  • Analysis of Heritability and Shared Heritability Based on Genome-Wide Association Studies for 13 Cancer Types

    • Background: Studies of related individuals have consistently demonstrated notable familial aggregation of cancer. We aim to estimate the heritability and genetic correlation attributable to the additive effects of common single-nucleotide polymorphisms (SNPs) for cancer at 13 anatomical sites. Methods: Between 2007 and 2014, the US National Cancer Institute has generated data from genome-wide association studies (GWAS) for 49 492 cancer case patients and 34 131 control patients. We apply novel mixed model methodology (GCTA) to this GWAS data to estimate the heritability of individual cancers, as well as the proportion of heritability attributable to cigarette smoking in smoking-related cancers, and the genetic correlation between pairs of cancers. Results: GWAS heritability was statistically significant at nearly all sites, with the estimates of array-based heritability, h(l)(2), on the liability threshold (LT) scale ranging from 0.05 to 0.38. Estimating the combined heritability of multiple smoking characteristics, we calculate that at least 24% (95% confidence interval [CI] = 14% to 37%) and 7% (95% CI = 4% to 11%) of the heritability for lung and bladder cancer, respectively, can be attributed to genetic determinants of smoking. Most pairs of cancers studied did not show evidence of strong genetic correlation. We found only four pairs of cancers with marginally statistically significant correlations, specifically kidney and testes (rho = 0.73, SE = 0.28), diffuse large B-cell lymphoma (DLBCL) and pediatric osteosarcoma (rho = 0.53, SE = 0.21), DLBCL and chronic lymphocytic leukemia (CLL) (rho = 0.51, SE = 0.18), and bladder and lung (rho = 0.35, SE = 0.14). Correlation analysis also indicates that the genetic architecture of lung cancer differs between a smoking population of European ancestry and a nonsmoking Asian population, allowing for the possibility that the genetic etiology for the same disease can vary by population and environmental exposures. Conclusion: Our results provide important insights into the genetic architecture of cancers and suggest new avenues for investigation.

  • Early symptoms and sensations as predictors of lung cancer : a machine learning multivariate model

    • The aim of this study was to identify a combination of early predictive symptoms/sensations attributable to primary lung cancer (LC). An interactive e-questionnaire comprised of pre-diagnostic descriptors of first symptoms/sensations was administered to patients referred for suspected LC. Respondents were included in the present analysis only if they later received a primary LC diagnosis or had no cancer; and inclusion of each descriptor required >= 4 observations. Fully-completed data from 506/670 individuals later diagnosed with primary LC (n = 311) or no cancer (n = 195) were modelled with orthogonal projections to latent structures (OPLS). After analysing 145/285 descriptors, meeting inclusion criteria, through randomised seven-fold cross-validation (six-fold training set: n = 433; test set: n = 73), 63 provided best LC prediction. The most-significant LC-positive descriptors included a cough that varied over the day, back pain/aches/discomfort, early satiety, appetite loss, and having less strength. Upon combining the descriptors with the background variables current smoking, a cold/flu or pneumonia within the past two years, female sex, older age, a history of COPD (positive LC-association); antibiotics within the past two years, and a history of pneumonia (negative LC-association); the resulting 70-variable model had accurate cross-validated test set performance: area under the ROC curve = 0.767 (descriptors only: 0.736/background predictors only: 0.652), sensitivity = 84.8% (73.9/76.1%, respectively), specificity = 55.6% (66.7/51.9%, respectively). In conclusion, accurate prediction of LC was found through 63 early symptoms/sensations and seven background factors. Further research and precision in this model may lead to a tool for referral and LC diagnostic decision-making.

  • Characterization of Large Structural Genetic Mosaicism in Human Autosomes

    • Analyses of genome-wide association study (GWAS) data have revealed that detectable genetic mosaicism involving large (>2 Mb) structural autosomal alterations occurs in a fraction of individuals. We present results for a set of 24,849 genotyped individuals (total GWAS set II [TGSII]) in whom 341 large autosomal abnormalities were observed in 168 (0.68%) individuals. Merging data from the new TGSII set with data from two prior reports (the Gene-Environment Association Studies and the total GWAS set I) generated a large dataset of 127,179 individuals; we then conducted a meta-analysis to investigate the patterns of detectable autosomal mosaicism (n = 1,315 events in 925 [0.73%] individuals). Restricting to events >2 Mb in size, we observed an increase in event frequency as event size decreased. The combined results underscore that the rate of detectable mosaicism increases with age (p value = 5.5 x 3 10(-31)) and is higher in men (p value = 0.002) but lower in participants of African ancestry (p value = 0.003). In a subset of 47 individuals from whom serial samples were collected up to 6 years apart, complex changes were noted over time and showed an overall increase in the proportion of mosaic cells as age increased. Our large combined sample allowed for a unique ability to characterize detectable genetic mosaicism involving large structural events and strengthens the emerging evidence of non-random erosion of the genome in the aging population.

    • To investigate large structural clonal mosaicism of chromosome X, we analysed the SNP microarray intensity data of 38,303 women from cancer genome-wide association studies (20,878 cases and 17,425 controls) and detected 124 mosaic X events >2 Mb in 97 (0.25%) women. Here we show rates for X-chromosome mosaicism are four times higher than mean autosomal rates; X mosaic events more often include the entire chromosome and participants with X events more likely harbour autosomal mosaic events. X mosaicism frequency increases with age (0.11% in 50-year olds; 0.45% in 75-year olds), as reported for Y and autosomes. Methylation array analyses of 33 women with X mosaicism indicate events preferentially involve the inactive X chromosome. Our results provide further evidence that the sex chromosomes undergo mosaic events more frequently than autosomes, which could have implications for understanding the underlying mechanisms of mosaic events and their possible contribution to risk for chronic diseases.

  • Expansion of medical vocabularies using distributional semantics on Japanese patient blogs

    • Background: Research on medical vocabulary expansion from large corpora has primarily been conducted using text written in English or similar languages, due to a limited availability of large biomedical corpora in most languages. Medical vocabularies are, however, essential also for text mining from corpora written in other languages than English and belonging to a variety of medical genres. The aim of this study was therefore to evaluate medical vocabulary expansion using a corpus very different from those previously used, in terms of grammar and orthographics, as well as in terms of text genre. This was carried out by applying a method based on distributional semantics to the task of extracting medical vocabulary terms from a large corpus of Japanese patient blogs. Methods: Distributional properties of terms were modelled with random indexing, followed by agglomerative hierarchical clustering of 3x100 seed terms from existing vocabularies, belonging to three semantic categories: Medical Finding, Pharmaceutical Drug and Body Part. By automatically extracting unknown terms close to the centroids of the created clusters, candidates for new terms to include in the vocabulary were suggested. The method was evaluated for its ability to retrieve the remaining n terms in existing medical vocabularies. Results: Removing case particles and using a context window size of 1 + 1 was a successful strategy for Medical Finding and Pharmaceutical Drug, while retaining case particles and using a window size of 8 + 8 was better for Body Part. For a 10n long candidate list, the use of different cluster sizes affected the result for Pharmaceutical Drug, while the effect was only marginal for the other two categories. For a list of top n candidates for Body Part, however, clusters with a size of up to two terms were slightly more useful than larger clusters. For Pharmaceutical Drug, the best settings resulted in a recall of 25 % for a candidate list of top n terms and a recall of 68 % for top 10n. For a candidate list of top 10n candidates, the second best results were obtained for Medical Finding: a recall of 58 %, compared to 46 % for Body Part. Only taking the top n candidates into account, however, resulted in a recall of 23 % for Body Part, compared to 16 % for Medical Finding. Conclusions: Different settings for corpus pre-processing, window sizes and cluster sizes were suitable for different semantic categories and for different lengths of candidate lists, showing the need to adapt parameters, not only to the language and text genre used, but also to the semantic category for which the vocabulary is to be expanded. The results show, however, that the investigated choices for pre-processing and parameter settings were successful, and that a Japanese blog corpus, which in many ways differs from those used in previous studies, can be a useful resource for medical vocabulary expansion.

    • Skolnära kan beskrivas som ett relativt nytt begrepp inom utbildning och forskning med skolanknytning. Det förekommer ofta i sammanhang där det tycks finnas en ambition att knyta skola och forskning närmare varandra. Det kan till exempel handla om olika forskarskolor, skolforskning och skolutvecklingsprojekt. Dock finns ingen enhetlig definition av begreppet utan det används parallellt med och i vissa fall synonymt med begreppen praktiknära och praxisnära. Syftet med denna artikel är att bidra till en problematisering av begreppet skolnära med fokus på såväl innebörder som användningsområden. Utifrån studier av olika typer av texter belyses hur begreppet skolnära används inom skolforskning och i andra utbildningsvetenskapliga sammanhang idag. Begreppet skolnära ser ut att uppkomma i samband med framväxten av forskarutbildningsämnet pedagogiskt arbete men återfinns idag även inom andra forskarutbildningsämnen. Till exempel kan en ”skolnära avhandling” vara skriven inom pedagogik eller något ämnesdidaktiskt ämne. Vad det innebär att forskningen är skolnära tycks ha skiftande innebörd och avgränsning i olika sammanhang. Det kan till exempel handla om huruvida forskningen bedrivs av eller för lärare. Begreppet används också inom skolvärlden i ett vidare perspektiv.

    • Approaches to determining the factuality of diagnoses and findings in clinical text tend to rely on dictionaries of marker words for uncertainty and negation. Here, a method for semi-automatically expanding a dictionary of marker words using distributional semantics is presented and evaluated. It is shown that ranking candidates for inclusion according to their proximity to cluster centroids of semantically similar seed words is more successful than ranking them according to proximity to each individual seed word.

    • Ekonomibyggnader har alltid behövts för de huvudbyggnader som finns inom våra bevarade kulturmiljöer. Några nyttobyggnader uppskattas och används fortfarande, andra betraktas som problematiska överloppsbyggnader, många rivs. Alltför få har dokumenterats eller fått sin historia klarlagd vilket undanhållit viktig kunskap om samhällets framväxt. Vi vill synliggöra och betona vikten av att bevara och utveckla hela bebyggelsemiljöer, ofta med ett antal hus utöver huvudbyggnaden och tillhörande yttre miljö i staden eller på landet. Denna rapport visar kursdeltagarnas projektarbeten om nyttobyggnader. De har dokumenterat med traditionella och nya arbetsmetoder, inventerat och intervjuat, läst och besökt arkiv, värderat, analyserat och därefter föreslagit hur man ska ta hand om och utveckla nyttobyggnaderna i sina kulturmiljöer.

    • Paediatric non-alcoholic fatty liver disease has increased in parallel with childhood obesity. Dietary habits, particularly products rich in sugars, may influence both hepatic fat and insulin resistance (homeostatic model assessment for insulin resistance (HOMA-IR)). The aim of the study was to examine the association of the consumption of foods and food components, dairy desserts and substitutes (DDS), sugar-sweetened beverages (SSB), as well as total and added sugars, with hepatic fat and HOMA-IR. Dietary intake (two non-consecutive 24 h-recalls), hepatic fat (MRI) and HOMA-IR were assessed in 110 overweight/obese children (106 (sd 11) years old). Linear regression analyses were used to examine the association of dietary intake with hepatic fat and HOMA-IR adjusted for potential confounders (sex, age, energy intake, maternal educational level, total and abdominal adiposity and sugar intake). The results showed that there was a negative association between cereal intake and hepatic fat (beta=-0197, Pamp;lt;005). In contrast, both SSB consumption (beta=0217; P=0028) and sugar in SSB (beta=0210, P=0035), but not DDS or sugar in DDS or other dietary components, were positively associated with hepatic fat regardless of potential confounders including total sugar intake. In conclusion, cereal intake might decrease hepatic fat, whereas SSB consumption and its sugar content may increase the likelihood of having hepatic steatosis. Although these observations need to be confirmed using experimental evidence, these results suggest that healthy lifestyle intervention programs are needed to improve dietary habits as well as to increase the awareness of the detrimental effects of SSB consumption early in life.

    • In this paper we discuss the theoretical rooting of present research on coopetition and point to the need for an integration of theories on competition dynamics, and cooperative interactions in social networks. We argue that the future growth of the coopetitive research field hinges on creatively combining existing theoretical approaches with novel research methods and contexts. In particular, we suggest that incorporating theories on the micro foundations of strategic action can substantially enhance the field. The aim of this paper is both to raise questions regarding the theory and practice of coopetition research and to give examples of new approaches and trends that may contribute to the advancement of the field in the future. We consider our research practice and explore avenues for further research starting from what, where and how we study coopetition, to when and who we study. In general, we call for a stronger focus on the centrality of multiple stakeholders in forming, executing, and developing coopetition, and on research methods that can investigate in depth the multitude of actors, interests, and interactions using a multi-level analysis, including the micro foundations of coopetition.

  • Ekonomi och ekosystemtjänster i gräsbaserad mjölk- och nötköttsproduktion

    • En stor del av den svenska jordbruksmarken utgörs av gräsmarker som används till foderproduktion åt nötkreatur. Denna markanvändning är mycket viktig för leveransen av en rad betydelsefulla ekosystemtjänster till samhället och för bevarandet av den biologiska mång-falden. Pågående igenväxningen av gräsmarker pga. olönsamhet, i synnerhet naturbetes-marker, hotar dessa tjänster och är en orsak till att miljömålet ”Ett varierat odlingslandskap” inte beräknas nås till 2020. Naturvårdsverkets bedömning är att det behövs starkare incitament för djurägare att hålla djur på naturbetesmarker. Även hos allmänheten finns en betalnings-vilja för att bevara naturbetesmarkerna. Projektet har, i dialog med lantbrukare, analyserat ekonomi och ekosystemtjänster i gräsbase-rade produktionssystem för nötkreatur, med bete i fokus. Studien inkluderar några möjliga framtida system för mjölkproduktion där intensiteten i avkastning per ko är lägre än dagens och istället inriktad på en kombinerad produktion av mjölk och kött. Ekonomisk lönsamhet, utan tillägg av några jordbruksstöd, har beräknats för fem olika eko-logiska typgårdar, fyra med mjölkproduktion och en med dikoproduktion. Typgården ”inten-siv” motsvarar dagens ekologiska produktion framräknad till år 2030 (10 ton mjölk/ko och år), medan typgårdarna ”gräs” producerar på 6, 7 respektive 8 ton mjölk per ko och år med stor andel grovfoder och bete i foderstaterna. Alla ungdjur (utöver rekrytering) föds upp till slakt på gården och gårdens areal motsvaras av djurens behov av grovfoder och spannmål. Produktionskalkyler för olika storlek på naturbetesmarker togs fram i projektet. Mjölkproduktionen visade ett positivt ekonomiskt resultat (utan tillägg av några jordbruks-stöd) för alla typgårdar, medan all köttproduktion gick med förlust. Det sammanlagda netto-resultatet för mjölk och kött var positivt för de två typgårdarna ”intensiv” och ”gräs 8 ton” medan det var negativt för övriga; särskilt för dikogården var resultatet kraftigt negativt. Den ekonomiska lönsamheten påverkades starkt av skiftesstorlek pga. stora skillnader i bruknings-kostnader. För att nå lönsamhet krävs det högre ersättningar för naturbetesmarker och för brukandet av små åker- och betesskiften än vad dagens jordbruksstöd ger. Typgården ”diko”, där naturbetesmarker utgjorde det mesta av betet, skulle t.ex. behöva en ersättning motsva-rande 6000 kronor per hektar för dessa marker för att få lönsamhet. Analysen av olika stödtypers inverkan visar att ett omkretsbaserat ”kantstöd”, istället för dagens arealbaserade, kan gynna brukning av hotade gräsmarker och de ekosystemtjänster de levererar. För att bevara jordbrukets viktiga ekosystemtjänster menar vi att det behövs en diskussion kring hur de totala stödbeloppen till lantbruket fördelas mellan de båda ytterligheterna intensivt odlad åkermark med stora skiften och naturbetesmarker som ofta utgörs av små skiften med höga skötselkostnader per hektar. Leveransen av reglerande och kulturella eko-systemtjänster skiljer sig radikalt åt mellan dessa två ytterligheter av markanvändning. Brister i tillgängligt kunskaps- och dataunderlag för gräsmarker behöver åtgärdas för att säkerställa att beräkningar av ersättningar fördelas rätt mellan olika markanvändning och gynnar gräs-markernas ekosystemtjänster. Summary A large share of Swedish agricultural land is grasslands used for cattle production. These grasslands deliver a range of important ecosystem services and they are crucial for biodiver-sity conservation. The maintenance of grasslands, especially permanent pastures, are threat-ened by a decrease in the number of grazing cattle and lack of profitability for farmers with small-scale fields which leads to land abandonment and regrowth of forest. This is one reason why the environmental objective "A varied agricultural landscape" is not expected to be reached by 2020. According to the Swedish Environmental Protection Agency there is a need of stronger incentives to farmers to keep grazing animals. There is also a public willingness to pay for conservation of permanent pastureland. The project has, in dialogue with farmers, analyzed the economy and ecosystem services in grass-based cattle production systems, with focus on grazing. The study includes scenarios for future milk production systems where the intensity of yield per cow is lower than today; instead milk production is focused on a combination of both milk and meat. Economic profitability, without any public support through the EU Farm Policy (CAP), was calculated for five modelled organic cattle farms, four with milk production and one with dedicated beef production. The most intensive dairy farm corresponded to current organic production projected at year 2030 (thus yielding 10 ton of milk/cow and year), while the "grass" farms produce 6, 7 and 8 ton of milk per cow and year respectively in 2030. In these grass scenarios, dairy cows´ feed rations had larger proportion of roughage and pasture. All young cattle (excl. replacement heifers) were bred at the farm for meat production and the acreage of the farm was matched by the animals’ need for pasture, roughage and grain. Cost estimates for pastureland were calculated based on data collected in the project. Milk production showed a positive economic result without public support through CAP, regardless of farm type and milk intensity, while all meat production yielded negative result. The total farm net income for milk and meat was positive for the intensive dairy farm and the modelled grass dairy farm producing 8 ton milk/cow and year, while it was negative for the other farm types. The dedicated beef farm showed a strong negative result. Economic profitability was strongly influenced by field size; the sensitivity analysis found this factor highly important for differences in operating costs. In order to reach profitability in grass-based cattle production, pastureland and small-scale fields need to be better compensated than present distribution of farm income support in the CAP. In order to maintain important ecosystem services in agriculture, a broad discussion is needed on how farm income support should be distributed between different uses of land, with inten-sively farmed cropland on large fields on one side and permanent pastureland on small fields (with high operating costs per hectare) on the other. The delivery of regulating and cultural ecosystem services differs significantly between these two very different types of land use. The project has identified several data gaps regarding grasslands, especially pasture, which needs to be addressed to ensure that cost-based payments and payments for ecosystem services for different land use are based on realistic numbers.

  • HEALTH BANK - A Workbench for Data Science Applications in Healthcare

    • The enormous amounts of data that are generated in the healthcare process and stored in electronic health record (EHR) systems are an underutilized resource that, with the use of data science applica- tions, can be exploited to improve healthcare. To foster the development and use of data science applications in healthcare, there is a fundamen- tal need for access to EHR data, which is typically not readily available to researchers and developers. A relatively rare exception is the large EHR database, the Stockholm EPR Corpus, comprising data from more than two million patients, that has been been made available to a lim- ited group of researchers at Stockholm University. Here, we describe a number of data science applications that have been developed using this database, demonstrating the potential reuse of EHR data to support healthcare and public health activities, as well as facilitate medical re- search. However, in order to realize the full potential of this resource, it needs to be made available to a larger community of researchers, as well as to industry actors. To that end, we envision the provision of an in- frastructure around this database called HEALTH BANK – the Swedish Health Record Research Bank. It will function both as a workbench for the development of data science applications and as a data explo- ration tool, allowing epidemiologists, pharmacologists and other medical researchers to generate and evaluate hypotheses. Aggregated data will be fed into a pipeline for open e-access, while non-aggregated data will be provided to researchers within an ethical permission framework. We believe that HEALTH BANK has the potential to promote a growing industry around the development of data science applications that will ultimately increase the efficiency and effectiveness of healthcare.

  • Early life programming of attention capacity in adolescents : The HELENA study

    • The study aims to examine the individual and combined association of early life factors (birth weight, birth length, and any and exclusive breastfeeding) with attention capacity in adolescents. The study included 421 European adolescents (243 girls), aged 12.5-17.5 years, who participated in the Healthy Lifestyle in Europe by Nutrition in Adolescence Study. Body weight and length at birth of adolescents were collected from parental records. The duration of any and exclusive breastfeeding were self-reported. The d2 Test of Attention was administered to assess attention capacity. The main results showed that birth weight, birth length, breastfeeding, and exclusive breastfeeding were related to attention capacity in boys (β ranging from 0.144 to 0.196; all p < .05) after adjustment for age, centre, gestational age, maternal education, family affluence scale, and body mass index. Among boys, differences in attention capacity were found according to tertiles of birth weight and birth length (p < .05), as well as borderline significant differences across groups of any and exclusive breastfeeding (p = 0.055 and p = 0.108, respectively) after adjusting for potential confounders. In addition, boys with 3 early life risk factors (low birth weight, low birth length, and <3 months of breastfeeding) had significantly lower scores in attention capacity compared with boys with 0 risk factors (percentile score - 15.88; p = 0.009). In conclusion, early life factors, both separately and combined, may influence attention capacity in male European adolescents. Importantly, the combination of the 3 early life risk factors, low birth weight, low birth length, and <3 months of breastfeeding, even in normal ranges, may provide the highest reduction in attention capacity.

  • Fitness, cortical thickness and surface area in overweight/obese children: The mediating role of body composition and relationship with intelligence

    • Cortical thickness and surface area are thought to be genetically unrelated and shaped by independent neurobiological events suggesting that they should be considered separately in morphometric analyses. Although the developmental trajectories of cortical thickness and surface area may differ across brain regions and ages, there is no consensus regarding the relationships of physical fitness with cortical thickness and surface area as well as for its subsequent influence on intelligence. Thus, this study examines: (i) the associations of physical fitness components (i.e., cardiorespiratory fitness, speed-agility and muscular fitness) with overall and regional cortical thickness and surface area; (ii) whether body composition indicators (i.e., body mass index, fat-free mass index and fat mass index) mediate these associations; and (iii) the association of physical fitness and cortical thickness with intelligence in overweight/obese children. A total of 101 overweight/obese children aged 8-11 years were recruited in Granada, Spain. The physical fitness components were assessed following the ALPHA health-related fitness test battery. T1-weighted images were acquired with a 3.0 Tesla Siemens Magnetom Tim Trio system. We used FreeSurfer software version 5.3.0 to assess cortical thickness (mm) and surface area (mm(2)). The main results showed that cardiorespiratory fitness and speed-agility were related to overall cortical thickness (beta = 0.321 and beta = 0.302, respectively; both P amp;lt; 0.05), and in turn, cortical thickness was associated with higher intelligence (beta = 0.198, P amp;lt; 0.05). Muscular fitness was not related to overall cortical thickness. None of the three physical fitness components were related to surface area (p amp;gt; 0.05). The associations of cardiorespiratory fitness and speed-agility with overall cortical thickness were mediated by fat mass index (56.86% amp; 62.28%, respectively). In conclusion, cardiorespiratory fitness and speed-agility, but not muscular fitness, are associated with overall cortical thickness, and in turn, thicker brain cortex is associated with higher intelligence in overweight/obese children. Yet, none of the three physical fitness components were related to surface area. Importantly, adiposity may hinder the benefits of cardiorespiratory fitness and speed-agility on cortical thickness. Understanding individual differences in brain morphology may have important implications for educators and policy makers who aim to determine policies and interventions to maximize academic learning and occupational success later in life.

    • Swedish medical language is rich with Latin and Greek terminology which has undergone a Swedification since the 1980s. However, many original expressions are still used by clinical professionals. The goal of this study is to obtain precise quantitative measures of how the foreign terminology is manifested in Swedish clinical text. To this end, we explore the use of Latin and Greek affixes in Swedish medical texts in three genres: clinical text, scientific medical text and online medical information for laypersons. More specifically, we use frequency lists derived from tokenised Swedish medical corpora in the three domains, and extract word pairs belonging to types that display both the original and Swedified spellings. We describe six distinct patterns explaining the variation in the usage of Latin and Greek affixes in clinical text. The results show that to a large extent affixes in clinical text are Swedified and that prefixes are used more conservatively than suffixes.

    • To sustain their proliferation, cancer cells become dependent on one-carbon metabolism to support purine and thymidylate synthesis. Indeed, one of the most highly upregulated enzymes during neoplastic transformation is MTHFD2, a mitochondrial methylenetetrahydrofolate dehydrogenase and cyclohydrolase involved in one-carbon metabolism. Because MTHFD2 is expressed normally only during embryonic development, it offers a disease-selective therapeutic target for eradicating cancer cells while sparing healthy cells. Here we report the synthesis and preclinical characterization of the first inhibitor of human MTHFD2. We also disclose the first crystal structure of MTHFD2 in complex with a substrate-based inhibitor and the enzyme cofactors NAD(+) and inorganic phosphate. Our work provides a rationale for continued development of a structural framework for the generation of potent and selective MTHFD2 inhibitors for cancer treatment.

    • To enable secondary use of healthcare data in a privacy-preserving manner, there is a need for methods capable of automatically identifying protected health information (PHI) in clinical text. To that end, learning predictive models from labeled examples has emerged as a promising alternative to rule-based systems. However, little is known about differences with respect to PHI prevalence in different types of clinical notes and how potential domain differences may affect the performance of predictive models trained on one particular type of note and applied to another. In this study, we analyze the performance of a predictive model trained on an existing PHI corpus of Swedish clinical notes and applied to a variety of clinical notes: written (i) in different clinical specialties, (ii) under different headings, and (iii) by persons in different professions. The results indicate that domain adaption is needed for effective detection of PHI in heterogeneous clinical notes.

    • For the purpose of post-marketing drug safety surveillance, which has traditionally relied on the volun- tary reporting of individual cases of adverse drug events (ADEs), other sources of information are now being explored, including electronic health records (EHRs), which give us access to enormous amounts of longitudinal observations of the treatment of patients and their drug use. Adverse drug events, which can be encoded in EHRs with certain diagnosis codes, are, however, heavily underreported. It is therefore important to develop capabilities to process, by means of computational methods, the more unstructured EHR data in the form of clinical notes, where clinicians may describe and reason around suspected ADEs. In this study, we report on the creation of an annotated corpus of Swedish health records for the purpose of learning to identify information pertaining to ADEs present in clinical notes. To this end, three key tasks are tackled: recognizing relevant named entities (disorders, symptoms, drugs), labeling attributes of the recognized entities (negation, speculation, temporality), and relationships between them (indication, adverse drug event). For each of the three tasks, leveraging models of distributional semantics – i.e., unsupervised methods that exploit co-occurrence information to model, typically in vector space, the meaning of words – and, in particular, combinations of such models, is shown to improve the predictive performance. The ability to make use of such unsupervised methods is critical when faced with large amounts of sparse and high-dimensional data, especially in domains where annotated resources are scarce.

    • Obscuring protected health information (PHI) in the clinical text of health records facilitates the secondary use of healthcare data in a privacy-preserving manner. Although automatic de-identification of clinical text using machine learning holds much promise, little is known about the relative prevalence of PHI in different types of clinical text and whether there is a need for domain adaptation when learning predictive models from one particular domain and applying it to another. In this study, we address these questions by training a predictive model and using it to estimate the prevalence of PHI in clinical text written (1) in different clinical specialties, (2) in different types of notes (i.e., under different headings), and (3) by persons in different professional roles. It is demonstrated that the overall PHI density is 1.57%; however, substantial differences exist across domains.

    • Antenatal depression affects similar to 9-19% of pregnant women and can exert persistent adverse effects on both mother and child. There is a need for a deeper understanding of antenatal depression mechanisms and the development of tools for reliable diagnosis and early identification of women at high risk. As the use of untargeted blood metabolomics in the investigation of psychiatric and neurological diseases has increased substantially, the main objective of this study was to investigate whether untargeted gas chromatography-mass spectrometry (GC-MS) plasma metabolomics in 45 women in late pregnancy, residing in Uppsala, Sweden, could indicate metabolic differences between women with and without depressive symptoms. Furthermore, seasonal differences in the metabolic profiles were explored. When comparing the profiles of cases with controls, independently of season, no differences were observed. However, seasonal differences were observed in the metabolic profiles of control samples, suggesting a favorable cardiometabolic profile in the summer vs. winter, as indicated by lower glucose and sugar acid concentrations and lactate to pyruvate ratio, and higher abundance of arginine and phosphate. Similar differences were identified between cases and controls among summer pregnancies, indicating an association between a stressed metabolism and depressive symptoms. No depression-specific differences were apparent among depressed and non-depressed women, in the winter pregnancies; this could be attributed to an already stressed metabolism due to the winter living conditions. Our results provide new insights into the pathophysiology of antenatal depression, and warrant further investigation of the use of metabolomics in antenatal depression in larger cohorts.

  • Structural and functional studies of a novel Botulinum neurotoxin and of MTH1

    • X-ray crystallography visualizes the three dimensional molecular structures of proteins at atomic resolution. Seeing the molecular structure of a biomedically interesting protein enables a higher understanding of its function. The process of producing pure protein from genetic material to generate crystals and determine the molecular structure can be a long and challenging process. My thesis involves structural and functional studies of two different proteins, which are both biomedically interesting and important to learn about. X-ray crystallography is the method which has been used to determine the majority of the protein structures that we know of today and is also the method used in the results presented in my thesis. Today there are no cancer therapies defeating all types of cancers and they do not come without side effects. Battling cancer diseases often include long and painful treatments. Finding an anti-cancer drug targeting phenotypes characteristic of cancer cells is a compelling thought. MutT homolog-1 (MTH1) is an enzyme present in all proliferating cells. The enzyme seems to be crucial for cancer cell survival but not for the viability of normal cells. MTH1 cleans out oxidized and thereby damaged nucleotides from the free nucleotide pool and stops them from being used in DNA synthesis. This process is very important in fast proliferating cancer cells. The hypothesis is to inhibit MTH1 and thereby allow a limitless amount of DNA damage in the cancer cells. This action will eventually kill cancer cells while not affecting normal cells. The molecular structure of MTH1 with (PDB ID: 3ZR0) and without a product bound (PDB ID: 3ZR1) was determined and is presented in my thesis. These two structures aided in the synthesis of inhibitors. Botulinum neurotoxins (BoNTs) are the most potent toxins known. As little as one gram of pure toxin could potentially kill one million people. Due to its potency BoNT is a potential  bioterrorism threat. The toxin is also a very potent drug used clinically to relieve the symptoms of an array of neuromuscular disorders. Most people know this neurotoxin by one of its commercial names: Botox™. Additionally BoNTs are the cause of botulism. BoNTs are neuro-specific enzymes that target neuromuscular signaling, inducing flaccid paralysis and potentially death. It is of importance to learn more about these toxins to enable the development of new countermeasures, vaccines or more efficient neuroparalytic drugs. BoNTs consist of three domains with different functions, all crucial for intoxication. The toxins are fragile and can easily be destroyed by harsh surroundings if not protected by non-toxic non-hemagglutinin (NTNH) proteins. The complex of some BoNT serotypes and their protective NTNH have proven to be pH-dependent. Parts of the intoxication process are not yet clear and their mechanisms are still puzzling researchers. Until recently seven BoNT serotypes were identified. We have now identified and characterized a novel serotype called BoNT/X. The molecular structure of the active domain is presented here (PDB ID: 6F47). The pH-dependent mechanism forming a complex as seen in other serotypes, is confirmed to be present in BoNT/X as well.

  • Cause-specific mortality in Swedish males diagnosed with non-psychotic mental disorders in late adolescence: a prospective population-based study.

    • While risk of premature death is most pronounced among persons with severe mental illness, also milder conditions are associated with increased all-cause mortality. We examined non-psychotic mental (NPM) disorders and specific causes of natural death in a cohort of late adolescent men followed for up to 46 years.Prospective cohort study of Swedish males (n=1 784 626) who took part in structured conscription interviews 1968-2005. 74 525 men were diagnosed with NPM disorders at or prior to conscription. Median follow-up time was 26 years. HRs for cause-specific mortality were calculated using Cox proportional hazards models.Risks in fully adjusted models were particularly elevated for death by infectious diseases (depressive and neurotic/adjustment disorders (HR 2.07; 95%CI 1.60 to 2.67), personality disorders (HR 2.90; 95%CI 1.96 to 4.28) and alcohol-related and other substance use disorders (HR 9.02; 95%CI 6.63 to 12.27)) as well as by gastrointestinal causes (depressive and neurotic/adjustment disorders (HR 1.64; 95%CI 1.42 to 1.89), personality disorders (HR 2.77; 95%CI 2.27 to 3.38) and alcohol-related/substance use disorders (HR 4.41; 95%CI 3.59 to 5.42)).Young men diagnosed with NPM disorders had a long-term increased mortality risk, in particular due to infectious and gastrointestinal conditions. These findings highlight the importance of early preventive actions for adolescents with mental illness.

  • Greenhouse gas emissions from beef production systems in Denmark and Sweden

    • The purpose of the study was to define and describe typical beef production systems in Denmark and Sweden and estimate greenhouse gas (GHG) emissions including contribution from soil carbon changes and land use change (LUC) in a life cycle perspective (LCA). Five typical Danish (DK) and four typical Swedish (SE) systems were identified; hereof three systems with beef from beef breed cattle and six systems with beef from bull calves derived from dairy production system (including steers). The beef breed systems include an extensive system (DK) and two intensive systems (SE, DK). In the systems with beef from dairy bull calves, the bull calves were slaughtered at different ages; 9.0 months (SE), 9.4 months (DK), 11.5 months (DK), 19.0 months (SE) and at 25.0 months in the two systems with steers (DK, SE). Feed use and carbon footprint (CF) per kg meat were positively correlated. Beef from dairy bull calves slaughtered between 9.0 and 19.0 months had the lowest CF (8.9–11.5kg CO2/kg carcass) and feed use (7.3–11.1kg DM/kg carcass). The steer systems had a CF of 16.6–17.0kg CO2/kg carcass and feed use of 13.2–15.5kg DM/kg carcass. The highest CF and feed use were seen for beef breed systems at 23.1–29.7kg CO2/kg carcass and 20.9–29.8kg DM/kg carcass, respectively. The GHG contribution from LUC was positively correlated to the use of arable land. Beef from dairy bull calves had the lowest LUC contribution (1.3–1.6kg CO2/kg carcass) from a land use of 9.4–11.5m2/kg carcass. The highest LUC contribution (2.5–3.5kg CO2/kg carcass) and land use of 17.3–24.7m2/kg carcass was seen for beef from beef breed systems, the Swedish dairy bull calf slaughtered at 19 month, and the Danish steer. Besides arable land, the beef breed systems also used permanent pastures that were assumed not to contribute to LUC. Carbon (C) sequestration from crop residues and use of manure had a mitigating effect on GHG emission in all beef systems. The lowest C sequestration was seen for systems with beef from dairy bull calves slaughtered between 9 and 19 months, making up 0.2–0.9kg CO2/kg carcass, and the highest C sequestration was for the steer production and the beef breed systems, contributing between 2.3 and 4.8kg CO2/kg carcass. The present study supports the hypothesis that feed use per kg carcass weight is a main driver for variation in greenhouse gas emission, land use change and soil carbon changes for beef meat sourced from different beef production systems. For the carbon footprint per kg carcass there was a positive correlation with feed use and therefore also the lowest carbon footprint per kg carcass in systems with the lowest feed intake, such as bull calves from dairy production.

    • This study investigates abstracted processes and introduces a new prototype abstraction model adapted to estimation tasks. This prototype abstraction model assumes that the processing of whole exemplar patterns supports the detection of the underlying statistics necessary for the abstraction of two extreme prototypes on the continuous criterion dimension of the task. The prototypes are stored in memory as valid reference points for future similarity-based judgments. This prototype model was compared with the cue abstraction model, which assumes that people abstract cue weights in learning and add the cue information from exemplars to infer their criterion values varying on the continuous dimension. This study hypothesises that the training mode and the number of exemplars in training interact and affect subsequent model performance at test. The results from an experiment confirmed this hypothesis and showed that observational training supports an efficient prototype abstraction and feedback training supports an efficient cue abstraction.

  • Human Rationality : Observing or Inferring Reality

    • This thesis investigates the boundary of human rationality and how psychological processes interact with underlying regularities in the environment and affect beliefs and achievement. Two common modes in everyday experiential learning, supervised and unsupervised learning were hypothesized to tap different ecological and epistemological approaches to human adaptation; the Brunswikian and the Gibsonian approach. In addition, they were expected to be differentially effective for achievement depending on underlying regularities in the task environment. The first approach assumes that people use top-down processes and learn from hypothesis testing and external feedback, while the latter assumes that people are receptive to environmental stimuli and learn from bottom-up processes, without mediating inferences and support from external feedback, only exploratory observations and actions.Study I investigates selective supervised learning and showed that biased beliefs arise when people store inferences about category members when information is partially absent. This constructivist coding of pseudo-exemplars in memory yields a conservative bias in the relative frequency of targeted category members when the information is constrained by the decision maker’s own selective sampling behavior, suggesting that niche picking and risk aversion contribute to conservatism or inertia in human belief systems. However, a liberal bias in the relative frequency of targeted category members is more likely when information is constrained by the external environment. This result suggests that highly exaggerated beliefs and risky behaviors may be more likely in environments where information is systematically manipulated, for example when positive examples are highlighted to convey a favorable image while negative examples are systematically withheld from the public eye.Study II provides support that the learning modes engage different processes. Supervised learning is more accurate in less complex linear task environments, while unsupervised learning is more accurate in complex nonlinear task environments. Study III provides further support for abstraction based on hypothesis testing in supervised learning, and abstraction based on receptive bottom-up processes in unsupervised learning that aimed to form ideal prototypes as highly valid reference points stored in memory. The studies support previous proposals that integrating the Brunswikian and the Gibsonian approach can broaden the scope of psychological research and scientific inquiry.

    • This multiple-cue judgment study investigates whether we can manipulate the judgment strategy and increase accuracy in linear and non-linear cue-criterion environments just by changing the training mode. Three experiments show that accuracy in simple linear additive task environments are improved with feedback training and intervention training, while accuracy in complex multiplicative tasks are improved with observational training. The observed interaction effect suggests that the training mode invites different strategies that are adjusted as a function of experience to the demands from the underlying cue-criterion structure. Thus, feedback and the intervention training modes invite cue abstraction, an effortful but successful strategy in combination with simple linear task structures, and observational training invites exemplar memory processes, a simple but successful strategy in combination with complex non-linear task structures. The study discusses adaptive cognition and the implication of the different training modes across a life span and for clinical populations.

    • The development of non-genotoxic therapies that activate wild-type p53 in tumors is of great interest since the discovery of p53 as a tumor suppressor. Here we report the identification of over 100 small-molecules activating p53 in cells. We elucidate the mechanism of action of a chiral tetrahydroindazole (HZ00), and through target deconvolution, we deduce that its active enantiomer (R)-HZ00, inhibits dihydroorotate dehydrogenase (DHODH). The chiral specificity of HZ05, a more potent analog, is revealed by the crystal structure of the (R)-HZ05/DHODH complex. Twelve other DHODH inhibitor chemotypes are detailed among the p53 activators, which identifies DHODH as a frequent target for structurally diverse compounds. We observe that HZ compounds accumulate cancer cells in S-phase, increase p53 synthesis, and synergize with an inhibitor of p53 degradation to reduce tumor growth in vivo. We, therefore, propose a strategy to promote cancer cell killing by p53 instead of its reversible cell cycle arresting effect.

    • Large epidemiological studies that use accelerometers for physical behavior and sleep assessment differ in the location of the accelerometer attachment and the signal aggregation metric chosen. This study aimed to assess the comparability of acceleration metrics between commonly-used body-attachment locations for 24 hours, waking and sleeping hours, and to test comparability of PA cut points between dominant and non-dominant wrist. Forty-five young adults (23 women, 18-41 years) were included and GT3X + accelerometers (ActiGraph, Pensacola, FL, USA) were placed on their right hip, dominant, and non-dominant wrist for 7 days. We derived Euclidean Norm Minus One g (ENMO), Low-pass filtered ENMO (LFENMO), Mean Amplitude Deviation (MAD) and ActiGraph activity counts over 5-second epochs from the raw accelerations. Metric values were compared using a correlation analysis, and by plotting the differences by time of the day. Cut points for the dominant wrist were derived using Lins concordance correlation coefficient optimization in a grid of possible thresholds, using the nondominant wrist estimates as reference. They were cross-validated in a separate sample (N = 36, 10 women, 22-30 years). Shared variances between pairs of acceleration metrics varied across sites and metric pairs (range in r(2) : 0.19-0.97, all p amp;lt; 0.01), suggesting that some sites and metrics are associated, and others are not. We observed higher metric values in dominant vs. non-dominant wrist, thus, we developed cut points for dominant wrist based on ENMO to classify sedentary time (amp;lt;50 mg), light PA (50-110 mg), moderate PA (110-440 mg) and vigorous PA (amp;gt;= 440 mg). Our findings suggest differences between dominant and non-dominant wrist, and we proposed new cut points to attenuate these differences. ENMO and LFENMO were the most similar metrics, and they showed good comparability with MAD. However, counts were not comparable with ENMO, LFENMO and MAD.

    • Objective To examine the associations of physical fitness and physical activity with executive function in children with overweight and obesity. Study design A cross-sectional study involving 100 children with overweight and obesity (10.1 +/- 1.1 years old; 58.0% boys). We assessed physical fitness components (ie, muscular strength, speed-agility, and cardiorespiratory fitness) using the ALPHA battery, and physical activity and sedentary time by accelerometry. Cognitive flexibility was measured by the Design Fluency Test and Trail Making Test, inhibition by the Stroop test, and planning ability by the Zoo Map Test. Results Handgrip strength was positively associated with planning ability (P = .025). Speed-agility was positively related to cognitive flexibility and inhibition (P amp;lt; .05). Cardiorespiratory fitness and an overall fitness Z-score were positively associated with indicators of cognitive flexibility (P amp;lt; .05). No associations were found for physical activity and sedentary time with executive function (P amp;gt;= .05). Conclusions Muscular strength, speed agility, and cardiorespiratory fitness are associated with executive function in children with overweight and obesity. Cognitive flexibility seems to be more robustly associated with all fitness components, whereas planning ability and inhibition might depend on the component analyzed. The positive associations found in the present study in children with overweight and obesity call for more exercise-based randomized controlled trials in this population.

  • Energy analysis of poplar production for bioenergy in Sweden

    • Poplar (Populus spp.) species are currently not widely grown in Sweden but offer interesting options for future large-scale biomass production for bioenergy. This study assesses the average annual net energy yield and the ratio between the gross energy yield and the total primary energy input for well-managed current commercial poplar production systems in Sweden, with and without fertilization with mineral nitrogen (N). The two systems perform similarly, mainly because the yield response to fertilization is estimated to be small. The average annual net energy yields with and without fertilization are estimated at 190 and 179 GJ ha−1 yr−1, respectively. The corresponding energy ratios are 32 and 34, i.e., the total primary energy inputs correspond to only ca. 3% of the gross energy yield, in both cases. Therefore, improving the net energy yield by increasing the harvest level may be considered the most important objective from an energy performance point of view. To do this, more research is needed in order to develop poplar clones that are better adapted to the Swedish climate and identify in which situations poplar responds well to fertilization. At the same time, by reducing the energy inputs associated with the most fuel-intensive activities (wood-chip transportation, chipping, harvesting, forwarding, and stump lifting), greenhouse gas emissions can be reduced. The results should be interpreted with caution since several parameters, especially the yield response to fertilization and fuel consumption rates, are associated with considerable uncertainties.

    • Energy from biomass, including lignocellulosic crops such as willow (Salix spp.), is expected to increase in importance in Sweden. This study assesses the average annual net energy yield and the ratio between gross energy yield and total primary energy input for well-managed current commercial willow production systems in Sweden subject to three levels of fertilization with mineral nitrogen (N): N-high, N-medium, and N-zero. The average annual net energy yields are estimated at 175, 133, and 86 GJ ha −1 yr −1 , and the (dimensionless) energy ratios are estimated at 19, 32, and 47, for N-high, N-medium, and N-zero, respectively. Thus, there is a trade-off between achieving a high net energy yield and achieving a high energy ratio. Since the total primary energy input amounts to ≤ 5% of the gross energy yield in all cases, and the amount of arable land is limited, productivity increases may be considered more important than energy efficiency improvements. Substantial improvements in energy performance can be achieved by increasing harvest levels and improving energy efficiency in ammonia production and biomass transportation. The results should be interpreted with caution since several input parameters, especially energy use in ammonia production, fuel consumption rates, and the yield response to fertilization, are associated with considerable uncertainties.

    • Cardiovascular fitness in late adolescence is associated with future risk of depression. Relationships with other mental disorders need elucidation. This study investigated whether fitness in late adolescence is associated with future risk of serious non-affective mental disorders. Further, we examined how having an affected brother might impact the relationship.Prospective, population-based cohort study of 1 109 786 Swedish male conscripts with no history of mental illness, who underwent conscription examinations at age 18 between 1968 and 2005. Cardiovascular fitness was objectively measured at conscription using a bicycle ergometer test. During the follow-up (3-42 years), incident cases of serious non-affective mental disorders (schizophrenia and schizophrenia-like disorders, other psychotic disorders and neurotic, stress-related and somatoform disorders) were identified through the Swedish National Hospital Discharge Register. Cox proportional hazards models were used to assess the influence of cardiovascular fitness at conscription and risk of serious non-affective mental disorders later in life.Low fitness was associated with increased risk for schizophrenia and schizophrenia-like disorders [hazard ratio (HR) 1.44, 95% confidence interval (CI) 1.29-1.61], other psychotic disorders (HR 1.41, 95% CI 1.27-1.56), and neurotic or stress-related and somatoform disorders (HR 1.45, 95% CI 1.37-1.54). Relationships persisted in models that included illness in brothers.Lower fitness in late adolescent males is associated with increased risk of serious non-affective mental disorders in adulthood.

    • BackgroundAnxiety disorders are common and associated with reduced quality of life, impaired physical and mental health and an increased economic burden for society. While evidence exists for the effectiveness of exercise treatment for depression, there is a need for high-quality randomized clinical trials (RCT) with a focus on anxiety disorders. Further research is also warranted regarding outcomes of cognitive function, other health-related variables, dose-response effects, work ability and potential mechanisms.Method/designUsing a parallel, RCT design with three assessment points (baseline, post-intervention and one-year follow-up), we aim to assess the effect of a 12-week exercise intervention in primary care patients with anxiety disorders (n=180), diagnosed using the Mini International Neuropsychiatric Interview (M.I.N.I; Swedish version 6.0.0d DSM-IV). Participants are randomly assigned to three physical exercise groups: one low-intensity training group, one moderate- to high intensity training group and one control non-exercise group. Assessments include measures of anxiety symptoms, cognitive function, physical health variables such as cardiovascular fitness, sick-leave and levels of hormones/cytokines in blood samples.DiscussionFindings from this study will provide novel insights regarding the effects of exercise treatment on not only anxiety symptoms but also other outcomes including mental and physical health, cognitive function, dose-response effects, work ability/sick leave and on biomarkers that may help explain underlying mechanisms.Trial registrationThe trial was registered at ClinicalTrial.gov NCT03247270 August 8, 2017.

    • Background Evidence suggests that participation in physical activity may support young peoples current and future mental health. Although previous reviews have examined the relationship between physical activity and a range of mental health outcomes in children and adolescents, due to the large increase in published studies there is a need for an update and quantitative synthesis of effects. Objectives The objectives of this study were to determine the effect of physical activity interventions on mental health outcomes by conducting a systematic review and meta-analysis, and to systematically synthesize the observational evidence (both longitudinal and cross-sectional studies) regarding the associations between physical activity and sedentary behavior and mental health in preschoolers (2-5 years of age), children (6-11 years of age) and adolescents (12-18 years of age). Methods A systematic search of the PubMed and Web of Science electronic databases was performed from January 2013 to April 2018, by two independent researchers. Meta-analyses were performed to examine the effect of physical activity on mental health outcomes in randomized controlled trials (RCTs) and non-RCTs (i.e. quasi-experimental studies). A narrative synthesis of observational studies was conducted. Studies were included if they included physical activity or sedentary behavior data and at least one psychological ill-being (i.e. depression, anxiety, stress or negative affect) or psychological well-being (i.e. self-esteem, self-concept, self-efficacy, self-image, positive affect, optimism, happiness and satisfaction with life) outcome in preschoolers, children or adolescents. Results A total of 114 original articles met all the eligibility criteria and were included in the review (4 RCTs, 14 non-RCTs, 28 prospective longitudinal studies and 68 cross-sectional studies). Of the 18 intervention studies, 12 (3 RCTs and 9 non-RCTs) were included in the meta-analysis. There was a small but significant overall effect of physical activity on mental health in children and adolescents aged 6-18 years (effect size 0.173, 95% confidence interval 0.106-0.239, p amp;lt; 0.001, percentage of total variability attributed to between-study heterogeneity [I-2] = 11.3%). When the analyses were performed separately for children and adolescents, the results were significant for adolescents but not for children. Longitudinal and cross-sectional studies demonstrated significant associations between physical activity and lower levels of psychological ill-being (i.e. depression, stress, negative affect, and total psychological distress) and greater psychological well-being (i.e. self-image, satisfaction with life and happiness, and psychological well-being). Furthermore, significant associations were found between greater amounts of sedentary behavior and both increased psychological ill-being (i.e. depression) and lower psychological well-being (i.e. satisfaction with life and happiness) in children and adolescents. Evidence on preschoolers was nearly non-existent. Conclusions Findings from the meta-analysis suggest that physical activity interventions can improve adolescents mental health, but additional studies are needed to confirm the effects of physical activity on childrens mental health. Findings from observational studies suggest that promoting physical activity and decreasing sedentary behavior might protect mental health in children and adolescents.     • Background: The number of notified cases of Tick-Borne Encephalitis (TBE) in Sweden has been increasing the past years despite the increased use of TBE-vaccine not subsidized by the healthcare system. Stockholm County is a high endemic area and an earlier study has shown that low-income households have lower vaccination coverage even when they are at high risk. This paper aims to determine the cost-effectiveness of a publicly funded TBE vaccination program in Stockholm. Methods: In three different cohorts with individuals aged 3, 40 or 50 years, long-term costs and health outcomes of an out-of-pocket strategy (53% of the cohort is vaccinated on their own expenses) and a structured vaccination program (full cohort is vaccinated covered by the publicly funded health care system), were estimated using a Markov model. The Markov model predicts the costs and effects in term of Quality-adjusted Life Years (QALYs) over a lifetime horizon using a third-party healthcare payer perspective. The primary results are presented as an incremental cost effectiveness ratio (ICER) indicating the additional cost required to achieve one additional QALY with the structured vaccination program. Results: The results show that the structured vaccination program is associated with a gain in QALYs and increased costs compared with an out-of-pocket strategy. The calculated ICERs were 27 761, 99 527 and 160 827 SEK/QALY in cohorts of age 3, 40 and 50, respectively. The sensitivity analyses showed that the results are robust when varying different parameters. Conclusion: Given the setting of Stockholm county, this analysis shows a cost per QALY of a free vaccinations program, especially for children of 3 years old, below generally acceptable cost-effectiveness thresholds in Sweden.
