    • This study deals with the consequences for household economy in connection with unemployment. Various mechanisms that influence the relationship between unemployment and the reception of supplementary benefits are described and analyzed. The labour market, the social insurance system and the adjustment strategies of families are fields of particular interest to this study.The economic systems of support that have developed are dominated by the unemployment insurance. Those, whose unemployment benefits have ceased or who for some other reason are not covered by the insurance, have to resort to supplementary benefits. The differences in level between working, receiving unemployment benefits or supplementary benefits can be described in terms of a scale of income. The aspect of the scale varies strongly between different types of families.The empirical point of reference for this dissertation is the close-down of a pulp industry (Ncb in Köpmanholmen 130 km south of Umeå in the north of Sweden). The development of those made redundant has been studied with regard to reception of supplementary benefits, income and adjustment strategies. Data about income and supplementary benefits were collected from registers at the local tax charge office and the local administration of social services. 83 out of 444 households were interviewed about adjustment strategies.The close-down in question proved to be exceptional in many aspects. The expected "social catastrophe" never occured due to the economic upswing, raised unemployment benefits and ambitious labour market policy efforts, among other things. Only a very small minority of those made redundant have received supplementary benefits after the close-down. This has been the case although many of those made redundant have been removed from the open labour market with a subsequent lowering of their income level.Amongst those made redundant many different patterns of adjustment havè been observed. It appears that those who lack work after a couple of years after the close-down are often characterized by passivity, whereas those who have been employed are often characterized by activity. Finally the variation in patterns of adjustment can be seen as a social policy problem. Judging from the results a combination of the institutional and marginal social policy strategies would be desirable. Reform schemes can be suggested that are based on a wish to supplement the preventive and institutional social policy with selective features where the adjustment strategies of families should provide an important resource potential.

    • Looking After Children (LAC) is an approach (care philosophy and working tools) used to assess the psycho-social development of children being cared for by child welfare agencies. It is an international initiative that was developed in England and then travelled and was translated into other contexts, most notably Australia, Canada and Sweden. This paper presents findings from an open-ended question in a survey distributed to social workers and managers using LAC and “cousin” systems in these countries. We asked respondents what advice they would give to others considering implementing these systems. Our qualitative content analyses showed that, regardless of the context, the 257 respondents gave voice to programmatic/normative arguments, reflecting mainly positive attitudes to the systems. However, managers and social workers voiced different arguments in their favour. Managers voiced normative arguments favouring the underlying principles, whereas social workers from all three countries identified the dual needs to remain flexible and to recognise the limitations of the systems, especially at the operational level. Results offer insights into approaches to change management in different contexts.

    • This article compares social work in countries representing four different welfare regimes: Chile, the Republic of Ireland (refer to elsewhere as ‘Ireland’), Lithuania and Sweden. The aim is to examine how social workers in different contexts refer to families’ complex needs, how contextual factors influence social workers’ positions and actions, and how they make sense of their work. Social workers in 15 focus groups, 4 per country except for Chile with 3, were interviewed about their conceptions of ‘family’, ‘families with complex needs’, and reasoning about interventions in relation to a fictitious complex case vignette. The understanding of complex needs appears relatively individualized in Chile and Lithuania, while contextual factors were more pronounced in the Irish and Swedish material. Chile, exemplifying a familialized family policy regime, reflects a poverty-compensatory social worker role that also supports familial reproduction; Ireland, a partly de-familialized regime, reflects a supportive and risk-reactive role; Lithuania, a re-familialized regime reflects a patriarchal risk-reducing role and Sweden, a de-familialized policy regime, reflects a rights-oriented and technocratic role. Welfare regimes shape different social work practice contexts. However, to some extent, social workers around the world share a common work ethos in how they, for the best interest of the people they work with, deal with the cross-pressure from social problems and political-ideological priorities.

    • Vid institutionen för socialt arbete i Umeå har utarbetats en modell för användning av korta narrativer (berättelser) vid uppföljning av praktikterminen i socionomprogrammet. Detta pedagogiska instrument har sitt ursprung i ett antal tidigare studier om socionomstudenters kunskapsanvändning under studiepraktiken. Socionomstudenter har till uppgift att inför sin avslutande praktikuppföljning skriva en narrativ som beskriver en problematisk händelse under praktikperioden samt den kunskap och de teoretiska redskap man använde för att hantera situationen. Under praktikuppföljningen bearbetar studenterna sina narrativer genom en strukturerad modell för kollegial handledning i grupp. Att använda narrativer i praktikuppföljning har visat sig varau ppskattat av studenterna och vi fortsätter att utveckla användandet av narrativer. I den här texten beskrivs bakgrund, genomförande och ett antal lärdomar. Avslutningsvis diskuteras hur korta narrativer kan användas i andra sammanhang.

    • Social workers around the world work with families and family complexities in their everyday practice. In this cross-national study, we explore social workers’ family intervention practices related to family definitions and functions, and how social workers balance children’s and parents’ rights and social policies in the proper context. Data derives from focus group interviews with child welfare workers from Norway, Lithuania, Chile and England based on discussions of a common fictitious complex family case (vignette). The four countries chosen for this comparative study are examples of four different welfare systems/regimes. The findings related to this broad area of caring topics are related to how the dimensions of a ‘private’ and a ‘public’ family manifest in social work in the four countries. Social workers in Chile and Lithuania refer to the idea of the private family, while their Norwegian counterparts lean more to the public family. English social workers combine public and private family conceptions in their focus groups, reflecting a system that is partly de-familialized.

    • The aim of this article is to reflect on the strengths and challenges in qualitative comparative research on personal social services. The specific methodological approach that these reflections emerge from is the application of case vignettes in focus group interviews with social workers, working in different welfare regimes.We describe the process of vignette construction and implementation in focus group interviews, and relate this to findings in a large international project with researchers and data from Chile, Mexico, Norway, Sweden, Lithuania, Bulgaria, Ireland and the UK.Findings reveal that some globally spread professional norms prevail when they are applied locally, while others are more formed through welfare systems with strong contextual norms and legal and socio-economic barriers. Furthermore, the project showed that to use case vignettes and focus groups, in order to compare ‘social work’ in its totality between countries, is really difficult. It appears more fruitful to use such research methods to compare subsectors and sub-disciplines instead of social work as a whole. The strength of the data retrieved from the study is that it makes it possible to separate information on actual practice from information on principles and system norms, thus providing in-practice and on-practice reflections.

    • Client violence towards social workers is common and its impact on their practice, physical and psychological health is well documented. The majority of research in this field has emerged from the UK and USA, and is limited in developing countries such as Iran. Thus, the aim was to determine the one-year prevalence of client violence in Iran and its association with the mental health status of social workers. A national survey of 390 social workers from the Centres for Socially Injured People affiliated to the Social Affairs Department of the State Welfare Organisation, Iran, was conducted. The results showed that 67 per cent of Iranian social workers had experienced violence with a considerably higher magnitude of psychological violence than physical violence. Psychological violence was associated with poorer health characterised by physical symptoms, anxiety and sleep disorders, and social dysfunction.

    • This paper explores different orientations to child-centered social work as conveyed in the training materials and guidelines of Looking After Children and Assessment Framework in Australia, Canada and Sweden. ‘Child centered’ is shaped by contextual factors and influences social work practices. We found differences in these approaches as needs based and/or rights based and in relation to how each emphasizes the three P's — Provision, Protection and Participation. Substantial differences were identified both in how references to a child-centered approach appear in theoretical frameworks, values, motives and use of concepts in training materials and guidelines, and in the instructions given as to how to apply these approaches. It appears that Australia balances needs and rights, while Canada is more needs-oriented and Sweden more rights-oriented. Swedish materials show a more explicit emphasis on participation than Australian and Canadian materials. Differences between the three countries indicate the importance of structural, contextual factors shaping orientations to child-centered practice.

    • Quality of life is a subjective and multidimensional concept that includes all aspects of an individual’s life. Many investigations indicate that marital satisfaction is an important determinant of quality of life and social support affects both marital satisfaction and quality of life. Since medical staff face job stressors in their everyday life, in the present cross-sectional study, the relationships between quality of life, marital satisfaction, and social support in medical staff in Tehran were analyzed. Data collection was performed in 653 medical staff using socio-demographic questions, the SF-36 questionnaire, the ENRICH Marital Satisfaction Inventory, and the Social Support Questionnaire. The results revealed that men reported significantly higher quality of life and marital satisfaction than women, but there was no gender difference in social support. The quality of life and marital satisfaction domains were found to be differentially correlated among male and female participants. All domains of marital satisfaction were found to be significantly associated with the “vitality” and “mental health” domains among females and “physical functioning” domain in the male. Also, social support was associated with almost all quality of life domains in both sexes. The multiple regression analyses indicated that all the study variables combined with socio-demographic factors can explain between 12 % and 28 % of the variance in quality of life domains. Therefore, focusing on these factors could be an effective approach to improve quality of life in medical staff as a group with a stressful job.

    • This paper investigates how children who have experienced instability in substitute family care describe their sense of belonging and relationships with adults who share responsibility for caring for them. Using an interpretive phenomenological methodology, we interviewed twelve children in Swedish foster and institutional care. Our study found that the children craved a close relationship with consistent adults and an opportunity to feel that they belonged somewhere. These needs were difficult for them to receive due to their parents' problematic life histories, instability in care which repeatedly placed them in new care situations and a lack of continuity of social workers. These children endured a repeated disconnection to those adults who were supposed to share the role of raising them and, at the same time, an incredible ability to adapt to new care environments was demanded of them. After continually losing relationships, some children finally decided to ‘hold off adults’ in order to not get hurt. Working with and caring for children who have experienced unstable care puts great demand on adults to develop relationships that children feel will be consistent and that they can trust.

    • The aim of this study was to assess possible determinants of quality of life in psoriasis patients in northern Sweden, and to analyse the association between these determinants and quality of life. A questionnaire was mailed to 1,707 subjects, resulting in a response rate of 74%. The study is therefore based on 1,060 subjects, since 195 denied having psoriasis. Quality of life was measured using a version of the Dermatology Life Quality Index. The extent of the disease was estimated using the patients' sketches. The association between determinants and quality of life was calculated in a risk analysis using logistic regression. Large disease extent and joint symptoms were the strongest indicators for impaired quality of life. Other indicators were withdrawal from medical treatment due to distance to treatment facilities, which strongly influenced the quality of life and choice of treatment.

    • In this study, the authors aimed at a sociological approach to the individual’s everyday life with psoriasis and to this end conducted qualitative interviews with 18 persons from a county in northern Sweden. The most difficult aspects of living with psoriasis were being marked by the visibility of psoriasis, especially in the younger ages, and the visibility of joint changes. Therefore, those with both rashes and joint changes felt marked and discredited in a twofold way. Commonly used coping strategies were routinization of both the treatment and the adjustment to the marking process, and acceptance, and these strategies developed with age. Most participants experienced a good quality of life but still could find nothing positive about psoriasis. The authors suggest efforts toward increased awareness among health care professionals of the marking process and future qualitative studies about experiences of psoriasis during adolescence.

    • During the 1990s priority discussions were actualized in Sweden due to increased demands on health care and limited resources. In the county of Västerbotten in northern Sweden, with large rural areas, the decision makers faced special challenges due to distances and cost. Despite discussions striving for fairness in priorities, decision makers are still dealing with limited resources and difficult priority decisions regarding different diseases and treatments. In this study we aimed at describing views on priorities in public psoriasis care and visions of a future care among politicians, administrators and professionals in the county of Västerbottten in northern Sweden. Qualitative research interviews were performed with 23 key-persons. The findings revealed priority dilemmas about issues on organization, accessibility and ethics. Visions of a future care appeared as ambitions of a more effective care with good accessibility, continued research, information and a holistic approach in priorities. We conclude that dilemmas revealed in this study were a reflection of a gap between intentions and practice. In efforts to reduce these dilemmas we suggest methods with fairness in economic planning and priority setting, with concrete, official statements about the dominating views on which the priorities are based, and public information about these statements.

    • The aim was to analyse use of psoriasis care and expectations from a gender perspective in a psoriasis population. The study is based on questionnaire data from 1060 subjects. The response rate was 74%. The relation between care consumption and studied variables was measured using regression and chi-square analysis. Care consumption was strongly influenced by age, quality of life, income and joint symptoms. Men visited a dermatologist more often, while women visited a general practitioner and treated themselves topically more frequently. Important expectations among both women and men concerned the receiving of professional care and amelioration, while more women wanted to be treated politely. Expectations were fulfilled, except those regarding amelioration, especially among men. Awareness of gender differences is important among professionals. In order to decrease such differences decision-makers should provide for a maintained specialized care as well as a local health care system of high standard.
