    • Context. It has been established that the classical gas-phase production of interstellar methanol (CH(3)OH) cannot explain observed abundances. Instead it is now generally thought that the main formation path has to be by successive hydrogenation of solid CO on interstellar grain surfaces. Aims. While theoretical models and laboratory experiments show that methanol is efficiently formed from CO on cold grains, our aim is to test this scenario by astronomical observations of gas associated with young stellar objects (YSOs). Methods. We have observed the rotational transition quartets J = 2(K) - 1(K) of (12)CH(3)OH and (13)CH(3)OH at 96.7 and 94.4 GHz, respectively, towards a sample of massive YSOs in different stages of evolution. In addition, the J = 1-0 transitions of (12)C(18)O and (13)C(18)O were observed towards some of these sources. We use the (12)C/(13)C ratio to discriminate between gas-phase and grain surface origin: If methanol is formed from CO on grains, the ratios should be similar in CH(3)OH and CO. If not, the ratio should be higher in CH(3)OH due to (13)C fractionation in cold CO gas. We also estimate the abundance ratios between the nuclear spin types of methanol (E and A). If methanol is formed on grains, this ratio is likely to have been thermalized at the low physical temperature of the grain, and therefore show a relative over-abundance of A-methanol. Results. We show that the (12)C/(13)C isotopic ratio is very similar in gas-phase CH(3)OH and C(18)O, on the spatial scale of about 40 '', towards four YSOs. For two of our sources we find an overabundance of A-methanol as compared to E-methanol, corresponding to nuclear spin temperatures of 10 and 16 K. For the remaining five sources, the methanol E/A ratio is less than unity. Conclusions. While the (12)C/(13)C ratio test is consistent with methanol formation from hydrogenation of CO on grain surfaces, the result of the E/A ratio test is inconclusive.

    • Isotopic ratios of elements are considered powerful tools, e.g. in tracing the origin of solar system body materials, or the degree of nucleosynthesis processing throughout the Galaxy. In interstellar molecules, some isotopic ratios like H/D and 12C/13C can also be used as indicators of their chemical origin. Isotope fractionation in gas-phase chemical reactions and gas-dust interaction makes observations of the ratio between 12C and 13C isotopologues suitable to distinguish between different formation scenarios. We will present observations of the 12C/13C ratio in methanol (CH3OH) and formaldehyde (CH2O) towards a sample of embedded, massive young stellar objects. In relation to this we also present results from theoretical modeling showing the usefulness of the 12C/13C ratio as a chemistry indicator.

    • With an expanding market for upgraded biofuel in many countries, it is important to develop efficient production methods for upgrading wet biomass. The possibilities for heat recovery can be improved if the upgrading process is integrated with other energy-intensive processes, as for example a pulp mill or a sawmill, in a biofuel combine. This work evaluates obstacles and Success factors for forming such biofuel combines with the forest industry. Case Studies and calculations on theoretical cases have been used together with literature references to evaluate how a biofuel combine can be realised and to compile obstacles and success factors for a combine. It could be seen from the case studies that an excess of by-products and waste heat, together with an existing need for investments are important driving forces for the formation of biofuel combines in the forest industry. The market was also identified as an important factor, which can be both an obstacle and a success factor depending on the situation. It was concluded that the existence of a small-scale pellet market near the plant is important for economic feasibility when sawdust is used as raw material. The conditions for the biofuel combine are different depending on the form of ownership. When a pulp mill or sawmill owns the pellet factory, it was concluded that minimising the risk by using well-known technologies can be an important factor for the realisation of the combine.

  • Adoption of change in endodontic practice after an educational program : A qualitative study

    • Objectives: The aim was to define the characteristics of successful implementation of new clinical endodontic routines within a public dental health organization, following an educational program. Materials and Methods: Fifteen staff members were strategically selected for the interview. All had completed a theoretical educational intervention including a complementary endodontic treatment strategy and, for the dentists, comprising training in the nickel-titanium-rotary-technique. All experienced the successful acceptance of new clinical routines. Two thematic in-depth audiotaped interviews were conducted, wherein the informants described the implementation process in their own words. The interviews were transcribed verbatim and analyzed according to Qualitative Content Analysis. Results: A theme was identified: A multiple flexible process with governance support and gradual reinforcement of motivation, with the following main categories: Firstly, contextual facilitation, with two subcategories (i) a multifaceted organizational foundation and (ii) a tolerance of flexibility. Secondly, emotional facilitation, with two subcategories (i) an experience of simplification and (ii) an experience of improvement. Conclusion: The results improve the understanding of a multifaceted process underlying the acceptance of changes to clinical endodontic procedures by dentists in a public dental health organization. Important contributing factors identified were governance support, a committed resource person with contextual knowledge, tolerance of flexibility in implementation, and permissive informal communication channels within the local workplace. These findings might be a valuable contribution to an evidence base, facilitating the selection of the most appropriate educational strategy and structure for a specified purpose.

  • Alleviation of acute dental pain from localised apical periodontitis : A prospective randomised study comparing two emergency treatment procedures

    • Objective: To compare the outcomes of two emergency treatment procedures to alleviate pain from localised symptomatic apical periodontitis: complete chemo‐mechanical disinfection (CMD) of the root canal system, or removal of necrotic tissue from the pulp chamber (RNT), that is without instrumentation of the root canals. Methods: Fifty‐seven consecutive patients from the emergency clinic at Malmö University met the inclusion criteria: spontaneous pain and/or pain on percussion and palpation, non‐bleeding pulp in the canal orifice, pain ≥4 on a Numeric Rating Scale and ≥18 years of age. The diagnosis was symptomatic apical periodontitis, in the absence of swelling and/or fever. Pre‐operative pain levels and intake of analgesics were registered. The patients were randomised to one of the two treatment groups. Three to five days post‐operatively, the patients were contacted by telephone and asked to grade their current pain level and report any post‐operative intake of analgesics and antibiotics. Results: Of the patients treated with CMD of the root canal system, 26/30 (87%) reported satisfactory pain relief, compared with 22/27 (81%) of those treated by RNT. There was no mean difference in pain relief between the two groups (P = 0.879). Post‐operatively, 37% in each group reported using analgesics and one in each group reported using antibiotics. Conclusion: Three to five days after treatment, a majority (>80%) in both groups reported adequate pain relief, in some cases in combination with analgesics. Removal of necrotic and infected tissue from the pulp chamber might therefore be a cost‐effective emergency treatment alternative to complete chemo‐mechanical disinfection.

    • Enligt aktuella nationella riktlinjer (Socialstyrelsen 2012) bör tandvården i första hand erbjuda antibiotikabehandling endast till personer med symtom på grund av apikal parodontit, där infeltionen kring rotspetsen visar på spridning och där personens allmäntillstånd är påverkat. Vid symtomatisk tand då infektionen inte tillfredsställande kan avlägsnas, till exempel då patienten inte kan gapa eller då rotkanalen inte är tillgänglig för infektionsbehandling kan sys- temisk antibiotika ordineras i avvaktan på att endodontisk behandling eller extraktion är möj- lig. Syftet med förskrivningen är då att begränsa spridning av befintlig infektion eller hindra progression av sjukdomen. När allmäntillståndet är påverkat är antibiotikabehandling indicerad eftersom detta är ett tecken på infektionsspridning. För exartikulerade och replanterade permanenta tänder har systemisk antibiotikabehandling tidigare rekommenderats, för att eventuellt underlätta den parodontala läkningen och minska risken för rotresorption. I en senare systematisk översikt har man inte kunnat påvisa några entydiga positiva effekter av detta. Utöver detta rekommenderas lokal antibiotikabehandling av roten före replantering av tand med oavslutad rotutveckling för att öka chanserna för revaskularisering.

    • The aim was to catalyze perceptions of oral health in adults who have been exposed to child sexual abuse. Eleven participants (10 women), 19 to 56 y of age, who had experienced sexual abuse as children were purposively selected and interviewed in-depth. The participants were encouraged to describe how they perceived the effect of the sexual abuse on their oral health as adults. The interviews were recorded digitally and transcribed verbatim. The collected material was analyzed according to qualitative content analysis. The theme "challenging conditions for maintaining oral health" was identified, comprising 2 categories: first, "the emotional significance," with the subcategories 1) emotional barriers and 2) powerful relief, and second, "the obstacles to oral health,' with the subcategories 1) daily self-care with complications and 2) dental appointments with dculties. The findings indicate that the experience of sexual abuse during childhood can have a negative impact on oral care in adulthood. The informants stated that oral health was of utmost importance but also associated with strong emotions. There were obstacles to maintenance of oral health that were difficult to surmount. Knowledge Transfer Statement: The study provides access to the attitudes of survivors of child sexual abuse regarding oral health and the needs and obstacles that they experience. This is important knowledge for dental professionals to optimize dental care.

  • Chronic orofacial pain. Understanding patients from two perspectives : the clinical view and the patient's experience

    • Det övergripande syftet har varit att studera patienter med långvarig ansiktssmärta dels från ett kliniskt perspektiv och dels från patientens perspektiv. Samtliga patienter som ingått i studierna var remitterade till Tandvårdshögskolan i Malmö pga långvarig ansiktssmärta. Det kliniska perspektivet erhölls genom en retrospektiv studie av 109 patienter (85 kvinnor, 24 män) som blivit undersökta i en smärtgrupp. Därvid studerades parametrar som diagnos, smärtkarakteristika, medicinering och utbildning. Uppmärksamhet riktades mot skillnader och likheter mellan kvinnor och män avseende de parametrar som studerats. En uppföljning av patienternas smärttillstånd gjordes 4-9 år efter den ursprungliga undersökningen. För att få patienternas perspektiv intervjuades fjorton patienter (11 kvinnor, 3 män) om sina personliga upplevelser av att leva med långvarig ansiktssmärta. Varje patient intervjuades vid två tillfällen under ca 1-1.5 timme. Nedan listas fynden från de kliniska studierna och intervjustudierna. Upplevelserna som de intervjuade patienterna förmedlade innebar ett annat perspektiv på flera av fynden från de kliniska studierna. - Kvinnorna var i majoritet bland de patienter som ingick i studierna. När kvinnor och män jämfördes registrerades fler likheter än olikheter för de parametrar som studerades. -Patienterna hade en lägre utbildningsnivå än svenska invånare av samma ålder. -En majoritet av patienterna som studerades kliniskt rapporterade ett upprepat vårdsökande med anledning av smärtan. De intervjuade patienterna upplevde misstro och avfärdande vid sina vårdbesök. De upplevde också smärtan som svårgripbar och svår att kommunicera. -En majoritet av patienterna som studerades kliniskt hade kvarstående smärta vid uppföljningen. De intervjuade patienterna uttryckte ett behov av att bli omhändertagna och hade ofta en begränsad förmåga att hantera smärtan konstruktivt. -De patienter som studerades kliniskt och medicinerade med centralt verkande läkemedel (sömnmedel, antidepressiva, lugnande eller starka smärtstillande tabletter) hade en risk för kvarstående smärta vid uppföljningen. De intervjuade patienterna förmedlade känslor av hopplöshet och hade en pessimistisk syn på framtiden. Att studera patienter med långvarig ansiktssmärta både från ett kliniskt perspektiv och från patientens eget perspektiv innebar en ökad förståelse för att långvarig ansiktssmärta är ett komplext tillstånd som påverkar patientens hela livssituation. Patienterna söker ofta tandvård pga sin ansiktssmärta. Det är därför angeläget att inom tandvården utveckla konstruktiva strategier för omhändertagandet, särskilt när sjukdomstillstånd i munhålan och angränsande vävnader är adekvat diagnostiserade och behandlade men patienten fortfarande har ont.

The second category was ‘The discoverable manifestations’,with two subcategories: (i) enigmatic communication; and (ii) expressions of bodilymemories. The dental appointment arouses similar psychological stressful reactionsas the episodes of abuse; both implicit and explicit expressions are recognizable.Dental staff can contribute to disclosure by improved understanding of the strain adental appointment can cause in patients who have been subjected to sexual abuseand familiarity with the associated bodily expressions.

    • The aim was to explore the experiences of sexually abused individuals as dental patients. Purposively selected were 13 informants (11 women) aged 19-56. All had experienced sexual abuse as children or adults and memories of this abuse had been triggered and expressed during a dental appointment. They were encouraged to relate in their own words their experiences of the dental appointment. The interviews were recorded digitally, transcribed verbatim, and analysed according to Qualitative Content Analysis. The overall theme illustrating the latent content was The dental appointment - a volatile base requiring predictability and a secure working alliance. The first category covering the manifest content was The dental care provider "assumes responsibility," with two subcategories: (i) contradictory disclosure, and (ii) alliance formation - a levelling of power. The second category was The patient is "in focus," with two subcategories: (i) alertness to signs of discomfort, and (ii) attention to obvious but subtle expressions of needs. On an understanding that the patient has been sexually abused, an individually tailored, patient-centered approach to treatment is suggested. Dental care providers may also need to be aware of and reflect on their position of power, in relation to the patient and its possible chairside implications.

  • Long-lasting orofacial pain : a study of 109 consecutive patients referred to a pain group

    • The aim was to register and evaluate the orofacial pain condition, with special focus on the equalities/inequalities between the sexes, in consecutive patients referred to the pain group at the Centre for Oral Health Sciences in Malmö, Sweden. The 109 patients, 85 (78%) women and 24 (22%) men, were referred to the pain group during the years 1988 to 1993 and were clinically examined using a standardised procedure. Pain duration of one year or more was experienced by 83% of the patients. The pain onset was related to dental care or toothache by 56% of the patients. Temporomandibular disorders were the most common diagnoses (52%) and were more common among women than among men (p < 0.05). Multiple pain locations were frequently reported, range 1-47 locations, and the women had significantly (p < 0.05) more pain locations than the men. Equalities between the sexes were more frequent than inequalities among patients referred for long-lasting orofacial pain. It can be discussed whether the sex difference in the prevalence of pain of musculoskeletal origin emerges from biological differences or can be explained by different psychosocial circumstances.

    • The aims were to follow-up, analyse and compare the pain status after 4-9 years with that at the baseline examination of 109 consecutive patients referred to the Pain Group at the Faculty of Odontology in Malmö, Sweden during the period 1988-1993 due to long-lasting orofacial pain. A further aim was to identify predictive factors of significance for pain alteration. 85 (78%) women with a median age of 51 years and 24 (22%) men with a median age of 60 years were included in the study. A survey of the pain status at the follow-up was conducted by means of a mailed questionnaire. The questionnaire covered the following aspects: pain alteration, pain intensity, pain location, medication and education. After one reminder, the non-responding patients were called for a telephone interview. A response level of 85% was obtained. Significant improvements were noted by the patients in the answers of the questionnaire in mainly three areas; the patients answered individually that pain relief had occurred, pain intensity rated on the VAS was lower at follow-up compared to the baseline examination and a decrease in drug use was reported. The responses indicated pain relief for 75% of the patients. However, only 27% of the patients experienced total disappearance of pain. Medication at baseline with opioids, muscle relaxants with central effect, antidepressants, neuroleptics, hypnotics or sedatives was found to be a predictive factor for persistent pain.

    • Kronisk ansiktssmärta är ett komplext tillstånd med konsekvenser som påverkar hela livssituationen. Syftet med studien var att med ett kvalitativt angreppssätt analysera upplevelsen av vardagssituationer för patienter med svårförklarlig kronisk ansiktssmärta. Elva kvinnor och tre män (21 till 77 år) valdes strategiskt ut bland de patienter med kronisk ansiktssmärta som remitterats till Orofaciala smärtenheten vid Odontologiska fakulteten, Malmö högskola, Malmö, Sverige. Samtliga utvalda informanter accepterade att delta. Data samlades in genom två tematiska djupintervjuer med varje informant. Intervjuerna spelades in med en digital ljudupptagare och skrevs ut ordagrant. Den text som berörde vardagssituationer identifierades i samtliga intervjuer och analyserades med kvalitativ innehållsanalys. Analysen av informanternas berättelser om vardagssituationer avslöjade en rädsla för konflikter, för den egna svagheten och för det okända. Analysen avslöjade också att informanterna uttryckte självanklagelser och att de undvek situationer som kunde orsaka rädsla. Åtta av de 14 informanterna nämnde inte någon vardagssituation de kände sig tillfreds med. Informanternas upplevelser av vardagssituationer dominerades av negativa känslor. Sammanfattningsvis kan kronisk långvarig svårförklarad ansiktssmärta inte förstås som ett isolerat fenomen utan måste beaktas i relation till den individ som drabbats av tillståndet.

    • Aims: To analyze the nonspecific chronic orofacial pain patient's experience of the pain condition and to gain knowledge on the complexity of the problem. Methods: Fourteen patients (1 I female, 3 male) aged 21 to 77 years were selected among those referred to a specialist clinic. All selected patients agreed to participate. Data were obtained through thematic in-depth interviews that exposed the context of the orofacial pain condition. The 2 interviews with each patient were audiotaped and transcribed verbatim. The text material was analyzed using a qualitative research strategy based on phenomenology. Results: The essence of the chronic orofacial pain was expressed by the patients as something that eludes perception and comprehension. The pain was difficult to grasp and to communicate, The consequence of the pain was experienced by the patients as to be stricken by the pain and was expressed as living a life permeated by hopelessness, resignation, and a lack of faith. Conclusion: The patients in this study experienced their chronic orofacial pain to have no limits and to repressively permeate all aspects of their existence: social, practical, and emotional.     • Aims: To use a qualitative research study to analyze the experiences of patients with nonspecific chronic orofacial pain with respect to consultations for their pain condition. Methods: Fourteen patients (11 women and 3 men; age range, 21 to 77 years) were strategically selected through a purposive sampling of the chronic orofacial pain patients referred to the Orofacial Pain Unit at the Faculty of Odontology, Malmo University, Malmo, Sweden. A qualitative research strategy based on phenomenological philosophy was chosen. Thematic in-depth interviews were conducted twice with each patient in order to expose the context of the orofacial pain condition. The interviews were audiotaped and transcribed verbatim. The text material was analyzed to determine the attitude of the patients concerning their experience from the consultations. Results: All selected patients consented to participate. The patients expressed dissatisfaction with the consultations and related many examples of poor communication and understanding. The patients also felt a great need to be taken care of and expressed contradictory statements concerning pain improvement. Conclusion: The results suggest that the communication between the patients and the care providers was unsatisfactory and that the patients were limited in their ability to develop a personal coping strategy.

  • Sexual Abuse Experiences and Dental Care Visits - Challenging!

    • Objectives: Victims of sexual abuse are more likely to experience dental fear as well as to suffer from an impaired oral health. Objective. To explicate and describe the persistent psychological and bodily memories of sexual abuse and how it is expressed during the visit to the dentist. Methods: Thirteen participants (11 women) who experienced sexual abuse as children or adults and in whom the abuse experiences were remembered and expressed during a dental appointment, were interviewed. The informants were encouraged to tell in their own words and in as much detail as possible about situations at the dental appointment when they were reminded of the sexual abuse experiences. To provide for as much empirical variation of the participants, some strategic selection criteria were applied; sex, age and class. The interviews were recorded digitally and transcribed verbatim. The collected material was analyzed according to Qualitative Content Analysis. Results: A pattern in the subjects’ experiences was identified and classified into the following theme and categories. The overall theme illustrating the latent content was An echo of sexual abuse transformed into (dys) functional reactions. The first main category covering the manifest content was The inner invisible struggle with two subcategories: (i) the abuse experience mental inscriptions and (ii) the consequences of the dental encounter. The second category was The discernible manifestations with two subcategories: (i) the enigmatic communication and (ii) the bodily expressions. Conclusions: During dental care the same reactions and negative emotions may arise as during abuse occasions. In the context of the abuse these are adequate defense responses, but in the dental clinic they are in a strict sense not adequate. The dental care situation contains both implicit and explicit expressions that are recognizable for dental staff. With knowledge of the pattern and expressions, dental care can provide better treatment and care.

    • Background/Aim: To analyse, from the perspective of adults with a history of sexual abuse, the impact of the experience on their general health and well-being.Material and Methods: The participants comprised 12 strategically selected informants (10 women), aged 19-56; all sexually abused during child- and/or adulthood. They were interviewed in-depth and encouraged to describe the impact of the sexual abuse on their daily lives. The consequences of sexual abuse on oral health have previously been reported. The interviews were recorded digitally, transcribed verbatim and analysed according to qualitative content analysis with an inductive approach.Results: The overall theme illustrating the latent content was Sexual abuse experience-limiting long-term consequences, always present in body and mind. The first category covering the manifest content was 'A lost foothold', with two subcategories: (i) emotional repercussions and (ii) physical health repercussions. The second category was 'The significance of distance to trauma', with two subcategories (i) keeping a distance and tending to escape and (ii) processing the trauma experience-a struggle towards balance.Conclusions: A history of sexual abuse cannot be understood in isolation: the long-term repercussions pervade daily life. However, after disclosure of the abuse and processing the trauma, it is possible to ameliorate some of the negative effects. In this context, the dental setting emerges as a potentially important venue for disclosure as almost every person, sooner or later, visits the dentist.

    • Background: Optical coherent tomography (OCT) technology is evolving with improved resolution and accuracy in segmentation between different cell layers in the retina. The ganglion cell layer in the macula region is a window to see what is happening in the visual pathways and a macula OCT is an examination that most children tolerate. This makes updated normative data necessary since variables may differ between different OCT devices and normative data for children is often not presented.The aim of this study was to develop normative data for macular ganglion cell layer thickness in children, measured with swept source OCT, and investigate the repeatability between measurements.Methods: Healthy Swedish children between 4 and 16 years old, with normal refraction, spherical equivalent mean:1.13 (sd:0.66) dioptre and normal visual acuity Logmar, mean: 0,015 (sd:0,05), were examined with swept source OCT. Macula OCT was performed three times in both eyes and the different retinal layers were evaluated.Results: Fifty-eight children were screened for inclusion. Fifty-five children were included in the study, 24 boys and 31 girls. Mean age was 8.9 years. Results from right eyes was analysed. The mean average thickness of macular ganglion cell layer thickness, retinal nerve fibre layer/ganglion cell layer boundary to inner plexiform layer/inner nuclear layer boundary, was 68.0 μm (sd:4.0, range:58-77). There was no correlation with sex or age. Fifty-three children manage to complete two, and 41 children three acceptable measurement and the mean coefficient of variation was low.Conclusion: The ganglion cell layer thickness differs according to which OCT device that is used, and the population examined. This makes normative data essential to accurately interpret results. Swept source OCT of the macula have excellent repeatability and the examination well tolerated in most children making it an investigation useful for diagnosing and following diseases in the optic pathways.

    • Pediatric movement disorders are still a diagnostic challenge, as many patients remain without a (genetic) diagnosis. Magnetic resonance imaging (MRI) pattern recognition can lead to the diagnosis. MEGDEL syndrome (3-MethylGlutaconic aciduria, Deafness, Encephalopathy, Leigh-like syndrome MIM #614739) is a clinically and biochemically highly distinctive dystonia deafness syndrome accompanied by 3-methylglutaconic aciduria, severe developmental delay, and progressive spasticity. Mutations are found in SERAC1, encoding a phosphatidylglycerol remodeling enzyme essential for both mitochondrial function and intracellular cholesterol trafficking. Based on the homogenous phenotype, we hypothesized an accordingly characteristic MRI pattern. A total of 43 complete MRI studies of 30 patients were systematically reevaluated. All patients presented a distinctive brain MRI pattern with five characteristic disease stages affecting the basal ganglia, especially the putamen. In stage 1, T2 signal changes of the pallidum are present. In stage 2, swelling of the putamen and caudate nucleus is seen. The dorsal putamen contains an "eye" that shows no signal alteration and (thus) seems to be spared during this stage of the disease. It later increases, reflecting progressive putaminal involvement. This "eye" was found in all patients with MEGDEL syndrome during a specific age range, and has not been reported in other disorders, making it pathognomonic for MEDGEL and allowing diagnosis based on MRI findings.
