    • Sepsis-induced acute kidney injury (SI-AKI) is common and associated with high mortality. Survivors are at increased risk of chronic kidney disease. The precise mechanism underlying SI-AKI is unknown, and no curative treatment exists. Toll-like receptor 4 (TLR4) activates the innate immune system in response to exogenous microbial products. The result is an inflammatory reaction aimed at clearing a potential infection. However, the consequence may also be organ dysfunction as the immune response can cause collateral damage to host tissue. The purpose of this review is to describe the basis for how ligand binding to TLR4 has the potential to cause renal dysfunction and the mechanisms by which this may take place in gram-negative sepsis. In addition, we highlight areas for future research that can further our knowledge of the pathogenesis of SI-AKI in relation to TLR4 activation. TLR4 is expressed in the kidney. Activation of TLR4 causes cytokine and chemokine release as well as renal leucocyte infiltration. It also results in endothelial and tubular dysfunction in addition to altered renal metabolism and circulation. From a physiological standpoint, inhibiting TLR4 in large animal experimental SI-AKI significantly improves renal function. Thus, current evidence indicates that TLR4 has the ability to mediate SI-AKI by a number of mechanisms. The strong experimental evidence supporting a role of TLR4 in the pathogenesis of SI-AKI in combination with the availability of pharmacological tools to target TLR4 warrants future human studies.

    • Laparoscopic hernioplasty has been criticized because of its technical complexity and increased costs. Disposable dissection balloons can be used to gain the initial working space in totally extraperitoneal endoscopic (TEP) hernioplasty, but this increases its cost. Forty-four men with bilateral, primary or recurrent inguinal hernias were randomized to undergo TEP with or without dissection balloon, There were two conversions to transabdominal preperitoneal hernioplasty, or open herniorrhaphy, in the group with balloon and four in the group without balloon. There was no difference in the postoperative morbidity or operation time between the two groups, and there were no major complications in either group. The recurrence rate was 4.3% in the group with the balloon and 7.1% in the group without the balloon. There were no statistically significant differences between the groups, Although our study population is too small to detect small differences between the groups, it seems that the use of a dissection balloon is not beneficial in a bilateral TEP.

    • Background: Laparoscopic hernioplasty has been criticized because of its technical complexity and increased costs. Disposable dissection balloons can be used to facilitate the creation of the initial working space in totally extraperitoneal endoscopic hernioplasty (TEP), but their use adds to the cost of the operation. Methods: A total of 322 men with unilateral, primary, or recurrent inguinal hernias were randomized to undergo TEP with or without a dissection balloon. Results: In the group with the balloon, three of 161 patients (2.5%) required conversion to transabdominal preperitoneal hernioplasty (TAPP), or open herniorraphy, whereas 17 of 161 patients (10.6%) were converted to TAPP or open herniorraphy in the group without the balloon (p = 0.002). The mean operation time was 55 min in the group with the balloon and 63 min in the group without the balloon (p = 0.004). There was no difference between them in postoperative morbidity, and there were no major complications in either group. The recurrence rate was 3.1% in the group with the balloon and 3.7 % in the group without the balloon (p = 0.8). Conclusion: The use of a dissection balloon in TEP reduces the conversion rate and may be especially beneficial early in the learning curve.

    • The Lucilia group sensu Anderberg and Freire comprises nine South American genera: Belloa, Berroa, Chevreulia, Cuatrecasasiella, Facelis, Gamochaetopsis, Jalcophila, Lucilia and Luciliocline. The aims of this contribution were, using DNA sequences from plastid (rpl32-trnL, trnL-F) and nuclear (ITS and ETS) markers, together with morphological characters, to test the monophyly of the Lucilia group and provide new insight into generic circumscriptions. Our studies, including a broad taxon sampling of Gnaphalieae species, suggest that the Lucilia group is paraphyletic, since Antennaria, Chionolaena, Gamochaeta, Loricaria, Micropsis, Mniodes and Stuckertiella are all nested within the Lucilia group. Morphology and molecular analyses combined showed that the traditional generic circumscription of most of the genera (e.g., Berroa, Chevreulia, Chionolaena, Cuatrecasasiella, Facelis, Jalcophila and Micropsis) correlates with the inferred phylogenetic relationships. Conversely, Lucilia and Luciliocline are non-monophyletic. Lucilia is nested in a clade with Berroa, Facelis and Micropsis. Luciliocline is strongly embedded within the clade Belloa pp + Mniodes. Our results are consistent with Dillon’s study that considered Belloa as a montotypic genus (B. chilensis). Luciliocline and the remaining species of Belloa are accommodated in the genus Mniodes, and the necessary combinations are proposed for the expanded Mniodes. All the analyses showed that the monotypic genera Stuckertiella and Gamochaetopsis are in a well-supported clade nested within Gamochaeta, which implies that taxonomic changes are required also for these genera. Internal relationships in the group and the key morphological characters used in the taxonomy of the group, as well as incongruences found between morphological and molecular analyses, are discussed.

  • Data validation in an economic evaluation of surgery for colon cancer

    • OBJECTIVES: This study aimed to validate the accuracy of data retrieved in a prospective multicenter trial, the purpose of which was an economic evaluation of two techniques of surgery for colon cancer. METHODS: Within the Swedish contribution of the COLOR trial (Colon Cancer Open or Laparoscopic Resection), an economic evaluation of open versus laparoscopic surgical techniques was conducted. Data were collected by case record forms (CRF), patient diaries, and telephone surveys every 2 weeks. The study period was 12 weeks, and the perspective was societal. Data from the first consecutive forty patients to complete the health economic study protocol were validated. Retrieved data were compared with data from medical records and data from local social security offices for agreement. RESULTS: Statistically significant differences were found for duration of anesthesia, length of surgery, number of outpatient consultations by doctors and district nurses, complication rate, and the use of central venous lines. No significant differences were observed concerning length of hospital stay, disposable instruments cost, and time off work, all of which heavily influence total costs. CONCLUSIONS: The present method of data collection regarding resources used in this setting seems to produce accurate data for economic evaluation; however, relative to complication rates, the method did not retrieve accurate data.

    • BACKGROUND: There has been no randomized clinical trial of the costs of laparoscopic colonic resection (LCR) compared with those of open colonic resection (OCR) in the treatment of colonic cancer. METHODS: A subset of Swedish patients included in the Colon Cancer Open Or Laparoscopic Resection (COLOR) trial was included in a prospective cost analysis; costs were calculated up to 12 weeks after surgery. All relevant costs to society were included. No effects of the procedures, such as quality of life or survival, were taken into account. RESULTS: Two hundred and ten patients were included in the primary analysis, 98 of whom had LCR and 112 OCR. Total costs to society did not differ significantly between groups (difference in means for LCR versus OCR euro1846; P = 0.104). The cost of operation was significantly higher for LCR than for OCR (difference in means euro1171; P < 0.001), as was the cost of the first admission (difference in means euro1556; P = 0.015) and the total cost to the healthcare system (difference in means euro2244; P = 0.018). CONCLUSION: Within 12 weeks of surgery for colonic cancer, there was no difference in total costs to society incurred by LCR and OCR. The LCR procedure, however, was more costly to the healthcare system.

  • Randomized trial of health-related quality of life after open and laparoscopic surgery for colon cancer

    • BACKGROUND: Randomized controlled trials (RCTs) have reported improved or unchanged three-year survival following laparoscopic colon resection (LCR) for colon cancer compared with that following open resection (OCR). The aim of this study was to determine health-related quality of life (HRQL) in patients randomized to laparoscopic or open resection for colon cancer. METHODS: In total, 285 patients (130 LCR, 155 OCR) from seven Swedish centers were included. HRQL was assessed preoperatively and at 2, 4, and 12 weeks postoperatively with the EQ-5D and EORTC QLQ-C30 instruments. RESULTS: The LCR patients did significantly better on the social function component of the EORTC QLQ-C30 at two and four weeks and on the role function component at two weeks. CONCLUSION: Laparoscopic resection for colon cancer improved quality of life during the first postoperative month.

    • OBJECTIVE: Analysis of reoperation and recurrence rates three years after repair of groin hernias.DESIGN: Prospective audit by questionnaire and selective follow-up.SETTING: Eight Swedish hospitals.SUBJECTS: All groin hernia operations done during 1992 on patients between the ages of 15 and 80 years.MAIN OUTCOME MEASURES: Postoperative complications, reoperation for recurrence, and recurrence.RESULTS: During 1992, 1565 hernia operations were done. The postoperative complication rate was 8% (125/1565). At 36 months postoperatively 108 recurrences had already been reoperated on, six patients with recurrences were on the waiting list for reoperation and a further 36 recurrences had been detected at follow-up. The interhospital variation in recurrence rate ranged from 3% to 20%. Postoperative complications, recurrent hernia, direct hernia and hospital catchment area over 100000 inhabitants were all factors associated with an increased relative risk of recurrence.CONCLUSIONS: The recurrence rate exceeded the reoperation rate for recurrence by almost 40% which should be taken into account if the reoperation rate is used as the endpoint after repairs of groin hernia. An audit scheme, based on prospective recording, reoperation rate, and (periodic) calculation of the recurrence rate may be used to identify risk factors for recurrence and areas in need of improvement.

  • The significance of meristic changes in the flower of Sapotaceae

    • Sapotaceae belongs to the heterogeneous order Ericales and exhibits extensive diversity in floral morphology. Although pentamery is widespread and probably the ancestral condition, some clades are extremely variable in merism, with fluctuations between tetramery to hexamery and octomery, affecting different floral organs to different degrees. We assessed the different states of merism in Sapotaceae to determine the evolution of this character among different clades. The floral morphology and development of nine species from eight genera were investigated using scanning electron microscopy (SEM). Furthermore, floral characters related to merism were mapped onto a phylogenetic tree to analyse the distribution and evolutionary significance of merism in the family. Developmental evidence shows that changes in merism are linked to a concerted multiplication of organs among whorls and an increase in whorls through the displacement of organs. Although pentamery is reconstructed as the ancestral condition, a reduction to tetramery or an increase to a higher merism (mainly hexamery or octomery) has evolved at least five times in the family. Fluctuations in merism between different whorls are not random but occur in a coordinated pattern, presenting strong synapomorphies for selected clades. Octomery has evolved at least twice, in Isonandreae from tetramery and in Sapoteae-Mimusopinae from pentamery. Hexamery has evolved at least three times, independently in Northia, the Palaquium clade of Isonandreae and derived from octomery in Sapoteae-Mimusopinae. Three possibilities of merism increase have been identified in Sapotaceae: (1) a concerted increase affecting all organs more or less equally (Palaquium clade of Isonandreae, Sapoteae); (2) a coordinated increase in petals, stamens and mostly carpels without effect on sepals (Labourdonnaisia, Payena–Madhuca clade of Isonandreae); (3) an increase in carpels independently of other organs (Burckella, Letestua, Labramia, etc.). A major shift affecting all Sapotaceae, except Isonandreae, is the sterilization or loss of the antesepalous stamen whorl. The presence of two fertile stamen whorls in Isonandreae indicates a possible reversal or a retained plesiomorphy. In a number of genera, stamens are secondarily increased independently of changes in merism. Descriptions of flowers listing only organ numbers are thus misleading in the inference of evolutionary relationships, as they do not differentiate between changes in merism affecting the number of perianth whorls and other changes affecting the androecium, such as sterilization, loss or occasional doubling of antepetalous stamens.

    • In an earlier paper we reported on a new septum magnet design with a magnetic flux filter [1]. Now the first septum magnet is in operation as in injection septum magnet for the MAX I storage ring. We dcscribe the actual mechanical construction and the field measurements of the new magnets. The fine Structure of the septum nose needed several trials in the construction until we found the final solution. The measured residual stray fields in the ring tube are even smaller than was expected from the model calculations.

    • At MAX-LAB the next synchrotron light source MAX-IV is currently studied (Proceedings of the seventh European Particle Accelerator Conference, EPAC 02, Paris, France, 2002). In this paper, we present a possible lattice with horizontal emittance of 1.2 nm rad at an energy of 3 GeV. The possibilities to realise the main magnetic system with high gradient integrated magnets are studied with 2D and 3D magnetic field programs. The magnet is introduced in the matching section of the 3 GeV storage ring lattice [Tarawneh, Proceedings of European Particle Accelerator Conference, July 2004, Lucerne, Switzerland] mainly to reduce the power of the synchrotron radiation hitting the upstream superconducting insertion devices.

    • Detta dokument innehåller ett ramverk och en stegvis me­tod för utformning och förvaltning av nätverk av marina skyddade områden.Ramverket syftar till att stötta utvecklingen av ett mer eko­logiskt representativt, sammanhängande och funktionellt nätverk av effektivt förvaltade marina skyddade områden i Sverige. Det möjliggör också en utvärdering av detta nätverk. Ramverket inbegriper principer för hur Havs- och vattenmyn­digheten, tillsammans med berörda kustlänsstyrelser, kan ar­beta strategiskt med marint skydd ur ett helhetsperspektiv. Med hjälp av ramverket kan tydliga målsättningar slås fast för att uppnå ett ekologiskt representativt, funktionellt och ef­fektivt nätverk av marina skyddade områden. Ramverket åt­följs av en stegvis metod för såväl utformning som förvaltning av marina skyddade områden, och lägger tydligt fram de an­taganden som ligger bakom metoden. Ramverket kan även vara ett stort stöd i den framtida ambitionshöjning av målet för marint områdesskydd till 30% – varav 10% strikt skydd – som föreslås till år 2030.Ramverket och metoden konkretiseras i två regionala planer, utarbetade av berörda kustlänsstyrelser. Implementeringen av de regionala planerna kommer att stötta Sverige i arbetet med att uppfylla nationella och internationella åtaganden om ett ekologiskt representativt, sammanhängande och funktionellt nätverk av effektivt förvaltade marina skyddade områden.Ramverket fungerar som ett stöd för dem som arbetar med marint områdesskydd i Sverige på nationell och regional nivå samt länsnivå. Det har i första hand utvecklats för att stötta ar­betet vid länsstyrelserna och Havs- och vattenmyndigheten, men även för Naturvårdsverket samt andra institutioner och universitet. Det fungerar även som ett exempel på hur meto­diken Conservation Standards (CS) kan tillämpas på nätverk av marina skyddade områden på både nationell, regional och lo­kal nivå. Ramverket kan fritt anpassas för andra sammanhang.Inom utformning och förvaltning av marint områdesskydd kan ramverket samt dess begrepp, vägledande principer och me­tod tillämpas på olika nivåer: för ett enskilt skyddat område, i ett län, i ett havsområde, nationellt eller till och med i gränsö­verskridande samarbete. I detta dokument beskrivs ramverket främst på regional nivå, i syfte att skapa separata regionala nät­verk av marina skyddade områden. Vissa komponenter är dock desamma för alla havsområden i Sverige.Vissa delar av ramverket är fortfarande under arbete. Ramverket kommer att förbättras kontinuerligt under dess implementering.

    • This document contains a Framework and step-by-step guid­ance to support the design and management of Marine Pro­tected Area Networks.The Framework is designed to support the development of a more ecologically representative, better-connected, and more functional network of effectively managed marine protected areas (MPAs) in Sweden. As such, the Framework also enables the evaluation of this Network. The Framework incorporates principles for how the Swedish Agency for Marine and Water Management, together with relevant coastal Country Admin­istrative Boards (CABs), can work strategically with marine pro­tection using an integrated approach. The Framework helps define clear objectives aimed to achieve ecological represent­ativity, functionality, and effectiveness of the MPA Network. It is accompanied by step-by-step guidance for both designing and managing marine protected areas, and transparently lays out the main assumptions behind this guidance. It can also be of great help to Sweden in meeting the potential future am­bition of protecting at least 30% of marine waters by 2030, of which 10% should be strictly protected.The Framework and the guidance are applied in two Regional Plans, developed by representatives of the relevant CABs. The implementation of the Regional Plans will help Sweden ful­fill its national and international commitments to ecologically representative, well-connected, and functional networks of marine protected areas.The Framework acts as a reference for professionals working with marine protected areas in Sweden on the national level, the regional level, and the county level. It has been primarily developed to support the work of the CABs and the Swed­ish Agency for Marine and Water Management, but also the Swedish Environmental Protection Agency and other insti­tutes and universities. It also serves as an example of how the Conservation Standards (CS) can be applied to the context of MPA Networks on a national, regional, and local level. Prac­titioners are encouraged to freely adapt this framework to fit other contexts.The Framework and the step-by-step guidance can be used for the design and management of a single MPA or a set of MPAs, of a regional or national MPA Network, and potentially even of an international network. In this document, the Framework is described primarily on a regional level, with the aim of creating distinct regional MPA Networks. Some components, however, are the same for all regions in Sweden.This document only represents a first attempt at a framework with the definitions, guiding principles, and methodology needed to guide MPA Network Design & Management. It will be refined over time, as it is put into practice and as learning is generated about what works and what does not.

    • BACKGROUND: The European Association of Endoscopic Surgery (EAES) initiated a consensus development conference on the laparoscopic resection of colon cancer during the annual congress in Lisbon, Portugal, in June 2002. METHODS: A systematic review of the current literature was combined with the opinions, of experts in the field of colon cancer surgery to formulate evidence-based statements and recommendations on the laparoscopic resection of colon cancer. RESULTS: Advanced age, obesity, and previous abdominal operations are not considered absolute contraindications for laparoscopic colon cancer surgery. The most common cause for conversion is the presence of bulky or invasive tumors. Laparoscopic operation takes longer to perform than the open counterpart, but the outcome is similar in terms of specimen size and pathological examination. Immediate postoperative morbidity and mortality are comparable for laparoscopic and open colonic cancer surgery. The laparoscopically operated patients had less postoperative pain, better-preserved pulmonary function, earlier restoration of gastrointestinal function, and an earlier discharge from the hospital. The postoperative stress response is lower after laparoscopic colectomy. The incidence of port site metastases is <1%. Survival after laparoscopic resection of colon cancer appears to be at least equal to survival after open resection. The costs of laparoscopic surgery for colon cancer are higher than those for open surgery. CONCLUSION: Laparoscopic resection of colon cancer is a safe and feasible procedure that improves short-term outcome. Results regarding the long-term survival of patients enrolled in large multicenter trials will determine its role in general surgery.
