    • Veljekset Keskinen bildades 1969 och är idag Finlands näst största varuhus beläget i Tuuri, en ort med 500 invånare i Österbotten i Finland. Företaget var från början en ”bybutik” eller vad vi skulle kalla lanthandel, men har över tid utvecklats till ett varuhus, hotell, livsmedelsbutik, trädgårdsbutik, restauranger, bensinstation och camping som attraherar kunder från hela Finland, men även internationella besökare. I denna rapport presenteras resultatet av finska och svenska forskares tvådagars besök på Veljekset Keskinen. Rapporten består av sex kapitel med olika perspektiv, teorier, metoder och reflektioner från besöket. Kapitel ett handlar om att attrahera shoppingturister genom upplevelser. Veljekset Keskinen är inte enbart ett varuhus som säljer fysiska produkter utan en turistdestination som erbjuder en kombination av olika upplevelser. Kapitel två handlar om bybutiken som gåva för bygden och turistattraktion. Besökarna möter idag både den traditionella bybutiken och det moderna varuhuset. Kapitel tre handlar om hur detaljhandelsföretaget Veljekset Keskinen är fullt av kontrastmarknadsföring i tre dimensioner: tradition och nytt, skala och storlek samt House of brands kontra Branded house. Kapitel fyra handlar om olika aspekter av tid och rytm på Veljekset Keskinen: synkronisering av tid och rum, tidslinjer, varornas tidsindelning, säsonger och framtiden. Kapitel fem handlar om negativa kundbeteenden på Veljekset Keskinen och beskriver hur kunder bär sig illa åt, möjliga orsaker till varför kunder bär sig illa åt och hur personalen hanterar situationer där kunder bär sig illa åt. Kapitel sex handlar om att attrahera shoppingturister med hjälp av sociala medier och belyser hur Veljekset Keskinen använder webb och sociala medier för att bygga sitt varumärke.

  • Quality of life and cost-effectiveness of a 3-year trial of lifestyle intervention in primary health care

    • Background: Lifestyle interventions reduce cardiovascular risk and diabetes but reports on long term effects on quality of life (QOL) and health care utilization are rare. The aim was to investigate the impact of a primary health care based lifestyle intervention program on QOL and cost-effectiveness over 3 years.Methods: 151 men and women, age 18-65 yr, at moderate-to-high risk for cardiovascular disease, were randomly assigned to either lifestyle intervention with standard care or standard care alone. Intervention consisted of supervised exercise sessions and diet counseling for 3 months, followed by regular group meetings during 3years. Change in QOL was measured with EuroQol (EQ-5D, EQ VAS), the 36-item Short Form Health Survey (SF-36), and the SF-6D.  The health economic evaluation was performed from a societal view and a treatment perspective. In a cost-utility analysis the costs, gained quality-adjusted life years (QALY) and savings in health care were considered. Cost-effectiveness was also described using the Net Monetary Benefit Method.Results: Significant differences between groups over the 3-yr period were shown in EQ VAS, SF-6D and SF-36 physical component summary but not in EQ-5D or SF-36 mental component summary. There was a net saving of 47 USD per participant. Costs per gained QALY, savings not counted, were 1,668 – 4,813 USD. Probabilities of cost-effectiveness were 89 – 100 %, when 50 000 USD was used as stakeholder’s threshold of willingness to pay for a gained QALY.Conclusion: Lifestyle intervention in primary care improves QOL and is highly cost-effective in relation to standard care.

  • Load applied on bone-anchored transfemoral prosthesis: Characterization of a prosthesis-A pilot study

    • The objectives of this study were to (1) record the inner-prosthesis loading during activities of daily living (ADLs), (2) present a set of variables comparing loading data, and (3) provide an example of characterization of two prostheses. The load was measured at 200 Hz using a multi-axial transducer mounted between the residuum and the knee of an individual with unilateral transfemoral amputation fitted with a bone-anchored prosthesis. The load was measured while using two different prosthetic knees, mechanical (PRO1) and microprocessor-controlled (PRO2), during six ADLs. The characterization of the prostheses was achieved using a set of variables split into four categories, including temporal characteristics, maximum loading, loading slopes, and impulse. Approximately 360 gait cycles were analyzed for each prosthesis. PRO I showed a cadence improved by 19% and 7%, a maximum force on the long axis reduced by 11% and 19%, and an impulse reduced by 32% and 15% during descent of incline and stairs compared with PRO2, respectively. This work confirmed that the proposed apparatus and characterization can reveal how changes of prosthetic components are translated into inner-prosthetic loading.

    • Bakgrunden till denna uppsats är en diskussion mellan oss författare vid ett litteraturseminarium vid Högskolan i Borås (HB). Diskussionen handlade om vår pedagogiska verksamhet och att ta hänsyn till motstridiga krav. Utbildning ska vara flexibel men samtidigt standardiserad, utbildning ska förbereda studenterna för specifika yrken men samtidigt ”lära studenterna för livet”. Vi har observerat att liknande resonemang kommer till uttryck också i andra sammanhang. Ett exempel är propositionerna ”Fokus på kunskap i den högre utbildningen”1 och ”En akademi i tiden - ökad frihet för universitet och högskolor”2. Ett annat är den seminarieserie – Från HB till Humboldt: Perspektiv på universitetsidén under 200 år – som genomförs nu i vår vid HB. Vi kan konstatera att de motsägelsefulla krav vi upplever i vår roll som pedagoger hänger samman med en mer övergripande utveckling inom den högre utbildningen. De ideal som typiskt förknippats med högre utbildning har på senare tid kommit att ställas mot mer politiskt orienterade modeller. Vad betyder denna utveckling för vår praktik? Vi tar utgångspunkt i centrala utvecklingstendenser inom den högre utbildningen i Europa. Detta för att visa på förekomsten av alternativa tolkningar och anpassningar till ”Bologna” som utgör den rådande styrmodellen för högre utbildning i Europa. Vi förflyttar oss därefter till HB och dess utbildning av civilekonomer. Genom exempel visar vi hur den svenska tolkningen av den högre utbildningens styrmodell påverkar vårt pedagogiska utvecklingsarbete. Syftet är att problematisera och ifrågasätta den ”svenska tolkningen” genom att lyfta fram dess för praktiken motsägelsefulla konsekvenser.

  • Cytokine mRNA expression in bronchoalveolar lavage cells during Dictyocaulus viviparus infection in calves

    • The purpose of this study was to monitor local cytokine responses to Dictyocaulus viviparus in calves during primary infection and re-infection. Bronchoalveolar lavage fluid (BALF) was collected weekly from experimentally infected calves and interleukin-2 (IL-2), IL-4, IL-5, IL-10 and IFN- mRNA expression was quantified in BALF cells. The major finding was a prominent transient increase in IL-4 mRNA expression, compared with that of uninfected calves, observed in BALF cells collected 2-3weeks post-primary D.viviparus infection. At 2weeks post-infection, macroscopic worms were also first observed in BALF. Calves re-infected after 10weeks were partially immune which was evident at slaughter 5weeks post-infection as a lower worm burden than in previously naive calves infected at the same time. IL-4 mRNA expression in BALF cells 2weeks post-re-infection was increased compared with that of uninfected animals but not as high as that of primarily infected calves. BALF cell expression of the other cytokines tested for was not as clearly effected by the D.viviparus infection. It seems likely that the strong IL-4 response observed during primary infection reflects an innate response to the worms that may initiate an ensuing Th2 response, which confers protective immunity.

  • Comparison of prosthetic costs and service between osseointegrated and conventional suspended transfemoral prostheses.

    • Background: Nowadays, a transfemoral amputation prosthesis can be fitted to the skeleton using an osseointegrated implant, that is, without a socket. Treated patients have reported improvements in quality of life.Objectives: To investigate differences in prosthetic costs and service of osseointegrated prostheses compared to socket-suspended prostheses.Study Design: Retrospective cost analysis and survey.Methods: Costs and noted visits during in mean ~10 years were taken from one prosthetic workshop and included 50 patients with unilateral transfemoral amputation (36 socket-suspended prostheses, 20 osseointegrated prostheses, 6 patients used both kinds of prostheses). A survey comprised 71 patients (69% males; mean age = 52.3 year; cause: 66% trauma, 23% tumour, 11% other).Results: Statistically significant fewer workshop visits were shown with osseointegrated prostheses compared to socket-suspended prostheses (cost analysis: 3.1 vs. 7.2 visits/year, survey: 3.4 vs. 9.2 visits/year). The mean total annual cost of new prostheses, services, repairs and adjustments was 14% lower for osseointegrated prostheses than socket-suspended prostheses (€3149 and €3672 respectively, p = 0.632). A subgroup analysis of recent produced prostheses revealed cost of material to account for 92.5% for osseointegrated prostheses and 70% for socket-suspended prostheses.Conclusion: Despite significantly fewer visits for prosthetic service the annual mean costs for osseointegrated prostheses were comparable with socket-suspended prostheses. This study suggests it is due to more advanced prosthetic components being used with osseointegrated prostheses.

  • Vibrotactile evaluation: Osseointegrated versus socket-suspended transfemoral prostheses

    • This study investigated detection thresholds of vibrometric stimuli in patients with transfemoral amputation supplied with osseointegrated (OI) and socket-suspended prostheses. It included 17 patients tested preoperatively with socket-suspended prostheses and after 2 yr with OI prostheses and a control group (n = 17) using socket-suspended prostheses, evaluated once. Assessments on the prosthetic and intact feet were conducted at six frequencies (8, 16, 32, 64, 125, and 250 Hz). Furthermore, measurements were conducted to investigate how vibrometric signals are transmitted through a test prosthesis. The results showed that the OI group had improved ability to detect vibrations through the prosthesis at 125 Hz (p = 0.01) at follow-up compared with the preoperative measurement. Compared with the control group, the 01 group at follow-up had better ability to detect high frequency vibrations through the prosthesis (125 Hz, p = 0.02; 250 Hz, p = 0.03). The vibrometric signal transmitted through the test prosthesis was reduced at 8, 125, and 250 Hz but was amplified at 16, 32, and 64 Hz. Differences between the OI and the control groups were found in the highest frequencies in which the test prosthesis showed reduction of the vibrometric signal. The study provides insight into the mechanisms of vibration transmission between the exterior and bone-anchored as well as socket-suspended amputation prostheses.

  • Anal carcinoma - Survival and recurrence in a large cohort of patients treated according to Nordic guidelines

    • Objective:To evaluate treatment outcome in a large population-based cohort of patients with anal cancer treated according to Nordic guidelines.Material:Clinical data were collected on 1266 patients with anal squamous cell carcinoma diagnosed from 2000 to 2007 in Sweden, Norway and Denmark. 886 of the patients received radiotherapy 5464 Gy with or without chemotherapy (5-fluorouracil plus cisplatin or mitomycin). according to different protocols, stratified by tumor stage.Results:High age, male gender, large primary tumor, lymph node metastases, distant metastases, poor performance status, and non-inclusion into a protocol were all independent factors associated with worse outcome. Among patients treated according to any of the protocols, the 3-year recurrence-free survival ranged from 63% to 76%, with locoregional recurrences in 17% and distant metastases in 11% of patients. The highest rate of inguinal recurrence (11%) was seen in patients with small primary tumors, treated without inguinal irradiation.Conclusions:Good treatment efficacy was obtained with Nordic, widely implemented, guidelines for treatment of anal cancer. Inguinal prophylactic irradiation should be recommended also for small primary tumors. (C) 2014 Elsevier Ireland Ltd. All rights reserved.

    • This study aims to add to knowledge on the relation between ageing and masculinity in society by looking at how older unmarried and childless men in a small Swedish rural community articulate their masculinity in relation to being old. The study is based on interviews with eleven men. An intersectional perspective is used to analyse how age and gender interplay in the self-presentations as well as how their identity is articulated in relation to the rural context. The interviewed men articulate their identity through the use of a logic we call ‘what I have done is who I am’. This reaffirms the rural aspect of their masculine identity as well as the work-centred values of midlife as a reference point in the identity construction in old age when their bodies to a diminishing degree can live up to the physically defined masculinity of the rural context. In this situation, the local rural community is important for the way the men perform both age and masculinity in their daily lives. The men’s place integration can have a mitigating effect as it makes it possible to have their physically defined rural masculinity accepted as the ‘truth’ of who they are. To be known, to have their history known, we argue is central for the resignification of the old, devalued, body as a masculine body. Their continued place integration is a resource in that it can sustain their self-presentation as men defined by their stoic work character and not by their age.

    • Canine herpesvirus (CHV) is a widespread infection among dogs that typically get latently infected after exposure and can reactivate the infection after stress. The aim of the present study was to study the effects of latent CHV infection during pregnancy on pregnancy outcome, and to study if there are signs of genital viral reactivation during pregnancy or during non-pregnant luteal phase. Twelve mated bitches and eight control bitches were followed and sampled regularly during pregnancy or non-pregnant luteal phase. Blood samples were taken for antibody analysis and vaginal swabs for real-time PCR analysis. Three of the pregnant bitches were vaccinated against CHV during pregnancy. All bitches had antibodies to CHV. Two pregnant bitches that were not vaccinated had a twofold or larger increase in CHV titre, with no negative effects detected on pregnancy. Higher titres were not associated with smaller litters or with vaccination. There was no consistent variation in antibody titres due to pregnancy or non-pregnant luteal phase. Vaginal excretion of CHV was not detected from any of the bitches.

  • T Cells in Tumors and Blood Predict Outcome in Follicular Lymphoma Treated with Rituximab

    • PURPOSE: T cells influence outcome in follicular lymphoma, but their contributions seem to be modified by therapy. Their impact in patients receiving rituximab without chemotherapy is unknown. EXPERIMENTAL DESIGN: Using flow cytometry, we evaluated the T cells in tumors and/or blood in a total of 250 follicular lymphoma patients included in two Nordic Lymphoma Group randomized trials that compared single rituximab with IFN-α2a-rituximab combinations. RESULTS: In univariate analysis, higher levels of CD3(+), CD4(+), and CD8(+) T cells in both tumors and blood correlated with superior treatment responses, and in multivariate analysis, tumor-CD3(+) (P = 0.011) and blood-CD4(+) (P = 0.029) cells were independent. CD4(+) cells were favorable regardless of treatment arm, but CD8(+) cells were favorable only in patients treated with single rituximab, because IFN-α2a improved responses especially in patients with low CD8(+) cell levels. Higher levels of blood-CD3(+) (P = 0.003) and blood-CD4(+) (P = 0.046) cells predicted longer overall survival, and higher levels of blood-CD8(+) cells longer times to next treatment (P = 0.046). CONCLUSIONS: We conclude that therapeutic effects of rituximab are augmented by tumor-associated T cells for rapid responses and by systemic T cells for sustained responses. CD4(+) and CD8(+) cells are both favorable in patients treated with rituximab. IFN-α2a abrogates the negative impact of few CD8(+) cells.
