  • Lessons for sleep medicine learned from the COVID-19 pandemic

    • During the coronavirus disease 2019 (COVID-19) pandemic, sleep medicine services were dramatically reduced worldwide. Implementation of mitigation strategies was suggested by expert organisations and helped to restart sleep medicine services. Distance communication and monitoring during diagnosis, treatment initiation, and treatment follow-up have been used widely, in particular for patients with sleep apnoea. Follow-up studies demonstrate both advantages and potential disadvantages with the use of these new technologies. Emerging epidemiological evidence suggests that obstructive sleep apnoea may be an independent risk factor for adverse outcome in COVID-19, including an increased risk for hospitalisation and elevated mortality. Experimental studies explaining the underlying mechanisms behind these findings are warranted. © ERS 2022.

    • Every year, the European Respiratory Society (ERS) organises the Lung Science Conference (LSC), in Estoril, to discuss basic and translational science. The topic of the 20th LSC was “Mucosal immunology of the lung: balancing protective immunity and chronic inflammation”. This was the first time that the LSC was organised as a hybrid congress with both in person and online attendance. In addition to an outstanding scientific programme, the LSC provides excellent opportunities for career development and inclusion of early career members (ECMs). All scientific and poster sessions are chaired by an ECM who is paired with a senior faculty to allow ECMs to become acquainted with session chairing, and there is a session organised by the Early Career Member Committee (ECMC) dedicated to career development. Moreover, travel bursaries are made available to abstract authors, and all bursary recipients and first-time attendees are invited to take part in a mentorship lunch. In this article, we provide the names of the ECM awardees and describe the scientific highlights of the LSC 2022 for those who could not attend.

  • Greenness exposure : beneficial but multidimensional

    • Many studies have shown that greenness has beneficial health effects, particularly on psychological and cardiovascular outcomes. In this narrative review, we provide a synthesis of knowledge regarding greenness exposure and respiratory health. The following outcomes were reviewed: respiratory mortality, lung cancer mortality, lung cancer incidence, respiratory hospitalisations, lung function, COPD, and asthma. We identified 174 articles through a literature search in PubMed, of which 42 were eligible for inclusion in this review. The most common marker for greenness exposure was the normalised difference vegetation index (NDVI), which was used in 29 out of 42 papers. Other markers used were tree canopy cover, landcover/land-use, plant diversity, density of tall trees and subjectively perceived greenness. We found beneficial effects of greenness in most studies regarding respiratory mortality, lung cancer incidence, respiratory hospitalisations and lung function. For lung cancer mortality, asthma and COPD, the effects of greenness were less clear cut. While many aspects of greenness are beneficial, some aspects may be harmful, and greenness may have different health effects in different population subgroups. Future studies of greenness and respiratory diseases should focus on asthma and COPD, on effects in different population subgroups and on disentangling the health effects of the various greenness dimensions.

In addition, 40 travel bursaries are made available to abstract authors and all bursary recipients are invited to take part in a mentorship lunch. Moreover, there is a session organised by the Early Career Members Committee (ECMC) dedicated to career development. Here, we describe the scientific highlights of LSC 2020 for those who could not attend. The ERS presents several awards at the LSC and here we will highlight all winners of the LSC 2020 awards. The five highest ranked abstracts from ECMs are presented during the Young investigator session. Patricia Ogger (UK) was presented with the William MacNee Award for the best presentation in this session. Several abstracts were selected for programmed oral presentations and Renata Jurkowska (UK) was presented with the inaugural Geoffrey Laurent Award for the best oral presentation. Moreover, the organisers presented eight Distinguished Poster awards to Anne-Sophie Lamort (Germany), Julia Frankenberg Garcia (UK), Johnatas Silva (UK), Pauline Esteves (France), Claudio Bussi (UK), Elodie Picard (France), Felix Ritzmann (Germany) and Alen Faiz (Australia) for their excellent contributions during the poster session.

  • Holistic management of patients with progressive pulmonary fibrosis

    • Progressive pulmonary fibrosis (PF) is a complex interstitial lung disease that impacts substantially on patients’ daily lives, requiring personalised and integrated care. We summarise the main needs of patients with PF and their caregivers, and suggest a supportive care approach. Individualised care, education, emotional and psychological support, specialised treatments, and better access to information and resources are necessary. Management should start at diagnosis, be tailored to the patient’s needs, and consider end-of-life care. Pharmacological and non-pharmacological interventions should be individualised, including oxygen therapy and pulmonary rehabilitation, with digital healthcare utilised as appropriate. Further research is needed to address technical issues related to oxygen delivery and digital healthcare.

    • Every year, the European Respiratory Society (ERS) offers grants to recognise the best overall abstracts of the 14 ERS Assemblies submitted for the ERS International Congress, covering all respiratory areas. The authors of the best abstract (i.e. the highest average score of abstract reviewers and only those who have not applied or were not eligible for a sponsored award) were invited to write a short summary about their virtual Congress experience and view on the evolving field of research in light of their respective Assembly. This article provides an overview of some of the Congress highlights and gives the stage to the promising best-abstract awardees as they are the future of the ERS.

    • For patients with end-stage respiratory diseases, such as COPD, interstitial lung diseases and cystic fibrosis, lung transplantation remains the only treatable option. However, due to increasing demand and limited availability of donor lungs, risk of complications such as acute and chronic rejection, and adverse effects of immunosuppressive treatments, this is not an alternative for the majority of this patient group [1, 2]. To meet the rising clinical demand new strategies to increase the number of available lungs for transplantation are needed [2]. One such strategy involves creating a functional lung ex vivo using different de- and recellularisation approaches. In this article, we will provide an overview of three landmark studies on bioartificial lungs published during 2010 that set the base for the direction of this relatively young field.

  • Need and baseline for harmonising nursing education in respiratory care : preliminary results of a global survey

    • Background The COVID-19 pandemic confirmed that respiratory nurses are critical healthcare providers.Limited knowledge is available about appropriate education to prepare nurses to deliver high-qualityrespiratory care. A survey was developed by the International Coalition for Respiratory Nursing (ICRN)group to identify the need for a respiratory nursing core curriculum.Method A 39-item survey was distributed to 33 respiratory nursing experts in 27 countries. Questionsasked about current roles, perception of need, expectations for a core curriculum project and respiratorycontent in nursing education in their countries.Results 30 responses from 25 countries were analysed; participants predominantly worked in academia(53.3%, 16/30) and clinical practice (40%, 12/30). In total, 97% (29/30) confirmed a need for a corerespiratory nursing curriculum. Post-registration nursing programmes at bachelor (83.3%, 25/30) andmasters (63.3%, 19/30) levels include internal/medical nursing care; less than half identified separaterespiratory nursing content. The core educational programme developed should include knowledge (70%,21/30), skills (60%, 18/30), and competencies (50%, 15/30), with separate paediatric and adult content.Conclusion Survey results confirm a wide variation in nursing education and respiratory nursing educationacross the world, with many countries lacking any formal educational programmes to prepare nursescapable of providing enhanced quality respiratory care. These findings support the need for a corerespiratory curriculum. To advance this significant work the ICRN group plans to conduct a Delphi studyto identify core curriculum requirements for respiratory nursing education at pre-registration and advancededucational levels to flexibly meet each country’s specific educational requirements for recognition ofrespiratory nursing speciality practice.
