  • Randomized clinical trial comparing 5-year recurrence rate after laparoscopic versus Shouldice repair of primary inguinal hernia

    • Background: The Shouldice technique is the 'gold standard' of open non-mesh hernia repair. The aim of this study was to compare 5-year recurrence rates after Shouldice and laparoscopic transabdominal preperitoneal patch (TAPP) repair for primary inguinal hernia. Method: Men with a primary unilateral inguinal hernia were randomized to either Shouldice or TAPP operation. An independent observer scored the surgeons' performance. Follow-up comprised clinical examination after 1 year, a questionnaire after 2 and 3 years, and a clinical examination after 5 years. Results: Between February 1993 and March 1996, 1183 patients were included. Nine hundred and twenty patients were followed for 5 years, 454 in the TAPP group and 466 in the Shouldice group. Recurrences were evenly distributed between groups throughout the follow-up period. The cumulative recurrence rate after 5 years was 6.6 per cent in the TAPP group and 6.7 per cent in the Shouldice group. Postoperative pain was a risk factor for recurrence after Shouldice operation but not after TAPP repair. There was a correlation between a low surgeon's performance score and recurrence. Conclusion: The 5-year recurrence rate is acceptable, with no difference between TAPP and Shouldice repair. Poor operative performance resulted in a higher recurrence rate. The TAPP operation represents an excellent alternative for primary inguinal hernia repair.

  • Multidimensional psychophysics : Surface feel of printing paper as a function of physical properties

    • The aim of this experiment was to explore the perception of tactile surface-feel of 21 printing papers. A multidimensional scaling (MDS) experiment was conducted with 20 women. They scaled similarity among all possible pairs of the papers. Similarity measurements were mapped by INDSCAL and modeled with PREFMAP. Test-retest and concordance coefficients were high. It is not yet established what physical properties best determine tactile feel. It seems likely though that finger friction and surface roughness are strong contenders. Finger friction for the papers was measured as a ratio of friction force to normal force;(while stroking a human finger on the surface. Average surface roughness was measured with a profilometer. Physical properties were rotated into the 3D INDSCAL solution. This solution identified and mapped the tactile surface feel of the papers in an interpretable way with regard to i.e. friction, surface roughness and weight.

    • This article describes how researchers, practitioners and technicians in West Sweden worked together with older people with early stage dementia and their family members to develop a user-friendly technology-based information, education and support service, based on the generic ACTION participatory design model. Ways of working in partnership with older people with early stage dementia and their family members are explored, and key lessons learned are highlighted. The overriding message is that elders with early stage dementia can be actively involved throughout the entire research and development process. Essential prerequisites are time and ongoing support by skilled practitioners and family members. Preliminary findings reveal that older people with early stage dementia can learn and benefit from user-friendly technology, especially when used together with others in a similar situation.

    • Applying a new theoretical and empirical approach to intrafirm knowledge transfers, this paper provides some initial insight to the little-researched phenomenon of why some subsidiaries are isolated from knowledge-transfer activities within the multinational corporation (MNC). Knowledge transfer is framed as a problemistic search process initiated by the recipient unit. We show that knowledge flows from units that are perceived to be highly capable to units that perceive themselves to be highly capable. Knowledge flows are also associated with existing levels of communication and reciprocity. Taken together, these findings suggest that knowledge transfers in MNCs typically occur between highly capable members of an "in crowd," and the isolated minority rarely, if ever, engages in knowledge-sharing activities. Finally, we show that the isolated minority underperforms other subsidiaries, suggesting the possibility of a "liability of internal isolation.".

    • We investigated relationships of biotic and abiotic factors to recruitment patterns of the endangered freshwater pearl mussel (Margaritifera margaritifera) in 10 Swedish streams. We found that the maximum proportion of gravid mussels did not differ between streams with and without recent recruitment. Moreover, the mean glochidial load on trout (Salmo trutta), which was positively related to adult mussel density, did not differ significantly between these stream types. Thus, the larval stages of the freshwater pearl mussel were not related to recruitment failure. Instead, recruitment is probably hindered at the next stage in the life history of the mussels, the benthic stage, and may be related to sedimentation as turbidity was four times greater in streams lacking recent recruitment than in streams with recent recruitment. Furthermore, we found that juvenile mussel density was positively related to the number of glochidial infections per stream area in streams with ongoing recruitment, indicating that successful recruitment in these streams may depend on both mussel and trout density. Future research should thus examine biotic interactions between mussels and trout as well as the effects of sedimentation on benthic-living mussels.
