    • This thesis consists of two separate yet connected studies which investigate how some adult second language speakers of Swedish with various backgrounds reflect on their experiences of language use. The studies are based on the same empirical data, which consists of questionnaires, diaries, essays and observations, but primarily of conversations conducted with a time interval of six years. This data is analysed from a theoretical framework based on Paul Ricœur’s ideas of interpretation, narrative analysis and identity creation, which recur as a leitmotif throughout the thesis.In the first study, data from three second language speakers are analysed by means of the concept of narrative identity, shedding light on the interplay between a static and a dynamic identity. The result is presented in the form of life narratives, which are commented on in relation to those strategies the participants express in relation to their second language use. The analysis shows a development over time moving from defiance to acceptance, from avoidance to flexibility and from planning to simplification. Adult language learning stands out as an arduous and time-consuming process with major consequences for the learner’s ego, a conclusion confirmed and clarified in the metaphor analysis of the second study in which language learning is related to hard work, a constant struggle and a long and strenuous journey. Here it also becomes evident that the participants equate language learning and language use to physical exertion and challenge as well as to individual achievement. This becomes particularly clear in the linked chains of metaphors in which language use is compared with skiing down the steepest slopes or appearing on stage.The participants in both studies are quoted by means of an ethnopoetic method which aims to bring out the creative aspects of their language use and to give second language speakers a voice – a voice which does not need to be corrected and gives the narratives a new dimension.

    • Self-directed learning denotes that the individual is in command of what should be learned and why it is important. In this study, guidelines for the design of Web 2.0 systems for supporting diabetic adolescents every day learning needs are examined in light of theories about information behaviour and social learning. A Web 2.0 system was developed to support a community of practice and social learning structures were created to support building of relations between members on several levels in the community. The features of the system included access to participation in the culture of diabetes management practice, entry to information about the community and about what needs to be learned to be a full practitioner or respected member in the community, and free sharing of information, narratives and experience-based knowledge. After integration with the key elements derived from theories of information behaviour, a preliminary design guideline document was formulated.

    • This article describes the use of the Swedish sån 'such' and såhär 'like', respectively, as a means to establish joint attention and cooperation among adolescents in multilingual settings. In the south of Sweden, in the city of Malmö, sån 'such' is used, primarily, as a determiner, and also a focus marker. In contrast, adolescents in Stockholm (approximately 600 kilometres north of Malmö) make frequent use of såhär 'like' as a focus marker. However, there is no corresponding development into a determiner. The two expressions have overlapping meaning and function. They both serve to direct the hearer's attention to a linguistic expression. Simultaneously, sån and såhär may function as an appeal to the hearer to co-construct a referent that is being introduced by the following expression.

  • Long-Term Results of a Multicenter Study on Subthalamic and Pallidal Stimulation in Parkinson's Disease

    • We report the 5 to 6 year follow-up of a multicenter study of bilateral subthalamic nucleus (STN) and globus pallidus internus (GPi) deep brain stimulation (DBS) in advanced Parkinson's disease (PD) patients. Thirty-live STN patients and 16 GPi patienis were assessed at 5 to 6 years after DBS surgery. Primary outcome measure was the stimulation effect on the motor Unified Parkinson's Disease Rating Scale (UPDRS) assessed with a prospective cross-over double-blind assessment without medications (stimulation was randomly switched on or off). Secondary outcomes were motor UPDRS changes with unblinded assessments in off- and on-medication states with and without stimulation, activities of daily living (ADL), anti-PD medications, and dyskinesias. In double-blind assessment, both STN and GPi DBS were significantly effective in improving the motor UPDRS scores (STN. P < 0.0001, 45.4%; GPi, P = 0.008, 20.0%) compared with of regardless of the sequence of stimulation. In open assessment. both STN- and GPi-DBS significantly improved the off-medication motor UPDRS when compared with before surgery (STN. P < 0.001, 50.5%; GPi, P = 0.002, 35.6%). Dyskinesias and ADL were significantly improved in both groups. Anti-PD medications were significantly reduced only in the STN group. Adverse events were more frequent in the STN group. These results confirm the long-term efficacy of STN and GPi DBS advanced PD. Although the surgical targets were not randomized, there was a trend to 1 better outcome of motor signs in the STN-DBS patients and fewer adverse events in the GPi-DBS group.     • Arctic ecosystems play a key role in the terrestrial carbon cycle. Our aim was to combine satellite-based normalized difference vegetation index (NDVI) with field measurements of CO2 fluxes to investigate changes in gross primary production (GPP) for the peak growing seasons 1992-2008 in Rylekaerene, a wet tundra ecosystem in the Zackenberg valley, north-eastern Greenland. A method to incorporate controls on GPP through satellite data is the light use efficiency (LUE) model, here expressed as GPP = epsilon(peak) x PAR(in) x FAPAR(green_peak); where epsilon(peak) was peak growing season light use efficiency of the vegetation, PARin was incoming photosynthetically active radiation, and FAPAR(green_peak) was peak growing season fraction of PAR absorbed by the green vegetation. The Speak was measured for seven different high-Arctic plant communities in the field, and it was on average 1.63 g CO2 MJ(-1). We found a significant linear relationship between FAPARgreen_peak measured in the field and satellite-based NDVI. The linear regression was applied to peak growing season NDVI 1992-2008 and derived FAPAR(green_peak) was entered into the LUE-model. It was shown that when several empirical models are combined, propagation errors are introduced, which results in considerable model uncertainties. The LUE-model was evaluated against field-measured GPP and the model captured field-measured GPP well (RMSE was 192 mg CO2 m(-2) h(-1)). The model showed an increase in peak growing season GPP of 42 mg CO2 m(-2) h(-1) y(-1) in Rylekaerene 1992-2008. There was also a strong increase in air temperature (0.15 degrees C y(-1)), indicating that the GPP trend may have been climate driven. (C) 2012 Elsevier B.V. All rights reserved.

    • Methane (CH4) fluxes 1997-2010 were studied by combining remotely sensed normalised difference water index (NDWI) with in situ CH4 fluxes from Rylekaerene, a high-Arctic wet tundra ecosystem in the Zackenberg valley, north-eastern Greenland. In situ CH4 fluxes were measured using the closed-chamber technique. Regression models between in situ CH4 fluxes and environmental variables [soil temperature (T-soil), water table depth (WtD) and active layer (AL) thickness] were established for different temporal and spatial scales. The relationship between in situ WtD and remotely sensed NDWI was also studied. The regression models were combined and evaluated against in situ CH4 fluxes. The models including NDWI as the input data performed on average slightly better [root mean square error (RMSE) = 1.56] than the models without NDWI (RMSE = 1.67), and they were better in reproducing CH4 flux variability. The CH4 flux model that performed the best included exponential relationships against temporal variation in T-soil and AL, an exponential relationship against spatial variation in WtD and a linear relationship between WtD and remotely sensed NDWI (RMSE = 1.50). There were no trends in modelled CH4 flux budgets between 1997 and 2010. Hence, during this period there were no trends in the soil temperature at 10 cm depth and NDWI.

  • NTCP modelling of lung toxicity after SBRT comparing the universal survival curve and the linear quadratic model for fractionation correction

    • Background. In SBRT of lung tumours no established relationship between dose-volume parameters and the incidence of lung toxicity is found. The aim of this study is to compare the LQ model and the universal survival curve (USC) to calculate biologically equivalent doses in SBRT to see if this will improve knowledge on this relationship. Material and methods. Toxicity data on radiation pneumonitis grade 2 or more (RP2+) from 57 patients were used, 10.5% were diagnosed with RP2+. The lung DVHs were corrected for fractionation (LQ and USC) and analysed with the Lyman-Kutcher-Burman (LKB) model. In the LQ-correction alpha/beta = 3 Gy was used and the USC parameters used were: alpha/beta = 3 Gy, D-0 = 1.0 Gy, (n) over bar = 10, alpha = 0.206 Gy(-1) and d(T) = 5.8 Gy. In order to understand the relative contribution of different dose levels to the calculated NTCP the concept of fractional NTCP was used. This might give an insight to the questions of whether "high doses to small volumes" or "low doses to large volumes" are most important for lung toxicity. Results and Discussion. NTCP analysis with the LKB-model using parameters m = 0.4, D-50 = 30 Gy resulted for the volume dependence parameter (n) with LQ correction n = 0.87 and with USC correction n = 0.71. Using parameters m = 0.3, D-50 = 20 Gy n = 0.93 with LQ correction and n = 0.83 with USC correction. In SBRT of lung tumours, NTCP modelling of lung toxicity comparing models (LQ, USC) for fractionation correction, shows that low dose contribute less and high dose more to the NTCP when using the USC-model. Comparing NTCP modelling of SBRT data and data from breast cancer, lung cancer and whole lung irradiation implies that the response of the lung is treatment specific. More data are however needed in order to have a more reliable modelling.

    • Background and purpose: Studies on accelerated fractionation (AF) in head and neck cancer have shown increased local control and survival compared with conventional fractionation (CF), while others have been non-conclusive. In 1998 a national Swedish group decided to perform a randomised controlled clinical study of AF. Materials and methods: Patients with verified squamous cell carcinoma of the oral cavity, oropharynx, larynx (except glottic T1-T2, N0) and hypopharynx were included. Patients with prior chemotherapy or surgery were excluded. Patients were randomised to either CF (2Gy/day, 5days/week for 7 weeks, total dose 68Gy) or to AF (1.1Gy+2.0Gy/day, 5days/week for 4.5weeks, total dose 68Gy). An extensive quality assurance protocol was followed throughout the study. The primary end point was loco-regional tumour control (LRC) at two years after treatment. RESULTS: The study was closed in 2006 when 750 patients had been randomised. Eighty-three percent of the patients had stages III-IV disease. Forty eight percent had oropharyngeal, 21% laryngeal, 17% hypopharyngeal and 14% oral cancers. There were no significant differences regarding overall survival (OS) or LRC between the two regimens. The OS at two years was 68% for AF and 67% for CF. The corresponding figures for LRC were 71% and 67%, respectively. There was a trend towards improved LRC for oral cancers treated (p=0.07) and for large tumours (T3-T4) (p=0.07) treated with AF. The AF group had significantly worse acute reactions, while there was no significant increase in late effects. Conclusion: Overall the AF regimen did not prove to be more efficacious than CF. However, the trend towards improved results in AF for oral cancers needs to be further investigated.
