    • BACKGROUND: Post-partum hemorrhage (PPH) is the major cause of maternal mortality in Ghana and worldwide. Active management of the third stage of labor (AMTSL) is a globally recommended three-step method that in clinical trials has been proven effective in prevention of PPH. The AMTSL guidelines were introduced in 2003, modified in 2006, and has been part of the national guidelines in Ghana since 2008. In 2012, the guidelines were modified a second time. Despite its positive effects on the incidence of PPH, the level of adherence to the guidelines seems to be low in the studied area. This appears to be a problem shared by several countries in the region. An in-depth understanding of midwives' experiences about AMTSL is important as it can provide a basis for further interventions in order to reach a higher grade of implementation.METHODS: Twelve in-depth interviews were conducted with labor ward midwives who all had previous training in AMTSL. The interviews took place in 2011 at three hospitals in Accra Metropolis and data was analyzed using qualitative latent content analysis.RESULTS: Our main finding was that the third step of AMTSL, uterine massage, was not implemented, even though the general attitude towards AMTSL was positive. Thus, despite regular training sessions, the midwives did not follow the Ghanaian national guidelines. Some contributing factors to difficulties in providing AMTSL to all women have been pointed out in this study, the most important being insufficiency in staff coverage. This led to a need for delegating certain steps of AMTSL to other health care staff, i.e. task shifting. The fact that the definition of AMTSL has changed several times since the introduction in 2003 might also be an aggravating factor.CONCLUSIONS: The results from this study highlight the need for continuous updates of national guidelines, extended educational interventions and recurrent controls of adherence to guidelines. AMTSL is an important tool in preventing PPH, however, it must be clarified how it should be used in countries with scarce resources. Also, considering the difficulties in implementing already existing guidelines, further modifications must be made with careful consideration.

    • AIMS AND OBJECTIVES: This study's purpose was to describe and analyze coach-parents' development process when supporting parents of children recently diagnosed with Type 1 Diabetes Mellitus (T1DM).BACKGROUND: It has been found repeatedly that providing social support for families with a child diagnosed with T1DM promotes health and wellbeing for both the child and the parents. Less explored are the processes experienced by those who provide this support. However, research has found that acting as a provider of social support promotes personal development, strengthens communication skills, and increases self-confidence.METHODS: The study design was based on Constructivist Grounded Theory and data were collected, through Repeated Focus Group Discussions, from eight coach-parents at a Swedish hospital from 2012-2015.RESULTS: The core category in the data was identified as a learning process where coach-parents emphasized their own learning in the dyad supporter - supported, and in the interaction with other parents in the Repeated Focus Group Discussions. The coach-parents' motivation for participation was a wish to learn more and to help other parents in a life-changing situation. They also pointed out hindrances and their frustration when unable to provide support.CONCLUSIONS: This study leads to the conclusion that people who provide support benefit from doing so. Encountering people with similar experiences in a supportive situation promotes a reciprocal learning process, based on the supporter's wish to help people in a situation they recognize. A further conclusion is that social support is not only essential initially, but is also important over a longer period and that it follows various life stages.RELEVANCE TO CLINICAL PRACTICE: Setting up repeated focus group discussions might be a relevant and effective tool for pediatric diabetes nurses to use in promoting health and wellbeing for both families with a newly diagnosed child and experienced families.     • Magnetic nanoparticles are the functional component for magnetic membranes, but they are difficult to disperse and process into tough membranes. Here, cellulose nanofibers are decorated with magnetic ferrite nanoparticles formed in situ which ensures a uniform particle distribution, thereby avoiding the traditional mixing stage with the potential risk of particle agglomeration. The attachment of the particles to the nanofibrils is achieved via aqueous in situ hydrolysis of metal precursors onto the fibrils at temperatures below 100 °C. Metal adsorption and precursor quantification were carried out using Induction Coupled Plasma-Optical Emission Spectroscopy (ICP-OES). FE-SEM was used for high resolution characterization of the decorated nanofibers and hybrid membranes, and TEM was used for nanoparticle size distribution studies. The decorated nanofibers form a hydrocolloid. Large (200 mm diameter) hybrid cellulose/ferrite membranes were prepared by simple filtration and drying of the colloidal suspension. The low-density, flexible and permanently magnetized membranes contain as much as 60 wt% uniformly dispersed nanoparticles (thermogravimetric analysis data). Hysteresis magnetization was measured by a Vibrating Sample Magnetometer; the inorganic phase was characterized by XRD. Membrane mechanical properties were measured in uniaxial tension. An ultrathin prototype loudspeaker was made and its acoustic performance in terms of output sound pressure was characterized. A full spectrum of audible frequencies was resolved.

    • Background:Various factors drive a care setting shift from institutional to home settings for dementia care until the latest stage of the disease, suggesting the critical role of family caregivers living with persons with dementia.Objectives:This study explored the characteristics and correlates of self-reported overall physical and psychological (ie, depressive symptoms and stress) health among family caregivers living with persons with dementia in Sweden.Methods:This cross-sectional, descriptive study used baseline data from an existing music-based intervention study of persons with dementia and their family caregivers (N = 76). Multivariable linear regression analyses were performed.Results:On average, caregivers (n = 38; mean age: 74.8 years) were slightly younger than persons with dementia (n = 38; mean age: 78.6 years). Most caregivers were female (n = 24; 63.2%) and spouses or partners of persons with dementia (n = 37; 97.4%). Caregivers’ perceived relationship with their family members with dementia was the only factor associated with caregivers’ self-reported overall physical health (b = −0.655, p = .046). This suggests caregivers’ more frequent feeling of a good relationship with the persons with dementia was linked to better self-rated physical health among family caregivers living with persons with dementia.Conclusions:This study highlights the importance of family caregivers’ perceived relationship with persons with dementia in the context of caregivers’ self-reported physical health. Future research is needed to explore the perceived relationship from the perspectives of persons with dementia and the determinants of caregiving dyads’ (persons with dementia and family caregivers) perceived relationship with each other.

  • Party membership and state subsidies : a comparative study

    • This thesis tests the hypothesis that increasing state subsidies to political parties cause a declined party membership. The theory that predicted this development was constructed by Epstein in 1967 and presented in his book Political Parties in Western Democracies. The theoretical propositions have been debated by political scientists since then, but no thorough test has been conducted.  In order to test the hypothesis state subsidies data and party membership data was collected for the national parties from eight states. The data was then analysed by use of the panel data models. Panel data models make use of the regression analysis technique. Examples of specialized such models are analysis through the fundamental parameter of change, first differencing and pooled analysis. The results provided by the models proved to not give a straight answer. Instead, the results pointed in different directions.  The most likely interpretation of the results is that increasing state subsidies to political parties does not cause a decline in party membership. To validate such a result would require that most of the results pointed in the same direction. Thus, the hypothesis of the investigation should be considered falsified. Instead the focus of future studies should probably be directed at the supply side of party membership. When party membership decline is explained by the supply side this means that the decline is explained by people stop having motive to become party members. However, it is important to remember that other interpretations of the results are possible as well.

  • Influences of income and employment on psychological distress and depression treatment in Japanese adults

    • Objectives: Psychological distress is a health issue of critical importance, especially in people of working age in developed countries, including Japan. This study examined the relationships of income and employment arrangement with psychological distress and treatment of depression in a national sample of Japanese adults.Methods: Data for 10,959 men and 11,655 women 25–59 years of age, obtained from the Comprehensive Survey of the Living Conditions of People on Health and Welfare in 2007, were examined. Health outcomes were psychological distress measured by the Japanese version of the K6, subjective complaints and medical treatment of depression. Explanatory variables included marital status, employment arrangement, and household income. The relationships between health outcomes and explanatory variables were examined using multiple logistic regression analyses.Results: Lower income and unemployment were associated with a higher prevalence of psychological distress and treatment of depression. The association between psychological distress and income showed a threshold: the lowest income quintile had an especially high prevalence, while other quintiles had similar prevalences. The prevalence of depression treatment in those with psychological distress was significantly lower in the highest income quintile than in all the other income groups, and the prevalence was also significantly lower in employed than in unemployed respondents.Conclusions: This study showed clear relationships of lower income and unemployment with psychological distress and depression treatment. It has been suggested that people with higher socioeconomic status and full-time work may be reluctant to consult professionals and receive medical treatment, despite their psychological distress. Comprehensive mental health interventions are required to prevent psychological distress in all socioeconomic strata of the population.

  • Analyzing the Social Structure and Dynamics of E-mail and Spam in Massive Backbone Internet Traffic

    • E-mail is probably the most popular application on the Internet, with everyday busi-ness and personal communications dependent on it. Spam or unsolicited e-mail has beenestimated to cost businesses significant amounts of money. However, our understandingof the network-level behavior of legitimate e-mail traffic and how it differs from spamtraffic is limited. In this study, we have passively captured SMTP packets from a 10Gbit/s Internet backbone link to construct a social network of e-mail users based on theirexchanged e-mails. The focus of this paper is on the graph metrics indicating variousstructural properties of e-mail networks and how they evolve over time. This study alsolooks into the differences in the structural and temporal characteristics of spam and non-spam networks. Our analysis on the collected data allows us to show several differencesbetween the behavior of spam and legitimate e-mail traffic, which can help us to under-stand the behavior of spammers and give us the knowledge to statistically model spamtraffic on the network-level in order to complement current spam detection techniques.

    • This paper proposes a novel formulation of the Chan-Vese model for pose invariant shape prior segmentation as a continuous cut problem. The model is based on the classic L 2 shape dissimilarity measure and with pose invariance under the full (Lie-) group of similarity transforms in the plane. To overcome the common numerical problems associated with step size control for translation, rotation and scaling in the discretization of the pose model, a new gradient descent procedure for the pose estimation is introduced. This procedure is based on the construction of a Riemannian structure on the group of transformations and a derivation of the corresponding pose energy gradient. Numerically, this amounts to an adaptive step size selection in the discretization of the gradient descent equations. Together with efficient numerics for TV-minimization we get a fast and reliable implementation of the model. Moreover, the theory introduced is generic and reliable enough for application to more general segmentation- and shape-models.

    • Purpose The aim of this study was to evaluate the possible risk factors for developing a parastomal hernia (PSH) in a cohort of rectal cancer patients with and without the application of a retro-muscular prophylactic mesh. The evaluated risk factors included the position of the stoma in the rectus abdominis muscle (RAM), RAM atrophy and shift of abdominal wall midline structures. Methods Rectal cancer patients treated with an abdominoperineal excision or Hartmann's procedure between 2002 and 2015 at Vastmanland Hospital, Sweden was included. Postoperative CT examinations were retrospectively reviewed regarding the presence of PSH and RAM atrophy and for measurements such as position of the stoma in the RAM. Results 116 patients were included, with a median age of 71 years. 70 patients received a prophylactic stoma mesh. 55 patients (47%) had a parastomal hernia at three-year follow-up. Rectus abdominis muscle atrophy was significantly higher in the mesh group compared with the non-mesh group (37% vs 2%; P = 0.04). RAM atrophy was a significant independent protective factor for developing a PSH (P = 0.007; OR 0.15; 95% CI 0.03-0.55). Conclusion Placement of a prophylactic retro-muscular stoma mesh resulted in a high frequency of RAM atrophy distal to the stomal aperture and patients with such atrophy had a lower risk of developing a PSH.

    • Legislation for the preservation of biodiversity has been instrumental to the recovery of multiple species and habitats. The European Habitats Directive 92/92/EEC is one of the strongest legal tools in nature conservation. This Directive seeks to achieve its biodiversity goals by requiring EU Member States to take measures to reach or maintain Favourable Conservation Status (FCS) of natural habitats and species in Europe. FCS is a legal concept, but must be understood and applied by scientists, managers and policy makers, and therefore a proper interpretation of this concept is crucial for biodiversity conservation and wildlife management. However, its definition contains several aspects that can lead to misinterpretation, being the core of controversies in determining whether or not populations have reached FCS. In this review, we provide legal and ecological clarifications of the most contested aspects of FCS that have not yet been conclusively settled by analyzing and weighting a variety of sources.

    • The drilling industry is steadily moving towards automation. To have a better control over the drilling operation and to optimize the drilling performance, it is necessary to have a good understanding of the physics involved in the process. A good simulation model of the drilling process/system would be of great value in assisting this and in enabling the development of strategies to optimize it. A representative simulation model can provide insights into various phenomena that appear during drilling for different drilling conditions. Such a model is also likely to be of assistance in preparing various measurements. Issues such as choosing the type of sensors, their positions and which quantities to measure are supported by a model. A well calibrated model in combination with information extracted from measured data are hence likely to assist in selecting control strategies for optimized drilling performance. In this paper we present a MATLAB based 1D simulation model of a Down The Hole (DTH) drill and compare it with an existing 1D model in terms of computational speed and accuracy. The emphasis is to make a 3D model of a DTH system that is computationally efficient and accurate.

    • Four different interaction styles for the social robot Furhat acting as a host in spoken conversation practice with two simultaneous language learners have been developed, based on interaction styles of human moderators of language cafés. We first investigated, through a survey and recorded sessions of three-party language café style conversations, how the interaction styles of human moderators are influenced by different factors (e.g., the participants language level and familiarity). Using this knowledge, four distinct interaction styles were developed for the robot: sequentially asking one participant questions at the time (Interviewer); the robot speaking about itself, robots and Sweden or asking quiz questions about Sweden (Narrator); attempting to make the participants talk with each other (Facilitator); and trying to establish a three-party robot–learner– learner interaction with equal participation (Interlocutor). A user study with 32 participants, conversing in pairs with the robot, was carried out to investigate how the post-session ratings of the robot’s behavior along different dimensions (e.g., the robot’s conversational skills and friendliness, the value of practice) are influenced by the robot’s interaction style and participant variables (e.g., level in the target language, gender, origin). The general findings were that Interviewer received the highest mean rating, but that different factors influenced the ratings substantially, indicating that the preference of individual participants needs to be anticipated in order to improve learner satisfaction with the practice. We conclude with a list of recommendations for robot-hosted conversation practice in a second language.

    • A new proof of the equivalence of the Taut String Algorithm and the one-dimensional Rudin–Osher–Fatemi model is presented. Based on duality and the projection theorem in Hilbert space, the proof is strictly elementary. Existence and uniqueness of solutions (in the continuous case) to both denoising models follow as by-products. The standard convergence properties of the denoised signal, as the regularizing parameter tends to zero, are recalled and efficient proofs provided. Moreover, a new and fundamental estimate on the denoised signal is derived. It implies, among other things, the strong convergence (in the space of functions of bounded variation) of the denoised signal to the in-signal as the regularization parameter vanishes.

    • The nuclear modification factor, R-AA, of the prompt charmed mesons D-0, D+ and D*+, and their antiparticles, was measured with the ALICE detector in Pb-Pb collisions at a centre-of-mass energy root s(NN) = 2 : 76 TeV in two transverse momentum intervals, 5 < p(T) < 8 GeV/c and 8 < p(T) < 16 GeV/c, and in six collision centrality classes. The R-AA shows a maximum suppression of a factor of 5{6 in the 10% most central collisions. The suppression and its centrality dependence are compatible within uncertainties with those of charged pions. A comparison with the R-AA of non-prompt J/psi from B meson decays, measured by the CMS Collaboration, hints at a larger suppression of D mesons in the most central collisions.

    • Measurements of charged jet production as a function of centrality are presented for p–Pb collisions recorded at √sNN = 5.02 TeV with the ALICE detector. Centrality classes are determined via the energy deposit in neutron calorimeters at zero degree, close to the beam direction, to minimise dynamical biases of the selection. The corresponding number of participants or binary nucleon–nucleon collisions is determined based on the particle production in the Pb-going rapidity region. Jets have been reconstructed in the central rapidity region from charged particles with the anti-kT algorithm for resolution parameters R= 0.2 and R= 0.4 in the transverse momentum range 20 to 120 GeV/c. The reconstructed jet momentum and yields have been corrected for detector effects and underlying-event background. In the five centrality bins considered, the charged jet production in p–Pb collisions is consistent with the production expected from binary scaling from pp collisions. The ratio of jet yields reconstructed with the two different resolution parameters is also independent of the centrality selection, demonstrating the absence of major modifications of the radial jet structure in the reported centrality classes. © 2016, CERN for the benefit of the ALICE collaboration.

    • The production of charged pions, kaons and (anti)protons has been measured at mid-rapidity (−0.510 GeV/c), the particle ratios are consistent with those reported for pp and Pb–Pb collisions at the LHC energies. At intermediate pT the (anti)proton RpPb shows a Cronin-like enhancement, while pions and kaons show little or no nuclear modification. At high pT the charged pion, kaon and (anti)proton RpPb are consistent with unity within statistical and systematic uncertainties. © 2016 The Author

  • Higher harmonic flow coefficients of identified hadrons in Pb-Pb collisions at √sNN=2.76 TeV

    • The elliptic, triangular, quadrangular and pentagonal anisotropic flow coefficients for π±, K± and p + p ¯ in Pb-Pb collisions at sNN=2.76 TeV were measured with the ALICE detector at the Large Hadron Collider. The results were obtained with the Scalar Product method, correlating the identified hadrons with reference particles from a different pseudorapidity region. Effects not related to the common event symmetry planes (non-flow) were estimated using correlations in pp collisions and were subtracted from the measurement. The obtained flow coefficients exhibit a clear mass ordering for transverse momentum (pT) values below ≈ 3 GeV/c. In the intermediate pT region (3 < pT< 6 GeV/c), particles group at an approximate level according to the number of constituent quarks, suggesting that coalescence might be the relevant particle production mechanism in this region. The results for pT< 3 GeV/c are described fairly well by a hydrodynamical model (iEBE-VISHNU) that uses initial conditions generated by A Multi-Phase Transport model (AMPT) and describes the expansion of the fireball using a value of 0.08 for the ratio of shear viscosity to entropy density (η/s), coupled to a hadronic cascade model (UrQMD). Finally, expectations from AMPT alone fail to quantitatively describe the measurements for all harmonics throughout the measured transverse momentum region. However, the comparison to the AMPT model highlights the importance of the late hadronic rescattering stage to the development of the observed mass ordering at low values of pT and of coalescence as a particle production mechanism for the particle type grouping at intermediate values of pT for all harmonics.[Figure not available: see fulltext.] © 2016, The Author(s).

    • The DO experiment enjoyed a very successful data-collection run at the Fermilab Tevatron collider between 1992 and 1996. Since then, the detector has been upgraded to take advantage of improvements to the Tevatron and to enhance its physics capabilities. We describe the new elements of the detector, including the silicon microstrip tracker, central fiber tracker, solenoidal magnet, preshower detectors, forward muon detector, and forward proton detector. The uranium/liquid -argon calorimeters and central muon detector, remaining from Run 1, are discussed briefly. We also present the associated electronics, triggering, and data acquisition systems, along with the design and implementation of software specific to DO.

    • Pedestal modelling is crucial to predict the performance of future fusion devices. Current modelling efforts suffer either from a lack of kinetic physics, or an excess of computational complexity. To ameliorate these problems, we take a first-principles multiscale approach to the pedestal. We will present three separate sets of equations, covering the dynamics of edge localised modes (ELMs), the inter-ELM pedestal and pedestal turbulence, respectively. Precisely how these equations should be coupled to each other is covered in detail. This framework is completely self-consistent; it is derived from first principles by means of an asymptotic expansion of the fundamental Vlasov-Landau-Maxwell system in appropriate small parameters. The derivation exploits the narrowness of the pedestal region, the smallness of the thermal gyroradius and the low plasma beta (the ratio of thermal to magnetic pressures) typical of current pedestal operation to achieve its simplifications. The relationship between this framework and gyrokinetics is analysed, and possibilities to directly match our systems of equations onto multiscale gyrokinetics are explored. A detailed comparison between our model and other models in the literature is performed. Finally, the potential for matching this framework onto an open-field-line region is briefly discussed.

    • Cells that grow together respond heterogeneously to stress even when they are genetically similar. Metabolism, a key determinant of cellular stress tolerance, may be one source of this phenotypic heterogeneity, however, this relationship is largely unclear. We used self-establishing metabolically cooperating (SeMeCo) yeast communities, in which metabolic cooperation can be followed on the basis of genotype, as a model to dissect the role of metabolic cooperation in single-cell heterogeneity. Cells within SeMeCo communities showed to be highly heterogeneous in their stress tolerance, while the survival of each cell under heat or oxidative stress, was strongly determined by its metabolic specialization. This heterogeneity emerged for all metabolite exchange interactions studied (histidine, leucine, uracil, and methionine) as well as oxidant (H2O2, diamide) and heat stress treatments. In contrast, the SeMeCo community collectively showed to be similarly tolerant to stress as wild-type populations. Moreover, stress heterogeneity did not establish as sole consequence of metabolic genotype (auxotrophic background) of the single cell, but was observed only for cells that cooperated according to their metabolic capacity. We therefore conclude that phenotypic heterogeneity and cell to cell differences in stress tolerance are emergent properties when cells cooperate in metabolism.
