  • Clinical and immunological characteristics of Autoimmune Addison's disease : a nationwide Swedish multicenter study

    • CONTEXT: Studies on clinical and immunological features of Autoimmune Addison's disease (AAD) are needed to understand the disease burden and increased mortality.OBJECTIVE: To provide upgraded data on autoimmune comorbidities, replacement therapy, autoantibody profiles and cardiovascular risk factors.DESIGN, SETTING AND PARTICIPANTS: Cross sectional, population-based study. 660 AAD patients were included utilizing the Swedish Addison Registry (SAR) 2008-2014. When analyzing cardiovascular risk factors, 3,594 individuals from the population-based survey in Northern Sweden, MONICA (MONItoring of Trends and Determinants of CArdiovascular Disease), served as controls.MAIN OUTCOME MEASURE: Prevalence of autoimmune comorbidities and cardiovascular risk factors. Autoantibodies against 13 autoantigens were determined.RESULTS: Sixty percent of the SAR cohort consisted of females. Mean age at diagnosis was significantly higher for females than for males (36.8 vs. 31.1 years). The proportion of 21-hydroxylase autoantibody positive patients was 83% and 62% of patients had one or more associated autoimmune diseases, more frequently coexisting in females (p<0.0001). AAD patients had lower BMI (p<0.0001) and prevalence of hypertension (p=0.027) compared with controls. Conventional hydrocortisone tablets were used by 89% of patients; with the mean dose 28.1±8.5 mg/day. The mean hydrocortisone equivalent dose normalized to body surface was 14.8±4.4 mg/m(2)/day. Higher hydrocortisone equivalent dose was associated with higher incidence of hypertension (p=0.046).CONCLUSIONS: Careful monitoring of AAD patients is warranted to detect associated autoimmune diseases. Contemporary Swedish AAD patients do not have increased prevalence of overweight, hypertension, T2DM or hyperlipidemia. However, high glucocorticoid replacement doses may be a risk factor for hypertension.

  • Cytokine Autoantibody Screening in the Swedish Addison Registry Identifies Patients With Undiagnosed APS1

    • Context: Autoimmune polyendocrine syndrome type 1 (APS1) is a monogenic disorder that features autoimmune Addison disease as a major component. Although APS1 accounts for only a small fraction of all patients with Addison disease, early identification of these individuals is vital to prevent the potentially lethal complications of APS1.Objective: To determine whether available serological and genetic markers are valuable screening tools for the identification of APS1 among patients diagnosed with Addison disease.Design: We systematically screened 677 patients with Addison disease enrolled in the Swedish Addison Registry for autoantibodies against interleukin-22 and interferon-α4. Autoantibody-positive patients were investigated for clinical manifestations of APS1, additional APS1-specific autoantibodies, and DNA sequence and copy number variations of AIRE.Results: In total, 17 patients (2.5%) displayed autoantibodies against interleukin-22 and/or interferon-α4, of which nine were known APS1 cases. Four patients previously undiagnosed with APS1 fulfilled clinical, genetic, and serological criteria. Hence, we identified four patients with undiagnosed APS1 with this screening procedure.Conclusion: We propose that patients with Addison disease should be routinely screened for cytokine autoantibodies. Clinical or serological support for APS1 should warrant DNA sequencing and copy number analysis of AIRE to enable early diagnosis and prevention of lethal complications.

  • Evaluation of reference genes for gene expression studies in human brown adipose tissue.

    • Human brown adipose tissue (BAT) has during the last 5 year been subjected to an increasing research interest, due to its putative function as a target for future obesity treatments. The most commonly used method for molecular studies of human BAT is the quantitative polymerase chain reaction (qPCR). This method requires normalization to a reference gene (genes with uniform expression under different experimental conditions, e.g. similar expression levels between human BAT and WAT), but so far no evaluation of reference genes for human BAT has been performed. Two different microarray datasets with samples containing human BAT were used to search for genes with low variability in expression levels. Seven genes (FAM96B, GNB1, GNB2, HUWE1, PSMB2, RING1 and TPT1) identified by microarray analysis, and 8 commonly used reference genes (18S, B2M, GAPDH, LRP10, PPIA, RPLP0, UBC, and YWHAZ) were selected and further analyzed by quantitative PCR in both BAT containing perirenal adipose tissue and subcutaneous adipose tissue. Results were analyzed using 2 different algorithms (Normfinder and geNorm). Most of the commonly used reference genes displayed acceptably low variability (geNorm M-values <0.5) in the samples analyzed, but the novel reference genes identified by microarray displayed an even lower variability (M-values <0.25). Our data suggests that PSMB2, GNB2 and GNB1 are suitable novel reference genes for qPCR analysis of human BAT and we recommend that they are included in future gene expression studies of human BAT.

  • Comparison Between the Montgomery-Asberg Depression Rating Scale–Self and the Beck Depression Inventory II in Primary Care

    • Objective: The Montgomery-Asberg Depression Rating Scale–Self (MADRS-S) and the Beck Depression Inventory II (BDI-II) are commonly used self-assessment instruments for screening and diagnosis of depression. The BDI-II has 21 items and the MADRS-S has 9 items. These instruments have been tested with psychiatric inpatients but not in outpatient primary care, where most patients with symptoms of depression initially seek treatment. The purpose of this study was to compare these 2 instruments in the primary care setting. Method: Data were collected from 2 primary care randomized controlled trials that were performed from 2010 to 2013 in Sweden: the Primary Care Self-Assessment MADRS-S Study and Primary Care Internet-Based Cognitive Behavioral Therapy Study. There were 146 patients (73 patients each from both trials) who had newly diagnosed mild or moderate depression (per DSM-IV recommendations) and who had assessment with both the MADRS-S and BDI-II at primary care centers. Comparability and reliability of the instruments were estimated by Pearson product moment correlation and Cronbach α. Results: A good correlation was observed between the 2 instruments: 0.66 and 0.62 in the 2 study cohorts. The reliability within the 2 study cohorts was good for both MADRS-S (Cronbach α: 0.76 for both cohorts) and BDI-II items (Cronbach α: 0.88 and 0.85). Conclusions: The 2 instruments showed good comparability and reliability for low, middle, and high total depression scores. The MADRS-S may be used as a rapid, easily administered, and inexpensive tool in primary care and has results comparable to the BDI-II in all domains.

  • Entrepreneurship in a School Setting : Introducing a Business Concept in a Public Context

    • Entrepreneurship has during the last decades gained an immense interest in academia, politics and practice. It is argued from politics that more entrepreneurs are necessary for the economic development. In addition, nowadays entrepreneurship is also perceived as a solution to social and societal challenges. This drives a need for entrepreneurial people everywhere in society who can cope with the inconstant and uncertain world of today. As a consequence, there are around the world numerous educational initiatives trying to inspire and fuel an entrepreneurial mind-set. Here, educations of all kind become relevant contexts since they provide an opportunity to affect children, youth’s and adult’s interest and attitudes towards entrepreneurship, and as such give a possibility to reach a vast number of people.Sweden is no exception, and in 2009 the Swedish Government launched a ‘Strategy for entrepreneurship in the field of education’ in which entrepreneurship is said to run like a common thread throughout education. The main focus is that self-employment is to become as natural as being an employee. As such the Government took an active stand for implementing entrepreneurship in the school setting on a broad front, from preschool to adult education.This development can be seen as part of New Public Management; a development where concepts from the private sector are lent and transferred to the public sector. Thus, when introducing entrepreneurship in the Swedish educational system, this at the same time means introducing a traditional business concept in a public setting. Therefore, the overall aim of this thesis is to increase knowledge of and insights on how a business concept – entrepreneurship – is operationalised and constructed in a public setting.When placing entrepreneurship in new societal contexts other questions arise and complexity intensifies. In this qualitative research, the empirical context in focus are schools. It investigates how entrepreneurship is constructed among teachers in their work. But also how this business concept is included in a non-business setting by studying how the entrepreneurship strategy is operationalised in educational practice.As such the thesis and its findings contribute to the scientific discussions on societal entrepreneurship and entrepreneurship education, as well as on strategy and strategising in a public context. The research also aspire to serve inspiration, insights and food for thoughts on discussions and reflections on entrepreneurship within the school practice. This compilation thesis include five papers. To be able to fulfil the aim this research use a broad theoretical base and multiple qualitative research methods. The combination of methods include semi-structured interviews, in-depth interview using the stimulated recall method, focus group interviews, participative meetings, observations, document studies, digital questionnaires, written inquiries, analysing texts and critical incidents questionnaires.

    • In Sweden, supervised visitation has been replaced with a new measure called supported visitation. In the reform process, it was emphasized that if face to face visitation cannot be organized without risk for the child, indirect visitation or no visitation are to be considered better options. The aim of this article is to explore social work practice regarding supported visitation in cases involving violence. It draws on a study of a local visitation centre and the data consists of case files from the social services regarding 37 children where a court ordered visitation support, interviews with seven members of staff, ten parents and three children, and local documents and guidelines. For 18 of the 37 children, case files contained credible information about a history of violence. The study shows that district courts sometimes order visitation support in cases where there is a risk for the child and where in the near future normalization of visitation is unlikely. Thus, the measure of visitation support is sometimes used in a way that was not intended. Regarding social work practice, the analysis indicates that, although the guidelines developed at the local support centre under study adhere to the national policy intentions, both professionals’ validation and invalidation of violence can be seen. For service users previously subjected to violence, the documented court and social services’ practices may actively contribute to children’s and residential parents’ continued vulnerability.

    • In this chapter, I argue that children’s voices and perspectives constitute a very fruitful point of departure in knowledge development in the field of intimate partner violence (IPV). Furthermore, that research and practice need to draw on a “double view” on these children as both objects for adults’ care, protection and support, and as social actors with their own perspectives, preferences and agendas.First, I review the extent to which it is possible to identify current research on intimate partner violence focusing on children’s voices and children’s agency, and outline a typology of different kinds of research on children exposed to intimate partner violence, as well as the “double view” on children exposed to intimate partner violence.Second, I discuss three different empirical areas to illustrate the point about possible knowledge gains. The first area is children’s exposure to intimate partner violence, specifically children’s ways of tackling their situation and experiences. This part of the discussion includes elaborations of the concept of “witnessing” or exposure to IPV, the relationship between the effects of exposure and children’s creation of meaning, and children’s agency. The second area is risk assessment and it is argued that such assessments must include not just immediate danger (or perpetrator dangerousness) but also the long-term risk associated with emotional violence, retraumatization or undermining of recovery, as well as the child’s sense of security. The third area is the critical assessment of support and treatment interventions, including perspectives on children exposed to IPV as services users, and to what extent their service user perspectives are included in the development and evaluations of interventions.

    • Aim: The paper discusses what the practices of family law social workers and school staff mean for children’s vulnerability and victimization at school (including preschool).Background: While there has been considerable discussion of issues about child protection and the cooperation between school and social services in that context, studies on the intersection between school and family law proceedings seem virtually non-existent. This is also the case when it comes to family law proceedings, school and different forms of childhood adversity.Methods: The analysis draws on a survey to 110 preschool managers and 110 school head teachers in two counties in Sweden, as well as qualitative interviews with school staff, individually or in groups (22 interviewees).Findings: Victimizing practices range from staff neglect of children’s possible fear due to previous exposure to violence, and the invalidation of experiences of violence, to outright dangerousness as when staff exposing children and their abused mothers to risks for further violence. Not just the children who are the objects of a family law dispute are subjected to victimizing practices at school, but other children as well. A key example is when children with disputing parents are subjected institutional violence by social workers trying to enforce contact, and this is witnessed by other children.Conclusion: The pattern of victimizing practices can be linked to institutional and professional hierarchies, as well as dominating discourses in the domain of education constructing domestic violence as a   problem of order affecting the collective, rather than as a threat to the rights of individual children.

  • Effektiva insatser för unga kvinnor som utsatts för sexuellt våld? : resultat från en pilotstudie

    • Sedan 2002 bedriver Järva Ungdomsmottagning en gruppverksamhet kallad ”NOOR-grupper”. Målgruppen är unga kvinnor 15-22 år som utsatts för sexuellt våld och kommit ur den akuta krisfasen. Insatsen pågår i cirka 13 veckor och sätter våldet och dess konsekvenser i fokus. Detta är en gruppintervention med både bearbetande och pedagogiska inslag. Den presenteras å ena sidan inte som en traumabearbetande insats och det finns inte heller något uttalat inklusionskriterium att gruppdeltagarna ska ha uppvisat någon form av symptom på posttraumatisk stress, samtidigt som arbetet å andra sidan är tydligt influerat av traumateori.Papret presenterar de första resultaten från en pilotstudie av insatsen. Studien bygger på strukturerade underlag om situation, våldsutsatthet, psykisk ohälsa generellt och traumasymptom specifikt före och efter grupp samt sex månader efter avslutad grupp, samt på brukarvärderingar och behandlarbedömningar av insatsen och intervjuer med deltagarna 6 månader efter att gruppen avslutades. I studien ingår deltagare från tre olika grupper under perioden 2013 - 2014, totalt 15 unga kvinnor. För att ge kunskap om vilka av mottagningens brukare som rekryteras till grupperna har också bakgrundsdata inhämtats om nybesök under perioden augusti – december 2013. Totalt har data om bakgrund och våldsutsatthet inhämtats för 69 unga kvinnor.I presentationen ligger fokus dels på vilka unga kvinnor som gått i NOOR-grupp, sett i förhållande till den bredare gruppen unga kvinnor som kommer till Järva Ungdomsmottagning, dels vilka tecken på psykisk ohälsa och traumasymptom gruppdeltagarna uppvisar och hur dessa tecken förändras över tid.Resultaten bekräftar att NOOR-gruppen når en specifik del av Ungdomsmottagningens våldsutsatta brukare. De våldsutsatta kvinnor som deltar i NOOR-grupp har i hög grad svensk bakgrund och relationerna till förövarna är blandade, med ungefär hälften bekanta, medan de våldsutsatta unga kvinnor som inte deltar i NOOR-grupp i högre grad har utländsk bakgrund och förövaren är ofta någon i familjen.När det gäller tecken på psykisk ohälsa och trauma visade de unga kvinnornas självskattningar före gruppstart höga nivåer av besvär. I jämförelse med ett svenskt representativt ungdomsmaterial bestående av 2,305 kvinnor i åldrarna 17-23 år och ett material med diagnostiserade patientgrupper med kvinnor i olika åldrar från olika enheter i Sverige låg undersökningsgruppen högre när det gäller generella besvär och antalet symtom, både jämfört med normeringsgruppen och den diagnostiserade gruppen, även om måttet som indikerar djupet av upplevda problem var lägre (NOOR-deltagarna har fler symptom generellt, men inte så allvarliga). Av 11 deltagare där det finns kompletta data om traumasymptom före gruppstart indikerade självskattningarna från 9 av dem att de sannolikt led av posttraumatisk stress och hade ett behandlingsbehov.Analysen av förändring över tid visar bland annat att det går att se en tydligare förbättring när det gäller psykisk ohälsa generellt, än traumasymtom specifikt. Implikationerna av dessa resultat för fortsatt utvecklingsarbete diskuteras i presentationen.

  • Family law proceedings, intimate partner violence, and children's victimization at school

    • Objectives: What do practices of family law professionals and school staff mean for children’s vulnerability and victimization at school? While there has been considerable discussion of issues about child protection and the cooperation between school and social services in that context, studies on the intersection between school and family law proceedings seem virtually non-existent. This is also the case when it comes to family law proceedings, school and different forms of childhood adversity. The paper explores how family law proceedings in cases involving intimate partner violence impact on school and how school staff tackle the issue of violence and family law.Methods: The empirical material consists of qualitative interviews with a sample of pre-school/school staff, individually or in groups, and a questionnaire to school head teachers and pre-school managers. In total 12 interviews were carried out with 22 interviewees from different staff categories. The questionnaire was sent out to 110 pre-schools and 110 schools in two Swedish counties (response rate 51 and 65 percent respectively). The analysis both concerns the children exposed to violence who are the objects of family law disputes, and the vulnerability of other children at school.Results: The analysis shows that children’s victimization at school can be due to actions by court staff, family law social workers and school staff, in addition to actions by parents. The problematic practices range from actions with victimizing effects, such as the neglect of children’s possible fear due to previous experiences of violence, or the invalidation experiences of violence, to outright dangerousness as when leaving children and their mothers without protection. In the worst case scenario, these practices enable further violence, possibly even lethal violence. Further violence may also be witnessed by other children present at school.Conclusion: The examples found in the data sets make it clear that it is not just the children who are the objects of a family law dispute that are subjected to victimizing practices at school, but other children as well. An issue raised by the analysis presented here is the question as to whether the concept of children “witnessing” violence as crime victims in their own right – well established in Swedish policy today - should be expanded to include also children who witness violence against their peers or their peers’ parents at school.

    • Aim: The aim of the paper is to discuss how the implementation of a structured risk- and safety interview may be affected by professionals’ views on domestic violence and risks for children, risk assessment and children’s treatment needs, as well as previous knowledge and experiences of other assessment instruments/models.Background: At present, there is ongoing work at a national level in Sweden to introduce evidence based methods in the field of domestic violence intervention. The paper draws on a recently completed research study in Sweden on the development of assessments of, and support interventions for, children subjected to violence, specifically the part of the project that aimed to implement a routine inquiry about parent reports on intimate partner violence and child abuse, and to try out a structured risk-/safety interview within social services and child and adolescent mental health units.Methods: The empirical data consists of group interviews with professionals from 18 different units that participated in the project.     • Coordinated, multidisciplinary collaboration teams have been developed in Sweden with the purpose of preventing or mitigating conflicts between parents and promoting effective parental cooperation. The screening and assessment tool, known as the Family Law Detection of Overall Risk Screen (FL-DOORS), was used to assess the children ’s and parents’ situation and need for support or protection. The overall results based on the children ’s and the parents’ situations and experiences demonstrate that a collaboration team is a promising model. The development project (2014 –2017) has demonstrated the importance of offering children and families preventative support at an early stage in order to avoid prolonged and conflict-ridden separations.

  • Taking children exposed to intimate partner violence seriously? : developments in BBIC from 2006 to 2015

    • To improve child protection investigations the framework for assessment ”BBIC – Barns Behov I Centrum” [Children’s needs in the centre] was introduced in Sweden just after the new millennium, with the first full training resource published in 2006. There is a lack of research in Sweden about child protection work in cases of intimate partner violence generally and as regards BBIC specifically. However, a number of different sources indicate that there is a need for improvement of the BBIC system when it comes to this group of children at risk. For example, the national inspections of the local authorities’ work with abused women and children exposed to intimate partner violence carried out so far point to serious problems in child protection practice. Since BBIC was introduced the system has been amended and revised several times. A major revision was carried out in 2015. An important question is what these revisions may mean for the handling of cases of intimate partner violence. The aim of the paper is to map and assess how the issue of children’s exposure to violence has been addressed in the different versions of BBIC between 2006 and 2015. Surveying training resources and other documents from the last decade, we outline how there has been a gradual and partial inclusion of the issue of children’s exposure to violence over time, and discuss to what extent these amendments constitute a shift in perspective and emphasis major enough to be likely to impact positively on practice.

Programmet Mentors in Violence Prevention kommer från USA och är ett utbildningsprogram som syftar till att förebygga killars och mäns våld. Organisationen Män för Jämställdhet har översatt programmet från engelska, anpassat det till svenska förhållanden och under perioden 2015-2017 spridit MVP i ett antal skolor i Sverige inom ramen för projektet ”En kommun fri från våld”. Det är den version av MVP som Män för Jämställdhet utvecklat och sprider som den här utvärderingen berör. Utvärderingen har genomförts på uppdrag av Skolverket.

    • Objectives: The aim of this study was to validate the use of survivin for preclinical recognition of rheumatoid arthritis (RA) among patients with unexplained arthralgia.Methods: Serum levels of survivin and the arthritis-specific autoantibodies RF and ACPA were measured in total of 5046 patients with musculoskeletal complains during 12 consecutive months in Gothenburg and in Umea. Among them, 303 arthralgia patients were identified and prospectively followed.Results: After 48 months, 12.2% of the arthralgia patients developed RA. Most of RA cases had high serum survivin, which increased the relative risk for RA (RR = 5.90,p = 3 x 10(-7)). Combination of survivin with autoantibodies was present in only 4.6% of the arthralgia patients and increased further the risk of RA and shortened time to RA development. Presence of any single autoantibody in the survivin-negative patients was associated with a minor risk for RA and had RA-free survival similar to the reference group.Conclusion: This study shows that measurement of survivin in serum improves estimation of RA risk and prospectively predicts RA development in patients with arthralgia. Survivin may indicate a phase preceding autoantibody production.

    • Objective. To identify a potential therapeutic target for type 2 diabetes by comparing the subcutaneous interstitial fluid from type 2 diabetes patients and healthy men. Methods. Proteomics was performed on the interstitial fluid of subcutaneous adipose tissue obtained by microdialysis from 7 type 2 diabetes patients and 8 healthy participants. 851 proteins were detected, of which 36 (including galectin-1) showed significantly altered expression in type 2 diabetes. We also measured galectin-1 expression in: (1) adipocytes isolated from adipose tissue biopsies from these participants; (2) subcutaneous adipose tissue of 24 obese participants before, during and after 16 weeks on a very low calorie diet (VLCD); and (3) adipocytes isolated from 6 healthy young participants after 4 weeks on a diet and lifestyle intervention to promote weight gain. We also determined the effect of galectin-1 on glucose uptake in human adipose tissue. Results. Galectin-1 protein levels were elevated in subcutaneous dialysates from type 2 diabetes compared with healthy controls (p < 0.05). In agreement, galectin-1 mRNA expression was increased in adipocytes from the type 2 diabetes patients (p < 0.05). Furthermore, galectin-1 mRNA expression was decreased in adipose tissue after VLCD (p < 0.05) and increased by overfeeding (p < 0.05). Co-incubation of isolated human adipocytes with galectin-1 reduced glucose uptake (p < 0.05) but this was independent of the insulin signal. Conclusion. Proteomics of the interstitial fluid in subcutaneous adipose tissue in vivo identified a novel adipokine, galectin-1, with a potential role in the pathophysiology of type 2 diabetes.     • Research is scarce on the suitability of the evidence-based components of child investigative interviews when used in non-forensic contexts such as social work or school, particularly in relation to children’s reports on emotional content.This explorative study investigated to what extent a structured forensic interview protocol aids children in verbalizing negative emotional experiences of distress or discomfort. To do this we assessed and compared children’s displayed distress or discomfort during a video-recorded health visit with the verbalized distress or discomfort in interviews 2-4 weeks later about this visit. The children, aged 4 and 5 years (N = 26), were interviewed with a forensic interview protocol. Children’s statements regarding distress and discomfort and the interviewer questions preceding these statements were analyzed qualitatively.The results showed that 46% of the 4-year-olds and 39 % of the 5-year-olds displayed discomfort or distress during their health visit. In the interviews, open-ended questions were posed to all children, however, these questions were sufficient to aid only some children (n = 6) to share evaluative content. None of the children who displayed distress or discomfort during the visit verbalized such experiences after an invitation only. Most children who described experiences of distress or discomfort did so in relation to evaluative questions.The results suggest that more research is warranted to investigate exactly how and when evaluative questions should be posed and whether this differs depending on severity of experience or the child’s age. The need for protocol development and its suitability when used in other fields of practice is discussed.

    • Interviewing children is a cognitively, socially, and emotionally challenging situation, especially for young and shy children. Thus, finding methods that aid rapport and increase these children's communication is important. The present study investigated whether children's verbal and non-verbal communicative behavior developed differently during the rapport phase, depending on whether children were situationally shy or not, and whether the interview was conducted using the computer-assisted interview In My Shoes (IMS) or a Standard verbal interview. The sample consisted of 60 children aged 4 to 5-years-old. The results showed that for the shy children in the IMS group their talkativeness increased and their answer latency decreased including the amount of encouragement the child needed to talk, while no changes were observed for the shy children in the Standard verbal interview group. There were no significant differences in the non-verbal behavior for the shy children regardless of the interview method used. For the non-shy children, overall, the interview method did not affect either the verbal or the non-verbal outcomes. Our findings indicate that IMS can be a useful tool during the rapport-building phase with shy children as it helps these children to improve their verbal communication.

    • This thesis is inter-disciplinary and proceed from the ongoing challenges of the increased digitalization, automation and robotization that impact the manufacturing industry's emergent need of high-qualified practitioners. Digitalization also challenges universities to open up to external collaboration and to design blended e-learning targeting industry knowledge needs. The studies take up on such challenges and explore inter-organizational collaborations and forms of knowledge construction to strengthen engineering competences integrated inwork in a way that enables manufacturing companies to remain effective and to be prepared for future industrial transformations. The objective is to explore how mutual construction of knowledge emerge through learning activities between multiple actors in a joint industry-university collaborative e-learning practice. The empirical setting is a new type of collaborative course concept developed within the project ProdEx. The project comprise a network of industries and one university in a longitudinal design and implementation process of blended and work-integrated e-learning. This initiative was explored with a collaborative action research approach integrated with five studies, from four perspectives, the industry managers, the practitioners, the research teachers and the course unit. Negotiated knotworking, from cultural-historical activity theory, became a central theoretical concept and a working tool to examine how managers, practitioners and research teachers together negotiated production technology knowledge content and e-learning design towards future workplace transformations. This concept was used to further understand how co-construction of knowledge was developing over time into a richer concept. The results contributes to a wider understanding of how co-construction of knowledge in an e-learning design practice was developing into stronger relations between actors and into more stable courses. Real learning cases and digital labs support theory-practical intertwining of mutual learning of active participation between practitioners and ix research teachers. Initial e-learning technology failures and pedagogical mistakes in the courses were easier to overcome, than issues concerning continuous company support for course participation. Matching industry competence needs with university research fields is continuously challenging. Practitioners' aiming for personal continuous competence development on university level created critical and high-qualitative performances and valuable engagement throughout the process of co-construction of knowledge. The knowledge co-construction became a two-way development, pushing research teachers to active involve and consider practitioners' industry experiences concerning learning content, pedagogical strategies and e-learning forms. While earlier research has discussed the problems of crossing boundaries between industry and university, overall findings show that industry and university actors are crossing boundaries when they mutually co-construct knowledge in an elearningpractice. Co-construction of knowledge entail mutual trust, sideways and interactive learning in a collaborative context. The main contribution suggested in the thesis is that co-constructing expertise entail three levels of activities among actors; to have insight into the purposes and practices of others (relational expertise), the capacity to transform the problems of a practice and together build common knowledge (distributed expertise), and finally the capacity of mutually co-construct knowledge acted upon in practice towards work-integrated transformations (co-constructing expertise).

    • Children who have parents with any kind of illness may become young carers who take a responsibility not expected of children for household tasks, or personal or emotional care for parents and siblings. So far, little is known about children in Sweden who are at risk of becoming young carers. The aim of this article is therefore to explore the extent and impact of children's caring activities as reported in a pilot study by a sample of children in Sweden. A number of international questionnaires measuring the amount of caring activities, impact of caring, quality of life, and psychological well‐being were translated and combined into a survey. The pilot survey was completed by 30 children 10–18 years of age. Also, when completing the survey, the children were interviewed concerning their experiences of caregiving. The participants report on a group level emotional symptoms such as fear and nervousness above the clinical cut‐off value. They also rate a lower level of caring compared with findings from the United Kingdom, but they report a higher degree of negative impact of caring than young carers in the United Kingdom.

The child’s right to information is also stated in the Swedish national guidelines for Barnahus and in Swedish national law. The child´s right to information is, however, connected to both legal dilemmas and dilemmas in practice. In this chapter, we discuss children’s right to participation from a child rights perspective in a criminal investigation in Barnahus.
