    • The Internet has become an increasingly important forum for societal activism, as event mobilization, member organization, and some actions have moved online. These new types of activities, often facilitated by diverse social media platforms such as Facebook, Twitter, and Instagram, form an increasingly important part of contemporary social movements’ and organizations’ communication, work, and expression. This rapid digitalization and the increase of online activities have created a dilemma for social movement archives and researchers: Born-digital material is necessary to understand our contemporary movements, yet the materials generated and available on the Internet are rarely systematically archived. To help find solutions to this problem, the project Labor Gone Digital (DigiFacket)! set out to construct an archiving system for material created on the Internet by the Swedish trade union movement (i.e., websites and social media feeds). This article reviews the creation of the DigiFacket system and explores the challenges of building a web archive that meets both the needs of the research community and the movements occurring online, and that is easy enough to maintain, even for small archives.

  • Secondary organic aerosol from VOC mixtures in an oxidation flow reactor

    • The atmospheric organic aerosol is a tremendously complex system in terms of chemical content. Models generally treat the mixtures as ideal, something which has been questioned owing to model-measurement discrepancies. We used an oxidation flow reactor to produce secondary organic aerosol (SOA) mixtures containing oxidation products of biogenic (α-pinene, myrcene and isoprene) and anthropogenic (m-xylene) volatile organic compounds (VOCs). The resulting volume concentration and chemical composition was measured using a scanning mobility particle sizer (SMPS) and a high-resolution time-of-flight aerosol mass spectrometer (HR-ToF-AMS), respectively. The SOA mass yield of the mixtures was compared to a partitioning model constructed from single VOC experiments. The single VOC SOA mass yields with no wall-loss correction applied are comparable to previous experiments. In the mixtures containing myrcene a higher yield than expected was produced. We attribute this to an increased condensation sink, arising from myrcene producing a significantly higher number of nucleation particles compared to the other precursors. Isoprene did not produce much mass in single VOC experiments but contributed to the mass of the mixtures. The effect of high concentrations of isoprene on the OH exposure was found to be small, even at OH reactivities that previously have been reported to significantly suppress OH exposures in oxidation flow reactors. Furthermore, isoprene shifted the particle size distribution of mixtures towards larger sizes, which could be due to a change in oxidant dynamics inside the reactor.

    • Die vorliegende Arbeit untersucht die Versionen der 29 Sonette von Andreas Gryphius (1616–1664), die in allen der zu seinen Lebzeiten erschienenen Drucke vorliegen. Drei Aspekte der Gedichte werden im Sinne eines erweiterten Interpunktionsbegriffs berücksichtigt: Die Sonettgraphie und Seitengestaltung, die Interpunktionszeichen und die Hervorhebung durch Großschreibung.Die Interpunktion der Sonette wurde ständig revidiert, und es lassen sich bedeutende Unterschiede feststellen. Wer dafür verantwortlich war – der Dichter oder die verschiedenen Buchdrucker – lässt sich mit letzter Sicherheit nicht entscheiden. Doch ist wahrscheinlich, dass den Buchdruckern größere Bedeutung für die Gestaltung der Interpunktion zuzumessen ist als dem Autor selbst.Die Analysen machen auch deutlich, wie sich die Unterschiede in der Interpunktion auf die Interpretation der Sonette auswirken.

    • It has been suggested that a genetic polymorphism in the angiotensin II type 1 receptor gene (ATRG) and the ACE gene DD genotype might have a synergistic influence on the risk of developing cardiovascular disease.To study the possible interaction between polymorphisms in the ACE gene and the ATRG, regarding survival and left ventricular function.Polymorphism of the two genes was studied in a population-based cohort of 194 patients with idiopathic heart failure, recruited from the western part of Sweden 1985-1988. The patients were investigated by echocardiography. The survival status was checked during the 7-year follow-up period.Although there was no statistically significant additive risk of the ATRG polymorphism, patients carrying the ACE gene DD genotype in combination with a C allele of the ATRG tended to have a poorer prognosis. DD +AA, OR 1.24 (95% CI 0.67-2.32, P = 0.49); DD +AC, OR 1.64 (95% CI 0.95-2.83, P = 0.08); DD + CC, OR 3.54 95% CI 0.78-16.1, P = 0.10); DD +AC/CC, OR 1.84 (95% CI 1.10-3.08, P = 0.02). Patients with the DD +AC/CC genotypes tended to have lower ejection fraction and increased left ventricular mass.There was a trend toward a worse prognosis in patients with the combination of a C-allele in the ATRG and the ACE gene DD genotype, suggesting an interaction of these two genetic polymorphisms on disease severity.

    • The ion flux obtained during reactive magnetron sputtering of an Al target in Ar/O2 gas mixtures was studied by energy-resolved mass spectrometry, as a function of the total and O2 partial pressures. The positive ions of film-forming species exhibited bimodal energy distributions, both for direct current and radio frequency discharges, with the higher energy ions most likely originating from sputtered neutrals. For the negative oxygen ions a high-energy peak was observed, corresponding to ions formed at the target surface and accelerated towards the substrate over the sheath potential. As the total pressure was increased the high-energy peaks diminished due to gas-phase scattering. Based on these results, the role of energetic bombardment for the phase constituent of alumina thin films are discussed.

    • Most chronic myeloid leukaemia (CML) patients are genetically characterized by the t(9;22)(q34;q11), generating the BCR/ABL1 fusion gene. However, a few CML patients with rearrangements of 9q34 and 12p13, leading to ETV6/ABL1 chimaeras, have also been reported. Here we describe the clinical and genetic response to imatinib mesylate treatment of an ETV6/ABL1-positive CML patient diagnosed in blast crisis (BC). A chronic phase was achieved after acute myeloid leukaemia induction therapy. Then, treatment with imatinib mesylate (600 mg/d) was initiated and the effect was assessed clinically as well as genetically, including by repeated interphase fluorescence in situ hybridization studies. Until d 71 of imatinib mesylate therapy, stable improvements in the clinical and laboratory features were noted, and the frequency of ABL1-rearranged peripheral blood cells decreased from 56% to 11%. At d 92, an additional t(12;13)(p12;q13), with the 12p breakpoint proximal to ETV6, was found. The patient relapsed into BC 126 d after the start of the imatinib mesylate treatment and succumbed to the disease shortly afterwards. No mutations in the tyrosine kinase domain of ABL1 of the ETV6/ABL1 fusion were identified in the second BC. However, whereas the ETV6/ABL1 expression was seemingly the same at diagnosis and at second BC, the expression of ETV6 was markedly lower at the second BC. This decreased expression of wild-type ETV6 may have been a contributory factor for the relapse.

Portionsstorlekarna för hamburgare, pommes frites, och läsk har ökat två till fem gånger under de senaste 50 åren.

    • Traumatic brain injury (TBI) is the leading cause of death and disability in the young, active population and expected to be the third leading cause of death in the whole world until 2020. The disease is frequently referred to as the silent epidemic, and many authors highlight the "unmet medical need" associated with TBI.The term traumatically evoked brain injury covers a heterogeneous group ranging from mild/minor/minimal to severe/non-salvageable damages. Severe TBI has long been recognized to be a major socioeconomical health-care issue as saving young lives and sometimes entirely restituting health with a timely intervention can indeed be extremely cost efficient.Recently it has been recognized that mild or minor TBI should be considered similarly important because of the magnitude of the patient population affected. Other reasons behind this recognition are the association of mild head injury with transient cognitive disturbances as well as long-term sequelae primarily linked to repeat (sport-related) injuries.The incidence of TBI in developed countries can be as high as 2-300/100,000 inhabitants; however, if we consider the injury pyramid, it turns out that severe and moderate TBI represents only 25-30 % of all cases, while the overwhelming majority of TBI cases consists of mild head injury. On top of that, or at the base of the pyramid, are the cases that never show up at the ER - the unreported injuries.Special attention is turned to mild TBI as in recent military conflicts it is recognized as "signature injury."This chapter aims to summarize the most important features of mild and repetitive traumatic brain injury providing definitions, stratifications, and triage options while also focusing on contemporary knowledge gathered by imaging and biomarker research.Mild traumatic brain injury is an enigmatic lesion; the classification, significance, and its consequences are all far less defined and explored than in more severe forms of brain injury.Understanding the pathobiology and pathomechanisms may aid a more targeted approach in triage as well as selection of cases with possible late complications while also identifying the target patient population where preventive measures and therapeutic tools should be applied in an attempt to avoid secondary brain injury and late complications.

    • Assuming that learning is done best as a collaboration activity, better technical support for communication should be in place. Today's communication support for netlearning is in most cases asynchronous. Support for audio and video for synchronous communication will make it possible to collaborate more natural as in face-to-face meetings. Adding possibilities for an electronic shared workspace will amplify this collaboration to get it, in some cases, even better than face-to-face meetings. One problem with this is that people are not aware of the technological tools that exist today. Another problem is that people also might have tried synchronous communication in earlier days, where neither network, nor computers were powerful enough, which gave poor performance and a bad experience with echoing audio and blocky video with very few frames/s.This is not the case today. By making people aware of that and by making them try and use the different technologies they will get trust in use of the technology and be able to develop methodologies that utilise the technology in a pedagogic way (http://www.meetings.sunet.se/). By making people use net-based meetings as a natural way for communication new possibilities opens for netlearning.

  • Variabillity in noise susceptibility in a Swedish population : the role of 35delG mutation in the Connexin 26 (GJB2) gene

    • Although it seems that genetic factors can influence individual susceptibility to noise, still very little is known about the genes or the mechanisms involved. The connexin 26 (Cx26) (GJB2) gene is of particular interest to study in relation to noise, since the gene encodes the gap junction protein Cx26. Noise has a metabolic and mechanical effect on the inner ear and may, therefore, interfere with gap junction channels. In order to investigate whether abnormally high susceptibility to noise induced hearing loss (NIHL) in humans is associated with the common 35delG mutation in the Cx26 gene, 1200 noise‐exposed workers were investigated in Sweden. Using a selection procedure based on audiometric analysis, noise exposure data and questionnaires, noise‐exposed workers were divided into two categories: noise susceptible and noise resistant. There was a correspondence in noise susceptibility between this noise‐exposed population and the international reference ISO Standard 1999. Blood samples were drawn from 245 highly selected male subjects (103 noise susceptible, 112 noise resistant and 30 randomized cases), and genomic DNA was analysed with respect to the Cx26 35delG mutation. The incidence of 35delG carriers among this cohort was determined by multiplex, allele‐specific PCR. Two of the 245 subjects (0.8% ‐ [95% confidence interval 0.1–2.9]) were found to be heterozygous carriers of the 35delG mutation, while the remaining 243 subjects were all non‐carriers. Both the heterozygous carriers were found in the noise susceptible group. Statistical evaluation of the results demonstrated no significant difference in carrier incidence between the noise susceptible and noise resistant individuals in our Swedish noise‐exposed population. In conclusion, there was no support for a major role of Cx26 35delG mutation in explaining the variability in noise susceptibility in this Swedish population.

    • Acoustic emission (AE) is a pivotal technique in condition-based maintenance (CBM) and recent years have witnessed a significant surge in the deployment of AE sensors in industrial applications. With this increase in availability, there comes a substantial challenge: evaluating the measurement capability of sensors within specific applications. As such, this study identifies a critical need for a structured approach to evaluate the measurement capabilities of AE sensors and subsequently judge their acceptability against guideline criteria. To address this need, we present an integrated approach that systematically guides the capability evaluation of AE sensors, incorporating both qualitative and quantitative analyses. The qualitative analysis aims to scrutinize the diagnostic accuracy of sensors by assessing the detectability of features critical for diagnostics. The quantitative analysis leverages the Gage repeatability and reproducibility (Gage R&R) to statistically evaluate sensor characteristics. A comprehensive experimental study further investigates the impact of measurement sources on the sensors’ repeatability, and reproducibility. This study illustrates the qualitative findings regarding sensor’s diagnostic accuracy in both time and frequency domains, revealing promising performance in diagnostic-based evaluations. In quantitative analysis, we demonstrate the results of sensor capability in terms of repeatability and reproducibility, providing the variations of different sources in statistics-based evaluations. We thoroughly investigate the influence of significant factors, quantifying their contributions to the sensor’s measurement capability. Furthermore, we introduce metrics designed to assess sensor’s acceptability, according to explicit acceptance and rejection criteria. Our experimental results affirm that root mean square measurements are within acceptable ranges for both sensors, while spectral entropy (SE) measurements for PK15I sensor satisfy the acceptable level. For HZ136I sensor, however, SE measurements are deemed conditionally acceptable. Ultimately, the proposed approach provides a robust framework for the comprehensive evaluation of AE sensor measurement capabilities, offering invaluable guidance for sensor selection and enhancement in industrial applications.

  • A simple and efficient incompressible Navier-Stokes solver for unsteady complex geometry flows on truncated domains

    • Incompressible Navier-Stokes solvers based on the projection method often require an expensive numerical solution of a Poisson equation for a pressure-like variable. This often involves linear system solvers based on iterative and multigrid methods which may limit the ability to scale to large numbers of processors. The artificial compressibility method (ACM) [6] introduces a time derivative of the pressure into the incompressible form of the continuity equation creating a coupled closed hyperbolic system that does not require a Poisson equation solution and allows for explicit time-marching and localized stencil numerical methods. Such a scheme should theoretically scale well on large numbers of CPUs, GPU'€™s, or hybrid CPU-GPU architectures. The original ACM was only valid for steady flows and dual-time stepping was often used for time-accurate simulations. Recently, Clausen [7] has proposed the entropically damped artificial compressibility (EDAC) method which is applicable to both steady and unsteady flows without the need for dual-time stepping. The EDAC scheme was successfully tested with both a finite-difference MacCormack'€™s method for the two-dimensional lid driven cavity and periodic double shear layer problem and a finite-element method for flow over a square cylinder, with scaling studies on the latter to large numbers of processors. In this study, we discretize the EDAC formulation with a new optimized high-order centered finite-difference scheme and an explicit fourth-order Runge-€“Kutta method. This is combined with an immersed boundary method to efficiently treat complex geometries and a new robust outflow boundary condition to enable higher Reynolds number simulations on truncated domains. Validation studies for the Taylor-€“Green Vortex problem and the lid driven cavity problem in both 2D and 3D are presented. An eddy viscosity subgrid-scale model is used to enable large eddy simulations for the 3D cases. Finally, an application to flow over a sphere is presented to highlight the boundary condition and performance comparisons to a traditional incompressible Navier-€“Stokes solver is shown for the 3D lid driven cavity. Overall, the combined EDAC formulation and discretization is shown to be both effective and affordable.

  • Physical Activity and Exercise in Mild Cognitive Impairment and Dementia : An Umbrella Review of Intervention and Observational Studies

    • Objectives: The aim of this umbrella review was to determine the effect of physical activity/exercise on improving cognitive and noncognitive outcomes in people with MCI (mild cognitive impairment) and dementia.Design: Umbrella review of systematic reviews (SR), with or without meta-analyses (MAs), of randomized controlled trials (RCTs) and observational studies.Settings and Participants: People with MCI or dementia, confirmed through validated assessment measures. Any form of physical activity/exercise was included. As controls, we included participants not following any prespecified physical activity/exercise intervention or following the same standard protocol with the intervention group.Methods: The protocol was registered in PROSPERO (CDR 164197). Major databases were searched until December 31, 2019. The certainty of evidence of statistically significant outcomes was evaluated using the Grading of Recommendations Assessment, Development and Evaluation approach. SRs' findings, without a formal MA, were reported descriptively.Results: Among 1160 articles initially evaluated, 27 SRs (all of RCTs, 9 without MA) for a total of 28,205 participants with MCI/dementia were included. In patients with MCI, mind-body intervention (standardized mean difference [SMD] = 0.36; 95% confidence intervals [CI] 0.20-0.52; low certainty) and mixed physical activity interventions (SMD = 0.30; 95% CI 0.11-0.49; moderate certainty) had a small effect on global cognition, whereas resistance training (SMD = 0.80; 95% CI 0.29-1.31; very low certainty) had a large effect on global cognition. In people affected by dementia, physical activity/exercise was effective in improving global cognition in Alzheimer disease (SMD = 1.10; 95% CI 0.65-1.64; very low certainty) and in all types of dementia (SMD = 0.48; 95% CI 0.22-0.74; low certainty). Finally, physical activity/exercise improved noncognitive outcomes in people with dementia including falls, and neuropsychiatric symptoms.Conclusions and Implications: Supported by very low-to-moderate certainty of evidence, physical activity/exercise has a positive effect on several cognitive and noncognitive outcomes in people with MCI and dementia, but RCTs, with low risk of bias/confounding, are still needed to confirm these relationships.

For research and screening purposes, the Satisfaction with Daily Occupations (SDO) instrument was developed. It addresses work, leisure, domestic tasks and self-care and generates a satisfaction and an activity level score. This study investigated its construct validity, internal consistency and test–retest reliability in 55 clients visiting occupational therapists in outpatient mental health care. Methods: The SDO was administered on two occasions with a 1-week interval, and data on the Canadian Occupational Performance Measure (COPM), psychosocial functioning and self-rated health were collected on the first of these occasions. Results: In contrast to what was expected, the relationships between the SDO scores and the COPM scores were low. The strongest relationship between the SDO satisfaction score and any other variable was to psychosocial functioning. These variables shared 23% of the variance. Internal consistency for the satisfaction score was acceptable, alpha = 0.75 on the second occasion, and the test–retest reliability was good; rs = 0.84 for the satisfaction score and rs = 0.92 for the activity level. Conclusions: The results indicated that the SDO has satisfactory reliability. The instrument targets a specific construct, as compared to the COPM and the measurements of self-rated health and psychosocial functioning. Thus, the SDO and the COPM satisfaction scale seem to assess different phenomena, probably because the SDO has predefined items, not specifically targeting a certain client's problematic occupations, whereas the COPM is based on self-defined problems. The instruments may therefore supplement each other.

    • Research findings suggest that in order to clarify the relationship between occupation and health, occupation needs to be framed in several different ways. One facet of occupation is satisfaction with daily occupations, assessed using the Satisfaction with Daily Occupations (SDO) instrument. The purpose was to investigate some of the SDO's psychometric properties when applied on people with persistent mental illness. Discriminant validity against occupational value and quality of life was investigated, as was internal consistency and ability to discriminate between people with different types of daily occupation. A total of 103 patients from a psychiatric outpatient unit were selected, representing people in work or studying, people visiting activity centres, and people with no organized daily occupation. The SDO showed no or weak relationships with occupational value and quality of life, and the internal consistency was alpha=0.80. Factor analysis revealed loadings between 0.44 and 0.

    • Impaired insulin secretion from the pancreatic β-cells is central in the pathogenesis of type 2 diabetes (T2D), and microRNAs (miRNAs) are fundamental regulatory factors in this process. Differential expression of miRNAs contributes to β-cell adaptation to compensate for increased insulin resistance, but deregulation of miRNA expression can also directly cause β-cell impairment during the development of T2D. miRNAs are small noncoding RNAs that posttranscriptionally reduce gene expression through translational inhibition or mRNA destabilization. The nature of miRNA targeting implies the presence of complex and large miRNA-mRNA regulatory networks in every cell, including the insulin-secreting β-cell. Here we exemplify one such network using our own data on differential miRNA expression in the islets of T2D Goto- Kakizaki rat model. Several biological processes are influenced by multiple miRNAs in the β-cell, but so far most studies have focused on dissecting the mechanism of action of individual miRNAs. In this Perspective we present key islet miRNA families involved in T2D pathogenesis including miR-200, miR-7, miR-184, miR-212/miR-132, and miR-130a/b/miR-152. Finally, we highlight four challenges and opportunities within islet miRNA research, ending with a discussion on how miRNAs can be utilized as therapeutic targets contributing to personalized T2D treatment strategies.

    • Vegetationskartan över de svenska fjällen ger en bra översiktlig bild över den storskaliga fördelningen av vegetationstyper inom fjällområdet. Den höga generaliseringsgraden i kartan medför dock att den karakteristiskt småskaliga variationen i fjällens vegetation döljs. Slutsatsen är att vegetationskartan är alltför generaliserad för att användas som underlag för att följa förändringar i vegetationen. Det är således tveksamt om vegetationskartan uppfyller kvalitetskraven för utgöra underlagsmaterial för basinventeringen inom Natura 2000.I rapporten visas att NILS har stor potential för att tillgodose informationsbehoven för Natura 2000 inom fjällområdet. För vanligt förekommande Natura 2000 habitat, bl a Fjällhedar och boreala hedar samt Nordisk fjällbjörkskog, bedöms NILS 1x1 km 2 rutor vara tillräckligt många för att ge ett godtagbart statistiskt underlag med avseende på tillstånd och förändringar i miljön. Den pågående datainsamlingen inom NILS bedöms kunna täcka merparten av informationsbehovet för vanliga vegetationstyper. I rapporten visas att NILS 5x5 km 2 rutor har stor potential för att följa tillståndet i sparsamt förekommande habitat, t ex kalkgräsmarker och rasbranter. Detta kan göras genom kompletterande flygbildstolkning och riktade fältinventeringar utöver vad som ingår i det ordinarie programmet. För sällsynt förekommande habitat behövs en förtätning av NILS stickprovsutlägg.

    • The Chinese Loess Plateau, the world's largest and oldest loess record, preserves evidence of Asia's long-term dust source dynamics, but there is uncertainty over the source of the deposits. Recent single-grain detrital zircon U-Pb age analysis has progressed this issue, but debates remain about source changes, and the generation and interpretation of zircon data. To address this, we analyze different groupings of new and existing datasets from the Loess Plateau and potential sources. We also present the results of a first high resolution sampling, multi-proxy provenance analysis of Beiguoyuan loess using U-Pb dating of detrital zircons and detrital garnet geochemistry. The data shows that some small source differences seem to exist between different areas on the Loess Plateau. However, sediment source appears to be unchanging between loess and palaeosols, supporting a recent material recycling hypothesis. Our zircon and garnet data demonstrates, however, that Beiguoyuan experienced a temporary, abrupt source shift during the last glacial maximum, implying that local dust sources became periodically active during the Quaternary. Our results highlight that grouping data to achieve bigger datasets could cause identification of misleading trends. Additionally, we suggest that multi-proxy single-grain approaches are required to gain further insight into Chinese Loess Plateau dust sources.

    • A novel high-frequency fiber Bragg grating (FBG) sensing interrogation system by using fiber Sagnac-loop-based microwave photonic filtering is proposed and experimentally demonstrated. By adopting the microwave photonic filtering, the wavelength shift of sensing FBG can be converted into amplitude variation of the modulated electronic radio-frequency (RF) signal. In the experiment, the strain applied onto the sensing FBG has been demodulated by measuring the intensity of the recovered RF signal, and by modulating the RF signal with different frequencies, different interrogation sensitivities can be achieved.

    • We present a theoretical study of the electronic excitation in a confined one-dimensional electron gas, which is utilized to model atomic chains created in recent experiments. The length dependence of the excitation spectra is obtained from the linear response theory within the random phase approximation and time-dependent density functional theory. As the length of the chains increases, the dipole excitation spectra show a transition from electron-hole pair excitations to collective plasmon excitation. The trend of the length-dependent plasmon resonance is predicted, and the nature of the plasmon resonance is also elaborated.

    • Breast cancer is a heterogeneous disease whose subtypes represent different histological origins, prognoses, and therapeutic sensitivity. But there remains a strong need for more specific biomarkers and broader alternatives for personalized treatment. Our study classified breast cancer samples from The Cancer Genome Atlas (TCGA) into three groups based on glycosylation-associated genes and then identified differentially expressed genes under different glycosylation patterns to construct a prognostic model. The final prognostic model containing 23 key molecules achieved exciting performance both in the TCGA training set and testing set GSE42568 and GSE58812. The risk score also showed a significant difference in predicting overall clinical survival and immune infiltration analysis. This work helped us to understand the heterogeneity of breast cancer from another perspective and indicated that the identification of risk scores based on glycosylation patterns has potential clinical implications and immune-related value for breast cancer.

    • Bone has a remarkable potential for self-healing and repair, yet several injury types are non-healing even after surgical or non-surgical treatment. Regenerative therapies that induce bone repair or improve the rate of recovery are being intensely investigated. Here, we probed the potential of bone marrow stem cells (BMSCs) engineered with chemically modified mRNAs (modRNA) encoding the hBMP-2 and VEGF-A gene to therapeutically heal bone. Induction of osteogenesis from modRNA-treated BMSCs was confirmed by expression profiles of osteogenic related markers and the presence of mineralization deposits. To test for therapeutic efficacy, a collagen scaffold inoculated with modRNA-treated BMSCs was explored in an in vivo skull defect model. We show that hBMP-2 and VEGF-A modRNAs synergistically drive osteogenic and angiogenic programs resulting in superior healing properties. This study exploits chemically modified mRNAs, together with biomaterials, as a potential approach for the clinical treatment of bone injury and defects.

    • This is a description of the technique used for laparoscopy-assisted colostomy in neonates with anorectal malformations (ARM), using 2 laparoscopic ports only. The technique is easy to use and has the advantages of enabling identification of the sigmoideum, the internal genitals and to establish a stoma leaving only minimal scars in the umbilicus. In our departments, we routinely perform stoma operations using this technique. We do not claim any benefits or disadvantages of LAC compared with the traditional open technique.

    • The purpose of this study was to acquire an understanding of how nurses' self-relation (view of themselves as nurses) was influenced in connection with implementation of a nursing theory, the sympathy-acceptance-understanding-competence model for confirming nursing. This model was developed by Gustafsson and Pörn. Twenty-two nurses' written statements evaluating mentoring during the six-month implementation process in elder care, were analyzed hermeneutically with the hypothetic-deductive method. An action-theoretic and confirmatory approach was used for facilitating theoretically specified hypotheses. The nurses increased their ability to describe nursing theoretically and gained a foundation of common nursing values. The results provided an understanding of how nurses' self-relation was strengthened by becoming theoretically competent.

    • OBJECTIVES: To investigate exposure-response relationships between measured movements and postures of the wrist and the incidence of carpal tunnel syndrome (CTS), and any modifications by sex.METHODS: In 2011, we established a historical cohort of 9364 members of the Painters' Union in Denmark. Self-reported task distributions were obtained by questionnaire (53% responded) and combined with sex-specific task exposure matrices to get individual estimates of exposure intensity, that is, velocity of wrist flexion/extension, mean power frequency (MPF) and non-neutral wrist postures. Exposure duration was assessed from yearly working proportions. Registered first-time hospital discharge CTS diagnoses and CTS surgery were collected as outcomes. The cohort was followed from 1994 to 2010. Log-linear Poisson regression was used.RESULTS: For CTS diagnoses, the adjusted incidence rate ratios (IRRs) increased with increasing wrist velocity (IRR=1.37 (95% CI 1.10 to 1.71) per °/s) and MPF (IRR=1.53 (95% CI 1.21 to 1.91) per 0.01 Hz). For CTS surgery, the results were similar. The outcomes were not related to non-neutral postures or exposure duration. The adjusted IRRs for women were higher than those for men. There were no multiplicative interaction effects between exposure intensity, exposure duration and sex. However, the absolute incidence rates (IRs) increased at a steeper rate for women than for men, indicating an additive interaction.CONCLUSIONS: The incidence of CTS increased with increasing velocity of wrist flexion/extension and MPF of wrist movements. The relative increase in incidence rates was the same for women and men, but the absolute incidence rates increased at a steeper rate for women than for men.

    • The Ncf1 gene, encoding the P47(PHOX) protein that regulates production of reactive oxygen species (ROS) by the phagocyte NADPH oxidase (NOX2) complex, is associated with autoimmunity and arthritis severity in rats. We have now identified that the single-nucleotide polymorphism (SNP) resulting in an M153T amino acid substitution mediates arthritis resistance and thus explains the molecular polymorphism underlying the earlier identified Ncf1 gene effect. We identified the SNP in position 153 to regulate ROS production using COSPHOX cells transfected with mutated Ncf1. To determine the role of this SNP for control of arthritis, we used the Wistar strain, identified to carry only the postulated arthritis resistant SNP in position 153. When this Ncf1 allele was backcrossed to the arthritis susceptible DA strain, both granulocyte ROS production and arthritis resistance were restored. Position 153 is located in the hinge region between the PX and SH3 domains of P47(PHOX). Mutational analysis of this position revealed a need for an -OH group in the side chain but we found no evidence for phosphorylation. The polymorphism did not affect assembly of the P47(PHOX)/P67(PHOX) complex in the cytosol or membrane localization, but is likely to operate downstream of assembly, affecting activity of the membrane NOX2 complex.
