    • BACKGROUND AND AIMS: The incidence of ulcerative colitis (UC) increased during the 20th century in Western Europe and the North America, but there are conflicting reports whether the incidence has declined, stabilized or continued to increase. The aim of this study was to evaluate the incidence of UC in the Uppsala Region, Sweden.METHODS: All new UC patients in Uppsala County (305,381 inhabitants) were prospectively registered during 2005-2006 and the same for all new UC patients in the Uppsala Region (642,117 inhabitants) during 2007-2009. The extent and severity of disease according to the Montreal classification, relapse rates and surgery were assessed.RESULTS: 526 UC patients were included. The mean overall incidence for the time period was 20.0 (95% CI: 16.1-23.9) cases per 100,000 inhabitants. The incidence among children <17 years of age was 8.9 per 100,000. The extent at diagnosis was evenly distributed (E1: n=167, 32%, E2: n=161, 31%, E3: n=163, 31%). Half of the cases had moderate to severe symptoms (S1: n=269, 51%, S2: n=209, 40%, S3: n=45, 8.6%). 228 (43%) relapsed and 13 (2.5%) required colectomy during the first year. Children had a higher proportion of extensive disease vs adults (27/42 vs 136/484), but no increased risk for severe symptoms or colectomy.CONCLUSION: In this prospective population-based study we found one of the highest incidences of UC in the world. The proportion of severe cases is comparable with historical data. The conclusion is that the nature of UC has not changed, only the incidence.

  • Mechanical complications after central venous catheterisation in the ultrasound-guided era : a prospective multicentre cohort study

    • BACKGROUND: Limited data are available on the incidence of mechanical complications after ultrasound-guided central venous catheterisation. We aimed to determine the incidence of mechanical complications in hospitals where real-time ultrasound guidance is clinical practice for central venous access and to identify variables associated with mechanical complications.METHODS: All central venous catheter insertions in patients ≥16 yr at four emergency care hospitals in Sweden from March 2, 2019 to December 31, 2020 were eligible for inclusion. Every insertion was monitored for complete documentation and occurrence of mechanical complications within 24 h after catheterisation. Multivariable logistic regression analyses were used to determine associations between predefined variables and mechanical complications.RESULTS: In total, 12 667 catheter insertions in 8586 patients were included. The incidence (95% confidence interval [CI]) of mechanical complications was 7.7% (7.3-8.2%), of which 0.4% (0.3-0.5%) were major complications. The multivariable analyses showed that patient BMI <20 kg m -2 (odds ratio 2.69 [95% CI: 1.17-5.62]), male operator gender (3.33 [1.60-7.38]), limited operator experience (3.11 [1.64-5.77]), and increasing number of skin punctures (2.18 [1.59-2.88]) were associated with major mechanical complication. Subclavian vein catheterisation was associated with pneumothorax (5.91 [2.13-17.26]). CONCLUSIONS: The incidence of major mechanical complications is low in hospitals where real-time ultrasound guidance is the standard of care for central venous access. Several variables independently associated with mechanical complications can be used for risk stratification before catheterisation procedures, which might further reduce complication rates.CLINICAL TRIAL REGISTRATION: NCT03782324.

    • INTRODUCTION: Central venous catheterisation is a common procedure in intensive care therapy and the use of central venous catheters is essential for treatment of many medical disorders. Although rare, central venous catheterisation is associated with mechanical complications that can be life-threatening if untreated. Real-time ultrasound guidance reduces the incidence of mechanical complications when compared with the anatomic landmark method. The purpose of this study is to determine the incidence of and potential risk factors associated with early mechanical complications of central venous catheterisation in an era where real-time ultrasound guidance has become clinical practice. METHODS AND ANALYSIS: This is a prospective, controlled, multicentre, observational study. All participating hospitals follow the same clinical guidelines for central venous catheterisation. Each central venous catheter insertion will be recorded in the common electronic chart system according to a recently revised template. An automated script-based search will identify all recorded central venous catheter insertion templates during the study period and relevant variables will be extracted. Outcome measures and independent variables are pre-defined in this study protocol. Multivariable and univariable logistic regression analysis will be used to determine associations and risk factors of mechanical complications. ETHICS AND DISSEMINATION: The Regional Ethical Review Board in Lund, Sweden has approved this study. The results will be submitted for publication in peer-reviewed medical journals and presented at national and international scientific meetings. TRIAL REGISTRATION NUMBER: NCT03782324.

Därmed får vi också ett bättre underlag även för dagens ställningsta­gande.Föremål för våra studier är Uppsala domkyrka, Gripsholms slott, Vreta kloster­kyrka, Gustav 11I:s paviljong i Haga, Kungapalatset i Vadstena och Skoklosters slott. Vi hoppas att denna utställning skall bidra till en kritisk hållning och en ökad kunskap om restaureringskonsten, som kvalificerad yrkesuppgift, tidsspegel för historiesyn och som gestaltningsideal.

    • Soil microorganisms living in close contact with minerals play key roles in the biogeochemical cycling of elements, soil formation, and plant nutrition. Yet, the composition of microbial communities inhabiting the mineralosphere (i.e., the soil surrounding minerals) is poorly understood. Here, we explored the composition of soil microbial communities associated with different types of minerals in various soil horizons. To this effect, a field experiment was set up in which mineral specimens of apatite, biotite, and oligoclase were buried in the organic, eluvial, and upper illuvial horizons of a podzol soil. After an incubation period of two years, the soil attached to the mineral surfaces was collected, and microbial communities were analyzed by means of Illumina MiSeq sequencing of the 16S (prokaryotic) and 18S (eukaryotic) ribosomal RNA genes. We found that both composition and diversity of bacterial, archaeal, and fungal communities varied across the different mineral surfaces, and that mineral type had a greater influence on structuring microbial assemblages than soil horizon. Thus, our findings emphasize the importance of mineral surfaces as ecological niches in soils.

    • We report an unusual robust ferromagnetic order above room temperature upon amorphization of perovskite [YCrO3] in pulsed laser deposited thin films. This is contrary to the usual expected formation of a spin glass magnetic state in the resulting disordered structure. To understand the underlying physics of this phenomenon, we combine advanced spectroscopic techniques and first-principles calculations. We find that the observed order-disorder transformation is accompanied by an insulator-metal transition arising from a wide distribution of Cr-O-Cr bond angles and the consequent metallization through free carriers. Similar results also found in YbCrO3-films suggest that the observed phenomenon is more general and should, in principle, apply to a wider range of oxide systems. The ability to tailor ferromagnetic order above room temperature in oxide materials opens up many possibilities for novel technological applications of this counter intuitive effect.

    • Perfluorinated alkyl substances (PFAS) were analyzed in muscle tissue from edible fish species caught in the second largest freshwater lake in Sweden, Lake Vättern (LV), and in the brackish water Baltic Sea (BS). Perfluorooctane sulfonate (PFOS) was the predominant PFAS found. PFOS concentrations were higher in LV (medians 2.9-12 ng g(-1) fresh weight) than in BS fish (medians 1.0-2.5 ng g(-1) fresh weight). Moreover, LV fish was more contaminated with several other PFAS than BS fish. This may be due to anthropogenic discharges from urban areas around LV. The PFAS pattern differed between LV and BS fish, indicating different sources of contamination for the two study areas. Human exposure to PFOS via fish intake was calculated for three study groups, based on consumption data from literature. The groups consisted of individuals that reported moderate or high consumption of BS fish or high consumption of LV fish, respectively. The results showed that PFOS intake strongly depended on individual fish consumption as well as the fish catchment area. Median PFOS intakes were estimated to 0.15 and 0.62 ng kg(-1) body weight (bw) d(-1) for the consumers of moderate and high amounts of BS fish, respectively. For the group with high consumption of LV fish a median PFOS intake of 2.7 ng kg(-1)bw d(-1) was calculated. Fish consumption varied considerably within the consumer groups, with maximum PFOS intakes of 4.5 (BS fish) or 9.6 ng kg(-1)bw d(-1) (LV fish). Comparison of our results with literature data on PFOS intake from food suggests that fish from contaminated areas may be a significant source of dietary PFOS exposure.

    • Currently, client and supplier firms are struggling with how they can make their information systems (IS) outsourcing engagements more valuable. This research points to the importance of successful practice and outcome in IS outsourcing for achieving engagements of value. To enhance the understanding of IS out­sourcing over time, a process perspective is applied. Further, this research is based on the contention that IS outsourcing is more likely to be successful if challenges are managed in a proactive manner and with a relationship perspective. Thus, understanding challenges in IS outsourcing requires a symmetric view on client and supplier.The thesis is based on two studies presented as three steps of research: development, validation and reconsideration of a conceptual framework. The licentiate study centred on the step of development whereas the present doctoral study has a focus on the steps of validation and reconsideration. The purpose of this study includes investigating how the management of successful practice and outcome in IS outsourcing can be enabled. To address the purpose, the IS outsourcing process is studied in a client–supplier relationship. This includes that different actor perspectives, including strategic, operative, and systems, are considered for a better understanding of the practice of IS outsourcing, the challenges, and how different actors convey successful IS outsourcing.The thesis contributes a framework for relational-oriented management of IS outsourcing, which provides the structure for understanding what needs to be managed in terms of key conditions, when and how in connection to actors, relationships, and process. Theoretical propositions, enhancing the understanding of the complexity of IS outsourcing as well as the number of challenges involved, are also presented. In brief, these propositions relate to the circumstance that IS outsourcing requires management on different levels and with different kinds of results in mind.

    • We have calculated spin and orbital moments for Fe clusters of sizes up to 700 atoms embedded as impurities in a bcc Co matrix. The calculations have been carried out using relativistic first-principles real-space density functional theory, and we have made a comparison with earlier experimental studies. For Fe atoms close to theFeCo interface, the spin moments are found to increase while atoms far from the interface exhibit bulklike moments. The Co moments remain essentially unchanged and close to the moment of bulk bcc Co. With increasing cluster size, the average moments of the cluster atoms decrease due to the decreased surface to volume ratio. The orbital moments of both Fe and Co are calculated to be small and they stay almost constant regardless of cluster size. Our results for spin moments agree with experimental data but the calculated orbital moments are slightly underestimated. A simplified model indicates that a compound of close-packed Fe clusters surrounded by Co show higher average total moments compared to bulk and multilayer systems with a similar concentration. This increase seems to disappear when cluster-cluster interactions are taken into account. The general trend is that for a given alloy concentration of FexCo1−x, clustering tends to lower the average magnetic moment compared to that of ordered structures and random alloys.

    • Amyloid precursor protein (APP) and its cleavage product amyloid beta (A beta) have been thoroughly studied in Alzheimer's disease. However, APP also appears to be important for neuronal development. Differentiation of induced pluripotent stem cells (iPSCs) towards cortical neurons enables in vitro mechanistic studies on human neuronal development. Here, we investigated expression and proteolytic processing of APP during differentiation of human iPSCs towards cortical neurons over a 100-day period. APP expression remained stable during neuronal differentiation, whereas APP processing changed. alpha-Cleaved soluble APP (sAPP alpha) was secreted early during differentiation, from neuronal progenitors, while beta-cleaved soluble APP (sAPP beta) was first secreted after deep-layer neurons had formed. Short A beta peptides, including A beta 1-15/16, peaked during the progenitor stage, while processing shifted towards longer peptides, such as A beta 1-40/42, when post-mitotic neurons appeared. This indicates that APP processing is regulated throughout differentiation of cortical neurons and that amyloidogenic APP processing, as reflected by A beta 1-40/42, is associated with mature neuronal phenotypes.

    • Matrix metalloproteinase (MMP)-12, S100A8/A9, and S100A12 are involved in innate immune responses. We addressed whether different aspects of oral health and non-disease-related covariates influence their levels in saliva. 436 participants were clinically examined, completed a health questionnaire, and provided stimulated saliva. Salivary levels of MMP-12, S100A8/A9, and S100A12 were determined by enzyme-linked immunosorbent assays. Lower MMP-12 levels were observed in individuals 40-64years old (yo) compared to < 40yo, and higher S100A8/A9 levels were found in individuals > 64yo compared to 40-64yo. Smokers exhibited lower MMP-12 and S100A12 levels compared to non-smokers. All three proteins were elevated in individuals with bleeding on probing (BOP)>20% compared to those with BOP/= 10% gingival pocket depths (PPD)>/=4mm compared to the ones with shallow pockets < 4mm. The extent of alveolar bone loss or presence of manifest caries did not alter any of the markers. MMP-12, S100A8/A9, and S100A12 levels were higher in participants with high periodontal inflammatory burden. All three proteins correlated positively to BOP, PPD, and to several inflammatory mediators. The explanatory variables for MMP-12 in saliva were age, smoking, presence of any tumor, and percentage of PPD>/=4mm. The determinant of salivary S100A8/A9 was percentage of BOP, while S100A12 levels were associated with percentage of BOP and presence of any tumor. Taken together, MMP-12 and the S100/calgranulin levels in saliva reflect different aspects of periodontal inflammation. Smoking and age should be taken into account in further investigation of these proteins as biomarker candidates of periodontal disease.

    • There is a clear clinical need for high-specificity plasma biomarkers for predicting risk of venous thromboembolism (VTE), but thus far, such markers have remained elusive. Utilizing affinity reagents from the Human Protein Atlas project and multiplexed immuoassays, we extensively analyzed plasma samples from 2 individual studies to identify candidate protein markers associated with VTE risk. We screened plasma samples from 88 VTE cases and 85 matched controls, collected as part of the Swedish Venous Thromboembolism Biomarker Study, using suspension bead arrays composed of 755 antibodies targeting 408 candidate proteins. We identified significant associations between VTE occurrence and plasma levels of human immunodeficiency virus type I enhancer binding protein 1 (HIVEP1), von Willebrand factor (VWF), glutathione peroxidase 3 (GPX3), and platelet-derived growth factor beta (PDGFB). For replication, we profiled plasma samples of 580 cases and 589 controls from the French FARIVE study. These results confirmed the association of VWF and PDGFB with VTE after correction for multiple testing, whereas only weak trends were observed for HIVEP1 and GPX3. Although plasma levels of VWF and PDGFB correlated modestly (rho similar to 0.30) with each other, they were independently associated with VTE risk in a joint model in FARIVE (VWF P < .001; PDGFB P = .002). PDGF. was verified as the target of the capture antibody by immunocapture mass spectrometry and sandwich enzyme-linked immunosorbent assay. In conclusion, we demonstrate that high-throughput affinity plasma proteomic profiling is a valuable research strategy to identify potential candidate biomarkers for thrombosis-related disorders, and our study suggests a novel association of PDGFB plasma levels with VTE.

    • Several methods for physical pre-treatments of source sorted organic fraction of municipal solid waste (SSOFMSW) before for anaerobic digestion (AD) are available, with the common feature that they generate a homogeneous slurry for AD and a dry refuse fraction for incineration. The selection of efficient methods relies on improved understanding of how the pre-treatment impacts on the separation and on the slurry’s AD. The aim of this study was to evaluate the impact of the performance of physical pre-treatment of SSOFMSW on greenhouse-gas (GHG) emissions and on the economy of an AD system including a biogas plant with supplementary systems for heat and power production in Sweden. Based on the performance of selected Swedish facilities, as well as chemical analyses and BMP tests of slurry and refuse, the computer-based evaluation tool ORWARE was improved as to accurately describe mass flows through the physical pre-treatment and anaerobic degradation. The environmental and economic performance of the evaluated system was influenced by the TS concentration in the slurry, as well as the distribution of incoming solids between slurry and refuse. The focus to improve the efficiency of these systems should primarily be directed towards minimising the water addition in the pre-treatment provided that this slurry can still be efficiently digested. Second, the amount of refuse should be minimised, while keeping a good quality of the slurry. Electricity use/generation has high impact on GHG emissions and the results of the study are sensitive to assumptions of marginal electricity and of electricity use in the pre-treatment.

    • Mining of pyrite minerals is a major environmental issue involving both biological and geochemical processes. Here we present a study of an artificial lake of a former uranium open pit mine with the aim to connect the chemistry and bacterial community composition (454-pyrosequencing of 16S rRNA genes) in the stratified water column. A shift in the water chemistry from oxic conditions in the epilimnion to anoxic, alkaline, and metal and sulfide-rich conditions in the hypolimnion was corresponded by a strong shift in the bacterial community, with few shared operational taxonomic units (OTU) between the water layers. The epilimnetic bacterial community of the lake (~20 years old) showed similarities to other temperate freshwater lakes, while the hypolimnetic bacterial community showed similarity to extreme chemical environments. The epilimnetic bacterial community had dominance of Actinobacteria and Betaproteobacteria. The hypolimnion displayed a higher bacterial diversity and was dominated by the phototrophic green sulphur bacterium of the genus Chlorobium (ca. 40 % of the total community). Deltaproteobacteria were only represented in the hypolimnion and the most abundant OTUs were affiliated with ferric iron and sulfate reducers of the genus Geobacter and Desulfobulbus, respectively. The chemistry is clearly controlling, especially the hypolimnetic, bacterial community but the community composition also indicates that the bacteria are involved in metal cycling in the lake.

The results of the study do not support a convincing treatment effect by KOS on IR.Objectives: Idiopathic rhinitis (IR) is a common disorder, affecting ∼10-20% of the population. A new method for treating IR, Kinetic Oscillation Stimulation (KOS), has been reported to have beneficial effects on total vasomotor symptom scores (TVRSS). The primary objective with this study was to evaluate if a mean pressure of 65 mbar, pressure amplitude of 100 mbar, and 68Hz treatment with KOS had a positive effect on total vasomotor symptom scores (TVRSS), as compared with a mean pressure of 65 mbar, pressure amplitude of 4 mbar, and 68Hz treatment in patients with idiopathic rhinitis.Methods: Two hundred and seven patients were randomized (Full Analysis Set, FAS) in the study, including five visits and lasting for approximate to 25 weeks. All patients had two treatment episodes, and all patients had at least one treatment, meant as active, with high amplitude pressure for 10min in each nostril. Group 1 had two such treatments, and Group 2 had one treatment with low amplitude pressure, initially meant as placebo, on one occasion. Because of numerical improvements in these two groups, a new control group, Group 3, was introduced. They had one new control treatment where the balloon was inserted into the nose, without any air inflation and without oscillations.Results: KOS treatment with high amplitude pressure did not have significant beneficial effects as compared to low amplitude pressure on TVRSS. Numerical improvements in TVRSS and SNOT 22 were found when comparing high and low amplitude pressure treatments with uninflated balloon treatment. However, this part of the study was initially single-blinded, and these results were secondary objectives.

    • Tailpipe particulate emissions, i.e., particle number, size distribution and total mass, from a series of four-cylinder engines with 2L displacement and power output of approximately 150 hp have been measured. The engines were in their respective vehicle installation, all midsize vehicles from various manufacturers, and represented different combustion concepts, i.e., port- and direct-injected vehicles and E5 and E85 fuels. The results are compared to post-Euro V emission standards for gasoline and biofuels using diesel as reference. The results show that the type of combustion and fuel significantly affect the particulate formation. In general, direct-injected engines show high particle numbers and mass compared to port-injected engines. The particulate number and total mass can be reduced by using biofuels, e.g., ethanol mixes, instead of gasoline. Moreover, an experimental procedure and setup facilitating precise studies of oxidation of particulates in realistic filter structures by well-controlled gas flow (composition and temperature) and sample (particulate load and temperature) conditions has been developed. The results from this method have been verified by using commercial soot as reference.

    • We discuss in this paper the magnetic and structural parameters of Fe/V and Fe/Co multilayers. The electronic structure, magnetic moments (spin and orbital) and Curie temperatures as well as the magneto-crystalline anisotropy are calculated using first principles theory. Although theory is fairly successful in reproducing the experimental data we argue that the observed difference between theory and experiment most likely is due to lattice imperfections and that the interface between e.g. Fe and V is not perfectly sharp. We also present a model, based on the theory of elasticity, for analysing the structural properties of multilayers.

  • Från sonett till drömtext : Gunnar Björlings väg mot modernismen

    • The subject of this dissertation is the Finland-Swedish author Gunnar Björling (1887-1960) and his journey towards Modernism. The principle thesis of this dissertation, the idea that Björling’s distinctive language cannot be understood without an underlying modernistic current, is outlined in Chapter I. It also discusses the idea that Modernism cannot be reduced to a matter of style, but encompasses particular values, ideological stances and practical matters, such as the forming of alliances, writing manifestos and publishing journals.Chapter II examines how and why Björling came to be regarded as a modernist. A strongly contributing factor was Björling’s language, which from the very first violated prevailing ideas on acceptable forms of expression. The fact that he published his first book through the modernist publishing firm Daimon, and contributed to its journal Ultra, further confirmed his modernistic placing.Chapter III focuses on Björling’s unpublished juvenilia from the 1910’s. The young Björling wrote traditional rhyming verse but was also influenced by writings of a more symbolistic and modern character. The chapter concludes with a study of bizarrerierna, Björling’s dream notes, originally written during the 1910’s but first published in 1928. With their bold enjambments and highly compressed form, these notes came to influence the continued development of his lyrical imagery.Chapter IV deals with his debut Vilande dag (Resting day) (1922) and its symbolistically coloured language problematic. Björling struggled to find a language that would convey his experience of the limitlessness of being, a language striving to transcend words, leaving space for silence.This struggle for the right words continues in the later Korset och löftet (The cross and the promise) (1925) and Chapter V demonstrates how this is connected with the quest for God. In the wake of German Expressionism and Dadaism, Björling adopts a more disjointed syntax and a bolder and more dissonant lyrical imagery. There also occurs a thematic expansion, giving greater prominence to the daily life of the metropolis, a life transformed by media. In accordance with modernistic patterns, Björling chose to end his book with a theoretical epilogue, which is a distinctive collation of life philosophy, anti-clericalism, passionate religiousness, moral-philosophical discussions and aesthetic expositions.

  • Blood inflammatory and endothelial markers in women with von Willebrand disease

    • Introduction: VWD-affected females often experience menorrhagia. Periodical fluctuations of the sex steroids during the menstrual cycle cause changes both in the coagulation and immune system. The aim of the current study was to assess the changes in selected inflammatory and endothelial markers in women with VWD during two phases of the menstrual cycle (follicular and luteal) and to compare it with corresponding data from healthy controls.Materials and methods: The study group included 12 VWD-affected females with regular menstrual cycle, with none of them being prescribed hormone treatment. They were not pregnant or breastfeeding. The control group consisted of 102 healthy females, matched for age and BMI.Results: Within the VWD group, endostatin was higher during the follicular phase, compared to the luteal phase, although the difference was not significant (p = 0.062). sICAM-1 and IL-6 were higher in VWD-affected females, compared to the controls, sVCAM-1, cathepsin S and sP-selectin were lower (p<0.003 for all cases). The pattern was constant throughout the menstrual cycle.Conclusions: Higher levels of endostatin during early follicular phase could potentially predispose women with VWD to the development of heavy menstrual bleeding, due to antiangiogenic properties and ability to suppress several coagulation factors. Lower p-selectin levels in VWD group, compared to controls, may also contribute to the bleeding tendency. Changes in other proteins, involved in angiogenesis are hypothetically related to the formation of angiodysplasia—common complication of VWF deficiency. The latter statement requires confirmation in larger studies.

  • The infant bow shock : a new frontier at a weak activity comet

    • The bow shock is the first boundary the solar wind encounters as it approaches planets or comets. The Rosetta spacecraft was able to observe the formation of a bow shock by following comet 67P/Churyumov-Gerasimenko toward the Sun, through perihelion, and back outward again. The spacecraft crossed the newly formed bow shock several times during two periods a few months before and after perihelion; it observed an increase in magnetic field magnitude and oscillation amplitude, electron and proton heating at the shock, and the diminution of the solar wind further downstream. Rosetta observed a cometary bow shock in its infancy, a stage in its development not previously accessible to in situ measurements at comets and planets.

    • The climate impact of bioenergy is debated, especially due to potential land use change effects and biogenic carbon fluxes. This study assessed the climate impact and energy efficiency of conventional long-rotation forest residues (branches, tops and stumps) and short-rotation forestry (willow) from a landscape perspective. A time-dependent life cycle assessment method, which considers the timing of biogenic carbon fluxes and the impact on global temperature over time, was combined with GIS mapping to assess the impact for a specific Swedish region (Uppsala County), i.e. a 'real' landscape. The results showed that harvesting forest residues decreased the forest carbon stocks over the landscape, while growing willow on previous fallow land increased the total carbon stocks. On average, energy ratios of 49 MJ MJ(-1) for branches and tops, and 30 MJ MJ(-1) for stumps and willow was found. Harvesting forest residues from the studied landscape resulted in climate impacts of around 0.8.10(-15) K MJ(-1) heat for branches and tops, and 1.3.10(-15) K MJ(-1) heat for stumps. Willow energy gave the lowest climate impact of about -0.6,10(-15) K MJ(-1) heat. The landscape analysis showed that spatial variations in the region had an effect on energy efficiency and climate impact, but that this effect was relatively small. A more important factor was the time frame chosen for the analysis, especially for long-rotation forest systems. Methodological choices such as spatial scale (stand or landscape perspective), allocation method and functional unit also influenced the results.

    • Background: In clinical neuroanaesthesia, the increase in cerebral blood flow (CBF) and intracranial pressure caused by the cerebral vasodilative effects of an inhalational anaesthetic agent is counteracted by the cerebral vasoconstriction induced by hypocapnia. Desflurane and sevoflurane may have advantages over the more traditionally used isoflurane in neuroanaesthesia but their dose-dependent vasodilative effects at hypocapnia have not been compared in the same model using truly equipotent minimal alveolar concentrations (MACs). Method: Desflurane, sevoflurane and isoflurane were administered in a randomized order to six pigs at 0.5 and 1.0 MAC. The intra-arterial xenon clearance technique was used to calculate CBF. Blood pressure was invasively monitored. Cerebral and systemic physiological variables were recorded first at normocapnia (PaCO2 5.6 kPa) and then at hypocapnia (PaCO2 3.5 kPa). Electroencephalographic (EEG) activity was continuously recorded. Results: None of the three agents abolished cerebrovascular reactivity to hyperventilation, and at 0.5 MAC all had similar effects on CBF at hypocapnia. Desflurane at 1.0 MAC was associated with 16% higher CBF (P = 0.027) at hypocapnia than isoflurane, and with 24% higher CBF (P = 0.020) than sevoflurane. There was no seizure activity in the EEG. Conclusion: More cerebral vasodilation at hypocapnia with high doses of desflurane than with sevoflurane or isoflurane indicates that desflurane might be less suitable for neuroanaesthesia than sevoflurane and isoflurane.

    • BACKGROUND: Fibreoptic laryngotracheoscopy via the laryngeal mask airway-previously reported in adults but not in children-gives a better endoscopic view of the upper airway than does endoscopy via an endotracheal tube. METHOD: The endoscopic procedure was carried out via a size 2 laryngeal mask in 4 spontaneously breathing children between 1 and 6 years old. Anaesthesia was induced and maintained with halothane in 50-100% oxygen. RESULTS: The laryngeal mask was found to enable laryngotracheoscopy with a flexible 5.0-mm fibreoptic endoscope with no technical difficulties. Spontaneous ventilation could be readily preserved throughout the endoscopic procedure. CONCLUSIONS: In children anaesthetized with halothane, flexible fibreoptic laryngotracheoscopy via a laryngeal mask is a useful method-offering technical advantages not achieved otherwise-provided that generally approved restrictions to the use of a laryngeal mask airway are taken into account.

  • Effect on orofacial pain in patients with chronic pain participating in a multimodal rehabilitation programme : a pilot study

    • OBJECTIVES: Orofacial pain in patients taking part in a multimodal rehabilitation programme (MMRP) due to chronic bodily pain is common but it is not known whether such a rehabilitation programme can also have an effect on the presence of orofacial pain. The first aim of this study was to evaluate the effect of an MMRP on orofacial pain frequency. The second aim was to evaluate differences in the effect on quality of life and on psychosocial factors related to chronic pain.METHODS: MMRP was evaluated through validated questionnaires from the Swedish Quality Registry for Pain Rehabilitation (SQRP). Fifty-nine patients participating in MMRP filled out the two screening questions for orofacial pain in addition to the SQRP questionnaires before and after participation in MMRP during the period August 2016 to March 2018.RESULTS: Pain intensity decreased significantly after the MMRP (p=0.005). Fifty patients (69.4 %) reported orofacial pain before MMRP and no significant decrease after the programme (p=0.228). Among individuals with orofacial pain, the self-reported level of depression decreased after participation in the programme (p=0.004).CONCLUSIONS: Even though orofacial pain is common among patients with chronic bodily pain, participation in a multimodal pain programme was not enough to reduce frequent orofacial pain. This finding implies that specific orofacial pain management including information about jaw physiology could be a justified component of patient assessment prior to a multimodal rehabilitation programme for chronic bodily pain.

  • Prostate specific antigen for early detection of prostate cancer : longitudinal study

    • OBJECTIVE: To evaluate if prostate specific antigen test attains validity standards required for screening in view of recent prostate cancer screening trial results.DESIGN: Case-control study nested in longitudinal cohort.SETTING: Västerbotten Intervention Project cohort, Umeå, Sweden.PARTICIPANTS: 540 cases and 1034 controls matched for age and date of blood draw.MAIN OUTCOME MEASURE: Validity of prostate specific antigen for prediction of subsequent prostate cancer diagnosis by record linkage to cancer registry.RESULTS: Blood samples were drawn on average 7.1 (SD 3.7) years before diagnosis. The area under the curve for prostate specific antigen was 0.84 (95% confidence interval 0.82 to 0.86). At prostate specific antigen cut-off values of 3, 4, and 5 ng/ml, sensitivity estimates were 59%, 44%, and 33%, and specificity estimates were 87%, 92%, and 95%. The positive likelihood ratio commonly considered to "rule in disease" is 10; in this study the positive likelihood ratios were 4.5, 5.5, and 6.4 for prostate specific antigen cut-off values of 3, 4, and 5 ng/ml. The negative likelihood ratio commonly considered to "rule out disease" is 0.1; in this study the negative likelihood ratios were 0.47, 0.61, and 0.70 for prostate specific antigen cut-off values of 3, 4, and 5 ng/ml. For a cut-off of 1.0 ng/ml, the negative likelihood ratio was 0.08.CONCLUSIONS: No single cut-off value for prostate specific antigen concentration attained likelihood ratios formally required for a screening test. Prostate specific antigen concentrations below 1.0 ng/ml virtually ruled out a prostate cancer diagnosis during the follow-up. Additional biomarkers for early detection of prostate cancer are needed before population based screening for prostate cancer should be introduced.

  • Swedish men and smoking : Views on screening-detected abdominal aortic aneurysm

    • Abdominal aortic aneurysms (AAA), most common among elderly male smokers, often show no symptoms before rupture. To facilitate better care and counselling targeted to smoking cessation for these patients, more knowledge is required about their views. Therefore, the aim of the present study was to describe the views on AAA and smoking among male smokers with screening‐detected AAA. A qualitative approach with individual interviews with 16 men with screening‐detected AAA was applied. Three had quit smoking by the time of the interviews. Thematic analysis was performed, and four themes emerged: (i) accepting the course of life; (ii) the elusive AAA: a disturbing experiencing or merely a minor inconvenience?; (iii) being in safe hands; and (iv) smoking as an unexpected topic of discussion. Most of the participants felt they were in “safe hands”, although thoughts about death were also evoked. However, the information about smoking cessation was unexpected, and the relationship between AAA and smoking unclear. Presenting the connection between AAA and smoking in a clear manner and motivating smoking cessation in an individually‐targeted way are important.

    • Perfluorinated alkyl substances (PFAS) are today known to beglobally distributed environmental contaminants. In the present study, concentrations of PFAS were analyzed in Swedish peregrine falcon eggs (Falco peregrinus), collected between 1974and2007.Analytesincludedinthestudywereperfluorinated carboxylates (PFCAs; carbon chain lengths C6-C15), perfluorinated sulfonates (PFSAs; C4, C6, C8, and C10), and perfluorooctane sulfonamide (PFOSA). The predominant PFAS was perfluorooctane sulfonate, PFOS (83 ng/g wet weight (w wt) mean concentration in samples from 2006), followed by perfluorotridecanoate, PFTriA (7.2 ng/g w wt) and perfluoroundecanoate, PFUnA (4.2 ng/g w wt). PFCA concentrations increased exponentially over the studied time. In contrast, concentrations of PFOS and perfluorohexane sulfonate (PFHxS) increased initially but leveled off after the mid 1980s. This is different from previously observed temporal trends in marine organisms. The present study is the first to establish temporal trends for PFAS in terrestrial biota. The results indicate potential differences between marine and terrestrial biota regarding sources of PFAS exposure and response to emission changes. The toxicological implications of PFAS exposure for the falcons are not known, but according to recent findings impaired hatching success and sublethal toxicological effects from PFOS exposure in the Swedish peregrine falcon cannot be ruled out.
