  • Otoneurological findings in psycho-organic syndrome caused by industrial solvent exposure

    • Nine subjects with long-term (8-30 years) occupational exposure to industrial solvents and a confirmed diagnosis of psycho-organic syndrome (POS) have been studied with audiological and otoneurological test batteries. The results were compared to a matched control group of nine industrial workers not exposed to solvents and to normal data from healthy volunteers. In the clinical examination, the Romberg test identified 5/9 workers as pathologic and concurrently the stabilometry showed significantly larger sway areas in the POS-group. In the audiological test battery, the significantly pathologic tests were discrimination of interrupted speech and evoked cortical responses to frequency glides (CRA-delta-f). The saccade test disclosed abnormal findings in 5/9 workers. In the smooth pursuit test, abnormality was found at some test frequencies using pseudorandomized stimulus. The VOR-suppression test was significantly abnormal at all test frequencies. The test battery used strongly indicates CNS lesions due to industrial solvents.

    • Three groups of subjects with long-term (5-41 years) occupational exposure to industrial solvents have been evaluated with extensive audiological and vestibular test batteries. Group A comprised 16 subjects with a confirmed diagnosis of psycho-organic syndrome (POS), while group B consisted of 7 subjects with suspected POS. Both groups had been exposed to mixtures of aliphatic and aromatic solvents. Eight subjects with long-term exposure to jet fuel constituted group C. In the audiological test battery, discrimination of interrupted speech and evoked cortical potentials in response to frequency glides were the tests that yielded significantly abnormal results. In the vestibular test battery, considerable pathology was seen in electronystagmography, and in addition, visual suppression test and saccade test indicated CNS disturbance. In general, when a test yielded pathological results, the incidence of pathology was highest in group A and lowest in group C.

    • The method of broad-frequency-band rotatory testing with results from normal subjects and patients with peripheral uni- and bilateral loss as well as central vestibular disorders are reviewed. The following conclusions are drawn: adequate testing of the vestibulo-ocular reflex including side detection of unilateral loss in light or in darkness can only be done with either random or high-frequency (2.5–3 Hz) sinusoidal stimulation. Measurements of compensatory eye movements at lower frequencies where vestibular and non-vestibular signals interact are of interest for central vestibular diagnosis. A decreased ability to suppress vestibular nystagmus is not an uncommon finding in patients with large acoustic neuromas or pathology in the brainstem or cerebellum.

    • Fifty-five patients with a unilateral acoustic neuroma were investigated preoperatively with visual suppression tests during rotatory oscillation and caloric irrigation. During a sinusoidal oscillation, 29% of the patients showed a reduced suppression compared to 9% during pseudo-random oscillation and 11 % in the caloric test. In the sinusoidal and caloric tests the majority of the patients with deficient suppression had large or medium-sized tumors. In a few patients with small tumors, pathology was observed in both sinusoidal and randomized tests, presumably as a sign of unconcentration. The study shows that the sinusoidal visual suppression test and to a lesser degree suppression during caloric irrigation are of value for identification of brainstem-cerebellum compression of acoustic neuromas.

  • Effects of Trichloroethylene on the human vestibulo-oculomotor system

    • Ten healthy volunteers were subjected to a vestibulo-oculomotor test battery before, during and 1 hour after trichloroethylene exposure. The concentration of trichloroethylene in in-spiratory air was 32–78 ppm (176429 mg/m3). The concentration of trichloroethylene in venous blood was followed throughout the experiment. The mean pulmonary uptake was estimated. Each test person was also subjected to a control experiment, breathing air free of trichloroethylene. A decreased ability to visually suppress the vestibulo-oculomotor reflex during sinusoidal stimulation was noticed during trichloroethylene exposure. One hour after exposure the test subjects showed a decreased maximum velocity of the voluntary saccade and a decreased ability to follow a sinusoidally moving target.

    • Human tracking eye movements were recorded in healthy volunteers of varying age. The target motion was either predictable (pure sinusoids or sinusoidal frequency sweep) or unpredictable (pseudorandomized). Gain and phase values for the tracking and the smooth pursuit eye movement signal were calculated as a function of target frequency from 0.2 to 2 Hz. Max. target velocity was 20 or 40 deg/sec. In the low frequency area the smooth pursuit gain for predictable stimulation was higher than the gain elicited by the pseudorandomized stimulation. In the high-frequency area, gain values did not differ significantly. For the predictable stimulation, the phase of the smooth component was always a lag, increasing with increasing frequency. At low frequencies of the pseudorandomized signal a phase lead was observed. At higher frequencies the lead turned into a lag that was greater than for the predictable stimulation. The young and old groups showed reduced smooth pursuit gain values, compared with the main group specially when the target waveform was pseudorandomized. The reduced smooth pursuit ability was mostly compensated for by an increased amount of saccades.

    • The short-term effect of exposure to toluene on the vestibulo- and opto-oculomotor system in rats was investigated. Stimulation of the vestibulo- and opto-oculomotor system was either a constant rotatory acceleration, a sinusoidal oscillation, a randomized oscillation or an optokinetic stimulation. Eye movements in response to the different stimulations were recorded by EOG and fed into a computer for analysis. Due to toluene exposure the slow phase velocity gain during constant acceleration increased and the post-acceleratory nystagmus response was prolonged. The optokinetic gain at stimulation velocities above 10 deg/sec decreased during toluene exposure. Toluene also prolonged the duration of optokinetic after-and after-afternystagmus. The findings in this study strongly suggest an effect of toluene on the cerebellum.

    • Postural control was investigated by static posturography in 18 workers exposed to industrial solvents, 9 patients with psycho-organic syndrome due to industrial solvent exposure, and 52 controls. Both groups of exposed subjects showed larger sway areas with eyes open as well as closed, compared with the controls. No differences were found in the Romberg quotient (the relationship between sway areas with eyes closed/open). The correlation between static posturography and the otoneurological test battery was positive for the visual suppression test in the styrene group. In the industrial solvent group no significant correlations were found. The visual suppression test and some auditory tests were pathological in the exposed groups. The otoneurological test battery—especially the visual suppression test and static posturography—seems to contribute in the assessment of solvent related CNS lesions.

    • Twelve subjects with bilateral vestibular loss, with a mean age of 27 years (18-49) were studied. The loss was based on symptoms of oscillopsia and ice-water caloric tests. Nine subjects were evaluated by low-frequency sinusoidal harmonic acceleration (SHA) rotatory tests (0.01-0.32 Hz), and 3 subjects with high broad-frequency band rotatory tests (0.25-3.25 Hz). During alertness tests (darkness) all subjects had absent or very low gains. When tested with a stationary light in the middle of the swing, the SHA tests showed perfect compensatory eye movements with gains of 1 and phases of approximately 0 degrees. In the broad-frequency test, the gain was near 1 below 1 Hz, decreasing rapidly at higher frequencies. In imaginary stationary target tests (darkness), the gain was enhanced to 'normal' values (0.5) with a phase lead in the low-frequency range (SHA). This could not be replicated in the high-frequency tests. Testing with stationary acoustic guidance (darkness), further enhanced the gain in the SHA tests. A small increase of gain could also be found in the high-frequency tests. Compensatory eye movements, when tested in low-frequency rotatory tests (less than 1 Hz), are to a large extent influenced by non-vestibular mechanisms. By voluntary modifications, normal gains could be produced by patients with 'bilateral vestibular loss'. The phase lead found is proposed to be of central non-vestibular origin. Testing in higher frequency ranges (greater than 1 Hz) could not replicate these findings, thus the broad-frequency band rotatory test should be preferred for adequate vestibular quantification.

    • Usher syndrome is an autosomal recessive disorder characterized by severe hearing loss or deafness and retinitis pigmentosa. Eleven families with 25 affected members were studied. The test battery included genetic studies, clinical examination, audiological, ophthalmologic, and otoneurological tests, and magnetic resonance imaging. Sixteen affected persons had profound hearing loss or were considered anacusic, with absent bilateral vestibular responses. These patients had varying degrees of retinitis pigmentosa. These 16 patients were considered to have type I Usher syndrome. Nine persons were diagnosed as Usher type II with a moderate to profound hearing loss, normal vestibular function, and retinitis pigmentosa of varying degree. Magnetic resonance imaging was normal in all cases. Otoneurological tests indicated no central nervous system disturbances. The conclusion is that hearing loss and balance problems in Usher syndrome are due to inner ear damage with no evidence of central nervous system disturbances. Furthermore, the ataxia seen in Usher type I is due to a combination of retinitis pigmentosa and bilateral peripheral vestibular deficiency.

    • Fifteen patients with disabling Meniere's disease were treated with local intratympanic administration of gentamicin once daily. They had suffered from frequent attacks of vertigo and vomiting, the hearing in the affected ears was decreased permanently, and spontaneous nystagmus was observed. The number of treatment days ranged between 3 and 11. Follow-up time was 1 to 6 years. For evaluation of the treatment, audiologic and vestibular examinations were used, including the broad frequency-band rotatory test (0.4-4.5 Hz). Fourteen patients were free from vertigo after treatment. In 5 patients, there was an increased hearing loss, and in 10 it remained unchanged. Tinnitus and fullness sensations were diminished. After treatment, all ears were unresponsive to caloric stimulation. The clinical examination and rotatory testing in light with sinusoidal stimulation revealed good central compensation of the vestibular loss. However, with pseudorandomized oscillations in darkness, the broad frequency-band rotatory test quantified the loss of peripheral vestibular function and was able to detect the side of the lesion in eight of nine patients.

    • Eye movements were recorded from rats with a magnetic search coil system before and after sectioning of the midline commissural pathways in the brain stem at the level of the vestibular nuclei. After lesion, the findings were as follows: 1) During sinusoidal vestibular stimulation the eyes moved in a sinusoidal way similar to the head movement without any regular saccades. There was a reduced gain and a phase lead. 2) During optokinetic stimulation the eyes moved in the stimulus direction to an excentric position and stayed there until stimulation ceased. 3) During acceleratory/deceleratory rotation in the light there was a drift of the eyes in the direction of the expected slow phase movement to an excentric position. In some animals there was a directional asymmetry. The findings may be explained by a failure of the central neural integrator for horizontal eye movements. The results support the hypothesis that vestibular commissural fibres are of crucial importance for the function of this integrator system.
