  • The Effects of Aspergillus fumigatus Colonization on Lung Function in Patients with Cystic Fibrosis

    • Aspergillus fumigatus is commonly isolated from CF airways. However, the impact on CF lung progression is not completely understood. In this study, using a 16-year retrospective observational cohort study (2000–2015) that included 132 patients, we determined the annual lung function, measured as percent predicted forced expiratory volume in the first second (ppFEV1), decline before and after the first colonization with A. fumigatus. Further, in the same individual, the ratios of lung function when patients were colonized with A. fumigatus and when they were not were calculated. The impact of eradication, with antifungal treatment or spontaneously, was assessed. The annual ppFEV1 was significantly lower after the first colonization with A. fumigatus. Furthermore, within the same individual, colonization with A. fumigatus for two and three years in a row was associated with 4.3% and 7.9% lower ppFEV1, respectively, compared to when not colonized. Finally, patients who eradicated A. fumigatus the following two years after colonization exhibited 9.9% and 14.5% higher ppFEV1 compared to patients who continued to produce cultures with A. fumigatus for two and three years. Our study demonstrated that A. fumigatus colonization was associated with a negative impact on lung function in the long term and eradication, spontaneously or with treatment, was associated with a better pulmonary outcome.

    • The HCl inner-valence photoelectron band at 26 eV binding energy has been recorded at high resolution. Discrete peaks arising from at least two separate vibrational progressions are superimposed on the broad continuum. Fano profiles are visible in one of the progressions. This indicates interference between superimposed electronic states, where weak avoided crossing allows two adiabatic states to couple. In the isotopic DCl molecular spectrum, the discrete lines are less pronounced, due to slower dissociation and therefore less coupling between the continuum and the bound state.

Electron-yield spectrum was obtained by setting a kinetic-energy window at 384 eV and scanning across the threshold region. At ground 410, 414, and 416 eV, photon energy, three dominant structures were observed. The increased signal-to-background ratio, intensity and resolution at third generation facilities were found to cast a different light on the dynamics of core photoionization in molecules.

    • The femtosecond dissociation of HCl after core excitation has been studied through the resonant Auger decay. The spectra contain contributions from decay occurring at both "molecular" and "atomic" internuclear distances. We have observed a new interference mechanism in these spectra: An atomic spectral line develops into a negative spectral contribution, a "hole," when detuning the excitation energy from the maximum of the Cl 2p(-1)sigma* resonance. Resonant x-ray scattering theory quantitatively explains this behavior as due to a novel destructive continuum-Continuum interference between molecular and atomic contributions to the Auger decay.

    • We present evidence for ultra-fast dissociation of molecular ammonia when photo-excited to the Nls --> 4a(1) core-hole state. This finding is based on resonant Auger spectroscopical results as well as qualitative arguments concerning the photon energy dependence of the Auger structures. Calculations of the excited state potential based on the Z + l approximation were performed. Both the calculations and the measurements indicate that the most likely fragmentation pathway for the core excited ammonia molecules leads to NH2* and H fragments. (C) 2003 Elsevier Science B.V.     • Aims/hypothesis Many cystic fibrosis patients are vitamin D-insufficient. Cystic fibrosis-related diabetes is a major complication of cystic fibrosis. The literature suggests that vitamin D might possess certain glucose-lowering properties. We aimed to assess the relationship between vitamin D and cystic fibrosis-related glucose intolerance. Methods We enrolled 898 cystic fibrosis patients from Sweden, Norway and Denmark. Vitamin D intake was assessed using a seven-day food record. Serum 25-hydroxyvitamin D (s25OHD) and HbA(1c) were measured, and an OGTT was carried out. Multiple linear and logistic regressions were used for HbA(1c) and cystic fibrosis-related diabetes/OGTT result as outcome variables, respectively. Each model was controlled for country, and for known cystic fibrosis-related diabetes risk factors: age, sex, genotype, liver dysfunction, long-term corticosteroid treatment, and lung and pancreatic function. Results Degree of vitamin D insufficiency (OR 1.36; p=0.032) and s25OHD<30 nmol/l (OR 1.79; p=0.042) were significant risk factors for cystic fibrosis-related diabetes. Accordingly, HbA(1c) value was positively associated with s25OHD<30 nmol/l and<50 nmol/l, as well as with degree of vitamin D insufficiency (adjusted R-2=20.5% and p<0.05 in all). In subgroup analyses, s25OHD<30 nmol/l determined the HbA(1c) value in paediatric patients (adjusted R-2=20.2%; p=0.017), but not in adults. Conclusions/interpretation Vitamin D status is associated with HbA(1c) and diabetes in cystic fibrosis, particularly in children. The study justifies prospective studies on the proposed role of vitamin D deficiency in the pathophysiology of diabetes mellitus.

Fast dissociation of core-excited ozone was confirmed by the presence of Auger emission from atomic oxygen in the decay spectrum from the ozone OT 1s-σ*(7a1) state. No such emission was observed after excitation of the corresponding state on the central oxygen atom. An angle-dependent energy shift of the atomic lines was observed. Analysis of the energy shift revealed that the kinetic energy released in the dissociation process is approximately 3.3 ± 0.3eV. An energy release of 8.45 was found by application of a Born-Haber cycle.

    • The severity of lung disease in cystic fibrosis (CF) may be related to the type of mutation in the cystic fibrosis transmembrane conductance regulator (CFTR) gene, and to environmental and immunological factors. Since pulmonary disease is the main determinant of morbidity and mortality in CF, it is important to identify factors that can explain and predict this variation. The aim of this longitudinal study of the whole Swedish CF population over age 7 years was to correlate genetic and clinical data with the rate of decline in pulmonary function. The statistical analysis was performed using the mixed model regression method, supplemented with calculation of relative risks for severe lung disease in age cohorts.The severity of pulmonary disease was to some extent predicted by CFTR genotype. Furthermore, the present investigation is the first long-term study showing a significantly more rapid deterioration of lung function in patients with concomitant diabetes mellitus. Besides diabetes mellitus, pancreatic insufficiency and chronic Pseudomonas colonization were found to be negative predictors of pulmonary function. In contrast to several other reports, we found no significant differences in lung function between genders. Patients with pancreatic sufficiency have no or only a slight decline of lung function with age once treatment is started, but an early diagnosis in this group is desirable.

By using time-resolved photoelectron spectroscopy to study the electronic state of the resulting ions we are able to monitor the system continuously during dissociation or rearrangement. We find that the predissociative lifetime for the 3R‴ (v′2 = 1) Rydberg state is about 150 fs. We demonstrate a powerful new technique using time-correlated femtosecond harmonic generation and laser light pulses to study the time evolution of ultrafast dynamic processes in molecules.
