    • Swedish institutions of higher education must promote widening participation. This goal is also expressed in policy documents at Lund University. To accomplish this, teachers at Lund University formed a group to strategically work towards student inclusion. This article reports on an interview study conducted by this group investigating students’ experiences and perceptions of entering education at two different programs. The theory of “the ideal student” was used to analyze the interview material. The result indicates the need for clarity and predictability in course layout and teaching.

  • Internet-delivered Cognitive Behavioral Therapy for insomnia in youth with autism spectrum disorder : A pilot study

    • Adolescents with ASD often suffer from sleep disorders affecting their development and quality of life. Research concerning psychological treatment of insomnia in this population is scarce. The objective of this pilot study was to examine the feasibility of internet-delivered CBT for insomnia (iCBT-I) and the participants' experiences after completing the treatment. Both quantitative and qualitative methods were used. Six adolescents with ASD and insomnia, aged 13 to 17, participated in the study. The results of the qualitative investigation showed general satisfaction with the iCBT-I. The participants experienced both better sleep and insights into their sleep patterns. Thematic analysis revealed five themes: experience of the structure of the treatment, treatment content, experienced outcomes, experienced difficulties, and suggested improvements. The results indicate the need for additional support for some participants and suggest distinct recommendations for further studies. The quantitative investigation showed large reductions in insomnia symptoms indicating the feasibility of the treatment in this population. The findings suggest promising results, but more studies are needed to define the efficacy of iCBT-I for adolescents with ASD.

    • Being the second leading cause of death and the leading cause of disability-adjusted life years worldwide, infectious diseases remain─contrary to earlier predictions─a major consideration for the public health of the 21st century. Resistance development of microbes to antimicrobial drugs constitutes a large part of this devastating problem. The most widely spread mechanism of bacterial resistance operates through the degradation of existing β-lactam antibiotics. Inhibition of metallo-β-lactamases is expected to allow the continued use of existing antibiotics, whose applicability is becoming ever more limited. Herein, we describe the synthesis, the metallo-β-lactamase inhibition activity, the cytotoxicity studies, and the NMR spectroscopic determination of the protein binding site of phosphonamidate monoesters. The expression of single- and double-labeled NDM-1 and its backbone NMR assignment are also disclosed, providing helpful information for future development of NDM-1 inhibitors. We show phosphonamidates to have the potential to become a new generation of antibiotic therapeutics to combat metallo-β-lactamase-resistant bacteria.

  • Quality assessment of radiotherapy in the prospective randomized SENOMAC trial

    • Background and purpose: Recommendations for regional radiotherapy (RT) of sentinel lymph node (SLN)-positive breast cancer are debated. We here report a RT quality assessment of the SENOMAC trial. Materials and Methods: The SENOMAC trial randomized clinically node-negative breast cancer patients with 1-2 SLN macrometastases to completion axillary lymph node dissection (cALND) or SLN biopsy only between 2015-2021. Adjuvant RT followed national guidelines. RT plans for patients included in Sweden and Denmark until June 2019 were collected (N = 1176) and compared to case report forms (CRF). Dose to level I (N = 270) and the humeral head (N = 321) was analyzed in detail.

  • Auditory pseudoword rhyming effects in bilingual children reflect second language proficiency : An ERP study

    • This study investigated second language (L2-English) phonological processing in 31 Spanish-English bilingual, 6- to 8-year-old schoolchildren in an event-related potential (ERP) auditory pseudoword rhyming paradigm. In addition, associations between ERP effects and L2 proficiency as measured by standardized tests of receptive language and receptive vocabulary were explored. We found a classic posterior ERP rhyming effect that was more widely distributed in children with higher L2 proficiency in group analyses and was larger for children with better L2 proficiency in correlation analyses. In contrast, the amplitude of an early (75–125 ms) auditory pos- itivity was larger in children with lower L2 proficiency. This pattern suggests differential use of early and late auditory/phonological processing resources in the pseudoword rhyme task associated with L2 proficiency, which is consistent with the predictions of the lexical restructuring model in a bilingual context.

    • Public media both refects and shapes societal perceptions and attitudes. Teachers and others around students in mathematics classrooms have expectations for the students, projected with what appears in these media. We are most concerned about the expectationsplaced on students who are identifed with minoritized groups—particularly students whoare Indigenous or migrated to Norway. We investigate how minoritized group contexts andmathematics education appear together in Norwegian news media texts. Our analysis usesthe notion of storylines to describe the expectations about minoritized groups that newsmedia project. We found seven entangled storylines: “the majority language and cultureare keys to learning and knowing mathematics,” “mathematics is language- and cultureneutral,” “minoritized groups’ mathematics achievements are linked to culture and gender,”“extraordinary measures are needed to teach students from minoritized groups mathematics,” “students from minoritized groups underachieve,” “students from minoritized groupsput in extraordinary efort and time to learn mathematics,” and “minoritized mathematicsstudents are motivated by gratitude.”

  • Mathematics curriculum discourses on democracy : Critical thinking in the age of digital traces

    • The use of people’s online digital traces has given rise to concerns for democracy. The digital traces may affect the individual’s life in unexpected and negative ways. Such traces may also be of importance for understanding the spread of disinformation and the like. This paper reports on a Foucault inspired discourse analysis of the Swedish upper secondary mathematics curriculum. Two discourses are construed in the intersection of critical thinking, democracy, and this new technology. Skovsmose’s concept of mathemacy is used to identify what is critical knowledge and what is not. The first construed discourse is, “With knowledge in formal mathematics, critical thinking on democracy will follow.” The second is, “Rather a personal career than a critical citizenship.” Neither of the discourses promotes a need for mathematics education to change due to new technology with regards to critical thinking.

    • Syftet med denna licentiatuppsats är att pröva en specifik strukturell modell som stöd för elevers utforskande av relationer mellan tal i additiva strukturer även när negativa tal är inkluderade. En intention är att resultatet ska kvalificera undervisningen och utgöra ett stöd vid planering och genomförande av en undervisning avseende ekvationer med additiv struktur utifrån algebraisk undervisning som alternativ till att i första hand finna en lösning med stöd av regler och procedurer. Under studien kartlades, kategoriserades och beskrevs elevernas erfarande av fenomenet relationer mellan kvantiteter. I studien prövades även om, och i sådant fall, på vilka sätt en specifik strukturell modell användes av elever under arbetet med att utforska ekvationers struktur.Elever från årskurserna 3, 8 och 9 deltog i semistrukturerade intervjuer samt i forskningslektioner. Tre grupper av lärare samt två forskare planerade, genomförde och reviderade forskningslektionerna baserat på ansatsen learning study.Fenomenografi var den teoretiska ansatsen för de inledande semistrukturerade intervjuerna. Variationsteori och lärandeverksamhet var de teoretiska ramverken för forskningslektionerna där Davydovs program var en inspirationskälla.För att uppnå syftet formulerades följande forskningsfrågor:Vilka skilda sätt att erfara fenomenet relationer mellan kvantiteter kan urskiljas i elevintervjuer?På vilka sätt använder elever en specifik strukturell modell för att utforska ekvationer?Den första forskningsfrågan besvaras i Artikel 1 som visar att elever erfar relationer mellan kvantiteter som någonting som ska beräknas alternativt någonting som ska relateras. Den andra forskningsfrågan besvaras i Artikel 2 som visar att elever använde sig av den i studien prövade specifika strukturella modellen såsom ett formulär att fylla i alternativt som en lärandemodell och redskap för att identifiera del-helhetsstrukturen mellan tal i en ekvation samt för att välja lämplig operation att lösa ut det obekanta talet.                          Resultaten visar på möjligheten, men även utmaningen, att introducera en algebraisk undervisning med fokus på analys och teoretiska resonemang även för elever med erfarenheter från en alternativ bakgrund.

  • Care experiences of younger people and next of kin following myocardial infarction

    • Background/Aims Coronary heart diseases, including myocardial infarction, remain a major health problem worldwide. Little is known about the cardiac care experiences post myocardial infarction of younger people and their next of kin. This study aimed to describe the cardiac care experiences of post-myocardial infarction younger people (<55 years old) and their next of kin. Method This study employed an exploratory inductive qualitative design involving individual interviews with younger people (n=17) and their next of kin (n=13). Results Three categories were identified and patients were deemed to be in need of: tailored information, recognition or a post-discharge rehabilitation plan Conclusions Findings demonstrated the importance of using the experiences of younger people and their next of kin as the starting point when planning care and cardiac rehabilitation. Health professionals must systematically and purposefully provide person-centred care, both in hospital and post-discharge settings. Next of kin must be regarded as an important resource in post-myocardial infarction care for younger people, as they tend to be engaged in providing their relatives with support, which is valuable for the recovery of young people post myocardial infarction. Further research is warranted to investigate what makes person-centred interventions directed towards post-myocardial infarction younger people and their next of kin effective.

  • Long emergency department length of stay : A concept analysis

    • INTRODUCTION: Emergency Department (ED) Length of stay (LOS) has been associated with poor patient outcomes, which has led to the implementation of time targets designed to keep EDLOS below a specific limit. The cut-offs defining long EDLOS varies across settings and seem to be arbitrarily chosen. This study aimed to clarify the meaning of long EDLOS.METHODS: A concept analysis using the Walker and Avant approach was conducted. It included a literature search aiming to identify all uses of the concept, resulting in a set of defining attributes and a way of measuring the concept empirically.RESULTS: Long EDLOS was primarily used as proxy for other phenomena, e.g. boarding or crowding. The definitions had cut-offs ranging between 4 and 48 h. The attributes defining long EDLOS was waiting, a crowded ED environment and an inefficient organization.DISCUSSION: Time targets are probably more suitable when directed towards and tailored for specific sub-groups of the ED population.

    • In this report, we have compiled our learnings and experiences of working with Policy Lab. Policy Labs have come about as an answer to the question "Can you work with policy and regulatory development in a better way than today?". Our answer to the question is a yes. Our hope with the report is that others will become interested and start their own Policy Lab. Abroad, there are many Policy Labs, but in Sweden there are only a few, which is why we believe there is room for more. There is not a given way to work with Policy Labs once and for all, but each Policy Lab is unique based on its context. Sweden's innovation agency Vinnova defines Policy Labs as follows: "Policy Labs can be explained as a group of actors with different competencies who want to develop a regulatory framework. In the Policy Lab, they use a set of user-centric methods and competencies to test, experiment, and learn in policy development."1 In our Policy Lab, we have worked in various research projects to: 1. analyse challenges/problems that arise between innovations, technology, market, and regulations, 2. develop one or more workable solutions and 3. interact with relevant actors to determine the next steps. What distinguishes our Policy Lab is that we never “own” the issue or solution. We must therefore always work with other actors who can take the results further. Our goal is to enable and skill people. This means that for us it is important to work concretely with real problems and needs owners and preferably test different solutions. We focus on the here and now perspective and not on what the future will look like in 10 years. It is about taking the next step forward towards the future, not creating the best rule, but instead creating the next rule. We also work consistently agile and use design as a method for problem solving. This means that the way we organize our work in the Policy Lab is circular and not linear. When it comes to using design as a method for problem solving, we use the concepts of "design thinking" and "double diamond". For us, it is also important that the members of the Policy Lab have different backgrounds and skills depending on what is needed in the individual project....

  • A transition of power in opioid substitution treatment : Clinic managers' views on the consequences of a patient choice reform

    • Objectives: Opioid substitution treatment (OST) is often described as a strict and highly regulated treatment method, in which patients have limited influence over their treatment. In 2014, a reform was introduced by the regional council of Skåne in southern Sweden, which allowed OST patients to choose their treatment provider, thus transferring power from care providers to patients. The aim of this study was to examine what this increase in patient influence has meant for the clinics that provide OST in Skåne, and how these clinics have dealt with the new competitive situation that has arisen following the introduction of the reform. Methods: The study is based on two waves of semi-structured interviews with clinic managers at all OST clinics in Skåne. Results: The clinic managers described the increase in patient influence as a positive change, which had led to the patients being treated with more respect. The competition among clinics was expressed, among other things, in the form of differing views on the prescription of benzodiazepines, which initially gave rise to dissatisfaction among clinics with a more restrictive approach to such prescriptions. The reform did not lead to any clear diversity between clinics, apart from different approaches to the prescription of benzodiazepines. The incentive for competition-based diversity is, however, limited by the strict national regulatory system and by the reimbursement system, which restricts the ways in which clinics can conduct treatment activities. Conclusion: OST-clinic managers were largely positive about the increased patient empowerment and the shift in power balance associated with the patient choice reform. The introduction of the reform did not lead to any clear diversity between treatment providers, apart from differing views on the prescription of benzodiazepines, which by some managers was regarded as unfair competition.

  • Addressing youth unemployment: what role for social work? : Policy responses to youth unemployment in Sweden and Europe

    • Unemployed youth are a heterogenous group facing varying and sometimes complex problems. Being young and unemployed can have a negative impact on future life chances and quality of life. Studies on youth unemployment have mostly focused on education and labour market responses, leaving the involvement of social work aside.   This thesis aims to explore policies for unemployed youth in three key policy areas: social work, education and labour market, in Europe and Sweden. The thesis consists of four separate studies highlighting how policy ideas are translated into agendas, and how responses are administered and organized at national and local level. Applying an institutionalist theoretic framework, the results are analysed with consideration to the structure of state and welfare state.   Study I explored the ideas inherent to the social policy concept ‘social investment’, outlining three central dimensions of the concept: temporal-perspective, principles of distribution and policy coherence. A key result of study I is that social investment ideas are nuanced, and that social investments can take different forms. In the following three studies, the dimensions of social investment are used as heuristic tools to examine policy responses for unemployed youth.   Study II examined how the EU recommendation on establishing a youth guarantee (YG), was translated in national YG plans. The results were analysed using tentative regime-types based on the structure of social work, the education system, and the social insurance system. The results showed that outreach as an early intervention was marginal across countries and regime types, and the involvement of social work was largely absent.   In study III, the coordination within and between policy areas was analysed between Sweden and the UK, over time. Policy documents on national labour market programs in both countries between 1998-2011, were analysed. The results showed that coordination between labour market programs and social security benefits/social assistance had strengthened over time in both countries. How authority to regulate and administer different policy areas, was also linked to the occurrence of coordination between different policy areas.   Study IV examined if and how specialization and coordination were part of organizing local level work with NEETs in Sweden. This was explored through structured interviews with local professionals in social work, education and labour market. The results showed a pattern of coordination and specialization among education and labour market actors. The involvement of social work was instead marginal, and primarily concerned social assistance.   In conclusion, the results of this thesis show that the agenda, content and administration of policy responses to youth unemployment consists mainly of wide, universal and reactive responses. They are also characterized by coordination between labour market measures and social insurance/assistance, in line with an activation trend. An important finding is also the very limited involvement of social work, as noted in national policy agendas and programs, and in local level work. Both activation and social work involvement did however vary somewhat with administrative levels and between different policy areas. The results thereby indicate that institutional aspects such as organizational structure and administration, matter for the involvement of different policy areas in responses to youth unemployment.

  • Changes in opioid-related deaths following increased access to opioid substitution treatment

    • Background Opioid-related mortality is high and increasing in the Western world, and interventions aimed at reducing opioid-related deaths represent an important area of study. In Skane County, Sweden, a patient choice reform resulted in increased access to opioid substitution treatment (OST). In addition, a gradual shift towards less restrictive terms for exclusion from OST has been implemented. The aim of this study was to assess the impact of these policy changes on opioid-related deaths. Methods Detailed data on opioid-related deaths in Skane during the 2 years prior to and following the policy change were obtained from forensic records and from health care services. Data on overdose deaths for Skane and the rest of Sweden were obtained using publicly available national register data. Time periods were used as the predictor for opioid-related deaths in the forensic data. The national level data were used in a natural experiment design in which rates of overdose deaths were compared between Skane and the rest of Sweden before and after the intervention. Results There was no significant difference in the number of deaths in Skane between the data collection periods (RR: 1.18 95% CI:0.89-1.57, p= 0.251). The proportion of deaths among patients enrolled in OST increased between the two periods (2.61, 1.12-6.10, p= 0.026). There was no change in deaths related to methadone or buprenorphine in relation to deaths due to the other opioids included in the study (0.92, 0.51-1.63, p= 0.764). An analysis of national mortality data showed an annual relative decrease in unintentional drug deaths in Skane compared to the rest of Sweden following the onset of the reform (0.90, 0.84-0,97, p= 0.004). Conclusions Opioid-related deaths, as assessed using forensic data, has not changed significantly in Skane following a change to lower-threshold OST. By contrast, national level data indicate that the policy change has been associated with decreased overdose deaths. The discrepancy between these results highlights the need for more research to elucidate this issue. The result that more patients die during ongoing OST following an increase in access to treatment underlines the need for further preventive interventions within the OST treatment setting.

    • Over the past decades, Europe has seen increasing interest in and pressure to coordinate social policy. Studies on the coordination of social policies have found that institutional settings affect coordination patterns when comparing different welfare state ideal-types. Adding to this body of research, this paper compares two countries with diverging authority to regulate social policy areas; Sweden representing a unitary decentralized state and the UK representing a regionalized state. With labour market programs for youth serving as a case, the purpose of this paper is to compare the development of inter- and intra-policy coordination over a period of 10 years, analysing the impact of regulatory authority. Policy documents which set the regulatory frame of the programs are analysed from the perspectives of instruments, and territorial levels of regulatory authority and how these connect to coordination patterns. The results show that levels of discretion and processes of decentralization and fragmentation of regulatory authority impact on coordination patterns, where inter- and intra-coordination develops under different circumstances.

    • Fatal opioid overdoses usually occur without anyone being present and able to intervene. Knowledge of the presence of witnesses is important for how naloxone programs should be designedDrug mortality has increased in Sweden during the 2000s. The vast majority of deaths are opioid overdoses. The National Board of Health and Welfare recommends that the antidote naloxone and a brief overdose education should be offered to people who are at risk of opioid overdose. A retrospective registry study of 193 forensically examined fatal opioid overdoses in Skåne showed that over 80 percent occurred in private residences, most often the deceased’s own home. Other people were present in just over 40 percent of the 193 deaths, but usually in another room or asleep. In most cases, the witnesses were friends, partners, parents, or other people close to the deceased. Naloxone programs should be expanded to include family members and other persons who are close to opioid users, and who therefore may witness or be present early in case of an overdose.

    • BACKGROUND: Opioid-related deaths have increased in Western countries over recent decades. Despite numerous studies investigating opioid-related mortality, only a few have focused on the lives of the deceased individuals prior to their deaths, specifically regarding contact with care-providing authorities such as health, social and correctional services. Furthermore, a change has been noted in the last two decades as to which opioids cause most deaths, from heroin to prescription opioids. However, studies comparing fatalities caused by different substances are rare. The aim of this study was to investigate contact with care-providing authorities during the year prior to death among individuals who died as a result of opioid intoxication and to analyse differences relating to which opioids caused their deaths. METHODS: The study is based on retrospective register data and includes 180 individuals with a history of illicit drug use, who died from opioid intoxication in Skåne, Sweden, between 1 January 2012 to 31 December 2013 and 1 July 2014 to 30 June 2016. Intoxications caused by heroin, methadone, buprenorphine and fentanyl were included. Data were collected from the National Board of Forensic Medicine, regional health care services, municipal social services and the Prison and Probation Service. Statistical testing was performed using Pearson's chi-square test, Fisher's exact test and the Mann-Whitney U test to analyse group differences. RESULTS: A total of 89% of the deceased individuals had been in contact with one or more of the care-providing authorities during the year prior to death; 75% had been in contact with health care, 69% with the social services, 28% with the Prison and Probation Service, and 23% had been enrolled in opioid substitution treatment at some point during their final year of life. Few differences appeared between the substance groups with regard to which opioid contributed to the death. In addition to opioids, sedatives were present in more than 80% of the cases. Individuals whose deaths were buprenorphine-related had been in contact with the social services to a significantly lesser extent during the year prior to death. CONCLUSIONS: The studied population is characterised by extensive contact with care-providing authorities, thus providing numerous opportunities for authorities to reach this group with preventive and other interventions. Few differences emerged between groups with regard to which opioid had contributed to the death.

  • Parental information work and wage work capacity in families with children with autism and ADHD

    • 5-7% of all children in Sweden have ADHD and 1% have autism (Gillbergcentrum, Göteborgs universitet, 2022; Socialstyrelsen, 2019). Previous research establishes that parents, primarily mothers, of these children suffer from reduced work capacity and need extended periods of sick leave (Findling et al., 2022). The causes of this reduced wage work capacity are yet to be explored.At the same time, surveys with parents indicate that information management and communication about their children with various societal actors such as the school, the child- and youth psychiatry, and the Social Insurance Agency conflicts with wage work (Riksförbundet Attention, 2016).Our study analyses the impact of parents’ information management and communication about their children with societal actors – called “information work” (Dalmer & Huvila, 2019) – on parents’ wage work capacity. The results are based on 50 semi-structured interviews with Swedish mothers of children with ADHD and autism.The mothers perceived that they had to gather, assess, use and communicate information about their children to and between numerous societal actors. The information work conflicted with their wage work as it was time consuming and often had to be carried out during work hours. It also encompassed emotion work (Hochschild 1979) as it stirred up feelings of for example frustration and sadness that the mothers needed to manage.The extent to which the information work conflicted with the mothers’ wage work depended on the mothers’ professions and the extent to which the mothers could draw on economic, social and cultural capital (Bourdieu 1979).

    • Patient choice of treatment provider was introduced within opioid substitution treatment in the southern Swedish county of Skåne in 2014. Substitution treatment has often been criticised for being strict, rule-based, and driven by an ethos of discipline. This study explores the patients’ views and experiences of patient choice, particularly as a potential means of empowerment. The study is based on semi-structured interviews with 33 patients at six substitution treatment clinics in two cities. Patient choice within substitution treatment has empowered the patients in that many are able to choose their treatment provider and transfer to another provider. The interviewees appreciated the possibility to choose and transfer, and felt that they had gained more influence on their treatment. Experiences of poor staff conduct and the new clinic’s policies and practice on prescribing benzodiazepines were important reasons for choosing and transferring between clinics. In particular, the patients stressed the importance of the possibility to leave a clinic they felt offered substandard treatment, and the psychologically important feeling of knowing that they could transfer to another facility. However, patient choice in addiction treatment is very rare in Sweden, and the demographic structure limits the development of patient choice within substitution treatment.

    • Problematic opioid use constitutes an extensive global problem. Correspondingly, opioid-related mortality is high and has increased in several Western countries, including Sweden, during the 2000s. In Sweden, the most effective treatment method, opioid substitution treatment (OST), was for a long time limited with respect to the number of patients. The treatment was also characterized by strict rules and conducted in a high-threshold manner, which has meant that it has not been fully appealing to people with problematic opioid use. Therefore, in Skåne County in southern Sweden, patient choice of treatment provider was introduced for OST in 2014 with the intention to increase the number of treatment places and strengthen patient empowerment. The overall aims of this thesis are (1) to investigate opioid-related deaths in Skåne with a focus on contact with care-providing authorities and in relation to increased access to OST, and (2) to examine patients’ and clinic managers’ attitudes towards the introduction of the patient choice reform for OST and their views of the reform's objectives of increased accessibility to OST and strengthening patients' empowerment and influence over their treatment.The four papers in this thesis are based on two research projects with various empiric material. Paper I and II are based on data on opioid overdose deaths from a period of two years before and two years following the introduction of the patient choice reform. Forensic data regarding the presence of various substances and which opioid caused the death, as well as demographic data and information on contact with care-providing authorities (health care, social services, and the Prison and Probation Service), were collected. Paper I examines clinical background and contact with care-providing authorities of opioid-related fatalities, as well as differences with regard to which opioid caused the death. Paper II examines the possible impact of the intervention on the development of opioid-related deaths in the region. National mortality data were also used in this study to investigate the development in Skåne compared to the rest of Sweden. The second research project focused on stakeholders’ views on the implementation of the patient choice reform. Paper III includes interviews with 33 OST patients, and paper IV consists of interviews with the managers of all OST clinics in Skåne. The results from paper I show that of the 180 deceased in opioid overdose included in the study, almost 90 per cent had been in contact with one of the examined care-providing authorities during the year prior to death. Few differences appeared with regard to which opioid contributed to the death. Paper II indicates that there has been no significant change in opioid-related deaths in Skåne after the patient choice reform and increased access to OST. An analysis on national mortality data however showed a significant yearly decrease in drug-related deaths in Skåne compared to other Swedish counties in the years following the reform (2015–2017). No change was noted in deaths related to methadone or buprenorphine in Skåne. The proportion of deaths among patients in OST increased after the introduction of the reform. The third paper indicates that patients in OST in Skåne have gained increased empowerment and influence over their treatment since the patient choice reform was introduced. Patients especially appreciated the knowledge that they could make an exit and change clinics if they so wished, even if they so far had chosen not to. In paper IV, the clinic managers were largely positive to the trend towards increased influence for patients over their treatment situation. They were more critical of the fact that there was no major differentiation between treatment providers, and that the competition that arose after the patient choice reform mainly was related to prescribing benzodiazepines.Conclusions drawn from the papers in this thesis include that patient choice of treatment provider can be viewed as a means of empowerment for patients in OST, which was regarded as positive by both patients and treatment providers. The limitations of such a system for providing OST that emerged were lack of diversity between clinics and that the competition between treatment providers largely comprised of differing views on the prescription of benzodiazepines. Further, improved access to low-threshold OST in Skåne was not associated with an increased overdose death-rate. The result that people who died from opioid overdose to a very large extent are known to society’s care-providing authorities suggests that there are considerable opportunities to reach people with problematic opioid use for therapeutic and harm reducing measures such as low-threshold OST and take-home naloxone.

    • Youth unemployment is a global issue, with supranational actors calling for policy responses, including early interventions such as outreach. This article examines how EU member states have translated the EU youth guarantee (YG) recommendation, focusing on early interventions in the form of outreach. Combining quantitative content analysis with corpus linguistics, we examined the YG implementation plans of 19 EU member states having differing institutional conditions, comparing if and how they have incorporated outreach in their policy agenda. Overall we found very little outreach in the YG-plans. In the outreach described, guidance and information on education and labour market efforts dominate, whereas social work and local level community work is largely absent. We found type of outreach to be related to institutional conditions. Our results also imply that getting member states to commit to early interventions such as outreach requires that the EU stress this issue with greater emphasis.

    • Platelet activation and the complement system are mutually dependent. Here, we investigated the effects of storage time on complement activation and platelet function in routinely produced platelet concentrates. The platelet concentrates (n = 10) were stored at 22 degrees C for seven days and assessed daily for complement and platelet activation markers. Additionally, platelet function was analyzed in terms of their responsiveness to protease-activated receptor-1 (PAR-1) and thromboxane A2 receptor (TXA(2)R) activation and their capacity to adhere to collagen. Complement activation increased over the storage period for all analyzed markers, including the C1rs/C1-INH complex (fold change (FC) = 1.9; p < 0.001), MASP-1/C1-INH complex (FC = 2.0; p < 0.001), C4c (FC = 1.8, p < 0.001), C3bc (FC = 4.0; p < 0.01), and soluble C5b-9 (FC = 1.7, p < 0.001). Furthermore, the levels of soluble platelet activation markers increased in the concentrates over the seven-day period, including neutrophil-activating peptide-2 (FC = 2.5; p < 0.0001), transforming growth factor beta 1 (FC = 1.9; p < 0.001) and platelet factor 4 (FC = 2.1; p < 0.0001). The ability of platelets to respond to activation, as measured by surface expression of CD62P and CD63, decreased by 19% and 24% (p < 0.05) for PAR-1 and 69-72% (p < 0.05) for TXA(2)R activation, respectively, on Day 7 compared to Day 1. The extent of platelet binding to collagen was not significantly impaired during storage. In conclusion, we demonstrated that complement activation increased during the storage of platelets, and this correlated with increased platelet activation and a reduced ability of the platelets to respond to, primarily, TXA(2)R activation.

    • Purpose: This study aims to explore challenges and opportunities in the digitalization of the business-to-business (B2B) customer journey in different buying situations. It also investigates where in the customer journey digital marketing is most efficient. Design/methodology/approach: This research adopts a single case study approach to examine a B2B company that implemented digitalization in its customer journey in different buying situations. Data were collected through semistructured interviews, complemented by internal documents and information from the company’s website and social media, to identify reasons for and against the decision to digitalize the B2B customer journey. Findings: Digitalization can offer firms a cost-effective and value-creating way to interact with customers in a B2B context. The B2B buying situation, however, plays a significant role in decisions on how to implement digitalization. Moreover, in the prepurchase phase, digital marketing is more effective in building awareness; in the purchase phase, personal selling is more effective in addressing customers’ needs. Research limitations/implications: The use of a single case study cannot produce results directly generalizable to other contexts. However, the findings are applicable to the digitalization of B2B customer journeys in similar industrial contexts. Practical implications: To successfully implement digitalization in the customer journey, B2B firms should choose digital tools according to different buying situations and phases in the customer journey, segment buyers by their needs rather than individual characteristics and integrate the sales and marketing functions. Originality/value: This study contradicts prior research that claims that digital marketing can be used in a similar way in both B2B and business-to-consumer contexts.     • BACKGROUND: In Europe, residential care homes (RCHs) rather than hospitals are the most common care facilities for the older adult and the place where most deaths occur in this age group. There is a lack of knowledge regarding end-of-life (EOL) discussions and how they correlate with symptoms and symptom relief.OBJECTIVE: The aim was to examine the correlation between EOL discussions and symptom occurrence, symptom relief and prescriptions or PRN drugs against symptoms for care home residents.METHODS: All expected deaths at RCHs from 1 October 2015 to 31 December 2016 and registered in the Swedish Register of Palliative Care were included. Univariate and multivariate logistic regression were used to compare the RCH residents, or their family members, who had received documented EOL discussions with a physician (the EOL discussion group) and the non-EOL discussion group.RESULTS: The EOL discussion group (n=17 071) had a higher prevalence of pain, nausea, anxiety, death rattles and shortness of breath reported, compared with the non-EOL discussion group (n=4164). Those with symptoms were more often completely relieved and had more often been prescribed PRN drugs against that symptom in the EOL discussion group. All differences remained significant when adjusting for age, time living in unit and cause of death.CONCLUSION: The results indicate that EOL discussions are correlated with higher prevalence of symptoms, but also with better symptom relief and prescription of symptom drugs PRN when symptomatic. A possible explanation for this is that the EOL discussion can work as an opportunity to discuss symptoms and treatment for symptom relief.

    • Objective: To translate and adapt the Serious Illness Conversation Guide for use within the Swedish healthcare setting and examine the validity and acceptability of the Swedish Serious Illness Conversation Guide. Methods: Three rounds of cognitive interviews were conducted (T1-3); patients (T1 n = 11; T2 n = 10; T3 n = 8), family members (T1 n = 5; T2 n = 2; T3 n = 2), and healthcare professionals (T1 n = 6; T2 n = 6; T3 n = 5). The guide was iteratively adapted based on interview feedback, clinical experience, and the literature. The guide was tested on training days with physicians and nurses. Results: The Swedish Serious Illness Conversation Guide was found to be useful in supporting serious illness conversations. Clinicians reported that some questions were emotionally challenging. Explicit questions about prognosis and timing were excluded. Instead, the dual approach of "hoping for the best and preparing for the worst" was used to explore patients' thoughts about the future. Conclusions: Patients, family members, and healthcare professionals found the Swedish Serious Illness Conversation Guide to be appropriate, sensitive, and responsive to their needs. The Swedish Serious Illness Conversation Guide may facilitate a more health-promoting approach to serious illness conversations. Further research is needed to understand the impact of these conversations on person-centered and goal-concordant care.

    • The Palliative Care Self-Efficacy Scale (PCSE) is a valid instrument in English for assessing healthcare professionals’ self-efficacy in providing palliative care; it has not been translated into Swedish. The aim of this study was to describe the translation, cultural adaptation, and content-validation process of the PCSE scale. In this study, forward and backward translations, pretesting including an expert panel (n = 7), and cognitive interviewing with possible healthcare professionals (physicians, nurses, and assistant nurses) (n = 10) were performed. Experts in palliative care rated items on a Likert scale based on their understandability, clarity, sensitivity, and relevance. The item-level content validity index (I-CVI) and modified kappa statistics were calculated. Healthcare professionals were interviewed using the think-aloud method. The translation and validation process resulted in the final version of the PCSE scale. The average I-CVI for sensitivity was evaluated and approved. Most of the items were approved for clarity, some items lacked understandability, but a majority of the items were considered relevant. The healthcare professionals agreed that the items in the questionnaire evoked emotions, but were relevant for healthcare professionals. Thus, the Palliative Care Self-Efficacy scale is relevant for assessing healthcare professionals’ self-efficacy in palliative care in a Swedish care context. Further research using psychometric tests is required.

    • INTRODUCTION: The aging population is a challenge for the healthcare system that must identify strategies that meet their needs. Practicing patient-centered care has been shown beneficial for this patient-group. The effect of patient-centered care is called patient-centered outcomes and can be appraised using outcomes measurements.OBJECTIVES: The main aim was to review and map existing knowledge related to patient-centered outcomes and patient-centered outcomes measurements for older people, as well as identify key-concepts and knowledge-gaps. The research questions were: How can patient-centered outcomes for older people be measured, and which patient-centered outcomes matters the most for the older people? STUDY DESIGN: Scoping review.METHODS: Search for relevant publications in electronical databases, grey literature databases and websites from year 2000 to 2021. Two reviewers independently screened titles and abstracts, followed by full text review and extraction of data using a data extraction framework.RESULTS: Eighteen studies were included, of which six with involvement of patients and/or experts in the process on determine the outcomes. Outcomes that matter the most to older people was interpreted as: access to- and experience of care, autonomy and control, cognition, daily living, emotional health, falls, general health, medications, overall survival, pain, participation in decision making, physical function, physical health, place of death, social role function, symptom burden, and time spent in hospital. The most frequently mentioned/used outcomes measurements tools were the Adult Social Care Outcomes Toolkit (ASCOT), EQ-5D, Gait Speed, Katz- ADL index, Patient Health Questionnaire (PHQ9), SF/RAND-36 and 4-Item Screening Zarit Burden Interview.CONCLUSIONS: Few studies have investigated the older people's opinion of what matters the most to them, which forms a knowledge-gap in the field. Future research should focus on providing older people a stronger voice in what they think matters the most to them.

    • Introduction: This report evaluates whether health related quality of life (HRQoL) and patient-reported arm morbidity one year after axillary surgery are affected by the omission of axillary lymph node dissection (ALND). Methods: The ongoing international non-inferiority SENOMAC trial randomizes clinically node-negative breast cancer patients (T1-T3) with 1-2 sentinel lymph node (SLN) macrometastases to completion ALND or no further axillary surgery. For this analysis, the first 1181 patients enrolled in Sweden and Denmark between March 2015, and June 2019, were eligible. Data extraction from the trial database was on November 2020. This report covers the secondary outcomes of the SENOMAC trial: HRQoL and patient-reported arm morbidity. The EORTC QLQC30, EORTC QLQ-BR23 and Lymph-ICF questionnaires were completed in the early postoperative phase and at one-year follow-up. Adjusted one-year mean scores and mean differences between the groups are presented corrected for multiple testing.

The latent theme Experiences of handling ethical challenges caused by the COVID-19 pandemic gave a deeper meaning to the categories.ConclusionDuring the pandemic, nursing home staff encountered ethical challenges that caused moral distress. Moral distress stemmed from not being given adequate conditions to perform their work properly, and thus not being able to give the residents adequate care. Another aspect of moral distress originated from feeling forced to act against their moral values when a course of action was considered to cause discomfort or harm to a resident. Alerting employers and policymakers to the harm and inequality experienced by staff and the difficulty in delivering appropriate care is essential. Making proposals for improvements and developing guidelines together with staff to recognize their role and to develop better guidance for good care is vital in order to support and sustain the nursing home workforce. The COVID-19 pandemic has affected both patients and staff in nursing homes, in Sweden and worldwide.Our study highlights that during the COVID-19 pandemic, nursing home staff encountered several ethical challenges which caused moral distress.Moral distress stemmed from not being given adequate conditions to perform their work, thus not giving the residents appropriate care.Moral distress could also originate from nursing home staff's feeling of being forced to act against their moral values.

To identify and prevent oral health problems among the residents, ROAG-J (Revised Oral Assessment Guide–Jönköping), a risk-assessment instrument, is used by nursing staff routinely, and the outcome is registered in the web-based Swedish quality register Senior Alert. This study aims to investigate the preventive actions registered when oral health problems are identified and the effect of these actions longitudinally. ROAG-J data registered at nursing homes in Sweden during 2011–2016 were obtained from the Senior Alert database. Out of 52,740 residents (≥65 years), 41% had oral health problems, of whom 62% had preventive actions registered. The most common action was “Assistance with cleaning teeth”. Longitudinally, during the five-year observation period, a slight increase in oral health problems assessed with ROAG-J was found. Registered preventive actions, however, led to significant improvement in the subsequent assessment for the ROAG items lips, tongue, and dentures. Standardised risk assessments like ROAG-J provide an opportunity to detect problems early and establish preventive actions. The study, however, indicates a further need for structured education and a continuous follow-up in ROAG-J. Moreover, increased collaboration between nursing and dental care to improve oral health for older residents at nursing homes is needed.

    • Aim: To investigate the extent to which the Revised Oral Assessment Guide-Jonkoping (ROAG-J) is used by nursing staff routinely in nursing homes in Sweden and to describe oral health status of the residents. Design: An observational, retrospective register-based study. Methods: Data from different validated health assessments instruments, including ROAG-J, for the period 2011-2016 were obtained from the Web-based national quality register Senior Alert. The basis for the analyses was 190,016 assessments. Results: About half of all residents had underwent at least one annual ROAG-J assessment (2014-2016). During the period 2011-2016, 42% of the residents (n = 92,827) were registered to have oral health problems. Significantly more oral health problems were found for men and for those with younger age, poorer physical condition, neurophysiological problems, underweight, impaired mobility and many medications. In conclusion, poorer oral health was found for more care-dependent individuals, which shows a need of preventive actions.

  • Modelling and optimisation in European Kidney Exchange Programmes

    • The complex multi-criteria optimisation problems arising in Kidney Exchange Programmes have received considerable attention both in practice and in the scientific literature. Whereas theoretical advancements are well reviewed and synthesised, this is not the case for practice. We present a synthesis of models and methods applied in present European Kidney Exchange Programmes, which is based on detailed descriptions we created for this purpose. Most descriptions address national programmes, yet we also present findings on emerging cross-national programmes. The synthesis provides a systematic and detailed description of the models and methods the programmes use, revealing important commonalities as well as considerable variation among them. Rather than distilling a single best practice from these results, we find that the variation in models and methods arises because of variation in country characteristics, policies, and ethics. The synthesised state of the art may benefit future national and cross-national initiatives and direct future theoretical contributions within and across the boundaries of the Operations Research discipline.

    • BACKGROUND: The Gram-negative bacterium Escherichia coli, commonly involved in severe sepsis and septic shock, shed endotoxin that upon detection by the host triggers an inflammatory cascade. Efficiency of albumin solutions to restore hypovolemia during sepsis has been debated. To aid identification of subgroups of sepsis patients that may respond positively or negatively to treatment with albumin we investigated if preparations of albumin for medical use could affect endotoxin-induced inflammatory response.METHODS: Isolated human omental arteries obtained during surgery were incubated with endotoxin in the presence or absence of albumin solution. Isolated human monocytes were incubated with endotoxin in the presence or absence of five different commercially available albumin solutions. Vascular contractile response to noradrenaline and release of interleukin (IL)-1β, IL-6, IL-8, IL-10, and tumor necrosis factor (TNF)-α were measured.RESULTS: Incubation with albumin together with endotoxin decreased median maximum contraction and increased release of IL-6 and IL-8 from the arteries compared to incubation with endotoxin alone. All albumin solutions except one significantly increased endotoxin-induced TNF-α release from monocytes. IL-6 and IL-10 were also increased and no concentration dependency of TNF-α release was observed above 2 mg mL-1 . Incubation with albumin alone did not affect contraction or release of cytokines while no potentially endotoxin-enhancing contaminant could be identified.CONCLUSION: We have shown that albumin solution in combination with endotoxin cause vasoplegia in human omental arteries, paralleled by an inflammatory response. This finding could explain the variable efficiency of albumin solutions for sepsis treatment.

  • Early Psychological Intervention After Rape : A Feasibility Study

    • Rape is the most common trauma leading to post-traumatic stress disorder (PTSD) among women, with a conditioned prevalence of up to 50%. PTSD is considered to be a lethal condition associated with increased risk of suicide, drug- and alcohol dependence, neurological- and vascular problems, as well as sick leave. Given the scope of this problem, novel and swiftly delivered interventions for this large vulnerable population are clearly warranted. One previous trial conducted in the United States (N= 137) showed that an adapted brief version of prolonged exposure (PE) to the fearful memory of the event and situations, provided in the immediate aftermath after trauma (<72 h after a traumatic event), was effective in reducing early PTSD symptoms in rape victims. The aims of the present study were to adapt the brief PE protocol to a Swedish context and investigate its feasibility and delivery in 10 executive patients recruited at the Emergency Clinic for Rape Victims in Stockholm. Ten participants were provided with three sessions of early PE with overall successful results in terms of session attendance, home-work compliance, and also symptom reduction of PTSD and depressive symptoms. However, only a fraction of the screened patients at the Emergency Clinic (5.2%) were eligible to be included in the study, where the majority (40%) were excluded due to the time criteria of 72 h. In this article, we will present detailed results of the intervention and elaborate on how to increase feasibility of preventive interventions for rape victims. In the current form, providing PE with the strict time criteria was not feasible in the clinical setting that constitutes the Emergency Department for rape.

    • The thesis addresses the issue of social work in Swedish civil society, focusing on the ambition and practice of helping others as an existential as well as a social, political and cultural phenomenon. The aim is to study the ambiguities and aporetic conflicts that imbue helping activities and the desire to help others, using ethnographic as well as philosophical method. An interview study comprising of 12 volunteers and activists, all working with begging EU-citizens in Sweden, was carried out between 2015 and 2016. The interviews aimed to gather context specific experiences of helping others from a vulnerable group, especially concerning questions of power asymmetry and moral and political dilemmas. Subsequently, a reading of the participants’ narratives informed by Jacques Derrida’s deconstruction was performed. Following Derrida’s understanding of deconstruction as a bipartite process, focusing on genealogy and representation as well as the logico-formal paradoxes, aporias or ‘undecidables’, that characterise certain phenomena, two strands of research questions guided the analysis: Firstly, how do the participants understand and create meaning regarding their helping activities? And secondly, what seemingly impossible paradoxes, aporias, emerge in the constituting ideas and practices of helping, as they are expressed in the participants' narratives?Resulting from this reading and a parallel literature study, three main areas of conflict were identified: altruistic gift-giving vs. reciprocity and exchange, objective and structural justice vs. personal relations and exceptions, and lastly, the meaning of empathy as well as the difficulty of representing the suffering of others. The main theoretical sources are Derrida, Emmanuel Levinas, Hélène Cixous and Jessica Benjamin, whose writings on economy, gifts, responsibility, desire and intersubjectivity respectively, are employed to understand and theoretically develop the participants' experiences.Connecting the difficulty of doing a good deed to feminist ethical elaborations on care and justice, I conclusively argue for a deconstructive ethics of care, that takes the aporetic character of helping others into account, while not relinquishing the responsibility towards the other and the possibility of solidarity.

  • Integrated experimental-computational analysis of a HepaRG liver-islet microphysiological system for human-centric diabetes research

    • Microphysiological systems (MPS) are powerful tools for emulating human physiology and replicating disease progression in vitro. MPS could be better predictors of human outcome than current animal models, but mechanistic interpretation and in vivo extrapolation of the experimental results remain significant challenges. Here, we address these challenges using an integrated experimental-computational approach. This approach allows for in silico representation and predictions of glucose metabolism in a previously reported MPS with two organ compartments (liver and pancreas) connected in a closed loop with circulating medium. We developed a computational model describing glucose metabolism over 15 days of culture in the MPS. The model was calibrated on an experiment-specific basis using data from seven experiments, where HepaRG single-liver or liver-islet cultures were exposed to both normal and hyperglycemic conditions resembling high blood glucose levels in diabetes. The calibrated models reproduced the fast (i.e. hourly) variations in glucose and insulin observed in the MPS experiments, as well as the long-term (i.e. over weeks) decline in both glucose tolerance and insulin secretion. We also investigated the behaviour of the system under hypoglycemia by simulating this condition in silico, and the model could correctly predict the glucose and insulin responses measured in new MPS experiments. Last, we used the computational model to translate the experimental results to humans, showing good agreement with published data of the glucose response to a meal in healthy subjects. The integrated experimental-computational framework opens new avenues for future investigations toward disease mechanisms and the development of new therapies for metabolic disorders.

  • An observational pilot study : Prevalence and cost of high frequency emergency department users at Örebro University Hospital, Sweden

    • BACKGROUND: There is little research on high frequency emergency department users (HEDU) in Sweden. We aim to determine the prevalence and costs of HEDU compared to non-HEDU at Örebro University Hospital (ÖUH). Additionally, we will determine the factors and outcomes associated with being a HEDU.METHODS: This was a retrospective, observational cohort study of ED patients presenting to ÖUH, Sweden between 2018-19. Analyses used electronic registry, ambulance, and cost data. The definition for HEDU was ≥4 visits/year. HEDUs were categorized further into Repeat, High and Super HEDU with 4-7, 8-18 and ≥19 visits/year, respectively. We used multivariable logistic regression to determine the adjusted odds ratios for factors and outcomes between HEDU and non-HEDU.FINDINGS: Of all ÖUH ED patients, 6.1% were HEDU and accounted for 22.4% of ED visits and associated costs. Compared to the mean cost of non-HEDU, the Repeat, High and Super HEDU were more costly by factors of 4, 8 and 27, respectively. The HEDUs were more likely to be male, self-referred, present with abdominal pain, arrive by ambulance, at night and from the Örebro municipal region. Super HEDU were more likely to be of adult age and assigned lower acuity scores. HEDU were more likely to be directed to the surgical zone, less likely to receive radiologic imaging or achieve a 4-hr time target. In contrast to the Repeat and High HEDU, Super HEDU were less likely to be admitted, but more likely to leave without being seen.CONCLUSION: ÖUH has a HEDU population with associated factors and outcomes. They account for a substantial proportion of ED costs compared to non-HEDU.

    • Introduction: Medulloblastoma is a malignant embryonal tumor of the cerebellum that occurs predominantly in children. To find germline genetic variants associated with medulloblastoma risk, we conducted a genome-wide association study (GWAS) including 244 medulloblastoma cases and 247 control subjects from Sweden and Denmark.Methods: Genotyping was performed using Illumina BeadChips, and untyped variants were imputed using IMPUTE2.Results: Fifty-nine variants in 11 loci were associated with increased medulloblastoma risk (p < 1 × 10–5), but none were statistically significant after adjusting for multiple testing (p < 5 × 10–8). Thirteen of these variants were genotyped, whereas 46 were imputed. Genotyped variants were further investigated in a validation study comprising 249 medulloblastoma cases and 629 control subjects. In the validation study, rs78021424 (18p11.23, PTPRM) was associated with medulloblastoma risk with OR in the same direction as in the discovery cohort (ORT = 1.59, pvalidation = 0.02). We also selected seven medulloblastoma predisposition genes for investigation using a candidate gene approach: APC, BRCA2, PALB2, PTCH1, SUFU, TP53, and GPR161. The strongest evidence for association was found for rs201458864 (PALB2, ORT = 3.76, p = 3.2 × 10–4) and rs79036813 (PTCH1, ORA = 0.42, p = 2.6 × 10–3).Conclusion: The results of this study, including a novel potential medulloblastoma risk loci at 18p11.23, are suggestive but need further validation in independent cohorts.

  • A Systematic Review of Longitudinal Trajectories of Mental Health Problems in Children with Neurodevelopmental Disabilities

    • To review the longitudinal trajectories - and the factors influencing their development - of mental health problems in children with neurodevelopmental disabilities. Systematic review methods were employed. Searches of six databases used keywords and MeSH terms related to children with neurodevelopmental disabilities, mental health problems, and longitudinal research. After the removal of duplicates, reviewers independently screened records for inclusion, extracted data (outcomes and influencing factors), and evaluated the risk of bias. Findings were tabulated and synthesized using graphs and a narrative. Searches identified 94,662 unique records, from which 49 publications were included. The median publication year was 2015. Children with attention deficit hyperactivity disorder were the most commonly included population in retrieved studies. In almost 50% of studies, trajectories of mental health problems changed by < 10% between the first and last time point. Despite multiple studies reporting longitudinal trajectories of mental health problems, greater conceptual clarity and consideration of the measures included in research is needed, along with the inclusion of a more diverse range of populations of children with neurodevelopmental disabilities.
