  • Alcohol Consumption and Survival after a Breast Cancer Diagnosis: A Literature-Based Meta-analysis and Collaborative Analysis of Data for 29,239 Cases

    • Background: Evidence for an association of alcohol consumption with prognosis after a diagnosis of breast cancer has been inconsistent. We have reviewed and summarized the published evidence and evaluated the association using individual patient data from multiple case cohorts. Methods: A MEDLINE search to identify studies published up to January 2013 was performed. We combined published estimates of survival time for "moderate drinkers" versus nondrinkers. An analysis of individual participant data using Cox regression was carried out using data from 11 case cohorts. Results: We identified 11 published studies suitable for inclusion in the meta-analysis. Moderate postdiagnosis alcohol consumption was not associated with overall survival [HR, 0.95; 95% confidence interval (CI), 0.85-1.05], but there was some evidence of better survival associated with prediagnosis consumption (HR, 0.80; 95% CI, 0.73-0.88). Individual data on alcohol consumption for 29,239 cases with 4,839 deaths were available from the 11 case cohorts, all of which had data on estrogen receptor (ER) status. For women with ER-positive disease, there was little evidence that pre-or postdiagnosis alcohol consumption is associated with breast cancer-specific mortality, with some evidence of a negative association with all-cause mortality. On the basis of a single study, moderate postdiagnosis alcohol intake was associated with a small reduction in breast cancer-specific mortality for women with ER-negative disease. There was no association with prediagnosis intake for women with ER-negative disease. Conclusion: There was little evidence that pre- or post-diagnosis alcohol consumption is associated with breast cancer-specific mortality for women with ER-positive disease. There was weak evidence that moderate post-diagnosis alcohol intake is associated with a small reduction in breast cancer-specific mortality in ER-negative disease. Impact: Considering the totality of the evidence, moderate postdiagnosis alcohol consumption is unlikely to have a major adverse effect on the survival of women with breast cancer. (C) 2014 AACR.

  • Duality in refined Watanabe-Sobolev spaces and weak approximations of SPDE

    • In this paper we introduce a new family of refined Watanabe- Sobolev spaces that capture in a fine way integrability in time of the Malliavin derivative. We consider duality in these spaces and derive a Burkholder type inequality in a dual norm. The theory we develop allows us to prove weak convergence with essentially optimal rate for numerical approximations in space and time of semilinear parabolic stochastic evolution equations driven by Gaussian additive noise. In particular, we combine Galerkin finite element methods with a backward Euler scheme in time. The method of proof does not rely on the use of the Kolmogorov equation or the It¯o formula and is therefore in nature non-Markovian. With this method polynomial growth test functions with mild smoothness assumptions are allowed, meaning in particular that we prove convergence of arbitrary moments with essentially optimal rate. Our Gronwall argument also yields weak error estimates which are uniform in time without any additional effort.

  • Weak convergence for a spatial approximation of the nonlinear stochastic heat equation

    • We find the weak rate of convergence of approximate solutions of the nonlinear stochastic heat equation, when discretized in space by a standard finite element method. Both multiplicative and additive noise is considered under different assumptions. This extends an earlier result of Debussche in which time discretization is considered for the stochastic heat equation perturbed by white noise. It is known that this equation only has a solution in one space dimension. In order to get results for higher dimensions, colored noise is considered here, besides the white noise case where considerably weaker assumptions on the noise term is needed. Integration by parts in the Malliavin sense is used in the proof. The rate of weak convergence is, as expected, essentially twice the rate of strong convergence.

  • Weak error analysis for semilinear stochastic Volterra equations with additive noise

    • We prove a weak error estimate for the approximation in space and time of a semilinear stochastic Volterra integro-differential equation driven by additive space-time Gaussian noise. We treat this equation in an abstract framework, in which parabolic stochastic partial differential equations are also included as a special case. The approximation in space is performed by a standard finite element method and in time by an implicit Euler method combined with a convolution quadrature. The weak rate of convergence is proved to be twice the strong rate, as expected. Our weak convergence result concerns not only the solution at a fixed time but also integrals of the entire path with respect to any finite Borel measure. The proof does not rely on a Kolmogorov equation. Instead it is based on a duality argument from Malliavin calculus.

  • Audiometric screening of a population with intellectual disability

    • Objective: Evaluation of pure-tone audiometry (PTA) in hearing screening of a population with mild to profound intellectual disability (ID).Design: PTA was performed at six frequencies at the screening level 20 dB HL. Referral criteria were threshold levels ≥ 25 dB HL at two or more frequencies for one ear or both.Study sample: 1478 participants aged 7–91 years were included.Results: 1470 (99.5%) people cooperated in screening of which 1325 (90%) could be tested on both ears at all six frequencies. A majority, 987 (66.8%), performed ordinary PTA, 234 (15.8%) conditioned play audiometry, and 249 (16.9%) behavioural observation audiometry. Six hundred and sixty-nine (45%) passed and 809 (55%) failed according to referral criteria. Of those failing, 441 (54.5%) accepted referral to clinical evaluation.Conclusions: PTA with slight modifications is applicable for screening of a population with mild to profound intellectual disability. The most challenging and time-consuming activity is to introduce the test procedure in a way that reduces anxiety and establishes trust.

  • Variational properties of a pumped dynamical system

    • We earlier constructed a generalized entropy concept to show the direction of time in an evolution following from a Markov generator. In such a dynamical system the entity found changes in a monotonic way starting from any initial state of the system. In this article, we generalize the treatment to the case when population is pumped into the system from levels not explicitly considered. These populations then pass through the coupled levels and exit by decay to levels outside the system. We derive the form of the equation of motion and relate it to our earlier treatments. It turns out that the formalism can be generalized to the new situation. Its physically relevant features are demonstrated and the behavior obtained is illustrated by a numerical treatment of the standard two-level system with pumping and relaxation included.

    • Early gender archaeology formulated two statements: men are visible, women are invisible, and men work in hard materials, women work in soft materials. We discuss these dichotomies in connection with nineteenth-century folklore and an excavated eighteenth-century cottage at a summer-farm. We conclude that much of the gendered order-of-work tasks broke down in pragmatic day-to-day life, especially by women crossing the gender border. However, social chaos was held at bay by ritual acts and magic objects.

    • This dissertation analyses social critique, communication critique and aestheticalcritique in television produced by artists. Theoretically it draws on researchon alternative media, TV studies, especially genre analysis and narratology,and media aesthetics. It conducts a text-production study of three examplesof alternative television from the period 2004-2008: ContemporaryArt Center TV (CAC TV): A show produced by the CAC in Vilnius, Lithuaniaand aired on a commercial TV-channel; Good TV who aired video art ona local public access channel in Stockholm, Sweden; and Candyland TV, apirate transmission from an art gallery in central Stockholm.Empirically it builds on TV-texts, web sites and documents, as well asinterviews with participants. Through a study of form and stylistics, relationto conventional genres and modes of narration, it engages in a discussionabout the features of a critical, alternative media text.The study shows how these televisions work in a tradition of alternativetelevision and connects them to tactics and aesthetical forms as found inhistorical examples, but also how this type of formalist media critiquemight inform an understanding of alternative media. From the analysis ofrelations between social and formalist aspects of alternative television, adistinction between alternative as ”alternative worldview” and as ”alternativeexpressions” is suggested, a distinction that contributes to the developmentof theory in the study of alternative media.

    • Denna inventering av violgubbe (Gomphus clavatus) och rödlistade fjälltaggsvampar (Sarcodon spp.) har utförts på uppdrag av Länsstyrelsen i Västmanlands län. Syftet har varit att öka kunskapen om arternas förekomst i länet samt behovet av skydd och andra åtgärder på de olika lokalerna. Inventeringen har genomförts inom ramen för åtgärdsprogrammen (ÅGP) för bevarande av violgubbe och rödlistade fjälltaggsvampar. Inventeringen har genomförts under september 2008 av Leif Andersson, Kurt- Anders Johansson, Rolf-Göran Carlsson, Stig Jacobsson och Tomas Fasth. Alla kända lokaler för violgubbe och rödlistade fjälltaggsvampar besöktes. Även lokaler för arter med likartade miljökrav har besökts och områden kända för kalkrik berggrund. Under inventeringen har mycel för de eftersökta arterna noterats och koordinater för dessa tagits. Även andra svampar har noterats om än i olika omfattning. Totalt har ett hundratal lokaler besökts varav svampar har noterats på 80 lokaler. Säsongen var mycket god för fjälltaggsvampar, möjligen med undantag för brödtaggsvamp (Sarcodon versipellis). För violgubbe var säsongen något sämre. Totalt noterades violgubbe och/eller rödlistade fjälltaggsvampar på 17 lokaler. Av de 19 kända lokalerna för violgubbe noterades den på sex och en ny lokal upptäcktes. Tidigare var 20 lokaler kända för de olika rödlistade fjälltaggsvamparna varav återfynd gjordes på sex av dessa. Dessutom gjordes 14 nya fynd av rödlistade fjälltaggvampar. Nya för Västmanlands län är blåfotad taggsvamp (Sarcodon glaucopus), sammetstaggsvamp (Sarcodon martioflavus) och den ännu obeskrivna Sarcodon ”modestum”. Den goda svamptillgången 2008 gjorde att särskilt många rödlistade spindelskivlingar noterades. Flera arter är nya för Västmanlands län. Även andra nyfynd av rödlistade svampar för Västmanlands län gjordes t. ex. narrvaxskivlingen (Hygrophorus subviscifer) som noterades på två platser. Totalt gjordes 146 fynd av 42 rödlistade svampar inklusive violgubbe och rödlistade fjälltaggsvampar. Ett antal lokaler och områden har kristalliserats ut som särskilt intressanta för de eftersökta svamparna. I Sala kommun finns många värdefulla områden, särskilt nära tätorten, ofta i närheten av där gruvdrift bedrivits. Särskilt Snarpsätraskogen norr om staden samt områden söder och sydväst om staden hyste många hotade arter. Områdena söder om staden ingår i Salakalkens Natura 2000-område, men skogsnaturtyperna är i dagsläget inte anmälda som naturtyp. I Fagersta och Norberg finns också värdefulla stråk för violgubbe och rödlistade fjälltaggsvampar. Särskilt ska här framhållas Jättåsarna i Fagersta kommun. Flera områdena är inte utpekade som naturvärden eller nyckelbiotoper och saknar också formellt områdesskydd. Ett antal lokaler för de eftersökta svamparna finns i naturreservat och biotopskydd.

    • BackgroundRadical prostatectomy reduces mortality among men with localized prostate cancer; however, important questions regarding long-term benefit remain. MethodsBetween 1989 and 1999, we randomly assigned 695 men with early prostate cancer to watchful waiting or radical prostatectomy and followed them through the end of 2012. The primary end points in the Scandinavian Prostate Cancer Group Study Number 4 (SPCG-4) were death from any cause, death from prostate cancer, and the risk of metastases. Secondary end points included the initiation of androgen-deprivation therapy. ResultsDuring 23.2 years of follow-up, 200 of 347 men in the surgery group and 247 of the 348 men in the watchful-waiting group died. Of the deaths, 63 in the surgery group and 99 in the watchful-waiting group were due to prostate cancer; the relative risk was 0.56 (95% confidence interval [CI], 0.41 to 0.77; P=0.001), and the absolute difference was 11.0 percentage points (95% CI, 4.5 to 17.5). The number needed to treat to prevent one death was 8. One man died after surgery in the radical-prostatectomy group. Androgen-deprivation therapy was used in fewer patients who underwent prostatectomy (a difference of 25.0 percentage points; 95% CI, 17.7 to 32.3). The benefit of surgery with respect to death from prostate cancer was largest in men younger than 65 years of age (relative risk, 0.45) and in those with intermediate-risk prostate cancer (relative risk, 0.38). However, radical prostatectomy was associated with a reduced risk of metastases among older men (relative risk, 0.68; P=0.04). ConclusionsExtended follow-up confirmed a substantial reduction in mortality after radical prostatectomy; the number needed to treat to prevent one death continued to decrease when the treatment was modified according to age at diagnosis and tumor risk. A large proportion of long-term survivors in the watchful-waiting group have not required any palliative treatment. (Funded by the Swedish Cancer Society and others.) The randomized Swedish trial of prostatectomy versus watchful waiting in disease detected mainly clinically (not by PSA screening) continues to show a benefit for early prostatectomy. The number of men younger than 65 needed to treat to prevent one death is now four. The Scandinavian Prostate Cancer Group Study Number 4 (SPCG-4), a randomized trial of radical prostatectomy versus watchful waiting in men with localized prostate cancer diagnosed before the era of prostate-specific antigen (PSA) testing, showed a survival benefit of radical prostatectomy as compared with observation at 15 years of follow-up.(1) By contrast, the Prostate Cancer Intervention versus Observation Trial (PIVOT), initiated in the early era of PSA testing, showed that radical prostatectomy did not significantly reduce prostate cancer-specific or overall mortality after 12 years.(2) PSA screening profoundly changes the clinical domain of study.     • Introduction: Today, a number of therapeutic options are available as the patient with myeloma relapses from initial treatment with high-dose melphalan and autologous stem cell transplantation (ASCT). For patients who experience a durable response to primary ASCT, retreatment with high-dose melphalan is recommended by many current guidelines. Yet, toxicity is an important aspect in the choice of relapse treatment, and a second ASCT in this setting could be associated with enhanced toxicity. As the goal for the treatment for relapsed myeloma should be disease control while maintaining quality of life, lower doses of melphalan might be preferable. Methods and Objectives: In this retrospective study, we account for the outcome of 66 patients with myeloma in first systemic relapse after ASCT, who were treated with intermediate-dose melphalan, 100mg/m(2) , and stem cell support (MEL 100). The aim was to evaluate this treatment in relation to prior response duration after initial ASCT and with respect to response rate, toxicity and survival. Results: The overall response rate was 62%. There was limited, mostly haematological, toxicity, and no treatment-related mortality was observed. The median progression-free survival (PFS) was 8.5months, and the median overall survival was 24months. Patients with time to progression of 34months or more (n=17; ≥75th percentile) after initial ASCT had a median PFS of 12.5months after MEL 100. Conclusion: For patients with a long-lasting response after ASCT, MEL 100 could be a therapeutic option with low toxicity and with efficacy comparable to newer immunomodulatory drugs.

    • The SBG 2000-1 trial is a randomised study that investigates if dose-tailored adjuvant FEC therapy based on the individual's leukocyte nadir value can improve outcome. The study has included 1535 women with medium and high-risk breast cancer. Patients and methods. After a first standard dosed FEC course (5-fluorouracil 600 mg/m(2), epirubicin 60 mg/mg(2) and cyclophosphamide 600 mg/m(2)), patients who did not reach leukopenia grade III or IV were randomised to standard doses (group standard) or doses tailored to achieve grade III leukopenia (group tailored) at courses 2 7. Patients who achieved leukopenia grade III or more after the first course were not randomised but continued on standard doses (group registered). Results. Both planned and actually delivered number of courses (seven) were the same in all three arms. The relative dose intensity was increased by a factor of 1.31 (E 1.22, C 1.43) for patients in the tailored arm compared to the expected on standard dose. Ninety percent of the patients in the tailored arm achieved leukopenia grade III-IV compared with 29% among patients randomised to standard dosed therapy. Dose tailoring was associated with acceptable acute non-haematological toxicity with more total alopecia, nausea, vomiting and fatigue. Conclusion. Dose tailoring according to leukopenia was feasible. It led to an increased dose intensity and was associated with acceptable excess of acute non-haematological toxicity.

  • Effects of early vaccination with Improvac (R) on the development and function of reproductive organs of male pigs

    • Gonadotropin-releasing hormone (GnRH) vaccine (Improvac (R)) is effective at diminishing boar taint by interfering with testis function. Early pre-pubertal vaccination at 10 and 14 weeks-of-age could be desirable if sufficient and sustained effects could be achieved. Crossbred male pigs (n = 24) were randomly assigned to three groups each with eight individuals: an unvaccinated control group, one group vaccinated with Improvac (R) early at ages 10 and 14 weeks, and a third group vaccinated with Improvac at the standard ages of 16 and 20 weeks. The average age at slaughter was 25 weeks. At slaughter, reductions in testes weight and bulbourethral gland length of vaccinated pigs compared with controls were observed (P andlt; 0.001), accompanied by lowered testosterone concentrations in peripheral blood (P andlt; 0.001). The diameter of tubuli seminiferi was affected: being 18% smaller in standard and 38% smaller in early vaccinated males, compared with controls (P andlt; 0.01). Leydig cells in vaccinated pigs became pycnotic, and their number decreased in early vaccinated pigs. Spermatogenesis was disrupted, evidenced by spermatocyte loss among standard vaccinated pigs to severe spermatogenic arrest among early vaccinated pigs. This histological picture was reflected in the absence of epididymal spermatozoa in 5 of 8 early vaccinated pigs and a dramatic reduction in the remaining 3 early vaccinated pigs. Among standard vaccinated pigs, 5% of the spermatozoa were morphologically normal (andgt;70% in controls, P andlt; 0.01). Early vaccination caused a more severe disruption of testicular structure and function than standard vaccination, thus providing an alternative for immunocastration of male pigs.

    • Background: Drug resistance is a common cause of treatment failure in cancer patients and encompasses a multitude of different mechanisms. The aim of the present study was to identify drugs effective on multidrug resistant cells. Methods: The RPMI 8226 myeloma cell line and its multidrug resistant subline 8226/Dox40 was screened for cytotoxicity in response to 3,000 chemically diverse compounds using a fluorometric cytotoxicity assay (FMCA). Follow-up profiling was subsequently performed using various cellular and biochemical assays. Results: One compound, designated VLX40, demonstrated a higher activity against 8226/Dox40 cells compared to its parental counterpart. VLX40 induced delayed cell death with apoptotic features. Mechanistic exploration was performed using gene expression analysis of drug exposed tumor cells to generate a drug-specific signature. Strong connections to tubulin inhibitors and microtubule cytoskeleton were retrieved. The mechanistic hypothesis of VLX40 acting as a tubulin inhibitor was confirmed by direct measurements of interaction with tubulin polymerization using a biochemical assay and supported by demonstration of G2/M cell cycle arrest. When tested against a broad panel of primary cultures of patient tumor cells (PCPTC) representing different forms of leukemia and solid tumors, VLX40 displayed high activity against both myeloid and lymphoid leukemias in contrast to the reference compound vincristine to which myeloid blast cells are often insensitive. Significant in vivo activity was confirmed in myeloid U-937 cells implanted subcutaneously in mice using the hollow fiber model. Conclusions: The results indicate that VLX40 may be a useful prototype for development of novel tubulin active agents that are insensitive to common mechanisms of cancer drug resistance.

    • This thesis focused on factors contributing to improved work environment in small-scale enterprises and sustainable health for the entrepreneurs. In Study I, implementation of the provision of Systematic Work Environment Management (SWEM) with and without support was investigated. Two implementation methods were used, supervised and network method. The effect of the project reached the employees faster in the enterprises with the supervised method. In general, the work environment improved in all enterprises. However, extensive support to small-scale enterprises in terms of advice and networking aimed at fulfilling SWEM regulations had limited effect – especially considering the cost of applying these methods. Studies II, III, and IV focused on entrepreneurs’ health, work conditions, strategies for maintaining good health, and utilisation of Occupational Health Service (OHS). A closed cohort of entrepreneurs in ten different trades responded to two self-administered questionnaires on health and work conditions, with five years between the surveys: at baseline, 496 entrepreneurs responded, and 251 entrepreneurs responded at follow-up. Differences were tested by Chi2-test, and associations estimated with logistic regression analyses. Qualitative interviews on entrepreneurs’ strategies for maintaining good health were included. In Study II, the most frequently reported complaints, musculoskeletal pain and mental health problems, were associated with poor job satisfaction and poor physical work environment. In Study III, consistent self-reported good health, i.e. good health both at baseline and at follow up, was associated with self-valued good social life when adjusted for physical work conditions and job satisfaction. Entrepreneurs’ strategies for maintaining good health included good planning and control over work, flexibility at work, good social contact with family, friends and other entrepreneurs, and regular physical exercise. Study IV concerned entrepreneur’s utilisation of OHS. Entrepreneurs affiliated to OHS had either better or more adverse work conditions than non-affiliated entrepreneurs. Medical care and health check-ups were the services most utilised. Affiliation to OHS correlated with use of specific information sources and active work environment management. The entrepreneurs were not consistently affiliated to OHS over the five-year-period.

    • Adsorption of atomic hydrogen has a remarkable influence on the structure and morphology of the Cu(I00) surface. We have examined two specific situations; the well-known reconstructed p(2 x 2)-H surface and a one-dimensional (6 X 1)-H surface resembling the stripe reconstruction observed in an electrochemical environment. High resolution electron energy loss spectroscopy and density functional calculations show that the H atoms occupy only surface sites and that H bonding in induced 3-fold Cu surface hollow sites is a structural key element. The calculations support a transient scenario in which H absorption in subsurface sites is a critical intermediate step in the reconstruction of the Cu surface. We find clear evidence in terms of characteristic properties, like the H-Cu bond lengths and the dipole excited vibrational mode spectra, that these structures consist of two-dimensional and one-dimensional structures of surface hydride.

    • Background: The stress of a breast cancer diagnosis and its treatment can produce a variety of psychosocial sequelae including impaired immune responses. Mindfulness Based Stress Reduction (MBSR) is a structured complementary program that incorporates meditation, yoga and mind-body exercises. Despite promising empirical evidence for the efficacy of MBSR, there is a need for randomized controlled trials (RCT). There is also a need for RCTs investigating the efficacy of psychosocial interventions on mood disorder and immune response in women with breast cancer. Therefore, the overall aim is to determine the efficacy of a Mindfulness Based Stress Reduction (MBSR) intervention on well-being and immune response in women with breast cancer.Methods and design: In this RCT, patients diagnosed with breast cancer, will consecutively be recruited to participate. Participants will be randomized into one of three groups: MBSR Intervention I (weekly group sessions + self-instructing program), MBSR Intervention II (self-instructing program), and Controls (non-MBSR). Data will be collected before start of intervention, and 3, 6, and 12 months and thereafter yearly up to 5 years. This study may contribute to evidence-based knowledge concerning the efficacy of MBSR to support patient empowerment to regain health in breast cancer disease.Discussion: The present study may contribute to evidence-based knowledge concerning the efficacy of mindfulness training to support patient empowerment to regain health in a breast cancer disease.     • Detailed physisorption data from experiment for the H2 molecule on low-index Cu surfaces challenge theory. Recently, density functional theory (DFT) has been developed to account for nonlocal correlation effects, including van der Waals (dispersion) forces. We show that the functional vdW-DF2 gives a potential-energy curve, potential-well energy levels and difference in lateral corrugation promisingly close to the results obtained by resonant elastic backscattering–diffraction experiments. The backscattering barrier is sensitive to the choice of exchange functional approximation. Further, the DFT-D3 and TS-vdW corrections to traditional DFT formulations are also benchmarked, and deviations are analyzed.

    • It is now well established that nicotine adversely affects the integrity of the blood–brain barrier (BBB). In contrast, nicotine has been reported to increase the transendothelial electrical resistance (TEER) of CaCo2 colon cancer cells. In the present study, the effects of nicotine upon the TEER and sucrose permeability of ECV304/C6 co-cultures and, for comparative purposes, CaCo2 cells has been investigated. Neither ECV304 nor C6 cells were found to express measurable membrane levels of nicotinic acetylcholine receptors, as assessed by [3H]–epibatidine binding. Nicotine treatment (0.01–1 µM) for up to 48 h had little or no effect upon the TEER or sucrose permeability of either ECV304/C6 co-cultures or CaCo2 cells. It is concluded that in contrast to the situation for the BBB, ECV304 cells lack nicotinic acetylcholine receptors and the barrier properties of ECV304/C6 co-cultures are not affected to any important extent by nicotine. This study underlines the conclusions made by other authors that the ECV304/C6 co-culture system is of limited validity as a model of the BBB.

    • Physically adsorbed H-2, HD, and D-2 desorbs via the low-energy (2)Sigma(+)(u) electron scattering resonance with characteristic electron energy dependent cross sections as we show by desorption and electron energy-loss measurements. Only H-2 and HD desorb, for energetic reasons, via excitation of the j = 0 -> 2 rotational mode and its subsequent decay into free translational motion, while all three isotopes desorb by direct transitions to unbound continuum states, with a probability close to unity and an unexpectedly weak mass dependence similar to m(-1/2).
