint surface on metapodia and phalanges was divided in 4-7 sites toexamine if different types of lesions were located on particular sites of the articular facets or ifdifferent types of lesions were noted in bone elements from fore- and hindlimbs. The results show thatmetatarsals exhibited a higher frequency of pathologies in both phases compared to metacarpals whileanterior phalanges 1 and 2 had a higher occurrence of lesions than the posterior elements. The studyalso demonstrates that the type and location of depressions on joint surfaces are unevenly distributedbetween bone elements. Furthermore the results show that the occurrence of skeletal lesions weremore common in robust animals.
