    • Implantation of demineralized bone matrix in rodents elicits a series of cellular events leading to the formation of new bone inside and adjacent to the implant. This process is believed to be initiated by an inductive protein present in bone matrix. It has been suggested that local growth factors may further regulate the process once it has been initiated. This investigation was designed to study the effect of adding a growth factor to the inductive implant. Pairs of demineralized rat femoral diaphyses were implanted intramuscularly in rats. the marrow canal of one implant in each pair was filled with a carboxymethyl cellulose gel containing 75 ng of recombinant human basic fibroblast growth factor (FGF). the other implant in each pair served as a control. It was either filled with the gel without FGF or left untreated. Bone formation was induced by all the implants after 3 weeks. the amount of mineralized tissue in the FGF-treated implants was 25 percent greater than in untreated controls. the carboxymethyl cellulose gel alone did not affect the bone yield.

    • Rabbit and rat bone matrix were implanted in athymic rat muscle, and the bone yield was measured as total calcium content after 4, 6, and 8 weeks. Matrix from both species induced equal amounts of new bone in the athymic rat. In rabbit and normal rat, the xenogenic matrix induced little or no bone formation. Thus, in the case of rabbit and rat, bone induction is species specific due to immunogenic mechanisms. the athymic rat can be used to measure inductive properties of bone matrix from different species.

    • During 1980-82, 739 ankle fractures occurred in Malmo. These were compared with 383 ankle fractures occurring in 1950-52. In men there was an increase in the age-specific incidence up to the age of 60, especially in fractures of the lateral malleolus. In women there was an increase in the age-specific incidence above the age of 50, both in lateral malleolar fractures and, even more pronounced, in bi- and tri- malleolar fractures.

    • The age and sex specific incidence of tibial condyle fractures and patellar fractures were compared between 1950-55 and 1980-83 in the urban population of Malmo. In elderly women there was an increased incidence over these 30 years for both fracture types.

    • The normal pinch strength was determined with the Mannerfelt-Ulrich intrinsic meter in 90 men and women aged 21-65 years. The pinch strength decreased with increasing age. The ratio dominant/nondominant hand was 1.12, 0.13 (mean, SD) for both men and women. The ratio would be useful in evaluating pinch strength under pathologic conditions.

    • A 9-year-old boy sustained a Salter-Harris Type IV fracture in the proximal tibia. the injury resulted in a progressive valgus deformity, detected already after 3 months by roentgen stereophotogrammetry. After temporary stapling medially, the growth rate increased laterally and the deformity was corrected. Unilateral stapling is an alternative procedure for correcting angular deformity following a physeal injury.

    • Forty patients who had had a patellar fracture during the years 1950-58 were reevaluated 30 years later. A clinical and radiographic examination was performed. Fourteen patients had subjective complaints. Two thirds of the patients who had more than 2-mm diastasis or 1-mm incongruity had complaints and reduced quadriceps strength. Radiographically, all the patients had a reduction in the lateral patellofemoral distance in the axial view with the greatest reduction in the knees with diastasis or incongruity of the fracture.

    • As pointed out in the preceding review (Nachemson and Rydevik 1988), the most serious and common complications of chymopapain treatment of patients with sciatica due to disc herniation are cerebral or subarachnoid hemorrhages and allergic manifestations. Recent progress in biochemical and physiologic studies of cysteine proteinases and their inhibitors allows some speculation on how these complications in the future might be alleviated or avoided and on how the efficiency of proteinase treatment of patients with sciatica might be increased.

    • Growth during adolescence was analyzed in 40 children with slipped capital femoral epiphysis using the Infancy Childhood Puberty growth model. the advantage of this model is that reference values for height can be adjusted for the individual age at pubertal maturation. In both sexes an above average height was found before the onset of the pubertal growth spurt. However, the pubertal gain in height was less than normal, and at maturity the heights were only slightly above the reference mean values. Most of the children had their first hip symptoms during the period of increasing gain in height at the initial stage of the pubertal growth spurt. Both boys and girls were overweight before puberty and remained so during puberty and at maturity.

    • 32 patients, suffering from Parkinson's disease, had internal fixation of femoral neck fractures. In 24 displaced fractures, 6 nonunions and 3 segmental collapses were seen; and in 8 undisplaced fractures, 1 case of segmental collapse was diagnosed. Healing complications were thus seen in one third. Total hip replacement for healing complication was performed in 3 of 32 patients. 9 patients died within 2 years. No difference in the rate of healing or mortality was detected compared with hip fracture patients without Parkinson's disease. Our study does not support primary arthroplasty for femoral neck fracture in patients with Parkinson's disease.

    • A good system for evaluating the degree of impairment, disability, and handicap of the patient with a cruciate ligament injury includes functional score, activity grading, stability testing, and measurements of performance and strength, all of which are relevant to different aspects of knee function. The symptom-related knee score gives a more differentiated picture of the disability than does a binomial rating of symptoms. A way of grading the disability in an objective way is to use a performance test. This test could also be used for monitoring rehabilitation before full activity has been resumed. The activity grading scale is very useful for grading the handicap

    • Fifty-three consecutive patients with troublesome old cruciate ligament lesions underwent a 3-month thigh and calf muscle training program. Before training, the diagnosis was established by arthroscopy and clinical examination under anesthesia. Significant improvement in strength, performance, knee score, and activity level took place; the majority were improved and declined surgery. A period of strength training is recommended before the decision to undertake surgery for cruciate ligament injury.

  • The effect of derotation braces on knee motion

    • Four different types of derotation braces and an elastic knee support were tested on ice-hockey players. The elastic support did not noticeably affect rotation and abduction-adduction of the knee. All four braces reduced rotation and abduction-adduction in test actions simulating sports situations. Flexion-extension was slightly affected by two of the individually made braces in one action. Running a figure eight was slower with two of the individually made braces. The best braces, one individually made and one ready-made, limited rotation and abduction-adduction effectively, but did not affect performance. Minor differences in design may account for differences in effect and may alter the protection afforded by a brace.

    • The Swedish Orthopaedic Society held its meeting on September 10-12,1986, in Östersund, which is situated on a large lake in central Sweden. This year, Ostersund celebrated its bicentennial; but more than 1,000 years ago the Vikings had a center here to worship their gods, as well as a fortress on the neighboring island of Frösö. Today, Östersund is the gateway to Sweden's most popular winter recreational area. Östersund has a single hospital that serves 135,000 inhabitants living in a 50,000 square kilometer catchment area. Finally, the meeting gathered 220 Swedish orthopedic surgeons. The Östersund Orthopedic Clinic, headed by Helge Semb, has, apart from the treatment of ski injuries, become renowned for its rheumatoid surgery. The latter topic was manifested by the invited speaker Professor Norbert Gschwend, Zurich, Switzerland, whose talk was titled “Surgery in rheumatoid arthritis,” and which was followed by papers on the subject. In addition, a special symposium entitled “Orthopedics - does it give cost benefit?” was held, and is reflected in the abstracts printed here. Further, the symposium included a debate in which the panel also consisted of health care administrators and politicians under the moderation of Erland Lysell, President of the Swedish Orthopedic Society. Other topics and contributions of this well-organized meeting are evident from the abstracts below. Finally, three greatly appreciated video tapes on orthopedic emergency care were presented by Jan Sandegård, Östersund.
