    • In a prospective study, 60 consecutive patients underwent a distal iliotibial band transfer for anterolateral knee instability. Knee function was evaluated with a score system, static stability tests and a standardized test including thigh muscle measurements, a one-leg-jump-test and a figure-of-8 running test. At the 40-month follow-up there was a significant increase in the mean knee score. The quadriceps in the treated leg was significantly weaker than in the non-operated leg, and the quadriceps strength was significantly correlated to the knee score. The functional outcome after operation was generally unsatisfactory. Few patients attained normal values in all tests, primarily because of poor restoration of stability.

    • The gleno-humeral joint space in standard antero-posterior images of 175 normal subjects was measured at three different sites. The average of the three measurements, the integral joint space, was calculated and was found to be between 4 and 5 mm. The value does not change with age, except in women, in whom it increases slightly. A narrow joint space in an elderly patient should not be expected simply because the patient is old. The measuring technique described is useful in population studies. In individual cases a simple measurement with a ruler serves the same purpose.

    • The acromioclavicular joint space in standard antero-posterior images of 151 normal subjects was measured. The joint space was significantly wider in men. There was a highly significant reduction of the joint space with age in both men and women and in persons past 60 a joint space of 0.5 mm or less is not pathological. A joint space wider than 7 mm in men and 6 mm in women is abnormal. The measuring technique described is useful in population studies. In individual cases a simple measurement with a ruler serves the same purpose.

    • The subacromial space in standard anteroposterior radiographs of 175 normal shoulders was between 9 and 10 mm. The space was significantly greater in men, with a slight reduction with age. In middle age, a subacromial space less than 6 mm is pathological, possibly indicating supraspinatus tendon rupture.

  • Two-year follow-up of conservative treatment of knee ligament injuries

    • Sixteen patients with old knee ligament injuries and symptoms of instability or pain were treated with a 3-month thigh muscle strength training program. Nine patients had a tear of the anterior and six patients a tear of the posterior cruciate ligament. One patient had a tear of both cruciates. Knee function was determined with a knee scoring scale, and thigh muscle strength with a Cybex-II dynamometer before training, after 1 and 3 months of training, and at a late follow-up after 2 years. Ten patients who increased their quadriceps strength by more than 15 per cent increased their score over 30 per cent. Three patients who showed a minor increase in strength did not increase their score significantly. Three patients did not increase their strength at all. All of these admitted a reluctance to train. Four patients, all with anterior cruciate ligament tears, were operated on after the 3-month training period. All four patients increased their strength. Two of them increased their functional score also, but they strove for a very high activity level and were therefore operated on. The other two patients had no symptomatic relief and were therefore also operated on. Improvements in muscle strength and knee function were unchanged at the 2-year follow up. Before planning a knee ligament reconstruction, a period of strength training is recommended.

  • Rising incidence of hip fracture in Uppsala, 1965-1980

    • The frequency and incidence of hip fracture in persons aged 55 years and older in the county of Uppsala during the years 1965, 1970, 1975 and 1980 are reported. For every 5-year interval, the number of hip fractures increased by 21-25 per cent. The ratio of women to men changed from 3.8 in 1965 to 3.1 in 1980. Trochanteric fractures were more common during the later years. The ratio of femoral neck fractures to trochanteric fractures decreased from 1.8 to 1.1 between 1965 and 1980. The incidence of hip fracture in the investigated part of the population increased from 43 per 10 000 in 1965 to 65 in 1980. The age-specific incidence increased especially in the group aged 85 years and older, in which fractures of the femoral neck were three times and trochanteric fractures four times more common in 1980 than in 1965. This investigation shows that the incidence of hip fracture has increased, particularly in the higher age groups. If the age-specific incidences continue to rise in the higher age groups, the frequency of hip fractures will be doubled within a 20-year period.

    • A prospective, randomized trial of the effect of graded compression of the calf was done in 62 patients operated on electively for hip disease. After total hip arthroplasty, compression reduced the number of positive fibrinogen uptake tests by two-thirds, significant only in males, who seem to run a higher risk of thrombosis than females.
