  • Adherence with advice and prescriptions in SLE is mostly good, but better follow up is needed: A study with a questionnaire.

    • The objective was to evaluate the efficacy of the information policy given to all systemic lupus erythematosus (SLE) patients. One hundred consecutive SLE patients were asked to answer anonymously a questionnaire covering demographic issues including education, adherence with prescriptions and advice given and methods of sourcing information. Seventy-three females and ten males responded. The demographic data showed that 34 had a university education, 29 high school and 19 primary school education. An inability to work due to disease was increased compared with the matched population (P < 0.001). Forty-two reported that they had received advice about physical training and forty of them followed this advice. Only 28 out of 46 smokers reported that they had got any advice about smoking, and out of these only 13 followed the advice. The patients with university background were less likely to smoke (P < 0.05) and followed the advice more often (P < 0.05). Most patients followed given advice about exposure to sunlight. Thirty-five percent of those prescribed glucocorticoids reported that they varied from the prescribed dosages without consultation with their specialist. This behaviour was more common in the university group (P < 0.05), this grouping also had a higher median dosage. Seventy-three patients had read the booklet about SLE provided by the clinic. Accessing internet information was more common for those with university education (P < 0.01). This study shows that on the whole SLE patients follow given advice, but adherence varies. Aside from the issue of glucocorticoid dosage adherence, educational level seems to be the most important predictor for adherence to advice. Thus, we conclude that a more individualized approach to delivery of information is required and better follow up is needed.

  • Social differentials in speed-premium effects in childbearing in Sweden

    • In Sweden, parents receive a parental-leave allowance of a high percentage (currently 80%) of their pre-birth salary for about a year in connection with any birth. If they space their births sufficiently closely, they avoid a reduction in the allowance caused by any reduced income earned between the births. The gain is popularly called a “speed premium”. In previous work we have shown that childbearing was sped up correspondingly. This is clear evidence of a causal effect of a policy change on childbearing behavior. In the present paper, we study how this change in behavior was adopted in various social strata of the Swedish population.

  • Positron emission tomography in clinical islet transplantation

    • The fate of islets in clinical transplantation is unclear. To elude on this positron emission tomography combined with computed tomography (PET/CT) was performed for 60 min during islet transplantation in five patients receiving six transplants. A fraction of the islets (23%) were labeled with 18F-fluorodeoxyglucose ([(18)F]FDG) and carefully mixed with unlabeled islets just prior to intraportal transplantation. The peak radioactivity concentration in the liver was found at 19 min after start of islet infusion and corresponded to only 75% of what was expected, indicating that islets are lost during the transplantation procedure. No accumulation of radioactivity was found in the lungs. A nonphysiological peak of C-peptide was found in plasma during and immediately after transplantation in all subjects. Distribution in the liver was heterogeneous with wide variations in location and concentration. Islets found in areas with concentrations of >400 IEQ/cc liver tissue varied between 1% and 32% of the graft in different subjects. No side effects attributed to the PET/CT procedure were found. Clinical outcome in all patients was comparable to that previously observed indicating that the [(18)F]FDG labeling procedure did not harm the islets. The technique has potential to be used to assess approaches to enhance islet survival and engraftment in clinical transplantation.

    • There is a need for tools that in a simple way can be used for the evaluation of image quality related to clinical requirements in mammography. The aim of this work was to adjust the present European image quality criteria to be relevant also for digital mammography images, and to use as simple and as few criteria as possible. A pilot evaluation of the new set of criteria was made with mammograms of 28 women from a General Electric Senographe 2000D full-field digital mammography system. One breast was exposed using the standard automatic exposure mode, the other using about half of that absorbed dose. Three experienced radiologists evaluated the images using visual grading analysis technique. The results indicate that the new quality criteria can be used for the evaluation of image quality related to clinical requirements in digital mammography in a simple way. The results also suggest that absorbed doses for the mammography system used may be substantially reduced.

  • Education and childlessness : The relationship between educational field, educational level, and childlessness among Swedish women born in 1955-59

    • In this paper we extend the concept of educational attainment to cover the field of education taken in addition to the conventional level of education attained. Our empirical investigation uses register records containing childbearing and educational histories of an entire cohort of women born in Sweden (about a quarter-million individuals). This allows us to operate with a high number of educational field-and-level combinations (some sixty in all). It turns out that the field of education serves as an indicator of a woman’s potential reproductive behavior better than the mere level attained. We discover that in each field permanent childlessness increases some with the educational level, but that the field itself is the more important. In general, we find that women educated for jobs in teaching and health care are in a class of their own, with much lower permanent childlessness at each educational level than in any other major grouping. Women educated in arts and humanities or for religious occupations have unusually high fractions permanently childless. Our results cast doubt on the assumption that higher education per se must result in higher childlessness. In our opinion, several factors intrinsic and extrinsic to an educational system (such as its flexibility, its gender structure, and the manner in which education is hooked up to the labor market) may influence the relationship between education and childlessness, and we would not expect a simple, unidirectional relationship.

  • Educational attainment and ultimate fertility among Swedish women born in 1955-59

    • This is the second of two companion papers addressing the association between educational attainment and fertility for some sixty educational groups of Swedish women, defined according to field of education as well as level of education. The first paper is about childlessness and education, the present one about the mean number of children ever born. We find that ultimate fertility decreases somewhat with an increasing educational level, but its dependence on the field of education is much more impressive. In general, educational groups with relatively little childlessness also have relatively high ultimate fertility, and educational groups with much childlessness have relatively low ultimate fertility. In particular, women educated for the teaching or health-care professions have less childlessness and a higher ultimate fertility than others. Conversely, women with an education for esthetic or (non-teacher) humanist occupations have unusually high fractions childless and low ultimate fertility. Women with religious educations stand out by having very high fractions childless but quite ordinary mean ultimate fertility nevertheless; such women have very little childbearing outside of marriage. Women with research degrees have remarkably ordinary childbearing behavior; they do not forego motherhood to the extent that some theories would predict.

  • Closed source experimental system for soft x-ray spectroscopy of radioactive materials

    • An instrumental and experimental setup for soft x-ray spectroscopy meeting the requirements of a closed source for radioactivity is described. The system consists of a vacuum sealed cell containing the sample, mounted on a tubing system to ensure compatibility with most standard manipulators. The soft x rays penetrate a thin x-ray window separating the interior of the cell from the vacuum in the experimental chamber. Our first results for single crystal PuO2 confirm the feasibility of experiments using the setup. The results are consistent with results of first principles calculations and previously recorded spectra obtained using a standard open source setup. The results show that the closed source experimental system can be used to collect valuable experimental data from radioactive materials.
