  • Relationship Between Everyday Health Information Literacy and Attitudes Towards Mobile Technology Among Older People

    • In order to benefit from advanced mobile information technology (AMIT) in e-health services, people need competencies in finding, evaluating, and understanding health-related information in varying everyday life situations, that is, everyday health information literacy (EHIL). This study focused on the relationship between EHIL and AMIT use and attitudes towards it among older adults. A paper questionnaire was posted to a random sample of 1,500 home dwelling subjects aged 65 or more living in Northern Finland. A variety of themes were addressed in the questionnaire including a 4-item version of an EHIL screening tool. The response rate was 61% (n=918). Older adults with negative attitudes towards or having less experience with mobile information technology were likely to have poor self-estimated EHIL skills. Older people are at risk of marginalization in regard to m-services, and these results should be utilized by decision-makers and software designers in digitalization of services.

    • New e-health services and technologies are developed around the world with expectations of multiple individual and system-wide benefits. Even if there is no single reason why many e-health projects have failed to deliver their expected gains, one central contributing factor has been their narrow focus on technology and a failure to understand the use of e-health in the context of citizens’ general health information behaviour. Here we report key findings from the ongoing research project Taking Health Information Behaviour into Account: implications of a neglected element for successful implementation of consumer health technologies on older adults, funded by the Academy of Finland (2015-2020). The project aims at explicating premises for the development of e-health services that are comprehensible, meaningful, and useful in the context of how older adults seek, use and manage information, and more specifically health information, in their everyday lives.

    • To explicate how experiences with patient-accessible electronic health records correspond to the expectations of the users, we present qualitative results of older adults’ experiences with the Finnish national patient- accessible health record My Kanta and similar services. 24 persons, 17 women and 7 men aged 55-73, took part in the study. We interviewed six focus groups of 3-5 participants with previous experience of My Kanta, in two cities in Finland. We used a convenience sample and video- and audio- recording as well as note taking. The interviews were transcribed verbatim. The inductive analysis was based on content analysis. We identified major uses, enablers, barriers, and outcomes of My Kanta. In addition to earlier reported barriers and enablers, the findings show that launch-time lack of useful content and features in systems still under development can cause frustration and hinder their effective use at the time and in the long run. Concerns and barriers relating to use were socio-techno-informational and tightly associated with the contents of the system. Improved security, usabil- ity and additional information and functions might increase use. Furthermore, coherent and timely information from health-care providers should be available in the e-health services.

  • Everyday Health Information Literacy and Attitudes Towards Digital Health Services Among Finnish Older Adults

    • Everyday health information literacy (EHIL) denotes people’s confidence and perceptions of their capability to obtain, evaluate, and use health information in everyday life. This paper presents results from a survey of older Finns on how EHIL relates to perceptions of digital health services. A postal survey was conducted with a random sample of 1,500 Finns aged 55– 70 years. In total, 373 completed surveys (25%) were returned. We used an adapted screening tool to measure EHIL and subject position scales to indicate attitudes towards digital health services. The data were analysed using PLS-SEM in SmartPLS 3.0. The analysis suggested that EHIL is positively related to more positive attitudes to digital health services and negatively to more negative attitudes to such services. The results emphasise the importance of EHIL as an enabler of the acceptance of digital health services. The study also introduces new methods for analysing EHIL.

    • The paper presents a study that examines older Finnish adults' self-perceived capability to access, evaluate, understand and use health-related information, and their perceived ability to influence their health themselves, and the relationship between these factors and their current health, health behaviour and information seeking. Questionnaires were mailed by post to 1000 Finns aged 65?79 years, and a total of 281 completed questionnaires (28%) were returned. Of these, 273 were included in the study. Cross-tabulation and chi-square tests were used for the analysis. Results showed that both a stronger belief in one?s capability to master health information and to act in a healthy manner were positively linked to better health and health behaviour as well as more active information behaviour. These beliefs can be important factors for influencing certain health behaviours and even behaviour change.

    • Despite older adults being one of the most important groups of users of eHealth services, including patient accessible electronic health records, these are claimed to not be designed to suit older users, and to not necessarily fulfill their information needs. User-centered design methods often utilized when developing eHealth services have also been claimed to not work well for older adults. This paper suggests an alternative way of collecting ideas and feedback on eHealth from older adults, by including open-ended questions in the data collection of larger studies on eHealth information behavior. When data are collected by methods like focus group interviews and surveys, older adults can be requested to envision an eHealth service that suits their needs. Results from studies in Finland are presented, and show that current eHealth services do not fulfill all information needs of older adults, as answers to the question in both focus group interviews and surveys contain practical suggestions for additional and more comprehensible information, easier ways of searching for needed information, and alternative channels for communication. Open-ended questions can be a cost-effective and timesaving way to help in the design and development of patient accessible electronic health records and other types of eHealth services.

    • Information literacy research is growing in importance but has been critiqued for remaining focused on practical topics of interest and attainment approaches to practice. Prior attempts to conceptualise information literacy have also often taken place without a comprehensive understanding of the ontological or epistemological foundations of theoretical work. The aim of this panel is to critically examine theory development and use within information literacy research through discussing the ways in which understandings of information literacy, including how it happens and how it shapes social life, are both enabled and constrained through critical, sociomaterial and discursive theoretical approaches. Providing a space to discuss and reflect on the impact of theory on information literacy scholarship, this panel creates a focal point for researchers, practitioners and students interested in the constructionand advancement of conceptually rich information literacy research and practice.

  • Artificial intelligence in the information ecosystem: affordances for everyday information seeking

    • In this conceptual article, we argue that artificial intelligence (AI) systems are contributing to the generation of an environment of affordances for everyday information practices through which they exert influence on people and the planet in ways that often are left unrecognised. We illustrate our insights by focusing on the practices of information seeking in everyday life, suggesting that the affordances of AI systems integrated into search engines, social media platforms, streaming services, and media generation, shape such practices. This shaping may, paradoxically, result both in the increase and reduction of diversity of and access to information. We discuss the potential implications of these developments in terms of the sustainability of information ecosystems and suggest solutions for addressing them through regulation and education. Drawing from the fields of library and information science and science and technology studies and research on affordances, everyday information practices, and sustainability, the article seeks to respond to the need for more nuanced theoretical insights on the impact and implications of AI on information practices and to develop conceptual tools with which to examine the co-evolution of humans and information systems from a systemic perspective.

  • Age-related differences in seeking clarification to understand medical record information

    • Introduction Patient accessible electronic health records can be used to inform and empower patients. However, their use may require complementary information seeking since they can be difficult to interpret. So far, relatively little is known of the information seeking that takes place in connection to health record use, and especially the way it varies in different age groups. A better understanding of patients' preferences of where and how to find explanatory information provides valuable input for the development of health information provision and counselling services. Method. The analysis is based on the results of a national survey of Swedish individuals (N=1,411) who had used a national patient accessible electronic health record system (Journalen). Analysis. The data were analysed in SPSS 24.0 using Kruskal-Wallis tests for detecting groupwise differences and Jonckheere-Terpstra tests for discovering age-related trends in the data. Findings. Older patients were more likely to use a telephone and younger patients to use socia l contacts to ask for clarification. Generally, older adults born between 1946-1960 appear as passive information seekers. Conclusion. Age gro ups differ in their preferences on how to seek clarification, which underlines the importance of a better understanding of individual differences in delivering not only technically but also intellectually accessible health information. Calling by telephone could be a habit of present older generations whereas, to a degree, searching information online could be a comparable habit of current younger generations.

  • Anticipating ageing : Older adults reading their medical records

    • In spite of the general interest in health information behaviour, there is little earlier research on how older adults, who are still active in working life but approaching retirement, differ from other age groups. A survey with Swedish patients who had ordered and read their medical record was conducted to map the preferences and motivations of older adults (born 1946-1960) ordering a copy of their medical record, and using medical records based e-health and information services in the future. The results do not indicate an obvious linear relationship between age and motivation to use online health information but show several differences between the age groups. Older adults were less interested in communication with their medical doctor by e-mail. Yet, they had searched health information in the Internet during the last week more likely than young. They were more inclined to read medical record to get an overview of their health than young, but less confident that they understood most of the content or turn to their family and friends to seek help than the elderly. When compared to younger adults and elderly people, older adults are the least confident and least motivated to use online health information. It is suggested that older adulthood can be seen as a transitory stage of life when the need of health information increases and engagement with health changes. The results agree with prior research on the potential usefulness of (online) medical records as a way to inform citizens. However, specific provision strategies may be necessary to match the needs and motivations of different age groups.

    • Current research on electronic patient portals and electronic health records shows a broad range of benefits to patients, healthcare professionals and healthcare when patients are allowed to take part of their medical record information. There are, however, shortcomings in the current knowledge about patients’ and other stakeholders’ information interactions with electronic health record systems and what motivates them to use these. We present a tentative classification of information interactions with an electronic patient portal, the stakeholders involved in the interactions and reasons that motivate patients to interact with electronic health record information. The purpose of the classification scheme is 1) to inform the design of useful health information access systems, and simultaneously to 2) contribute to the broader information interaction research by acting as a first step in developing a more generic classification that brings together information interactions, their stakeholders and stakeholder motivations to engage with information.

  • Differences in Health Information Literacy Competencies Among Older Adults, Elderly and Younger Citizens

    • To address the research gap on age-based differences in health information literacy (HIL), we investigated how younger (born 1960–) and older adults (1946–1960), and elderly citizens (–1945) differed from each other by their HIL competencies. Data were collected with an online survey of patients using the Swedish national electronic health record system. Altogether, 2,587 users responded. One-way ANOVA with post hoc tests revealed several differences between the groups: younger adults were less likely to value health information than older adults; older adults and elderly were least likely to compare information from multiple sources and had trouble in determining health information needs; older adults were most likely to have trouble understanding health terminology and the elderly to have difficulties in understanding medicinal package labels. The study shows that HIL is not necessarily improving or declining but adapting to challenges of advanced age. © 2019, Springer Nature Switzerland AG.

    • Information behavior and practices (IBP) research has been repeatedly criticized for having little impact on information systems development (ISD). Claiming that there is a complete disconnect would be an exaggeration but it is apparent that it is not always easy to translate findings of IBP research to workable design recommendations. Based on a reading of earlier literature and a closer investigation of three illustrative example contexts, this article underlines that the value of IBP research for ISD lies in its capability to inform ISD of the variety of ways people deal with information beyond individual systems, their own wants and designers' assumptions. Moreover, it highlights that the implications of information systems go beyond their primary users. Instead of overemphasizing the contextuality of findings, a part of IBP research would benefit from an increased focus on explicating its epistemological extents and limits and identifying, which findings are transferable, what distinguishes specific contexts, what are their defining constraints and priorities, and what aspects of their uniqueness are assumptions and simple cliches.

    • A common critique is that insights from information behaviour and practises (IBP) research have difficulties to find their way to inform the design of new technologies, systems and services. There is a certain seed of truth of these statements but the situation is much more complex as are the requirements to improve the relevance of empirical observations of information activities for design and development. This panel enquires into how different approaches to IBP research can inform technology and service development in different ways, how to support interdisciplinary dialogue between IBP and systems and service design, and what novel insights from the state-of-the-art of IBP research can be drawn to support technology and service development.

    • The aim of this article is to bring forward the benefits of a better integration of a comprehensive understanding of individuals information behaviour in the design and development of e-health services. This study is a descriptive review based on a non-exhaustive selection of literature that describes the state-of-the-art, problems and opportunities identified in e-health, health information behaviour and health information literacy research. By focusing on how to tailor the information provided and the technological devices to fit the information behaviour, the approach has also potential to uncover new insights into how to adequately implement and integrate ICTs into everyday life practices of other hard-to-reach groups in society. We presuppose that it will be possible to give practical recommendations based on a combined understanding of individual differences in health information behaviour and users expectations and experiences, acquired through empirical studies focusing on older adults. Moreover, the usefulness of health information literacy as an indicator of the patterns and competences related to health information behaviour is highlighted.

    • There is not much doubt that information, information services and systems need to be useful. In this light, the relatively lack of conceptual elaboration of the concept of usefulness in the library and information science literature can be regarded as somewhat surprising. This paper provides a conceptual overview of the use of the notion of usefulness in library and information science literature, explicates its relation to key parallel concepts, and on the basis of an empirical vignette in the context of health information research, discusses the potential limits and advantages of referring to usefulness instead of and together with other related concepts. A review of literature relating to the concept of usefulness was conducted to examine how it has been used in library and information science. A close reading of the literature shows an overlap between related concepts but at the same time, diverging foci of interest in and emphasis on what and how information, information services and, for instance, information systems are considered beneficial or suitable for their users and particular uses. There is a need for better conceptual clarity in the literature regarding usefulness and related concepts. The review shows that usefulness can be literally a useful concept for addressing the user and use (versus e.g. system, content or topic) perspective to engagements with people, services, systems and beyond.

    • PurposeThis study addresses how algorithmic recommendations and their affordances shape everyday information practices among young people.Design/methodology/approachThematic interviews were conducted with 20 Finnish young people aged 15–16 years. The material was analysed using qualitative content analysis, with a focus on everyday information practices involving online platforms.FindingsThe key finding of the study is that the current affordances of algorithmic recommendations enable users to engage in more passive practices instead of active search and evaluation practices. Two major themes emerged from the analysis: enabling not searching, inviting high trust, which highlights the how the affordances of algorithmic recommendations enable the delegation of search to a recommender system and, at the same time, invite trust in the system, and constraining finding, discouraging diversity, which focuses on the constraining degree of affordances and breakdowns associated with algorithmic recommendations.Originality/valueThis study contributes new knowledge regarding the ways in which algorithmic recommendations shape the information practices in young people's everyday lives specifically addressing the constraining nature of affordances.

  • Everyday Life Information Practice : Affordances and Strategies within a Facebook Group

    • Social networking sites are integral in reshaping how we access and interact with information and others. This doctoral thesis aims to offer an in-depth understanding of engagement in an everyday life information practice within a private Facebook group. To achieve this aim, I conducted in-depth semi-structured interviews with 20 members of a private Facebook group for foreign mothers situated in Sweden.    The thesis consists of a framing essay and four research articles exploring different aspects of how the group has formed, managed, and navigated engagement in information activities within the Facebook group. Grounded in a sociocultural perspective of mediated action, the research draws on specialised concepts and theories to further unpack key themes in the study. These include affordances, cognitive authority, situated learning, community of practice, communication privacy management, the imagined audience, and context collapse. These concepts and theories form the theoretical framework for the thesis, enabling interpretations of members’ accounts of opportunities and challenges entailed with engagement in information activities within the Facebook group and the ways these were managed and navigated by the group.  The findings show that the Facebook group offers a distinctive online space providing affordances that simultaneously facilitate and constrain joint information activities. The study highlights six key affordances offered by the group: visibility, persistence, associations, accessibility, invisibility, and inaccessibility. Negotiation of mutual and shared goals and rules is found to be essential for sustaining a space that facilitates members’ engagement in information activities. However, three complex phenomena within the Facebook group are highlighted as limiting and complicating this engagement: context collapse, time collapse, and spatial collapse. These phenomena relate to issues concerning lack of anonymity; control over information quality, flow, and privacy boundaries; and the presence of large, diverse, and evolving audiences. Several challenges and risks are identified as a result, relating to the assessment of information credibility, management of privacy, and management of conflicts. The study discusses strategic ways the group manages and navigates these opportunities and challenges. Overall, the study offers an understanding of the complex formation, engagement, and management of an everyday life information practice within a Facebook group. This understanding contributes theoretical and practical insights into broader discussions on the use of Facebook groups for informational purposes in everyday life.

These include climate change, political extremism, and health threats –in many of which information plays a crucial role. The panel asks, first, what is the role andresponsibility of information behaviour research in addressing societal challenges, and second, is therea risk of jeopardising the conceptual coherence of the research field if information behaviourresearchers to a greater extent direct their research to address major societal challenges? In the panel,five panelists will present their specific position to the questions after which all attendees are invited toparticipate in the discussion. A broad audience is expected, as the topic should be of interest toeveryone at the conference.
