- Flow linear dichroism (LD) provides new structural information on 7β,8α-dihydroxy-9α, 10α-epoxy-7,8,9,10-tetrahydrobenzo[a] pyrene (anti-BPDE) covalently bound to DNA: by resolving the LD spectrum with respect to the polarized absorption components of the pyrenyl chromophore, we have determined the orientations of the in-plane symmetry axes of the bound compound relative to the helix axis of DNA [they are on average oriented at 70°(short axis) and 30°(long axis)]; for both (+)- and (–)-anti-BPDE adducts the flexibility of the DNA is increased.

L-carnitine and haemodialysis: double blind study on muscle function and metabolism and peripheral nerve function

- Twenty-eight haemodialysis patients were randomized to L-carnitine, 2 g i.v. three times a week, and saline over a 6-week period. No obvious deficiency of carnitine was found in vastus lateralis with a median value of 12.9 mmol/kg dry weight; range 6.2-21.4. Female patients had lower total plasma carnitine compared to female controls, p less than 0.002, whereas no decrease was found in males. No relationship was found between muscle and total plasma carnitine. After carnitine administration the muscle carnitine level increased about 60%, p less than 0.01, and the total plasma carnitine level more than tenfold, whereas the initially high degree of acylation decreased, p less than 0.02. Maximum dynamic muscular strength was reduced with a mean value of 44% compared with healthy controls. Total metabolic activity of isolated skeletal muscle fibres, measured as heat production with a new technique using a perfusion microcalorimeter, showed a median value of 0.40 mW/g, 25% lower than normal, p less than 0.02. Carnitine administration had no effect on several different tests of muscular function. Neurophysiologically, discrete improvements in the temperature responses were recorded, but no changes in sensory and motor nerve conduction velocities or in vibration thresholds were noted. No symptomatic improvement was observed even in patients with the lowest carnitine levels prior to treatment. Our data do not support the hypothesis that carnitine deficiency contributes to muscle and nerve dysfunction in patients on chronic haemodialysis.

- The Stockholm Cancer Prevention Program is to our knowledge the first community intervention program in the world aiming of a reduction in the incidence of cancer. A community diagnosis model has been used in the planning. A thorough description of different organizations in the local communities of Stockholm has been made. A model for describing the organizations or collaborators has been worked out, including description of the organizations and its environment, objectives of activities, possible activities to reach the objectives, resources needed, incentives for collaborative partners, formative evaluation criteria and anticipated problems and solutions. The main method of description has been by use of keyperson interviews. This was found to be a good method to reach commitment to the program.

Measures of continuity of care. A register-based correlation study.

- In an empirical study using data from a health center in Sweden, correlation coefficients were computed among nine different measures of continuity of care, five of them visit-based and four individual-based. Generally, the correlations were high. This may be due, in part, to the similar behavior of the measures for people making few visits. The correlations were also quite high, however, when the sample was restricted to people with many visits. Several measures display a significant dependence on utilization level. The results suggest that, for general purposes, the measure COC should be preferred among the individual-based measures and fraction-of-care continuity among the visit-based measures. On grounds of flexibility and ease of interpretation, the authors recommend fraction-of-care measures.

- The cytoskeleton of human ventricular myocardium in normal and pathological hearts has been analysed with immunocytochemical techniques. Specific antibodies against the intermediate filament proteins desmin (Mr 55 000) and vimentin (Mr 58 000) and antibodies against two cytoskeleton-associated proteins, a spectrin-like protein (Mr 230 000) and vinculin (Mr 130 000), have been used. We show that desmin is localized in the myocytes as an intermyofibrillar lattice at the Z disk level of the myofibrils, and at the intercalated disks. The spectrin-like protein is localized as a transverse striated pattern interlinked with fine longitudinal strands in the subplasmalemmal region of the myocytes. Vinculin is abundant in the intercalated disks and in myotendinous junctions but occurs also at the peripheral sarcolemma in the form of a regular repeat of dots and of fine bar-like extensions into the cytoplasm from the dots. These patterns were observed both in normal and in abnormal hearts, but a number of altered patterns in pathological myocytes were also seen. It is concluded that the intermediate filament system has important implications in the structural function of normal and abnormal hearts but that further studies are needed to elucidate how the different components are related to each other and how they are influenced by different disease processes.

- We examined the relationship between pulmonary density, measured with computerized tomography, and pulmonary mechanics (static pulmonary volume; pulmonary resistance) in 39 normal subjects (20 nonsmokers and 19 smokers). Pulmonary density decreased with increasing static elastic recoil pressure, and smokers consistently showed higher pulmonary density than nonsmokers. Pulmonary density, measured at full inspiration, correlated inversely with total lung capacity. Pulmonary density showed a ventrodorsal gradient, which was greater at low elastic recoil pressure than at high recoil pressure. The study shows that pulmonary density is related to the mechanical properties of the lung in normal subjects. Increased pulmonary density appears to be a sensitive indicator of pulmonary damage induced by smoking. Further studies of the relationship between pulmonary density and pulmonary mechanics in disease seem warranted.
