    • We present Magnetospheric Multiscale observations of an electron-scale current sheet and electron outflow jet for asymmetric reconnection with guide field at the subsolar magnetopause. The electron jet observed within the reconnection region has an electron Mach number of 0.35 and is associated with electron agyrotropy. The jet is unstable to an electrostatic instability which generates intense waves with E-vertical bar amplitudes reaching up to 300mVm(-1) and potentials up to 20% of the electron thermal energy. We see evidence of interaction between the waves and the electron beam, leading to quick thermalization of the beam and stabilization of the instability. The wave phase speed is comparable to the ion thermal speed, suggesting that the instability is of Buneman type, and therefore introduces electron-ion drag and leads to braking of the electron flow. Our observations demonstrate that electrostatic turbulence plays an important role in the electron-scale physics of asymmetric reconnection.

  • Strong current sheet at a magnetosheath jet : Kinetic structure and electron acceleration

    • Localized kinetic-scale regions of strong current are believed to play an important role in plasma thermalization and particle acceleration in turbulent plasmas. We present a detailed study of a strong localized current, 4900 nA m(-2), located at a fast plasma jet observed in the magnetosheath downstream of a quasi-parallel shock. The thickness of the current region is similar to 3 ion inertial lengths and forms at a boundary separating magnetosheath-like and solar wind-like plasmas. On ion scales the current region has the shape of a sheet with a significant average normal magnetic field component but shows strong variations on smaller scales. The dynamic pressure within the magnetosheath jet is over 3 times the solar wind dynamic pressure. We suggest that the current sheet is forming due to high velocity shears associated with the jet. Inside the current sheet we observe local electron acceleration, producing electron beams, along the magnetic field. However, there is no clear sign of ongoing reconnection. At higher energies, above the beam energy, we observe a loss cone consistent with part of the hot magnetosheath-like electrons escaping into the colder solar wind-like plasma. This suggests that the acceleration process within the current sheet is similar to the one that occurs at shocks, where electron beams and loss cones are also observed. Therefore, electron beams observed in the magnetosheath do not have to originate from the bow shock but can also be generated locally inside the magnetosheath.

  • First observations and performance of the RPW instrument on board the Solar Orbiter mission

    • The Radio and Plasma Waves (RPW) instrument on the ESA Solar Orbiter mission is designed to measure in situ magnetic and electric fields and waves from the continuum up to several hundred kHz. The RPW also observes solar and heliospheric radio emissions up to 16 MHz. It was switched on and its antennae were successfully deployed two days after the launch of Solar Orbiter on February 10, 2020. Since then, the instrument has acquired enough data to make it possible to assess its performance and the electromagnetic disturbances it experiences. In this article, we assess its scientific performance and present the first RPW observations. In particular, we focus on a statistical analysis of the first observations of interplanetary dust by the instrument's Thermal Noise Receiver. We also review the electro-magnetic disturbances that RPW suffers, especially those which potential users of the instrument data should be aware of before starting their research work.

    • The Radio and Plasma Waves (RPW) instrument on the ESA Solar Orbiter mission is described in this paper. This instrument is designed to measure in-situ magnetic and electric fields and waves from the continuous to a few hundreds of kHz. RPW will also observe solar radio emissions up to 16 MHz. The RPW instrument is of primary importance to the Solar Orbiter mission and science requirements since it is essential to answer three of the four mission overarching science objectives. In addition RPW will exchange on-board data with the other in-situ instruments in order to process algorithms for interplanetary shocks and type III langmuir waves detections.

    • Background The 'extreme male brain' theory suggests that autism spectrum disorder (ASD) is an extreme variant of male intelligence. However, somewhat paradoxically, many individuals with ASD display androgynous physical features regardless of gender. Aims To assess physical measures, supposedly related to androgen influence, in adults with and without ASD. Method Serum hormone levels, anthropometry, the ratio of 2nd to 4th digit length (2D:4D) and psychiatric symptomatology were measured in 50 adults with high-functioning ASD and age- and gender-matched neurotypical controls. Photographs of face and body, as well as voice recordings, were obtained and assessed with respect to gender coherence, blindly and independently, by eight assessors. Results Women with ASD had higher total and bioactive testosterone levels, less feminine facial features and a larger head circumference than female controls. Men in the ASD group were assessed as having less masculine body characteristics and voice quality, and displayed higher (i.e. less masculine) 2D:4D ratios, but similar testosterone levels to controls. Androgynous facial features correlated strongly and positively with autistic traits measured with the Autism-Spectrum Quotient in the total sample. In males and females with ASD dehydroepiandrosterone sulfate did not decrease with age, in contrast to the control group. Conclusions Women with ASD had elevated testosterone levels and several masculinised characteristics compared with controls, whereas men with ASD displayed several feminised characteristics. Our findings suggest that ASD, rather than being characterised by masculinisation in both genders, may constitute a gender defiant disorder.

    • We present observations of electron energization within a sub-ion-scale magnetosheath current sheet (CS). A number of signatures indicate ongoing reconnection, including the thickness of the CS (similar to 0.7 ion inertial length), nonzero normal magnetic field, Hall magnetic fields with electrons carrying the Hall currents, and electron heating. We observe localized electron acceleration and heating parallel to the magnetic field at the edges of the CS. Electrostatic waves observed in these regions have low phase velocity and small wave potentials and thus cannot provide the observed acceleration and heating. Instead, we find that the electrons are accelerated by a parallel potential within the separatrix regions. Similar acceleration has been reported based on magnetopause and magnetotail observations. Thus, despite the different plasma conditions in magnetosheath, magnetopause, and magnetotail, the acceleration mechanism and corresponding heating of electrons is similar. Plain Language Summary Magnetic reconnection is an important physical energy conversion process in astrophysical and laboratory plasmas. The easiest place to analyze magnetic reconnection is in near-Earth space. Due to lack of sufficient electron resolution of previous spacecraft missions, there are many unanswered questions regarding electron heating and acceleration processes at small scales. In particular, the regime where thermal pressure dominates over magnetic pressure, the most common state of plasmas in the Universe, is little explored. In this letter we study such a regime using the four-spacecraft Magnetospheric Multiscale mission. We analyze a reconnecting current sheet in the magnetosheath. We show that electrons are energized by a parallel potential, similar to what has been observed in the different plasma regimes the magnetopause and magnetotail. Thus, despite different plasma conditions, a similar acceleration mechanism and corresponding heating of electrons is occurring in all these regions.

  • 15-year follow-up of the Second Nordic Mantle Cell Lymphoma trial (MCL2) : prolonged remissions without survival plateau

    • In recent decades, the prognosis of Mantle Cell Lymphoma (MCL) has been significantly improved by intensified first-line regimens containing cytarabine, rituximab and consolidation with high-dose-therapy and autologous stem cell transplantation. One such strategy is the Nordic MCL2 regimen, developed by the Nordic Lymphoma Group. We here present the 15-year updated results of the Nordic MCL2 study after a median follow-up of 114years: For all patients on an intent-to-treat basis, the median overall and progression-free survival was 127 and 85years, respectively. The MCL International Prognostic Index (MIPI), biological MIPI, including Ki67 expression (MIPI-B) and the MIPI-B including mIR-18b expression (MIPI-B-miR), in particular, significantly divided patients into distinct risk groups. Despite very long response durations of the low and intermediate risk groups, we observed a continuous pattern of relapse and the survival curves never reached a plateau. In conclusion, despite half of the patients being still alive and 40% in first remission after more than 12years, we still see an excess disease-related mortality, even among patients experiencing long remissions. Even though we consider the Nordic regimen as a very good choice of regimen, we recommend inclusion in prospective studies to explore the benefit of novel agents in the frontline treatment of MCL.

  • Long-term progression-free survival of mantle cell lymphoma after intensive front-line immunochemotherapy with in vivo-purged stem cell rescue : a nonrandomized phase 2 multicenter study by the Nordic Lymphoma Group

    • Mantle cell lymphoma (MCL) is considered incurable. Intensive immunochemotherapy with stem cell support has not been tested in large, prospective series. In the 2nd Nordic MCL trial, we treated 160 consecutive, untreated patients younger than 66 years in a phase 2 protocol with dose-intensified induction immunochemotherapy with rituximab (R) + cyclophosphamide, vincristine, doxorubicin, prednisone (maxi-CHOP), alternating with R + high-dose cytarabine. Responders received high-dose chemotherapy with BEAM or BEAC (carmustine, etoposide, cytarabine, and melphalan/cyclophosphamide) with R-in vivo purged autologous stem cell support. Overall and complete response was achieved in 96% and 54%, respectively. The 6-year overall, event-free, and progression-free survival were 70%, 56%, and 66%, respectively, with no relapses occurring after 5 years. Multivariate analysis showed Ki-67 to be the sole independent predictor of event-free survival. The nonrelapse mortality was 5%. The majority of stem cell products and patients assessed with polymerase chain reaction (PCR) after transplantation were negative. Compared with our historical control, the Nordic MCL-1 trial, the event-free, overall, and progression-free survival, the duration of molecular remission, and the proportion of PCR-negative stem cell products were significantly increased (P < .001). Intensive immunochemotherapy with in vivo purged stem cell support can lead to long-term progression-free survival of MCL and perhaps cure. Registered at www.isrctn.org as #ISRCTN 87866680.

    • Mantle cell lymphoma (MCL) is a heterogenic non-Hodgkin lymphoma entity, with a median survival of about 5 years. In 2008 we reported the early based on the median observation time of 4 years results of the Nordic Lymphoma Group MCL2 study of frontline intensive induction immunochemotherapy and autologous stem cell transplantation (ASCT), with more than 60% event-free survival at 5 years, and no subsequent relapses reported. Here we present an update after a median observation time of 6.5 years. The overall results are still excellent, with median overall survival and response duration longer than 10 years, and a median event-free survival of 7.4 years. However, six patients have now progressed later than 5 years after end of treatment. The international MCL Prognostic Index (MIPI) and Ki-67-expression were the only independent prognostic factors. Subdivided by the MIPI-Biological Index (MIPI + Ki-67, MIPI-B), more than 70% of patients with low-intermediate MIPI-B were alive at 10 years, but only 23% of the patients with high MIPI-B. These results, although highly encouraging regarding the majority of the patients, underline the need of a risk-adapted treatment strategy for MCL.

    • The electron dynamics within thin current sheets plays a key role both for the process of magnetic reconnection and other energy transfer mechanisms but, from an observational point of view, is not well understood. In this paper we report observations of a reconnecting current sheet with intermediate guide field B-G = 0.5B(in), where B-in is the magnetic field amplitude in the inflow regions. The current sheet width is comparable to electron spatial scales. It shows a bifurcated structure and is embedded within the magnetopause current layer with thickness of several ion scales. The electron scale current sheet has strong out-of-plane and in-plane currents, Hall electric and magnetic fields, a finite magnetic field component normal to the current sheet, and nongyrotropic electron distributions formed due to finite gyroradius effects at the boundary of the current sheet. Comparison between test particle simulations and electron data shows that electrons approaching from the edge of the largest magnetic curvature are scattered to perpendicular pitch angles in the center of the current sheet while electrons entering from the opposite side remain close to field aligned. The comparison also shows that an observed depletion in phase space at antiparallel pitch angles can be explained if an out-of-plane electric field, which due to the guide field is close to antiparallel to the magnetic field, is present in the center of the current sheet. This electric field would be consistent with the reconnection electric field, and we therefore interpret the depletion of electron phase space density as a manifestation of ongoing reconnection.

    • The Universe is permeated by hot, turbulent, magnetized plasmas. Turbulent plasma is a major constituent of active galactic nuclei, supernova remnants, the intergalactic and interstellar medium, the solar corona, the solar wind and the Earth's magnetosphere, just to mention a few examples. Energy dissipation of turbulent fluctuations plays a key role in plasma heating and energization, yet we still do not understand the underlying physical mechanisms involved. THOR is a mission designed to answer the questions of how turbulent plasma is heated and particles accelerated, how the dissipated energy is partitioned and how dissipation operates in different regimes of turbulence. THOR is a single-spacecraft mission with an orbit tuned to maximize data return from regions in near-Earth space - magnetosheath, shock, foreshock and pristine solar wind - featuring different kinds of turbulence. Here we summarize the THOR proposal submitted on 15 January 2015 to the 'Call for a Medium-size mission opportunity in ESAs Science Programme for a launch in 2025 (M4)'. THOR has been selected by European Space Agency (ESA) for the study phase.

    • This article seeks to move beyond the Euro/North-centrism recurrent in methodological discussions on what we may learn from the COVID-19 pandemic. Such debates often centre on uncertainty and involuntary immobility – aspects which are hardly new for many researchers. In this article, we argue that the pandemic offers an opportunity to rethink research relations between what we term ‘contracting researchers’ in the Global North and ‘facilitating researchers’ in the Global South. Such relations are often marked by rampant inequalities in remuneration, working conditions, and visibility/authorship. Drawing upon experiences in DR Congo, Sierra Leone, and India, we argue that the pandemic increased the dependence on – and highlighted the invaluable contributions and skills of – facilitating researchers, in part slightly refiguring bargaining power. We also propose pathways for change, arguing for a strong collaborative approach and the need for institutional change, without discarding the responsibilities of individual researchers.

  • Pre-Emptive Treatment With Rituximab of Molecular Relapse After Autologous Stem Cell Transplantation in Mantle Cell Lymphoma

    • Purpose Minimal residual disease (MRD) is predictive of clinical progression in mantle-cell lymphoma (MCL). According to the Nordic MCL-2 protocol we prospectively analyzed the efficacy of pre-emptive treatment using rituximab to MCL patients in molecular relapse after autologous stem cell transplantation (ASCT). Patients and Materials MCL patients enrolled onto the study, who had polymerase chain reaction (PCR) detectable molecular markers and underwent ASCT, were followed with serial PCR assessments of MRD in consecutive bone marrow and peripheral blood samples after ASCT. In case of molecular relapse with increasing MRD levels, patients were offered pre-emptive treatment with rituximab 375 mg/m(2) weekly for 4 weeks. Results Of 160 MCL patients enrolled, 145 underwent ASCT, of whom 78 had a molecular marker. Of these, 74 were in complete remission (CR) and four had progressive disease after ASCT. Of the CR patients, 36 underwent a molecular relapse up to 6 years (mean, 18.5 months) after ASCT. Ten patients did not receive pre-emptive treatment mainly due to a simultaneous molecular and clinical relapse, while 26 patients underwent pre-emptive treatment leading to reinduction of molecular remission in 92%. Median molecular and clinical relapse-free survival after pre-emptive treatment were 1.5 and 3.7 years, respectively. Of the 38 patients who remain in molecular remission for now for a median of 3.3 years (range, 0.4 to 6.6 years), 33 are still in clinical CR. Conclusion Molecular relapse may occur many years after ASCT in MCL, and PCR based pre-emptive treatment using rituximab is feasible, reinduce molecular remission, and may prevent clinical relapse.

    • We investigate the generation of waves in the lower hybrid frequency range by density gradients in the near plasma environment of comet 67P/Churyumov-Gerasimenko. When the plasma is dominated by water ions from the comet, a situation with magnetized electrons and unmagnetized ions is favourable for the generation of lower hybrid waves. These waves can transfer energy between ions and electrons and reshape the plasma environment of the comet. We consider cometocentric distances out to a few hundred km. We find that when the electron motion is not significantly interrupted by collisions with neutrals, large average gradients within tens of km of the comet, as well as often observed local large density gradients at larger distances, are often likely to be favourable for the generation of lower hybrid waves. Overall, we find that waves in the lower hybrid frequency range are likely to be common in the near plasma environment.

The evidence supporting best practise guidelines is based on studies with low-level evidence. A consensus group of experts was convened to collaboratively advance towards consensus opinions regarding the best available evidence. The purpose of this article is to report the resulting consensus statements.Methods: Twenty-five experts participated in a process based on the Delphi method of achieving consensus. Questions and statements were drafted via an online survey of two rounds, for initial agreement and comments on the proposed statements. An in-person meeting between the panellists was organised during the 2022 ESSKA congress to further discuss and debate each of the statements. A final agreement was made via a final online survey a few days later. The strength of consensus was characterised as: consensus, 51–74% agreement; strong consensus, 75–99% agreement; unanimous, 100% agreement.Results: Statements were developed in the fields of patient assessment and indications, surgical considerations and postoperative care. Between the 25 statements that were discussed by this working group, 18 achieved unanimous, whilst 7 strong consensus.Conclusion: The consensus statements, derived from experts in the field, represent guidelines to assist clinicians in decision-making for the appropriate use of mini-implants for partial resurfacing in the treatment of femoral chondral and osteochondral lesions. We aimed to characterize the preclinical systemic inflammation in ulcerative colitis, using a comprehensive set of proteins.Methods: We obtained plasma samples biobanked from individuals who developed ulcerative colitis later in life (n = 72) and matched healthy controls (n = 140) within a population-based screening cohort. We measured 92 proteins related to inflammation using a proximity extension assay. The biologic relevance of these findings was validated in an inception cohort of patients with ulcerative colitis (n = 101) and healthy controls (n = 50). To examine the influence of genetic and environmental factors on these markers, a cohort of healthy twin siblings of patients with ulcerative colitis (n = 41) and matched healthy controls (n = 37) were explored.Results: Six proteins (MMP10, CXCL9, CCL11, SLAMF1, CXCL11 and MCP-1) were up-regulated (P < .05) in preclinical ulcerative colitis compared with controls based on both univariate and multivariable models. Ingenuity Pathway Analyses identified several potential key regulators, including interleukin-1β, tumor necrosis factor, interferon-gamma, oncostatin M, nuclear factor-κB, interleukin-6, and interleukin-4. For validation, we built a multivariable model to predict disease in the inception cohort. The model discriminated treatment-naïve patients with ulcerative colitis from controls with leave-one-out cross-validation (area under the curve = 0.92). Consistently, MMP10, CXCL9, CXCL11, and MCP-1, but not CCL11 and SLAMF1, were significantly up-regulated among the healthy twin siblings, even though their relative abundances seemed higher in incident ulcerative colitis.Conclusions: A set of inflammatory proteins are up-regulated several years before a diagnosis of ulcerative colitis. These proteins were highly predictive of an ulcerative colitis diagnosis, and some seemed to be up-regulated already at exposure to genetic and environmental risk factors.

    • Essential for detection of relevant external stimuli and for fear processing, the amygdala is under modulatory influence of dopamine (DA). The DA transporter (DAT) is of fundamental importance for the regulation of DA transmission by mediating reuptake inactivation of extracellular DA. This study examined if a common functional variable number tandem repeat polymorphism in the 3' untranslated region of the DAT gene (SLC6A3) influences amygdala function during the processing of aversive emotional stimuli. Amygdala reactivity was examined by comparing regional cerebral blood flow, measured with positron emission tomography and [(15)O]water, during exposure to angry and neutral faces, respectively, in a Swedish sample comprising 32 patients with social anxiety disorder and 17 healthy volunteers. In a separate US sample, comprising 85 healthy volunteers studied with blood oxygen level-dependent functional magnetic resonance imaging, amygdala reactivity was assessed by comparing the activity during exposure to threatening faces and neutral geometric shapes, respectively. In both the Swedish and the US sample, 9-repeat carriers displayed higher amygdala reactivity than 10-repeat homozygotes. The results suggest that this polymorphism contributes to individual variability in amygdala reactivity.

    • AIM: To investigate whether professional training and/or clinical experience affect the ability of caregiver to assess clinical signs of pre-emptive morphine analgesia. METHODS: In the Neurological Outcomes & Pre-emptive Analgesia In Neonates trial preterm infants undergoing mechanical ventilation were randomized to receive continuous infusion, either of morphine or placebo blinded. Staff from centres in Sweden (Stockholm and Orebro) completed an assessment form. RESULTS: A total of 360 assessment forms were collected from 52 neonates. In 59% of the cases, caregivers correctly identified patients group. Comparable proportion of answers were correct between physicians, nurses and assistant nurses (63, 60 and 54%, respectively, p = 0.60). Staff with Neonatal intensive care unit experience <1 year identified 63%, as compared to 65% for working 1-5 year, and 55% that has been working >5 years (p = 0.28). Staff's ability to correctly identify group assignment was reduced by amount of additional morphine (p < 0.01) and severity of illness (p = 0.01). CONCLUSIONS: Clinical medical staffs, including neonatologists, have great difficulties in assessing the presence and severity of pain. Further studies should focus on the methods for assessment of prolonged pain in preterm neonates, define the effects of adequate analgesia, and investigate the clinical factors that may alter neonatal responses to acute and prolonged pain.

    • An integrated stratigraphic, paleoecological, and geochronological study of lake and fen sediments just beyond the terminus of Berendon Glacier provides insights into late Holocene climate, vegetation, and glacier change in the northern Coast Mountains of British Columbia. Cores collected from two small lakes in the foreland of Berendon Glacier and pits dug in a nearby fen record Little lee Age and older glacier advances. The first Little Ice Age advance in this area began more than 500 years ago and peaked in the early 17th century. An earlier Neoglacial advance began about 2800-3000 cal yr ago and may have lasted for hundreds of years. There is also evidence for an intervening advance of even smaller magnitude around 1200-1300 cal yr ago. The advances are broadly synchronous with those in other parts of western North America, indicating that they were caused by regional, possibly global, changes in climate. Plant communities within the study area did not change dramatically during the late Holocene. The ranges of some plants, however, likely retracted or extended near treeline in response to changes in mean temperatures of perhaps 1-2degreesC, as A well as changes in summer snow cover. The greatest changes in vegetation occurred within and just beyond the forefields of Berendon, Frank Mackie, and other nearby glaciers. The largest climate shifts of the last 3000 years took place during the late Little Ice Age and the last century. Climate warmed about 1-2degreesC during the 20th century, accompanied by a rise in treeline, an increase in coniferous tree cover in the subalpine zone, and an increase in the temperature and biological productivity of ponds.     • The Rosetta dual Langmuir probe instrument, LAP, utilizes the multiple powers of a pair of spherical Langmuir probes for measurements of basic plasma parameters with the aim of providing detailed knowledge of the outgassing, ionization, and subsequent plasma processes around the Rosetta target comet. The fundamental plasma properties to be studied are the plasma density, the electron temperature, and the plasma flow velocity. However, study of electric fields up to 8 kHz, plasma density fluctuations, spacecraft potential, integrated UV flux, and dust impacts is also possible. LAP is fully integrated in the Rosetta Plasma Consortium (RPC), the instruments of which together provide a comprehensive characterization of the cometary plasma.

    • With the radionuclide tracers available today, 50-90 per cent of neuroendocrine tumours of the gastro-intestinal tract can be visualised with PET (positron-emission tomography). PET also enables the effect of tumour treatment to be monitored in terms of biochemical and functional variables, which is not possible with other radiological techniques. Owing to the very good tumour resolution possible with PET, it serves as a complement to other routine methods such as computed tomography and ultrasonography, and can be used to screen the chest and abdomen for small primary tumours that can not be detected with other methods. In several pre-operative trials PET has been shown to demonstrate more changes in the pancreas and liver than was possible with other methods. In the near future it will be possible to demonstrate the presence of and quantify growth factor receptors, hormones, enzymes, DNA synthesis, mRNA synthesis and protein synthesis. Access to these tumour biological data will be of crucial importance to the individualisation of treatment.

    • We study Magnetospheric MultiScale observations in the outflow region of magnetotail reconnection. We estimate the power density converted via the three fundamental electron acceleration mechanisms: Fermi, betatron, and parallel electric fields. The dominant mechanism, both on average and the peak values, is Fermi acceleration with a peak power density of about +200 pW/m3. The magnetic field curvature during the most intense Fermi acceleration is comparable to the electron gyroradius, consistent with efficient electron scattering. The peak power densities due to the betatron acceleration are a factor of 3 lower than that for the Fermi acceleration, the average betatron acceleration is close to zero and slightly negative. The contribution from parallel electric fields is significantly smaller than those from the Fermi and betatron acceleration. However, the observational uncertainties in the parallel electric field measurement prevent further conclusions. There is a strong variation in the power density on a characteristic ion time scale.

    • Background: Intraosseous (IO) access is a recommended method when venous access cannot be rapidly established in an emergency. Experimental data suggest that major hemorrhage and catecholamine administration both reduce bone marrow blood flow. We studied the uptake of gentamicin as a tracer substance administered IO following adrenaline administration in hemorrhagic shock and in cardiac arrest.Methods: Twenty anesthetized pigs underwent hemorrhage corresponding to 50% of the blood volume. They then received injections of either; adrenaline IO (n=5), saline IO n=5), adrenaline IO during cardiac arrest and cardiopulmonary resuscitation (CPR, n=5), or intravenous adrenaline. The injections were followed by an injection of gentamicin by the same route. Doses and volumes were equivalent among the groups. In all animals, mixed venous antibiotic concentrations were analyzed at 5, 15 and 30min after administration.Results: Mean (SD) plasma gentamicin concentrations (mg x L-1) at 5min were 26.4 (2.3) in the group with previous IO adrenaline administration, 26.6 (4.5) in the IO saline group, 31. 2 (12) in the IO adrenaline + CPR group and 23 (4.5) in the IV group. Concentrations in the CPR group were significantly higher than the others.Conclusions: No impairment of drug uptake with IO administration after recent IO adrenaline exposure was demonstrable in this shock model.

  • Troponin I can be Determined in Intraosseous Aspirates in a Porcine Shock Model

    • BACKGROUND: Determination of troponin I may be important in the management of the critically ill patient. In medical emergencies, especially when vascular access is difficult to achieve, the use of intraosseous (10) needles is recommended. We aimed to perform a descriptive study, aiming to elucidate whether IO needles can be used to evaluate troponin I in a porcine model of human shock.METHODS: Eight pigs were anesthetized and challenged with a 6 hours continuous intravenous infusion of E. coli endotoxin. An IO needle (EZ-IO®) was inserted in the proximal tibia of each pig. Circulatory variables were monitored and troponin I was sampled from arterial and venous blood and also from bone marrow aspirates.RESULTS: Circulatory deterioration developed in all endotoxemic animals, which was reflected by a profound deterioration of left ventricular stroke work index. Troponin I levels were nearly identical in both arterial, venous, and IO samples during the first hour of endotoxemia. At 1 hour, all mean troponin I levels had more than doubled as compared to baseline. The troponin I levels continued to increase over time and were markedly elevated versus baseline levels during the 2nd and 6th hours, regardless of sampling site. At 3 hours, IO troponin I reached a plateau, whereas troponin I in both arterial and venous blood continued to increase.CONCLUSIONS: This investigation has shown that troponin I can be analyzed in bone marrow aspirates in a shock model. This may be useful in medical emergencies, where cardiac damage is suspected to be involved. The levels of IO troponin I increased during the first 3 hours of shock, after which it remained at a high level. During this initial period there was, in parallel, a progressive circulatory deterioration.

    • Although the potential of adult neural stem cells to repair damage via cell replacement has been widely reported, the ability of endogenous stem cells to positively modulate damage is less well studied. We investigated whether medium conditioned by adult hippocampal stem/progenitor cells altered the extent of excitotoxic cell death in hippocampal slice cultures. Conditioned medium significantly reduced cell death following 24 h of exposure to 10 microM NMDA. Neuroprotection was greater in the dentate gyrus, a region neighboring the subgranular zone where stem/progenitor cells reside compared with pyramidal cells of the cornis ammonis. Using mass spectrometric analysis of the conditioned medium, we identified a pentameric peptide fragment that corresponded to residues 26-30 of the insulin B chain which we termed 'pentinin'. The peptide is a putative breakdown product of insulin, a constituent of the culture medium, and may be produced by insulin-degrading enzyme, an enzyme expressed by the stem/progenitor cells. In the presence of 100 pM of synthetic pentinin, the number of mature and immature neurons killed by NMDA-induced toxicity was significantly reduced in the dentate gyrus. These data suggest that progenitors in the subgranular zone may convert exogenous insulin into a peptide capable of protecting neighboring neurons from excitotoxic injury.

    • The spacecraft potential is often used to infer rapid changes in the thermal plasma density. The variations in spacecraft potential associated with large-amplitude Langmuir and upper hybrid waves are investigated with the Magnetospheric Multiscale (MMS) mission. When large-amplitude Langmuir and upper hybrid waves are observed, the spacecraft potential increases. The changes in spacecraft potential are shown to be due to enhanced photoelectron escape from the spacecraft when the wave electric fields reach large amplitude. The fluctuations in spacecraft potential follow the envelope function of the Langmuir and upper hybrid waves. Comparison with the high-resolution electron moments shows that the changes in spacecraft potential associated with the waves are not due to density perturbations. Indeed, using the spacecraft potential as a density probe leads to unphysically large density fluctuations. In addition, the changes in spacecraft potential are shown to increase as density decreases: larger spacecraft potential changes are observed in the magnetosphere, than in the magnetosheath and solar wind. These results show that external electric fields can lead to unphysical results when the spacecraft potential is used as a density probe. The results suggest that fluctuations in the spacecraft potential alone cannot be used to determine whether nonlinear processes associated with Langmuir and upper hybrid waves, such as the ponderomotive force and three-wave decay, are occurring.

    • An important source of magnetospheric plasma is cold plasma from the terrestrial ionosphere. Low energy ions travel along the magnetic field lines and enter the magnetospheric lobes where they are convected toward the tail plasma sheet. Recent observations indicate that the field aligned ion outflow velocity is sometimes much higher than the convection toward the central plasma sheet. A substantial amount of plasma therefore escapes downtail without ever reaching the central plasma sheet. In this work, we use Cluster measurements of cold plasma outflow and lobe convection velocities combined with models of the magnetic field in an attempt to determine the fate of the outflowing ions and to quantify the amount of plasma lost downtail. The results show that both the circulation of plasma and the direct tailward escape of ions varies significantly with magnetospheric conditions. For strong solar wind driving with a southward interplanetary magnetic field, also typically associated with high geomagnetic activity, most of the outflowing plasma is convected to the plasma sheet and recirculated. For periods with northward interplanetary magnetic field, the convection is nearly stagnant, whereas the outflow, although limited, still persists. The dominant part of the outflowing ions escape downtail and are directly lost into the solar wind under such conditions.

    • Low-energy ions of ionospheric origin constitute a significant contributor to the magnetospheric plasma population. Measuring cold ions is difficult though. Observations have to be done at sufficiently high altitudes and typically in regions of space where spacecraft attain a positive charge due to solar illumination. Cold ions are therefore shielded from the satellite particle detectors. Furthermore, spacecraft can only cover key regions of ion outflow during segments of their orbit, so additional complications arise if continuous longtime observations, such as during a geomagnetic storm, are needed. In this paper we suggest a new approach, based on a combination of synoptic observations and a novel technique to estimate the flux and total outflow during the various phases of geomagnetic storms. Our results indicate large variations in both outflow rates and transport throughout the storm. Prior to the storm main phase, outflow rates are moderate, and the cold ions are mainly emanating from moderately sized polar cap regions. Throughout the main phase of the storm, outflow rates increase and the polar cap source regions expand. Furthermore, faster transport, resulting from enhanced convection, leads to a much larger supply of cold ions to the near-Earth region during geomagnetic storms.

    • Background: Bipolar disorder is a chronic condition with recurring episodes that often lead to suffering, decreased functioning, and sick leave. Pharmacotherapy in the form of mood stabilizers is widely available, but does not eliminate the risk of a new depressive or (hypo) manic episode. One way to reduce the risk of future episodes is to combine pharmacological treatment with individual or group psychological interventions. However, access to such interventions is often limited due to a shortage of trained therapists. In unipolar depression there is now robust evidence of the effectiveness of Internet-based psychological interventions, usually comprising psychoeducation and cognitive behavioral therapy (CBT). Internet-based interventions for persons suffering from bipolar disorder could increase access to psychological treatment.Objective: The aim of this study was to investigate the feasibility of an Internet-based intervention, as well as its effect on residual depressive symptoms in persons diagnosed with bipolar disorder type II (BP-II). The most important outcomes were depressive symptoms, treatment adherence, and whether the patient perceived the intervention as helpful.Methods: A total of 7 patients diagnosed with bipolar disorder type II at a Swedish psychiatric outpatient clinic were offered the opportunity to participate. Of the 7 patients, 3 (43%) dropped out before treatment began, and 4 (57%) were treated by means of an online, Internet-based intervention based on CBT (iCBT). The intervention was primarily aimed at psychoeducation, treatment of residual depressive symptoms, emotion regulation, and improved sleep. All patients had ongoing pharmacological treatment at recruitment and established contact with a psychiatrist. The duration of BP-II among the treated patients was between 6 and 31 years. A single-subject design was used and the results of the 4 participating patients were presented individually.Results: Initiating treatment was perceived as too demanding under current life circumstances for 3 patients who consequently dropped out during baseline assessment. Self-ratings using the Montgomery-sberg Depression Rating Scale-Self-rated (MADRS-S) showed symptom reduction in 3 (75%) of the 4 treated cases during iCBT. In the evaluation of the treatment, 2 patients reported that they perceived that the treatment had reduced symptoms a little, 1 that it had reduced symptoms very much, and 1 not at all. Treatment adherence (ie, module completion) was fairly high in 3 cases. In general, the modules were perceived as fairly helpful or very helpful by the patients. In one case, there was a reliable change-according to the Reliable Change Index-in self-rated symptoms of depression and perseverative thinking.Conclusions: The treatment seemed to have acceptable feasibility. The iCBT intervention could be an effective way to treat residual symptoms in some patients with bipolar disorder type II. This should be investigated in a larger study.

    • Background: Many patients with aggressive B-cell lymphomas and high clinical risk score still die of lymphoma after conventional R-CHOP chemoimmunotherapy. We hypothesized that intensified chemoimmunotherapy including systemic central nervous system (CNS) prophylaxis improves outcome and reduces the incidence of CNS-related events. Patients and methods: Inclusion criteria were age 18-65 years, primary diffuse large B-cell lymphoma or grade III follicular lymphoma without clinical signs of CNS disease and negative cerebrospinal fluid cytology, age-adjusted International Prognostic Index 2-3 and WHO performance score 0-3. Treatment consisted of six courses of R-CHOEP-14 followed by a course of high-dose cytarabine and a course of high-dose methotrexate. Primary end point was failure-free survival (FFS) at 3 years. Results: A total of 156 eligible patients with a median age of 54 years (range 20-64) were included. Three toxic deaths were observed. Three-year overall survival (OS) and FFS rates (median observation time 52 months for survivors) were 81% and 65%, respectively. Seven patients experienced CNS relapse, all within 6 months. Conclusions: The results are promising with favorable 3-year OS and FFS rates, a low toxic death rate and a lower than expected number of CNS events.     • The aim of this study was to achieve realistic levels of two different types of aerosols commonly abundant in indoor environments in an experimental chamber intended for human exposure studies and aerosol characterization. The aerosols chosen were particles from candle lights (in particle number dominated by inorganic water soluble particles) and from ozone-terpene reactions (organic particles). The aerosol generation and characterization system consisted of a controlled air tight stainless steel 22 m(3) chamber, to which the generation set-ups were connected. No air could enter or leave the chamber except through a conditioning system by which temperature, relative humidity and air exchange rate could be controlled. Candle smoke aerosol was generated from ten candles burning in a 1.33 m(3) glass and stainless steel chamber. The aerosol was diluted by clean air from the conditioning system before entering the chamber. Terpene vapor was generated by passing pure nitrogen through a glass bottle containing limonene oil. Ozone was generated by a spark discharge using pure O-2, and was added to the ventilation air flow downstream the inlet for terpene vapors and upstream the inlet to the chamber. Both aerosols were characterized with respect to number and mass concentrations, size distribution and chemical composition. Particle number concentration in the size range 10-650 nm could be varied from <10 cm(-3) to more than 900,000 cm(-3) (for candle smoke) or to more than 30,000 cm(-3) (for particles formed in a 160 ppb terpene/40 ppb ozone mixture). Furthermore, the set-ups were evaluated by, for each source, repeating the generation at six three-hour long events. For both aerosols repeatable generations at pre-determined concentration levels, that were stable over time, could be achieved. The results show that realistic concentrations of aerosols from real-world environments could be reproduced in a well-controlled manner and that this set-up could be used both for aerosol characterization and for human exposures.   • Chewing gum and human hair as retrospective dosimeters

    • Retrospective dosimeters are sometimes needed after radiological/nuclear (RN) exposures to determine the doses to individuals. Conventional dosimeters may not be at hand or may not be applicable calling for alternative materials.The possible exposure situations can be divided into external and internal; the radiation field stems either from outside the body or from a source within. This thesis investigates the possibility to use chewing gum and hair as retrospective dosimeters. The chewing gum would be used after an unexpected radiation event of external type whereas human hair is examined after chronic intake of uranium. Chewing gum containing xylitol and sorbitol was analyzed using electron paramagnetic resonance (EPR) and the hair was analyzed by alphaspectrometry following radiochemistry and by synchrotron radiation microbeam x-ray fluorescence (SR μ-XRF).Xylitol and chewing gum (in this particular case, V6) are in the present work found to be valuable dosimeters after unexpected radiation events. The xylitol signal linearity with dose in the interval 0-10 Gy was confirmed (r2=1.00). The doses to the coating of the chewing gums were determined 4-6 days after irradiation with an uncertainty of less than 0.2 Gy (1 SD). Spectral dependence with time after exposure was found, but was, however, minimal between 4-8 days.Hair was evaluated and compared with urine as biodosimeter after ingestion and inhalation intake of uranium. Concentrations of 234U and 238U and their activity ratios were measured in the hair, urine and drinking water sampled from 24 drilled bedrock well water users in Östergötland, Sweden, as well as among 8 workers at a nuclear fuel fabrication factory, Westinghouse Electric Sweden. The results show that there is a stronger correlation between the uranium concentrations in the drinking water of the well water and the users’ hair (r2 = 0.50) than with their urine (r2 = 0.21). There is also a stronger correlation between the 234U/238U activity ratios of water and hair (r2 = 0.91) than between water and urine (r2 = 0.56). The individual absorbed fraction of uranium, the ƒ value, calculated as the ratio between the excreted amount of uranium in urine and hair per day and the daily drinking water intake of uranium stretched from 0.002 to 0.10 with a median of 0.023. The uranium concentrations of the fuel factory workers’ hair and urine were also obtained as well as that of personal air sampler (PAS) filters for the determination of inhaled uranium activity. A large day-to-day variation (7-70 Bq d-1) of the inhaled 234U activity was seen over a 6 week period. Over a 12 week period the 234U activity concentration in urine was similarly seen to vary from 2 to 50 mBq kg-1. Four hair samples from the same subject and period showed less variation (100-240 mBq g-1). The uranium inhalation to urine and hair factors finh,u and finh,h were found to be 0.0014 and 0.0002 respectively given by calculations based on the measured PAS, urine and hair data from two individuals. The SR μ-XRF measurements showed that uranium is present in an outer layer of the hair shaft, about 10-15 μm wide. The  measurements also revealed particles containing uranium being present on the surface of unwashed hair shafts. However, the washed hair shafts showed few, if any, particles.This thesis concludes that chewing gum and hair can be used as retrospective dosimeters after external radiation and after intake of uranium respectively.

    • The growth mechanism and dependence on precursor conditions are vital for creation of high quality crystalline materials in many fields. Here the growth from nano sized quantum dots to micro crystalline methyl ammonium lead tri-iodide (MAPbI(3)) perovskites prepared from isopropanol solution are reported. Isopropanol is more environmental friendly compared to the commonly used solvents DMF or DMSO, both with relatively high toxicity and the proposed method can be a useful new route to prepare hybrid perovskites. Three different molar ratios of MAPbI3 perovskite solution (MAI:PbI2 of 1: 1, 2: 1 and 0.5: 1) were applied to give insights in the crystal formation mechanism also under non-stoichiometric conditions. Perovskite crystal growth is followed by TEM. From XRD powder diffraction the lattice constants have been determined and compared with results from electron diffraction (ED). Interestingly, there seems to be an occurrence of the cubic phase besides the common tetragonal phase at room temperature. (C) 2016 The Electrochemical Society. All rights reserved.

    • BackgroundGlobally, approximately 6,700 newborn deaths and 5,400 stillbirths occur daily. The true figure is likely higher, with under reporting of adverse pregnancy outcomes (APOs) noted. Decision-making in health is influenced by various factors, including one's social networks. We sought to understand APOs disclosure within social networks in Uganda, Ghana, Guinea-Bissau and Bangladesh and how this could improve formal reporting of APOs in surveys.MethodsA qualitative, exploratory multi-country study was conducted within four health and demographic surveillance system sites. 16 focus group discussions were held with 147 women aged 15-49 years, who had participated in a recent household survey. Thematic analysis, with both deductive and inductive elements, using three pre-defined themes of Sender, Message and Receiver was done using NVivo software.ResultsDisclosure of APOs was a community concern, with news often shared with people around the bereaved for different reasons, including making sense of what happened and decision-making roles of receivers. Social networks responded with comfort, providing emotional, in-kind and financial support. Key decision makers included men, spiritual and traditional leaders. Non-disclosure was usually to avoid rumors in cases of induced abortions, or after a previous bad experience with health workers, who were frequently excluded from disclosure, except for instances where a woman sought advice on APOs.ConclusionsCommunities must understand why they should report APOs and to whom. Efforts to improve APOs reporting could be guided by diffusion of innovation theory, for instance for community entry and sensitization before the survey, since it highlights how information can be disseminated through community role models. In this case, these gatekeepers we identified could promote reporting of APOs. The stage at which a person is in decision-making, what kind of adopter they are and their take on the benefits and other attributes of reporting are important. In moving beyond survey reporting to getting better routine data, the theory would be applicable too. Health workers should demonstrate a more comforting and supportive response to APOs as the social networks do, which could encourage more bereaved women to inform them and seek care.
