  • Auditory brainstem implant pitch discrimination and auditory outcome

    • We present a pitch discrimination test performed by five experienced adult auditory brainstem implant (ABI) users with neurofibromatosis type 2 (NF2). The ability to discriminate frequency/pitch from different channels on the implant may be an important factor in improving speech performance. The pitch discrimination ability was evaluated by using a triangle test compared to adjacent contacts and the speech perception was measured by the Swedish three-digit test. The test was easy to perform, and all patients were able to answer reliably, even though it cannot be ruled out that patients used attributes other than pitch to differentiate between sounds. Due to the limited number of patients and small variation in results, no conclusive correlations could be made regarding pitch discrimination and auditory outcome. There was a tendency for poorer ability to discriminate pitch (discrimination of tonotopically adjacent electrodes) at testing to result in poorer speech results.

  • Do Patients Aged 85 Years and above Benefit from Their Cochlear Implants?

    • The present study aims to investigate the usage and benefits of cochlear implants (CIs) in elderly patients aged >= 85 years, including their device-handling issues, follow-ups, and the influence on their well-being. The patients answered one questionnaire regarding quality of life, EQ5D-3L, and one questionnaire, obtained from the Swedish CI quality register, regarding usage, handling, satisfaction, remaining difficulties, etc. The medical records were searched for the implantation date, implant model, speech processor model, monosyllabic (MS) word scores, infections over the implant, and compliance regarding scheduled visits to the clinic. The results show that most elderly patients are satisfied full-time users of their implants. Even though most patients had no problems handling their CI, handling issues must be considered. Recurring guidance and training on device operation are needed. We suggest that follow-up visits are essentially needed for this group of patients on a regular basis. CI surgery is considered a safe treatment, even for the elderly. Upgrads to new external equipment (e.g., sound processors) should not be excluded because of their age. The results suggested that the CI positively affected their well-being. This study was approved by the Swedish Ethical Review Authority (5/10-2021, Dnr: 2021-04970).

  • Pneumolabyrinth following cochlear implantation resolved after shunt adjustment

    • We present a case of a male with a history of repeated surgeries for a cerebellar astrocytoma, leading to profound deafness and facial paresis on the left side. A ventriculoperitoneal (VP) shunt was inserted to manage hydrocephalus. At the age of 46 the hearing suddenly disappeared on the right side, where he received a cochlear implant (CI). At implant activation, impedances showed atypical high values. One month later impedance levels had further increased and the patient had no benefit from the CI. A computed tomography (CT) scan suggested air in the cochlea (pneumolabyrinth). The shunt was adjusted to elevate the cerebrospinal fluid (CSF) pressure and the pressure in the perilymph to prevent air from entering the cochlea via the round window. One year after activation the electrode impedances were normal and the hearing outcome from the implant was successful.
