  • Aging-in-Place: Residents’ Attitudes and Floor Plan Potential in Apartment Buildings From 1990 to 2015
  • 2021
  • Ingår i: HERD. - : SAGE Publications. - 2167-5112 .- 1937-5867. ; 14:4, s. 211-226
  • Tidskriftsartikel (refereegranskat)abstract
    • Objectives: We investigated apartment designs in apartment blocks built 1990-2015 in Gothenburg, Sweden. We investigated the residents' attitudes toward their previous, present, and future housing and their perceived possibilities for aging-in-place. We analyzed their apartments, focusing on the possibilities for aging-in-place in future care situations concerning bedroom capacity in a care situation; spatial proximity between bathroom, bedroom, storage, and entrance; and functional autonomy in a care situation without too much disturbance for a partner. Background: Since the 2000s, the ambition in Sweden is to enable older people to remain in ordinary housing. The possibilities for aging-in-place should therefore be considered already in the design stage, also when producing standard apartments. Methods: Semi-structured interviews were made with 30 households, with one or more resident 65 years or older. Floor plan analyses were made of their present apartments. Results: The majority displayed a pragmatic attitude toward aging, high satisfaction with their present housing situation, and good chances for aging-in-place in future homecare scenarios. The floor plan analysis shows that the three concepts of bedroom capacity, spatial proximity, and functional autonomy can be used to determine the potential for aging-in-place. Conclusions: The results suggest that architectural qualities related to aging-in-place are not automatically connected to floor size or number of rooms. Small apartments can perform better than larger ones, depending on spatio-functional organization and connections between different functions. The residents' perceived chances for aging-in-place confirm this relation. Future studies should compare different locations, production periods, and relations between size, space efficiency, and accessibility.

  • Assisted living for the elderly-Features of a swedish model
  • 2020
  • Ingår i: Architecture for Residential Care and Ageing Communities: Spaces for Dwelling and Healthcare. - New York, NY : Routledge, 2021. : Routledge. ; , s. 131-141
  • Bokkapitel (övrigt vetenskapligt/konstnärligt)abstract
    • In Sweden, the provision of assisted living (AL) for older people is a municipal responsibility. Most eldercare provision is given directly by the municipalities, but a fast-growing private sector is emerging. Sweden shares the demographic problems related to ageing with other countries but has specific challenges to meet concerning economic, demographic and organizational issues. Like in many other countries, care provision is moving from the hospitals out to the older patients in ordinary and special housing. This necessitates a discussion of what AL should provide and how to address residential aspects, caregiving and service provision in AL, targeting the physical environments. Home aspects in AL must be related to aspects of control, at-homeness and personalization. There are also trends and challenges on different levels, concerning, for example, family structures, existing facilities in use, quality of life and alternative options for housing and care in advanced age. To address the current situation, we need to increase the attractivity of AL, both concerning residential and workplace aspects. We also need to stimulate long-term planning and cooperation in order to update AL schemes, for example, concerning tenure, options and choices in order to have effective, usable and resilient facilities over time.

  • Bygglov Göteborg 2017.
  • 2022

    • Assisted living facilities for elderly represents the major costs within the budget for eldercare for municipalities in Sweden. As the municipalities are responsible for public eldercare, which consumes a fifth of the total municipal budget, the consequence of the demographic development within the coming decades will imply major challenges for this sector. It also stresses the importance of creating adequate living conditions for the growing number of frail, elderly people. Political measures are underway and involve new housing concepts for senior citizens and changed conditions for assisted living. This PhD-project is ongoing 2009-2013 and is jointly financed by the City of Gothenburg and Chalmers University of Technology. The City has a great interest in research about assisted living facilities, being responsible for the construction and maintenance, as well as the daily use represented by the eldercare providers. It is therefore crucial to understand the prerequisites for usability in existing facilities, in order to create new and adequate environments. The project is expected to contribute to building programming, eldercare facilities planning and property management in the eldercare sector. This thesis is based upon an observation study in five assisted living facilities in Gothenburg. The results provide partial answers to the question: "How are the environments used and how is the usability affected by the physical environment and by the users’ activities?" The results highlight a number of discrepancies between the actual use and the intended use of the environments and their various functions. It also highlights a number of conflicts in use between different users or groups of users, along with differing purposes for the use of the premises. The thesis points to the complex relationship between use and activity and emphasizes the need for adequate planning concerning eldercare facilities. Particular attention must to be given to the relationship between facilities, staff and elderly and their use of the facilities. From a practical point of view the study shows how changes in use over time can have a negative effect on the daily activities. To develop an understanding of processes evolving in daily use of assisted living facilities, further studies needs to include the users' attitudes in order to better understand the context for activities in the facilities.

  • Assisted Living: Long-term housing solution or short-term care at the end of life?
  • 2014
  • Ingår i: 22nd Nordic Congress of Gerontology, Gothenburg, 25-28 May 2014.
  • Konferensbidrag (övrigt vetenskapligt/konstnärligt)abstract
    • This research project displays the objectives for AL – residence and care – from the architectural discipline. These dichotomous objectives bring about conflicts in the use of the physical environments. It is suggested that the long-term perspective of residence and home is in conflict with the short-term care perspective. Assisted Living (AL) in Sweden displays great similarities with AL in other countries, although eldercare may be differently organized. The similarities entail a physical structure with subdivision in residential groups of apartments with shared common spaces for communal activities. They also entail an organizational structure with common activities, medical care and front-line staff (FLS) available around the clock. Residents in Swedish AL facilities are increasingly old and more multi-diseased, mainly due to increased efforts in home-based care and services. In order to meet with the changing user requirements the typology of AL has to be discussed both in a national and international context. The research is based on empirical material from observations, interviews and questionnaires in Swedish eldercare and relates to the ongoing discussion on special housing for older people in Sweden and other countries. The results suggest that efforts to create a long-term home-like residential environment are discernible on several levels: in legislation and guidelines, in the organizations and in the local routines. In contrast to these efforts, short-term care aspects tend to dominate as more AL residents become in need of end-of-life (EOL) care. This entails changing user requirements with increased focus on medical measures and more use of supportive technology. Swedish legislation equals AL and ordinary housing, with the exception that AL contains common spaces in addition to the individual apartments. The typological discussion contains issues concerning physical structure; size, number of residents, the design of common spaces and differences between specialized units or groups for people with dementia (SDUs) and ordinary AL.

  • Common spaces in assisted living for older persons. Aspects of usability from the residential and workplace perspectives.
  • 2013
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • This thesis is about space. It assumes that space forms a necessary part of the structure of existence. The architectural space becomes an existential space in the encounter with the user. Common unit spaces in assisted living (AL) for older persons are the principal venue for social interaction and have no parallel in ordinary housing. The usability of these spaces is discussed here from the residential and workplace perspectives. These perspectives are explored in relation to the concept of AL and involve residents, staff and other stakeholders in buildings in use. Special housing for older people is currently in focus worldwide due to demographic develop-ments, entailing an increasing proportion of older people. Residents in AL are increasingly old and multi-diseased, which changes the conditions for residential care in its present form.Four methods were used, involving 14 AL facilities (ALFs) in Sweden: observations, group interviews, individual interviews and self-completion questionnaires. An explorative strategy was adopted, combining qualitative and quantitative methods to enhance the validity of the results. The results were analysed using statistical analyses, Qualitative Content Analysis (QCA) and triangulation. The results show functional demarcations between apartments, dining rooms, sitting rooms and kitchens in a private-public continu¬um, with varying adherence to the respective residential and workplace perspectives. It also shows that these perspectives entail diverging objectives for use of the common spaces. The dining rooms were utilized most by the residents, but the kitchens were not used at all. The daily activities promote a focus on collective aspects above individual needs. The increasing use of assistive technology causes space shortages and suggests a mismatch between the actual users and the users conceptualised at the planning stage, which entails a focus on care aspects at the expense of the residential perspective. It shows that the common spaces are not perceived as a part of the home environment; rather an addition. The dementia and somatic units presented diverging objectives for use, but the physical environ-ments did not differ significantly. The functions of the common spaces were perceived differently among users, planners and architects. This suggests that these spaces have ambiguous meanings in a social context and may lead to ineffective use of the spaces. Along with a complicated body of regulations, this calls for a redefinition of AL. Design strategies are suggested that are more up-to-date with the users’ needs in relation to the usability of the common spaces. The residential and workplace perspectives have to be considered concomitantly when planning ALFs, otherwise inherent conflicts will become manifest as a result of the physical design, which affects usability.

  • 2014
  • Ingår i: HERD. - : SAGE Publications. - 2167-5112 .- 1937-5867. ; 7:3, s. 98-119
  • Tidskriftsartikel (refereegranskat)abstract
    • OBJECTIVE: This exploratory study examines the function and use of common spaces in assisted living facilities (ALFs) from the residential and workplace perspectives.BACKGROUND: The impact of the physical environment on human activities in healthcare settings has been emphasized in many studies. Few studies, however, have explored the daily use of common spaces and theimpact on the usability of ALFs.METHODS: Four explorative methods–observation, group interviews, individual interviews, and questionnaires—were used to investigate 14 ALFs in Sweden. The study involves residents, staff, relatives, architects, and people responsible for planning and construction of eldercare. This research strategy combines quantitative and qualitative methods to enhance the validity of the results. Method triangulation and data triangulationwere used and the data were analyzed using Qualitative Content Analysis (QCA).RESULTS: The results show that residents and staff have different objectives for use and these differences affect usability, although explicit conflicts are rare. The residents, staff, and other stakeholders have differentviews about the demarcation of home and workplace and the role of common spaces as venues for social interaction.CONCLUSIONS: Both the residential and the workplace perspective must be considered when planning assisted living facilities. Otherwise, inherent conflicts between these perspectives will manifest as a result of thephysical design. Common spaces have diverse functions that are reflected in their spatial organization. Therefore, ALFs should be designed so the intended function of a specific space is apparent to all users.

  • Is it possible to feel at home in a patient room in an intensive care unit? Reflections on environmental aspects in technology-dense environments
  • 2019
  • Ingår i: Nursing Inquiry. - : Wiley. - 1440-1800 .- 1320-7881. ; 26:4
  • Tidskriftsartikel (refereegranskat)abstract
    • This paper focuses on the patient's perspective and the philosophical underpinnings that support what might be considered optimal for the future design of the intensive care unit (ICU) patient room. It also addresses the question of whether the aspects that support at-homeness are applicable to ICU patient rooms. The concept of “at-homeness” in ICUs is strongly related to privacy and control of space and territory. This study investigates whether the sense of at-homeness can be created in an ICU, when one or more patients share a room. From an interdisciplinary perspective, we critically reflect on various aspects associated with conflicts surrounding the use of ICU patient rooms. Thus, from an architectural and a caring perspective, the significance of space and personal territory in ICU patient rooms is emphasized. Recommendations for further research are suggested. In conclusion, privacy and control are deemed to be essential factors in the stimulation of recovery processes and the promotion of well-being in situations involving severe illness or life-threatening conditions.

  • 2013
  • Ingår i: Facilities. - : Emerald. - 0263-2772 .- 1758-7131. ; 31:1/2, s. 24-38
  • Tidskriftsartikel (refereegranskat)abstract
    • Purpose – The purpose of this paper is to map significant features of the physical design of nine recently planned facilities for forensic psychiatric care in Sweden. The present paper is focused on differences in the physical design, static security adaptations, visions and goals for the projects, economy and steering processes. Design/methodology/approach – In June 2008, records concerning major forensic psychiatric construction projects, planned or carried out between 1970 and 2008, were requested from all 21 regions in Sweden. The documents were collected, organized, critically examined, and analyzed in their contexts. Extensive data have also been retrieved from the internet. Findings – In spite of the common national legislation governing forensic psychiatric care, the projects show great diversity in the physical design and, after 2006, increasing emphasis on static security. The collected material indicates different visions and goals and little coordination between them. It also suggests that the decisions rarely have been preceded by scientific studies or, as it appears, systematic needs assessments. There were also considerable variations in the interpretation of the legislation stipulating public access to official documentation and the time-frame allowed for such requests. Research limitations/implications – Limitations of the project especially relate to the incompleteness of the documents received. Originality/value – This mainly descriptive paper provides an overview of contemporary Swedish forensic psychiatric construction projects, planned for or realized after 2000. This paper points out structural and physical differences between these projects. The systematised documents are made available for research purposes within different disciplines.

  • Konferensbidrag (refereegranskat)abstract
    • Relation to the complexity and usability of common spaces in assisted living facilities (ALFs). Background: ALFs for older people in Sweden contain both the residential and workplace perspectives. The common spaces retain physical and organizational features that are related to traditional institutional care environments. The use of common spaces in ALFs focuses heavily on communal activities, entailing a complex relation to the concept of home in a social context and of the home as a place. Approach: A mixed QUAL/QUAN approach was applied containing method and data trian-gulation. Participant observations, semi-structured interviews, questionnaires and statistical analyses were used. The project encompasses 14 ALFs in Sweden and includes residents, staff, relatives, planners and architects. Results: When the relative context of the building in use diverges from the normative context of planning and conceptualization, it affects the usability. Residential and workplace per-spectives entail different objectives for use, which also affects the usability. The results also show different objectives for use between dementia and somatic units. Although Swedish regulations include the common spaces as part of the housing unit, the different functions are used and perceived differently and to a varying degree. The spaces represent functions with varying proximity to the concept of home; placing the dining rooms closer and the kitchens further away. Higher degree of use on the dementia units places the common spaces closer to the concept of home here, compared to the somatic units.Discussion: The normative aspects of architecture represent a fixation in time and space as they are manifested in the design processes and, as a result, in the physical structures. Usability, contrarily, represents relational aspects as it is created in continuous negotiation between the users, residents and staff, and the physical environment; or in the interaction between human and non-human actors. The prerequisites for use are continuously changing in relation to the planning context. Common spaces must be understood from an extended concept of home that takes into consideration the context of the building in use.

  • 2014
  • Ingår i: Proceedings of the RGS-IBG Annual International Conference 2014. London 26-29 August. Session title: The geographies of architecture, knowledge and care..
  • Konferensbidrag (övrigt vetenskapligt/konstnärligt)abstract
    • Usability is created in negotiation between environment and users, or between human and non-human actors. In contrast to this relational approach, architecture practices focus on normative aspects of the built environment from artistic and technological perspectives. The usability of care environments, here in assisted living, is related to a necessary architectural generality of institutional contexts, which counteracts personalization. Common rooms in assisted living are designed for shared social activities, in which ambiguous meanings emerge to the users and position the rooms in an unclear location in a private-public continuum. The usability of these rooms is generated both by the users’ disparate understanding of the spaces in a social context and the evasiveness of a private-public dichotomy. Normative aspects of the design processes are discernible, in contrast to relational aspects of usability, which are the result of user-artefact interaction over time.

    • This study explores how common spaces in assisted living schemes for older persons are used by the residents. Observation studies, group interviews with staff and individual interviews with residents, relatives, architects and key stakeholders in the context of Swedish elder-care were conducted. Common spaces are the major location for in-house social interaction on the units. The results show a higher presenceon the dementia units, compared to the somatic units. No significant correlation was found between the residents’ mobility limitations and their degree of presence in the common spaces. The results also suggest a contradiction between the staff’s intentions to provide a social context and the capabilities of the residents. Although common spaces are not used much between meals, the residents stress their importance for social interaction, suggesting that common spaces have important qualitative aspects, rather than quantitative. The results also how that few of the residents used the common spaces together with their relatives. The increasing use of assistive technology creates a shortage of space, suggesting a conflict between the efforts to create a home-like environment and the use of assistive technology.

Participant observation is used to study the daily use of common spaces in 14 assisted living units in Gothenburg. The results show examples where the currentuse diverges from the intended use and suggest diverging objectives and conflicts of use. The observations indicate that the degree of usability is determined by the physical environment itself in some cases, by the users and by changes in use in other cases, and by a combination of both in some cases.

  • Genes regulating molecular and cellular functions in noninfectious nonallergic rhinitis.
  • 2009
  • Ingår i: Allergy. - : Wiley. - 1398-9995 .- 0105-4538. ; 64:9, s. 1301-8
  • Tidskriftsartikel (refereegranskat)abstract
    • Chronic noninfectious, nonallergic rhinitis (NINAR) is a complex syndrome with a principally unknown pathophysiology. New technology has made it possible to examine differentially expressed genes and according to network theory, genes connected by their function that might have key roles in the disease.

  • TOTALL: high cost of allergic rhinitis-a national Swedish population-based questionnaire study.
  • 2016
  • Ingår i: NPJ primary care respiratory medicine. - : Springer Science and Business Media LLC. - 2055-1010. ; 26
  • Tidskriftsartikel (refereegranskat)abstract
    • Allergic rhinitis is a global illness with a well-recognised impact on quality of life and work performance. Comparatively little is known about the extent of its economic impact on society. The TOTALL study estimates the total cost of allergic rhinitis using a sample representing the entire Swedish population of working age. A questionnaire focused on allergic rhinitis was mailed out to a random population of Swedish residents, aged 18-65 years. Health-care contacts, medications, absenteeism (absence from work) and presenteeism (reduced working capacity at work) were assessed, and the direct and indirect costs of allergic rhinitis were calculated. Medication use was evaluated in relation to the ARIA guidelines. In all, 3,501 of 8,001 (44%) answered the questionnaire, and 855 (24%) of these reported allergic rhinitis. The mean annual direct and indirect costs because of allergic rhinitis were €210.3 and €750.8, respectively, resulting in a total cost of €961.1 per individual/year. Presenteeism represented 70% of the total cost. Antihistamines appear to be used in excess in relation to topical steroids, and the use of nasal decongestants was alarmingly high. The total cost of allergic rhinitis in Sweden, with a population of 9.5 million, was estimated at €1.3 billion annually. These unexpectedly high costs could be related to the high prevalence of disease, in combination with the previously often underestimated indirect costs. Improved adherence to guidelines might ease the economic burden on society.

  • Central NMU signaling in body weight and energy balance regulation: evidence from NMUR2 deletion and chronic central NMU treatment in mice.
  • 2009
  • Ingår i: American journal of physiology. Endocrinology and metabolism. - : American Physiological Society. - 1522-1555 .- 0193-1849. ; 297:3
  • Tidskriftsartikel (refereegranskat)abstract
    • To investigate the role of the central neuromedin U (NMU) signaling system in body weight and energy balance regulation, we examined the effects of long-term intracerebroventricular (icv) infusion of NMU in C57Bl/6 mice and in mice lacking the gene encoding NMU receptor 2. In diet-induced obese male and female C57BL/6 mice, icv infusion of NMU (8 microg x day(-1) x mouse(-1)) for 7 days decreased body weight and total energy intake compared with vehicle treatment. However, these parameters were unaffected by NMU treatment in lean male and female C57BL/6 mice fed a standard diet. In addition, female (but not male) NMUR2-null mice had increased body weight and body fat mass when fed a high-fat diet but lacked a clear body weight phenotype when fed a standard diet compared with wild-type littermates. Furthermore, female (but not male) NMUR2-null mice fed a high-fat diet were protected from central NMU-induced body weight loss compared with littermate wild-type mice. Thus, we provide the first evidence that long-term central NMU treatment reduces body weight, food intake, and adiposity and that central NMUR2 signaling is required for these effects in female but not male mice.

  • Transforming knowledge systems for life on Earth : Visions of future systems and how to get there
  • 2020
  • Ingår i: Energy Research & Social Science. - : Elsevier. - 2214-6296 .- 2214-6326. ; 70
  • Tidskriftsartikel (refereegranskat)abstract
    • Formalised knowledge systems, including universities and research institutes, are important for contemporary societies. They are, however, also arguably failing humanity when their impact is measured against the level of progress being made in stimulating the societal changes needed to address challenges like climate change. In this research we used a novel futures-oriented and participatory approach that asked what future envisioned knowledge systems might need to look like and how we might get there. Findings suggest that envisioned future systems will need to be much more collaborative, open, diverse, egalitarian, and able to work with values and systemic issues. They will also need to go beyond producing knowledge about our world to generating wisdom about how to act within it. To get to envisioned systems we will need to rapidly scale methodological innovations, connect innovators, and creatively accelerate learning about working with intractable challenges. We will also need to create new funding schemes, a global knowledge commons, and challenge deeply held assumptions. To genuinely be a creative force in supporting longevity of human and non-human life on our planet, the shift in knowledge systems will probably need to be at the scale of the enlightenment and speed of the scientific and technological revolution accompanying the second World War. This will require bold and strategic action from governments, scientists, civic society and sustained transformational intent.

  • 2016
  • Ingår i: The Journal of laryngology and otology. - 1748-5460. ; 130:3, s. 296-301
  • Tidskriftsartikel (refereegranskat)abstract
    • Symptoms of sleep-disordered breathing in children, such as frequent snoring, apnoea and choking, may lead to health problems if untreated. The caregiver's level of awareness of these symptoms has been poorly studied. This study aimed to study healthcare provider contact related to sleep-disordered breathing symptoms in a population of children aged 0-11 years.

  • Perceived safety in extra-care housing for senior residents
  • 2018
  • Ingår i: Journal of Housing for the Elderly. - : Informa UK Limited. - 0276-3893 .- 1540-353X. ; 32:1, s. 58-72
  • Tidskriftsartikel (refereegranskat)abstract
    • Extra-care housing (ECH) is a new housing model in Sweden, falling in between ordinary homes and sheltered housing. The perception of safety among residents living in ECH was explored with the use of in-depth interviews (n = 28, on average 83 years old). A qualitative thematic analysis resulted in a model of perceived safety in the housing environment consisting of four themes: being able to manage on my own; a safe social context; being able to stay; and protection and safety. The model can be used for information, planning, and development of ECH in general.

  • The impact of the physical environment for caregiving in ordinary housing: Experiences of staff in home- and health-care services
  • 2021
  • Ingår i: Applied Ergonomics. - : Elsevier BV. - 0003-6870 .- 1872-9126. ; 92
  • Tidskriftsartikel (refereegranskat)abstract
    • The strong driving forces for ageing in place demand sustainable solutions for the housing and care of older people and the health and safety of home- and health-care staff. The aim of the study was to elucidate staff experiences of providing home- and health-care to older people living in ordinary housing. This study was part of a larger project investigating the relation between home design and conditions for care in ordinary housing. The data were gathered through focus group interviews with staff in home- and health-care. Three main themes were found according to staff experiences of particular rooms' sizes and proportions, spatial configurations, and aspects to consider when designing new housing. This study contributes important knowledge about essential features of the physical environment for staff providing home- and health-care for older people in their own homes and to aid the development of functionally sustainable housing to minimise injuries to staff.

  • #metoo-management
  • 2018
